심부정맥혈전증에 대한 혈부축어탕의 치험 1례 (Case Study of a Deep-vein Thrombosis Patient Treated with Combinatorial Hyulbuchuko-tang)

  • Objective: This study reports on the clinical effects of combinatorial Hyulbuchuko-tang on a deep-vein thrombosis patient.Method: The deep-vein thrombosis patient was treated using combinatorial Hyulbuchuko-tang and pharmacopuncture (water-soluble carthami flos, WCF) three times a day for 33 days. The effects of deep-vein thrombosis were measured using a 0-10 numerical rating scale (NRS) and external appearance.Result: Improvements in the NRS from 7 to 2 were observed after the combinatorial Hyulbuchuko-tang treatment.Conclusions: From these results, combinatorial Hyulbuchuko-tang may be considered to show improvement for patients with deep-vein thrombosis.

  • Background: This case report shows the effect of Hyulbuchuko-tang on a patient with a contraindication to anticoagulants who complained about dyspnea caused by pulmonary embolism (PE). Case report: A PE patient with dyspnea was treated with herbal medication, Hyulbuchuko-tang, for 28 days. Evaluations took place by assessing time for oxygen application time per day, follow-up chest CT, and D-dimer test results. Oxygen application time per day decreased, and oxygen therapy ended on the 14th day of Hyulbuchuko-tang treatment. Follow-up chest CT showed resolution of PE. The D-dimer level decreased on the 24th day and decreased more after 1 month later. Conclusion: This clinical case study suggests that Hyulbuchuko-tang might be effective in the resolution of PE and can be an option as a treatment for PE patients with contraindications to anticoagulants.

  • Objectives: Deep vein thrombosis (DVT) is a common complication among stroke patients. The implication of DVT progressing into a fatal pulmonary embolism is one of the main reasons treatment cannot be delayed. However, when there is a contradiction for anticoagulants, such intracranial hemorrhage (ICH), it is difficult to determine the course of treatment. Our team reports a case with both acute DVT and ICH who improved with herbal medicine Hyulbuchuko-tang. Methods : A patient with a variety of thrombosis risk factors (atrial fibrillation, DVT, Cb-inf with intracranial hemorrhage due to thrombolytic complications) showed classic symptoms of DVT (pain, edema, discoloration), disorientation and chest discomfort. The patient was administered Hyulbuchuko-tang three times a day for 24 days without any anticoagulants. Conservative therapy including elastic stocking and leg elevation was co-administered. Laboratory tests and extremity vascular Doppler sonography were carried out 3 times during the treatment period. Results : After our treatment period, both popliteal vein DVT and calf vein DVT were not discovered by sonography, and thrombosis derived factors (eg. D-dimer, fibrinogen) decreased. There was no sign of edema or discoloration after treatment, and the patient no longer complained of leg pain, disorientation or chest discomfort. Conclusion : From these results, we suggest that there is a positive effect of Hyulbuchuko-tang on DVT. Hyulbuchuko-tang should be considered as a treatment option when western medical procedures are unavailable.

항응고제 치료에도 임상증상이 호전되지 않은 뇌경색 후 심부정맥혈전증에 대한 한의 치험 1례 (A Case Report of Korean Medicine Treatment on Clinical Symptoms in a Patient with Post-stroke Deep Vein Thrombosis)

  • This case report shows the effect of Korean medicine treatment in a patient with deep vein thrombosis (DVT) after cerebral infarction who is suspected of having post-thrombotic syndrome (PTS) due to the loss of thrombus after receiving new oral anticoagulant (NOAC) treatment. The patient was treated with Korean medicine (Boyanghwano-tang and Hyulbuchuko-tang) three times a day. Acupuncture, moxibustion, infrared, and extra-physical therapy were given to the patient for 24 days. Clinical assessment - grading of edema and circumference of the left lower extremity were observed twice a week until the end of the treatment. NOAC treatment was continued for 24 days. After treatment, the patient's edema grade and circumference of the extremity improved. On the 24th day, the patient's chief complaints improved, and she requested termination of treatment. The results suggest that Korean medicine treatments, including Boyanghwano-tang, Hyulbuchuko-tang, and acupuncture therapy, can be effective in improving the clinical symptoms of DVT and PTS.

  • Objectives: As Korea transitions into an aging society, the incidence of cerebrovascular disease is expected to increase. Herbal medicine is commonly used in Oriental medicine to treat cerebrovascular disease. However, there is insufficient clinical evidence to actively support the safety of herbal medicine in clinical practice. Therefore, the aim of this study was to determine the toxicity and safety of four herbal medicines (Cheongsimyeonja-tang, Dodam-tang, Hyeolbuchukso-tang, and Boshiniknai-tang) in patients with cerebrovascular disease. Methods: This study used electronic medical records to analyze patients admitted to an oriental medicine hospital from April 1, 2017, to December 31, 2020. Liver and renal function values at the time of admission and discharge were compared. Results: A total of 25 patients were included in this study. We found no significant differences in various variables, such as complete blood count, liver-renal function test, and urine, before and after the administration of the four herbal medicines. Additionally, no significant adverse events related to herbal medicine were observed. Conclusions: This study confirmed the safety of the four herbal medicines in patients with cerebrovascular disease who were hospitalized in a single Oriental medicine hospital.

제초제 음독 후 발생한 신경학적 후유증으로 사지마비, 인지저하를 호소한 환자의 한의치료 증례보고 1례 (A Case Report on Korean Medical Treatment for a Patient with Quadriplegia and Impaired Cognition as Neurological Sequelae after Ingestion of Herbicides)

  • This study is a case report of Korean medicine treatment for a patient with quadriplegia and impaired cognition as neurological sequelae after ingestion of herbicides. A 59-year-old man with toxic encephalopathy and hypoxic encephalopathy after the ingestion of herbicides was treated with acupuncture, Hyulbuchuko-tang mixed with Ondam-tang-gami (血府逐瘀湯合溫膽湯加味), cupping, moxibustion, and rehabilitation. Clinical symptoms were measured with the Manual Muscle Test (MMT), Korean Mini Mental Status Exam (K-MMSE), Functional Independence Measure (FIM), and Modified Barthel Index (MBI). After three months of treatment, clinical symptoms improved. The motor function improved (MMT on both sides, Gr. 3+F/4G→Gr. 4-G~4G/4+G), cognition improved (K-MMSE, 13→21), and ADL (Activities of Daily Living) scores also improved (FIM 41→74; MBI 20→63). Korean medical treatment could effectively treat neurological sequelae after ingestion of herbicides in this case.
