Development of a Basic Standard Tool for Pattern Identification in Hyperthyroidism (갑상선 기능 항진증 변증 도구 개발 기초 연구)

  • Objective: The purpose of this study was to develop a basic standard tool for pattern identification in hyperthyroidism. Method: It was based on a review of the published literature in China and Korea. The advisory committee on this study included 8 internal medicine professors from Korean Medicine Schools and 2 who had a doctor's degree and specialized in internal medicine. We were advised about the importance of pattern identification, weight gain as a symptom, the importance of treatment, changes in symptoms, rare changes in symptoms, and frequency of prescriptions regarding hyperthyroidism. Results: A Korean instrument of pattern identification was completed. Conclusion: We sincerely look forward to improving the instrument through continuous clinical studies.

The Concentration of Serum Lipids and Protein Electrophoresis Fractions in Thyroid Disease Patients

  • We analyzed and compared the concentration of total cholesterol (CHOL), high density lipoprotein (HDL) cholesterol, low density lipoprotein (LDL) cholesterol and triglyceride (TG) in serum and the serum protein electrophoresis fractions of thyroid disease patients. In comparison with the average of reference, our data showed that the average concentration of CHOL, LDL cholesterol and TG in hyperthyroidism patients were decreased significantly, but HDL cholesterol was increased significantly. In hypothyroidism patients, CHOL, HDL cholesterol, LDL cholesterol and TG were all increased significantly. In comparison of the concentration of lipids in each patient to reference range, 28.3% of hyperthyroidism patients showed abnormally low level of total cholesterol. In the patients with hypothyroidism, the percentage of patients showed abnormally high level of CHOL, HDL cholesterol, LDL cholesterol and TG were 37.7%, 10%, 68.8% and 49.1%, respectively. In our studies of serum protein electrophoresis, the average of ${\alpha}_2$-globulin and $\gamma$-globulin in hyperthyroidism patients were increased and $\beta$-globulin was decreased significantly. In hypothyroidism patients, the average of $\gamma$-globulin was increased and $\beta$-globulin was decreased significantly. In comparison of protein fractions of each patient to reference range, 38.3% and 50.0% of hyperthyroidism patients showed abnormally high levels of ${\alpha}_2$-globulin and $\gamma$-globulin, but 73.3% of patients showed abnormally low level of $\beta$-globulin. In hypothyroidism patients, 70.4% of patients were abnormally decreased in $\beta$-globulin and 63.9% of patients were abnormally increased in $\gamma$-globulin. These data suggest that the concentrations of CHOL, HDL cholesterol, LDL cholesterol and TG are not critical data for clinical interpretation of hyperthyroidism, but the levels of them are useful for interpretation of hypothyroidism patients. Our results of serum protein electrophoresis suggest that the concentration of serum protein electrophoresis fractions can be useful to understand the thyroid disease.

Diagnosis and Management of Feline Hyperthyroidism Complicated with Chronic Kidney Disease (고양이에서 만성 신부전에 병발한 갑상선 기능 항진증의 진단과 치료)

  • A 14-year-old spayed, mixed breed, female cat was admitted for evaluation of a polyphagia, hyperactivity and chronic weight loss. Physical examination revealed a tachycardia and mild elevated systemic blood pressure. This cat had azotemia and mild increased total thyroxin (TT4) and free thyroxin concentration. However triiodothyronine (T3) level was normal, the T3 suppression test for definite diagnosis were made. No changes of TT4 serum concentration before and after the exogenous T3 administration in this cat showed hyperthyroidism. This cat was diagnosed as mild hyperthyroidism concurrent with chronic kidney disease (CKD) and antithyroid drug, methimazole, was used for medical management. This is first case report describing clinical and laboratory characteristic features of feline hyperthyroidism complicated with CKD and its clinical outcome using medical management in our country.

Clinical Case Report of Hyperthyroidism Patient Treated with Only Traditional Korean Herbal Medicine (한약물 단독치료로 호전된 갑상선기능항진증 치험 1례)

  • This study was designed to evaluate the effects of traditional korean herbal medicine(Jaumkunbi-tang) therapy on a hyperthyroidism patient. Clinical data was analyzed on a patient with hyperthyroidism whose main symptoms were fatigue, palpitation, up flame, hyperhidrosis, hypersomnia and dyspepsia. The patient visited our hospital 9 times from November 10, 2011 and remianed until January 30, 2012. He was treated with Jaumkunbi-tang, the traditional korean herbal medicine. After treatment, improvement was seen in clinical symptoms(such as fatigue, palpitation, up flame, hyperhidrosis, hypersomnia and dyspepsia) and thyroid function test. The study suggests that Jaumkunbi-tang, one of traditional korean herbal medicines, could be effective on the treatment of hyperthyroidism.

A Case Report on The Relieving Effect of Herbal Medicine on Clinical Symptoms in Patients with Hyperthyroidism Taking Methimazole (메티마졸 투여 중인 갑상선기능항진증 환자에서 한약의 임상증상 완화효과에 대한 증례보고)

  • Objectives : The purpose of this study is to report the symptomatic relief effect of a herbal prescription combined with methimazole for hyperthyroidism caused by Graves' disease. Methods : After diagnosis of hyperthyroidism, methimazole was initially administered alone. As the clinical symptoms continued, a herbal medicine combining Gamiondam-tang and Cheongsimyeonja-eum was administered together with methimazole. Blood concentrations of hormones were measured, and the degree of clinical symptoms was measured using the NRS scale. The effect of herbal medicine on heat intolerance was analyzed by Wilcoxon rank sum test, and the relationship between thyroid hormone and heat intolerance was analyzed by Pearson's correlation coefficient. Results : Heat intolerance symptoms were significantly reduced when the herbal prescription was administered in parallel than when methimazole was administered alone (w=296, p=0.001). The decrease in heat intolerance was not related to thyroid hormone levels (p=0.27, 0.37). Conclusions : It was found that the herbal medicine combining Gamiondam-tang and Cheongsimyeonja-eum was effective in treating hyperthyroidism symptoms including heat intolerance.

A Case of Subclinical Hyperthyroidism Treatment with Herbal Medicine (한약 단독치료로 호전된 불현성 갑상선기능항진증 증례 보고)

  • Objective: This study was performed to evaluate the effects of herbal medicine on an elderly patient with subclinical hyperthyroidism who could not use antithyroid medication due to an adverse reaction to methimazole. Methods: Herbal medicine was administered and a thyroid function test was used to evaluate the effects of the treatment. Results: After treatment with herbal medicine and therapy, the patient showed improvements in TSH levels. Conclusion: This case suggested that herbal medicine can be used for patients who have adverse reactions to antithyroid medication; however, additional studies with more patients are required for verification of this finding.

  • The presence of polyostotic fibrous dysplasia of bone, hyperpigmented skin macules, and precocious sexual development in children is known as the McCune-Albright syndrome. In addition to the described in McCune-Albright syndrome, other endocrinopathies have been reported including hyperthyroidism, acromegaly, Cushing syndrome and vitamin D resistant rickets. The case describes a 6-year-old boy showing bony deformities due to polyostotic fibrous dysplasia, hyperpigmented skin macules, hyperthyroidism and vitamin D resistant rickets. The purpose of this report is to describe a patient of McCune-Albright syndrome with vitamin D resistant rickets which is very rare.

Iodine Intake and Excretion of the Patients with Thyroid Disease (갑상선질환 환자의 요오드섭취량과 배설량)

  • Dietary iodine intake and urinary iodide excretion were meassured from 110 patients with various thyroid hormone diseses(hypothyroidism, hyperthyroidism, simple goiter and thyroid adenoma) and 67 normal control subjects. Iodine intake was assessed on the 24-hour recall dietary data using the compiled lists of food iodine values developed from various countries. Urinary iodide concentrations of drink water samples were measured with the iodide-selective electrode. The average iodine intake of the thyroid patients was 411$\mu\textrm{g}$, which was 87% higher(p<0.05) than that of the control subjects(220$\mu\textrm{g}$). Patients with hyperthyroidism and hypothyroidism or simple goiter excreted the most(0.6442ppm) amount of iodide respectively in the urine, with the control subject in the middle(0.5229ppm). Iodide concentrations of the drinking water samples were found to be in the range of 0.0015ppm to 0.0214ppm, which seemed to vary depending on the kind(underground water vs public water) and the location.

Incidental Visualization of Thyroid Gland on Bone Scan Caused by Graves' Hyperthyroidism (뼈 스캔상 우연히 발견된 그레이브스 갑상선기능항진증에서 갑상선의 섭취증가)

  • A 45-year-old man presented with fatigue and weight loss underwent a Tc-99m MDP bone scan because of increased serum alkaline phosphatase. Delayed images at 4 hours demonstrated diffuse increased activity throughout both lobes of the thyroid in the absence of activity of the stomach and salivary glands. Thyroid laboratory indices and a Tc-99m pertechnetate thyroid scan suggested Graves' hyperthyroidism.

  • The patient in this case, 38 years-old female was adimitted for 8 days(2001. 7. 16 ${\sim}$ 2001. 7. 23). Her chief complain was lumbago, shoulder pain, headache, palpitation, general body weakness, insomnia, whole body heating and dyspnea. we diagnosed this patient as hyperthyroidism with originated from yinhuhwawang(陰虛火旺) and bigihu(脾氣虛). we treated this hyperthroidism patient with herbal medicine(Chonglijagamtang-gamibang(淸離滋坎湯加味方), bojungikgitang-gamibang(補中益氣湯加味方)), Acupunture, Moxa, etc. after oriental treatment during 8 days, pain(shoulder pain, headache, lumbago, whole body pain) was decreased and the symptoms were all diminished. in the days to come, we need to study further study about the superiority of Oriental Medicine healing a hyperthyroidism patient.
