Acute Onset of Intracerebral Hemorrhage due to Autonomic Dysreflexia

  • Autonomic dysreflexia is a clinical emergency syndrome of uncontrolled sympathetic output that can occur in patients who have a history of spinal cord injury. Despite its frequency in spinal cord injury patients, central nervous system complications are very rare. We report a man with traumatic high level incomplete spinal cord injury who suffered hypertensive right thalamic hemorrhage secondary to an episode of autonomic dysreflexia. Prompt recognition and removal of the triggering factor, the suprapubic catheter obstruction which led to hypertensive attack, the patient had a favorable functional outcome after the resorption of the hematoma and effective rehabilitation programme.

Analysis of Serum Lipids and Lipoprotein Electrophoresis Fractions in Hypertensive Patients

  • We analyzed the concentration of serum total cholesterol (TC), high density lipoprotein cholesterol (HDLC), low density lipoprotein cholesterol (LDLC), triglyceride (TG) and the serum lipoprotein electrophoresis fractions in hypertensive patients (hypertension group, n=182). The average concentration of lipids and lipoprotein fractions in the hypertension group was compared to that of the normal group. The average concentrations of serum TC, LDLC and TG in the hypertension group were significantly higher than those of the normal group (3.3%, 11% and 70%, respectively) (P<0.05). But HDLC was nonsignificantly lower (2%) (P<0.05). In the hypertension group, the percentages of patients who had an abnormally high level of total cholesterol, HDLC, LDLC and TG were 12.1%, 8.2%, 24.2% and 44.5%, respectively. The average of ${\beta}$-lipoprotein (27.5%) and pre ${\beta}$-lipoprotein (12.6%) were significantly higher, whereas ${\alpha}$-lipoprotein (15.2%) was significantly lower in the hypertension group than in the normal group (P<0.05). The percentages of patients who had an abnormally high level of very low density lipoprotein (pre ${\beta}$-LP), LDLC (${\beta}$-LP) and chylomicron were 31.3%, 17.0% and 12.6%, respectively. This study suggests that an increasing of total cholesterol, LDLC and TG, pre ${\beta}$-LP, ${\beta}$-LP and chylomicron are associated with a risk factor for hypertension.

고혈압 환자의 건강행태와 생활습관 실천정도 (The Health Care Status and Healthy Life Practices of Hypertensive Patients)

  • Purpose: The purpose of this study was to examine the health care status and healthy life practices among hypertensive patients in our South Korea in a bid to provide some information on the efficient management of hypertension. Methods: The data of the fifth Korea National Health and Nutrition Examination Survey of 2011 (KNHANES V-2) was utilized to analyze the state of hypertension management of 1,932 adults who were at the Western age of 19 and up and suffered from hypertension in order to identify what factors were related to their health care status and healthy life practices. IBM SPSS/WIN Statistics 20 software was employed, and frequency analysis and a ${\chi}^2$-test were carried out. Results: According to the results of the analysis, subjective health status, depression, and suicidal thoughts were significantly related to variable income, while health status and healthy life practices were not significant in town. Furthermore Depression, alcohol dependency, and walking were significant in hypertension patients who had experienced hypertension education programs. Conclusion: This study will provide database on hypertension management. Furthermore, multidisciplinary approaches should be taken for efficient and effective care for hypertension patients.

고혈압을 동반하는 중고도 비만 환자의 중의학 임상연구 체계적 고찰: CNKI 검색을 중심으로 (Systematic Review of TCM on Moderate to Severe Obese Patients with Hypertension in Chinese Medical Journal (CNKI))

  • Objectives: The purpose of this study is to review clinical studies related to the effect of Traditional Chinese Medicine (TCM) on moderate to severe obese patients with hypertension. Methods: Clinical studies from the China National Knowledge Infrastructure (CNKI) were searched by specific keywords and criteria. Total of 7 randomized controlled trials were selected and analyzed. Results: Various evaluation methods were used to see the effect of TCM compared to conventional western medicine. Most of the studies used herbal medicine combined with conventional anti-hypertensive western medicine. The effective rate of anti-hypertension was higher when TCM was added to conventional anti-hypertensive medicine. Traditional medicine treatment showed significant effect on lowering blood pressure and body mass index. Conclusions: Based on the results of the clinical studies from China, TCM can be a valuable option for moderate to severe obese patients with hypertension. Applying Traditional medicine has a significant effect on lowering blood pressure and weight loss. Combining herbal medicine can be worthy of clinical promotion and application for moderate and severe obese patients with hypertension.

한국삼, 중국삼, 서양삼이 고혈압 환자의 혈압에 미치는 영향 (The Effect of korean, Chinese and American Ginseng on Blood Pressure of Hypertensive Patients)

  • Objectives : We conducted a randomized, double-blinded clinical trial to assess the anti-hypertensive effect of Ginseng and to know the difference of the effect according to it's growing districts md species by 24-hour ambulatory blood pressure measurement (ABPM). Methods : We allocated 96 hypertensive patients enrolled in this trial to Korean ginseng(KG), American ginseng (AG), Chinese ginseng (CG), and Korean red ginseng (KRG) groups by randomization. Each subject was administered 4.5mg/day of encapsulated ginseng for 4 weeks. We assessed anti-hypertensive effect, blood pressure variability using ABPM and toxicity by blood chemistry before and after treatment. We also evaluated changes of symptoms due to hypertension and adverse effect in all groups at the first visit, 2 weeks later and 4 weeks later. Results : Blood pressure after treatments showed significant decrease of systolic blood pressure (sBP) in the CG-group (p<0,05) and diastolic blood pressure (dBP) in the KRG-group (p<0.05). However, there were no significant changes of sBP (or dBP) after treatment in the other groups and no significant difference in changes of BP between before and after treatment among the 4 groups. Blood pressure variability in the CG-group showed significant decrease after treatment but not in the KRG-group. Symptoms such as headache or neck stiffness and heating sensation due to hypertension improved significantly in all groups, especially in the KRG-group. A3l patients had no adverse effect after treatment and there was no liver or kidney toxicity. Conclusions : CG and KRG seem to have anti-hypertensive effects, but there was no significantly different effect depending on growing district and species of Ginseng.

Clinical Effects of Korean Ginseng, Korean Red Ginseng, Chinese Ginseng, and American Ginseng on Blood Pressure in Mild Hypertensive Subjects

  • Background : Ginseng has traditionally been used in oriental countries to recover vital energy from Qi deficiency, and has shown various biomedical effects in the scientific literature. Recent reports suggest that ginseng could regulate blood pressure (BP), but much controversy still remains. Therefore, we intended to assess the anti-hypertensive effect of several ginseng types frequently used in clinics. We also investigated the anti-hypertensive effect on Koreans and Chinese, and by the body type according to Sasang Constitution Medicine (SCM). Methods : The study subjects were recruited from mildly hypertensive patients who exhibited pre-hypertension(120/80 to 139/89 mmHg) and stage I hypertension (140/90 to 159/99 mmHg) in Korea and China. After assigning the subjects into a Korean, a Chinese, a red, and an American ginseng group by randomization, we prescribed ginseng at a dose of 4.5 g per day for 4 weeks. To assess the anti-hypertensive effect, we compared the mean of systolic and diastolic BP between before and after ginseng medication using a 24-hour ambulatory blood pressure monitor (24 hr ABPM. We also monitored adverse effect and laboratory findings to secure the subjects' safety. In addition, all of the subjects in Korea consulted a specialist of Sasang Constitution Medicine to identify their constitutional type. Results : There were 64 subjects treated with Korean ginseng, 58 treated with Chinese ginseng, 33 treated with red ginseng, and 64 treated with American ginseng. Korean, Chinese, and American ginseng all reduced subjects' BP; Korean and Chinese ginseng showed more effect. The secondary analysis on the subjects' nationality revealed that all of the ginseng types showed more significant anti-hypertensive effect in Chinese patients than in Koreans. The third analysis on the constitutional type of SCM showed there was no significant difference in the effectiveness and the safety of ginseng among the constitutional types. Conclusions : We suggest ginseng, especially Panax ginseng without any steaming-drying process, could be useful for mild hypertension. Further, ginseng is safe regardless of subjects' constitutional type or type of ginseng within a dosage of 4.5g per day.

일 농촌지역 고혈압 환자들의 건강행태와 약물복용 이행도에 관한 연구 (A Study on Health Behaviors and Medication Compliances of Hypertensive Patients in a Rural Area)

  • This study was performed to identify the health behaviors and medication compliances of hypertensive patients in a rural area, from May 1 to July 31. 2001. The subjects were 100 hypertensive patients who were registered at the Health Center in Goeje City. The data was collected by face-to-face interviews with a 25-item questionnaire on health behaviors, and analyzed by the Chi-square test on each variable. The results were as follows: Approximately 76% of the subjects were currently taking drug medication. In the comparison of health behavior rates between male and female, there were statistically significant differences in smoking (p< .000), alcohol(p< .003), low salt diet(p< .014), and the health behavior rates of female were higher than those of male. In the comparison of socioeconomic factors by medication compliance, there was statistically significant difference in sex(p< .001), and the medication compliance rate of female was higher than that of male. In the comparison of subjects' perception by medication compliance, there were statistically significant differences in seriousness of hypertension (p< .001) and medication period for hypertension care (p< .004), The medication compliance rate of the group of subjects who took the threat on their hypertension seriously was higher than that of the group who didn't, and the medication compliance rate of the group who knew that they should take medication for the rest of their life was higher than that of the group who didn't. These results suggest that community health education programs and distribution of information must be emphasized in order to increase medication and to encourage behavior changes for promoting health.

장애인 복지관을 이용하는 장애인 고혈압 환자 대상 전화상담 고혈압 자가관리 프로그램의 효과 (The effect of telephone counseling hypertension self-management program for hypertensive patients with disabilities using the welfare center for the disabled)

  • 본 연구는 장애인 복지관을 이용하는 지체장애인 및 뇌병변장애인 중 고혈압 환자를 대상으로 복지관 간호사와 간호학생 자원봉사자를 통해 제공된 8주 전화상담 고혈압 자가관리 프로그램의 효과를 파악하는데 목적이 있다. 비동등성 대조군 전후 유사실험설계 연구로, 2019년 12월부터 2020년 5월까지 사전, 사후 조사를 완료한 대조군 28명, 실험군 29명의 자료를 SPSS 프로그램을 이용하여 분석하였다. 주요 연구결과로, 프로그램 적용 후 실험군이 대조군에 비해 고혈압 관련 지식, 건강신념 중 심각성과 민감성, 삶의 질 EQ-VAS에서 통계적으로 유의한 차이를 나타나며 높아진 것으로 나타났다. 따라서 장기적인 건강관리가 필수적인 장애인 고혈압 환자를 대상으로 다양한 자가관리 프로그램을 적용하여 관련 지식, 건강신념 및 삶의 질 등을 높이는 노력이 계속적으로 필요할 것이다.

건강 인센티브 프로그램이 고혈압 환자의 혈압 조절에 미치는 영향 (Health Incentive Program and Hypertensive Patients' Blood Pressure Control)

  • 만성질환관리사업에서 건강 인센티브 프로그램에 따른 고혈압 환자의 특성을 분석하고, 고혈압 환자의 혈압 조절에 미치는 영향을 고찰하였다. 인천광역시의 웹기반 만성질환등록관리체계(iCDMS)에 등록관리된 고혈압 환자의 진료정보에서 고혈압 환자의 성, 연령, 건강포인트 적립 수준, 의원 방문 횟수 등의 변수가 혈압 조절이 이루어지고 있는 환자((n=17,278)와 그렇지 않은 환자(n=5,020)에서 통계적으로 유의한 차이를 나타냈으며, 성, 연령, 건강포인트 적립 수준이 고혈압 환자의 혈압 조절에 통계적으로 유의하게 영향을 주었다. 본 연구는 고혈압 환자의 자가 관리역량을 강화하고 지속치료를 유도하기 위한 다양한 만성질환관리사업의 개발에 활용될 수 있으며, 특히 향후 만성질환관리사업에서 건강포인트제도와 유사한 인센티브 시스템의 체계적인 개발에 유용한 자료가 될 것으로 기대된다.
