A Proposal of Dietary Supplement from Choto-san, a Kampo Medicine

  • Therapeutic effect of a Kampo medicine, Choto-san, in patients with vascular dementia was demonstrated by a double-blind and placebo-controlled clinical trial. To clarify the therapeutic efficacy of Choto-san, anti-ischemic effect in mice, hypotensive effect in spontaneously hypertensive rats (SHR), anti-oxidative effects in vitro, and N-methyl-D-aspartate (NMDA) receptor-blocking activity using Xenopus oocytes were studied. (1) Pretreatment with Choto-san (0.75-6.O g/kg, P.O.) or a component herb Chotoko (Uncaria genus: 75 - 600 mg/kg, P.O.) prevented ischemia-induced impairment of spatial learning behaviour in mice. Indole alkaloids- and phenolic fractions extracted from Chotoko also improved significantly the learning deficit. (2) Subchronic administration of Choto-san (0.5 g/kg, p.o.) caused a significant hypotensive effects in SHR. (3) Choto-san, Chotoko, and the phenolic constituent, (-) epicatechin, significantly protected the NG108-15 cell injury induced by $H_20_2$ exposure in vitro and also inhibited lipid peroxidation in the brain homogenate. (4) Indole alkaloids, rhynchophylline and isorhynchophylline (1-100 uM), reversibly reduced NMDA-induced current in the receptor-expressed Xenopus oocytes. These results suggest that anti-vascular dementia effects of Choto-san are mainly due to the effect of Chotoko. From these results, it is possible to make a novel dietary supplement through several extraction steps from Chotoko.

Study of the Herbal Medicines for Hypertension on the Chinese Paper (중의논문에 나타난 고혈압의 한약 치료)

  • Objective: The purpose of this study is to find out Herbal medicine effective to the hypertensive patients. Methods: The present study was done over 42 chinese papers about hypertension from 2000 to 2006. We calculated frequency of the herbs used to treatment for hypertension. Result & Conclusion: 1. In 42 herbal medicines, Gamibanhabaekchulcheonmatang and Gamitaeksatang showed high frequency. 2. In 125 herbs, RADIX SALVIAE MILTIORRHIZAE(丹蔘) showed highest frequency. And RAMULUS ETUNCUS UNCARIAE(釣鉤藤), RADIX ACHYRANTHIS BIDENTATAE(牛膝), RADIX PAEONIAE ALBA(白芍藥), RHIZOMA ALISMATIS(澤瀉), RHIZOMA CNIDII(川芎), PORIA(白茯苓), RHIZOMA GASTRODIAE(天麻), SPICA PRUNELLAE(夏枯草), CONCHA HALIOTIDIS(石決明) showed high frequency.

Open Heart Surgery in Infancy (영아기 개심술)

  • Fourty-three infants underwent intracardiac repairs within the first 12 months of life. Mean age was 7 months and mean weight was 6.7 kg. A quarter of them were below 5 kg. Overall mortality including complex anomalies was 17.9%. Thirty-four infants with a ventricular septal defect[VSD were treated. Three infants[8.8% died in the early postoperative period, but only one[3.7% of the infants without having associated lesions was dead. VSD was repaired either by the incision on the right atrium, pulmonary artery, or right ventricle. Postoperative right bundle branch block[RBBB occurrence were different according to the approach: 30.8 % in pulmonary arteriotomy, 37.5 % in atriotomy, and 42.8 % in ventriculotomy. Surgically induced heart block did not occur in any patient, but 10 had temporary arrhythmia, and 11 patients were under temporary pacing from one day to 4 days without any persisting rhythm disturbances. Pulmonary hypertension was present in 29 infants preoperatively, and seven of them developed postoperative elevation of the pressure. Infants complicated with pulmonary hypertensive crisis were managed successfully. Inotropics were necessary in 55.8 % of the infants and vasodilators in 37.2 %. Platelet transfusion were needed in 10 cases who required 272 ml in average, but one who died from sepsis consumed 5,370cc of platelets. And we discuss complications and causes of deaths.

Association between Genetic Variation in the Human Antithrombin III Gene and Essential Hypertension in Korean Population

  • In view of the effect of antithrombin III on essential hypertension, we investigated the 5' untranslated exon 1 length (I/D) polymorphism and intron 5'-DdeI RFLP of the human antithrombin III gene in the Korean patients with essential hypertension. There were no significant differences in the allele, genotype and haplotype frequencies of these polymorphisms between normotensive and essential hypertensive subjects. The significant linkage disequilibrium was however, detected between I/D polymorphism and Dde I RFLP. The I/D polymorphism was also significantly associated with BMI, total cholesterol (TC) and HDL- cholesterol levels, while DdeI polymorphism with age and BMI. Therefore, our results suggest that the significant association between the genetic polymorphisms in the antithrombin III gene and variable cardiovascular risk factors may reflect the potential role of human antithrombin III gene in cardiovascular function.

Pharmacokinetics of Acebutolol and Its Main Metabolite, Diacetolol After Oral Administration of Acebutolol in Rabbits with Carbon Tetrachloride-Induced Hepatic Failure

  • Pharmacokinetic characteristics of Acebutolol and its main metabolite, diacetolol, following a single 10 mg/kg oral dose, were investigated in rabbits with carbon tetrachloride-induced hepatic failure. Plasma concentrations of acebutolol and diacetolol were determined by a high performance liquid chromatography assay. The area under the plasma concentration-time curves (AUC) and maximum plasma concentration ($C_{max}$) of acebutolol were significantly increased in moderate and severe carbon tetrachloride-induced hepatic failure rabbits. The ratio of the diacetolol to total acebutolol in plasma (i.e., metabolite percentage rate) was significantly decreased in moderate and severe carbon tetrachloride-induced hepatic failure rabbits. Volume of distribution ($V_{d}$) and total body clearance ($CL_{t}$) of acebutolol were significantly decreased in moderate and severe carbon tetrachloride-induced hepatic failure rabbits. Slope of terminal phase ($\beta$) of acebutolol was significantly decreased in hepatic failure rabbits. These findings suggest that the $V_{d},{\;}CL_{t}$ and $\beta$ of acebutolol were significantly decreased as a result of inhibition of the hepatic metabolism in moderate to severe hepatic failure rabbits. Therefore, dose adjustment may be necessary for acebutolol in hypertensive patients with hepatic damage.

Recent Research Trends of Sayeok-tang Used in Ischemic Heart Disease (허혈성 심질환에 활용된 사역탕(四逆湯)의 최신 연구 동향)

  • Purpose : Sayeok-tang (Sini decoction, SND) is a cold-dispelling formula used for cold deficiency syndrome and is composed of Aconiti Lateralis Radix Preperata, Zingiberis Rhizoma, and Glycyrrhizae Radix et Rhizoma. It is used for diseases such as myocardial infarction, heart failure, acute and chronic gastroenteritis, and gastric effusion. This study proposes the possibility of expansion of basic research and clinical applications for ischemic heart disease (IHD) through systematic analysis of domestic and foreign studies on SND. Methods : We collected studies within the last 10 years on the use of SND in IHD and excluded those lacking relevance. Selected studies were classified by research method and the main themes of the studies were analyzed for each classification. Result: Out of 15 studies, there were 5 animal studies, 8 metabolite analyses in animals, 1 in vitro study, and 1 systematic review. Our review suggests that SND may be used as an adjuvant to nitroglycerin and percutaneous transluminal coronary angioplasty, and may improve symptoms and quality of life of patients with IHD. Myocardial protective effects through antioxidant, anti-inflammatory, anti-apoptotic, and anti-hypertensive actions were confirmed through these studies. Effects on carbohydrate, protein, and lipid metabolism were also reported. Conclusions : This study suggests that SND has potential as a treatment for IHD.

A Case Report for the Effects of the Modified Fasting Therapy (Gamrosu) on Obese Patients with Hypertension (감로수 절식요법을 적용한 고혈압 비만환자 증례보고)

  • Gamrosu was originally conceptualized from Jeho-tang, a selected thirst quencher of the kings in Chosun Dynasty and Saeng-Maek-san, a qi-vigorating summer beverage recommended by Dongeuibogam. It is a modified fasting therapy beverage which is manufactured from the single herbal medicines composed of those two prescriptions. This study was conducted on 6 obese patients with hypertension. A modified fasting therapy with Gamrosu was practised on them for 10 consecutive days. After the therapy, their average blood pressures were descended from 148/89 mmHg to 119/79 mmHg. The modified fasting therapy with Gamrosu is supposed to be more effective than general diet program or dietary sodium reduction on controlling hypertension. And, Gamrosu improves anti-hypertensive effect by reducing the side effects, such as fatigue, electrolyte imbalance, heartburn, nausea, and headache, during the modified fasting therapy.

Comparison of functional Images obtained by radionuclide angiocardiography and gated blood pool scan (방사성핵종 심혈관조영술의 기능적영상화에 대한 고찰)

  • Radionuclide cardiac studies lend themselves exceptionally well to functional imaging. This is especially true for gated blood pool scan (GBP). Making functional images is also possible in radionuclide angiocardiography (RNAC). In this study we tried to validate the functional images obtained from RNAC by comparing it with GBP. Twenty three patients (16 patients with coronary artery diseases, 5 with hypertensive heart diseases, and 2 with nonspecific chest pains) underwent simultaneous RNAC and GBP at the same position (LAO $45^{\circ}$). From both studies, global ejection fraction, regional ejection fraction, phase image, amplitude image, stroke image, paradox image, maximum ejection and maximum filling rates were obtained. Global ejection fraction are almost same in both studies. Regional ejection fractions of apex and inferior portion of left ventricle calculated from RNAC are well correlated with those of GBP. Phase and paradox image, maximum ejection and maximum filling rates were obtained. Global ejection fraction are almost same in both studies. Phase and paradox images of RNAC are very similar to those of GBP. However, amplitude and stroke images are different. Regional ejection fractions of the left ventricular base, maximum ejection and maximum filling rates obtained from RNAC are significantly different from those of GBP. In conclusion, albeit all of functional images of RNAC is not same as GBP, regional walt motions and global left ventricular function are expected to be successfully analyzed by phase and paradox image and ejection fraction.

  • Hypertension excessively affects national health is the basic disease that causes death and disablement after the middle-aged. Therefore, The detection and control of hypertension is essential to health care. In general, It is known that the systolic blood pressure should be below 140mmHg, the diastolic blood pressure should be below 90mmHg. Presently, the treatment of hypertension is mostly taking anti-hypertension drug, diet and stability. but Taking anti-hypertension drug has such side effects as depression. and that, We were making a study of various treatments of hypertension. In a course of hypertension study, We applied aromatherapy to the control of hypertension. Aromatherapy is the practice of using of volate plant oils including essential oils, for psychological and physical well-being. Essential oils are drawn into the body by the sense smell not only does the aroma of the true plant stimulate the brain to the trigger a positive effect, but supply physical benefits also. In Europe and North America, The study and development of aromatherapy is lively in progress. In the control of hypertension, We applied Lavender, Majoram, Ylang-Ylang to forty-two hypertensive patients. After aromatherapy for controlling hypertension. We measured the changes of the systolic blood pressure & the diastolic blood pressure of patients by time. As the result of above study, We identified the significance of blood pressure down by aromatherapy.

Study on Relationship between Iris Constitution and Apolipoprotein E Gene Polymorphism

  • lridology, a form of complementary and alternative medicine (CAM), is the diagnosis of medical conditions through noting irregularities of the pigmentation in the iris. lridological constitution has a strong familial aggregation and is implicated in heredity. Apolipoprotein E (apoE) gene polymorphism is one of the most well studied genetic markers of vascular disease. I investigated the relationship between iridological constitution and apoE polymorphism. I classified 87 hypertensive patients with family history of cerebral infarction and 79 controls according to iris constitution, and determined apoE genotype. Neurogenic type in hypertensives was 32.2% compared with 16.5% in controls (P<0.001). No differences in the apoE genotypes frequencies were observed in patients compared with those in controls ($x^2=0.726$, df-=2, P=0.696). However, in a population with ${\varepsilon}3/{\varepsilon}4$ genotype, the frequency of neurogenic constitution was significantly higher in hypertensives than in controls (60% vs. 0%) ($x^2=5.265$, df=l, P=0.022). These results could imply that apoE ${\varepsilon}3/{\varepsilon}4$ genotype and neurogenic iris constitution are risk factors for hypertension.
