Comparison of hs-CRP and Lipid Values Between Hypertensive and Normotensive Groups (고혈압군과 정상혈압군 환자에서 hs-CRP와 지질 측정값의 비교)

  • Lipids such as total cholesterol (TC), high density lipoprotein-cholesterol (HDL-C) and high sensitivity C-reactive protein (hs-CRP) are known as good risk factors for coronary artery diseases caused by arthrosclerosis. We measured the serum concentrations of hs-CRP, TC, and HDL-C of 6,128 persons who underwent medical examinations in the Health Screening Center at Kangbuk Samsung Hospital. We classified the subjects into normotensive and hypertensive groups according to the blood pressure measured. We investigated whether there was a difference of hs-CRP and lipids between the normotensive and hypertensive groups. There was significant difference of TC, TC/HDL-C ratio, and hs-CRP concentration between the normotensive and hypertensive groups in both men and women, but there was no significant difference of HDL-C concentration between two groups. Our results indicated that the concentrations of hs-CRP and lipids were higher in patients with hypertension and confirmed the usefulness of lipids and hs-CRP in predicting the risk for coronary artery diseases.

Health Approaches for Weight Perception and Weight Loss Efforts in Hypertensive Patients with Obesity: The 2016-2019 Korea National Health and Nutrition Examination Survey

  • This study was conducted to identify factors influencing weight loss efforts in hypertensive patients with obesity in Korea using data from the 2016-2019 National Health and Nutrition Examination Survey. Weight perception, weight loss efforts, and weight control methods were investigated for 1,910 subjects. Data were analyzed using descriptive analysis, cross-tabulation analysis and logistic regression. Among obese hypertensive patients, 12.6% perceived their weight as normal. Weight loss efforts were 2.03 times (95% CI: 1.48 to 2.78) higher in people with overweight perception than those with normal weight perception, and 1.74 times (95% CI: 1.33 to 2.26) higher in women than in men. In addition, those with class 1 obesity were 1.50 times (95% CI: 0.85 to 2.65) higher than those with class 3 obesity, and those with class 2 obesity were 2.16 times (95% CI: 1.16 to 4.00) higher than those with class 3 obesity. These results suggest that weight management approaches for hypertensive patients with obesity should be individually designed according to weight perception, gender, and obesity class.

Genome-Wide Association Analyses on Blood Pressure Using Three Different Phenotype Definitions

  • Hypertension is the most prevalent disease worldwide and is itself a risk factor for cerebral, cardiac, and renal diseases. The inconsistency of candidate genes suggested by previous genomewide association studies (GWASs) may be due to not only differences in study design and genetic or environmental background but also the difference in the power of analysis between continuous traits and discrete traits. We analyzed 352,228 single nucleotide polymorphisms (SNPs) in 8842 unrelated Koreans obtained from Ansan and Ansung cohorts. We performed a series of GWA analyses using three different phenotype models; young hypertensive cases (278 subjects) versus elderly normotensive controls (680 subjects); the upper 25% (2211 hypertensive cases) versus the lower 25% of the SBP distribution (2211 hypotensive controls); and finally SBP and DBP as continuous traits (8842 subjects). The numbers of young hypertensive cases and elderly normotensive controls were not large enough to achieve genomewide significance. The model comparing the upper 25% subjects to the lower 25% of subjects showed a power that was approximate to that of QTL analysis. Two neighboring SNPs of the ATP2B1 gene, rs17249754 (SBP, p=$2.53^{-10}$; DBP, p=$1.28{\times}10^{-8}$) and rs7136259 (SBP, p=$1.30{\times}10^{-9}$; DBP, p=$6.41{\times}10^{-8}$), were associated with both SBP and DBP. Interestingly, a SNP of the RPL6 gene, rs11066280, revealed a significant genomewide association with SBP in men only (p=$3.85{\times}10^{-8}$), and four SNPs located near the MAN2A1 gene showed a strong association with DBP only in elderly men aged 60-70 years (e.g., rs6421827, p=$4.86{\times}10^{-8}$). However, we did not observe any gene variant attaining genomewide significance consistently in the three phenotype models except for the ATP2B1 gene variants. In general, the association signal with blood pressure was stronger in women than in men. Genes identified in GWASs are expected to open the way for prevention, early diagnosis, and personalized treatment of hypertension.

Estimating the Burden of Diseases due to High Alcohol Consumption in Korea: (고도음주로 인한 우리나라 국민의 질병부담 측정)

  • Objectives: This study estimated the burden of disease due to high alcohol consumption using DALY, a composite indicator recently developed by the Global Burden of Disease study group. The results were analyzed by age and sex. Methods: Firstly, high alcohol consumption-related diseases, and their relative risk (RR), were selected. Secondly, population attributable fractions (PAFs) were computed using formulae, including the relative risk (RR) and prevalence of exposure (Pe). Thirdly, the DALYs of high alcohol consumption-related diseases were estimated. Lastly, the attributable burdens of diseases due to high alcohol consumption wereconcluded as being the sum of the products that multiplied the DALYs of high alcohol consumption-related diseases by their population attributable fraction (PAF). Results : The burden of high alcohol consumption in Korea was 2992.3 person years (PYs) per 100,000 persons in men, and 1426.6 in women. For men, the high alcohol consumption-induced diseases with the five biggest burdens were liver cirrhosis, hypertensive disease, liver cancer, cerebral infarction and intracerebral hemorrhage. For women, these were cerebral infarction, intracerebral hemorrhage, hypertensive disease, liver cirrhosis and liver cancer. Conclusion: This study highlighted the attributable fraction of diseases due to exposure to high alcohol consumption, by quantifying the results of exposure to risk factors. Therefore, it is now possible to assess interventions for risk factors in quantifiable terms in each population. Finally, measuring the risk factor burdens was expected to contribute to priority setting and effective resource allocation in public health policy.

Effect of Uric Acid on the Development of Chronic Kidney Disease: The Korean Multi-Rural Communities Cohort Study

  • Objectives: Several studies have investigated the effects of serum uric acid (SUA) levels on chronic kidney disease (CKD), with discrepant results. The effect of SUA levels on CKD development was studied in the Korean rural population. Methods: A total of 9695 participants aged ${\geq}40years$ were recruited from 3 rural communities in Korea between 2005 and 2009. Of those participants, 5577 who participated in the follow-up and did not have cerebrovascular disease, myocardial infarction, cancer, or CKD at baseline were studied. The participants, of whom 2133 were men and 3444 were women, were grouped into 5 categories according to their quintile of SUA levels. An estimated glomerular filtration rate of < $60mL/min/1.73m^2$ at the time of follow-up was considered to indicate newly developed CKD. The effects of SUA levels on CKD development after adjusting for potential confounders were assessed using Cox proportional hazard models. Results: Among the 5577 participants, 9.4 and 11.0% of men and women developed CKD. The hazard ratio (HR) of CKD was higher in the highest quintile of SUA levels than in the third quintile in men (adjusted HR, 1.60; 95% confidence interval [CI], 1.02 to 2.51) and women (adjusted HR, 1.56; 95% CI, 1.14 to 2.15). Furthermore, CKD development was also more common in the lowest quintile of SUA levels than in the third quintile in men (adjusted HR, 1.83; 95% CI, 1.15 to 2.90). The effect of SUA was consistent in younger, obese, and hypertensive men. Conclusions: Both high and low SUA levels were risk factors for CKD development in rural Korean men, while only high levels were a risk factor in their women counterparts.

A Study on the Nutrients Intake and Blood Pressure of Korean Farmers in Summer (농업인의 여름철 영양소 섭취 및 혈압상태에 관한 연구)

  • This study was carried out to investigate the nutrients intakes, blood pressure, and correlations between these two factors in Korean rural adults(60 men, 60 women). Nutrients intakes were measured by analyzing with CAN Pro after weighing the each diets of the subjects had for 24 hours. We measured body mass index(BMI), waist to hip ratio(WHR), and fat content in the body besides blood pressure, and considered their relationship with blood pressure. This examination includes collections and analysis of serum and urine, and considers the correlations between blood pressure and serum or urinary factors. These results were as follows ; 1) The mean ages of men and women were 50.$\pm$12.6 and 46.0$\pm$12.5, respectively. The percentage of hypertensive was higher in men(43%) than in women(37%). 2) The values of WHR, body water and lean body mass were higher in men and the value of body fat was higher in women. Weight, BMI, waist or hip circumferences, and WHR were correlated with blood pressure in these subjects. 3) Though the levels of serum triglyceride, sodium, potassium and iron levels were higher in men and urinary Na was higher in women, these factors didn't showed any significant correlations with blood pressure in both sexes. 4) The values of calcium and vitamin B$_2$ intake in men and women, and of iron intake in women didn't meet the Korean recommended allowances(RDA). Animal protein and Na intakes were negatively correlated with blood pressure. These results showed that the levels of calcium and vitamin B$_2$ intake in both sexes and iron intake in women were lower than those of RDA, and weight, BMI, waist or hip circumferences and WHR were significantly correlated with blood pressure. Therefore we suggest that the farmers of this research need to increase intake of nutrients like calcium and vitamin B$_2$ and control the weight.

The Prevalence, Awareness, Treatment, and Control of Hypertension, and Related Factors in Rural Korea (농촌 지역의 고혈압 유병률, 관리 양상 및 그 관련요인)

  • Objectives : To estimate the prevalence of hypertension, rates of awareness, treatment modalities, and control of hypertension, and also to identify factors in rural Korea that are related to these aspects of hypertension. Methods : A cross-sectional survey, including blood pressure(BP) measurements and interviews, investigating the variables in the health belief model was peformed from August to November, 1999 on 1,426 (79.4%) study subjects out of 1,797 registered residents over 30 years old in Ibansong-Myun, Chinju City. A second survey was peformed from January to March, 2000 on 376 (80.0%) subjects out of 470 hypertensive (or suspected hypertensive) subjects found in the first survey. Two BP readings were taken in each survey using a standard mercury sphygmomanometer. Hypertension was defined as either a BP reading $\geq$ 140/90 mmHg in both surveys, or as subjects on oral hypertensive medication. Results : Estimated hypertension prevalence was 24.9% for men, and 30.4% for women. Rates of hypertension awareness, treatment (the regular use of oral hypertensives), and control (reduction of BP to <140/90 mmHg) were 52.5%, 34.4%, and 12.9%, respectively. The factors related to lower hypertension awareness in the logistic regression analysis were male gender, farming occupation, and higher perceived barrier to medical treatment (those for whom visiting health professionals is a burden). Conclusions : To improve the low awareness, treatment, and control of hypertension, a more active and out-reaching hypertension control program, including routine BP measurements for every visitor to primary care facilities, is needed in rural Korea.

Relationship between Social Support and Self-care of Patients with Hypertension (고혈압 환자의 사회적 지지와 자가간호행위와의 관계)

  • Purpose: This descriptive correlational study was done to analyze the relationship between social support and self-care in hypertensive patients aged 30 years or older. Methods: Data were collected from April 09 to April 16, 2020 through an online survey (Naver Form) at Internet cafes and SNS Eighty 80 adults aged 30 or older who had been diagnosed with high blood pressure at a medical institution participated in the study. Data were analyzed using t-test, one-way ANOVA, Scheffé's test and Pearson correlation coefficients with SPSS statistics 26.0. Results: The study results showed that social support for hypertensive patients was significantly higher for men (t=-2.17, p=.033), according to religious status (t=-2.33, p=.023), and the number of people in the household (F=6.05, p=.001). In addition, there was a significant positive correlation between social support and hypertensive self-care (r=.24, p=.036). Conclusion: The results confirm that the social support of patients with hypertension is related to self care for hypertension management. As the number of elders and single-person households who cannot manage their health well increases, it is necessary to establish a long-term and continuous social support system for these clients.

A SMR study of Korean public servants (우리 나라 공무원의 표준화 사망비에 대한 연구)

  • In Korea, sudden deaths of middle-aged and older male workers who are the responsible persons at home as well as workplace, have aroused social concern. Besides, U. N. has reported recently that mortality of Korea male in 40-50's was one of the highest among newly developed countries in 1992. Not much is, however, known about the nature of the work contributing to the mortality of different groups of workers. Therefore, this study was done to examine mortality of public servants, comprising about 5% of all the employed in Korea, according to their job titles and grades. The datas of 1753 official deaths, comprised 323 (only disease-oriented deaths) applicants of survivors' compensation, were used to examine age-adjusted Standardized Mortality Ratios(SMRs) according to their job titles, grades, tenures and cause of deaths. Controlling age and sex difference was conducted using 26,950,481 general population, 95,340 general deaths and 864,560 working public servants. All the groups were aged 20-64, who were being observed January - December in 1993, at the same time. Results and discussions are as follows. 1. SMRs standardized by general population was significantly low (SMR 44.9 CI 42.8-52.7) for all job titlses. Of public servants, 90.0% was graduated from high school, although 17.5% in general population. The distinction of social status such a education may produce a strong healthy worker effect. Besides, SMRs for different tenure groups showed a steady increase as tenure increases. This suggests that the magnitude of healthy-worker effect may be greater with increasing tenures. 2. SMRs standardized by own public servants was significantly elevated for work-men(SMR 121.0, CI 110.2-132.6) in solitue. When SMRs for different grade, of work-men was examined, 9th(SMR 124.2, CI 104.4-146.7) and 10th(SMR 137.9 CI 120.8-156.8)grade, lower grade in workmen, showed significantly elevated SMRs. Of workmen, 57.0% were graduated from high school and 50.1% in 9th grade, as well. These mean that low economic states made up social class, education may increase mortality rate. 3. Of SMRs according to all causes of death, only policemen on 'cause of death related hypertensive disease'(SMR 282.5, CI 121.6-556.7) was significantly high except for' cause of death related other signs, symptoms and ill-defined conditions'. 4. When SMRs on cause of death related hypertensive disease for different grade of policemen was examined, senior policemen(SMR 241.9), in charge of the front service, showed elevated SMR, in spite of statistical no significance. Especially, the working hour of senior policemen is quite long and also the work schedule is even more irregular for policemen. The results of this study showed that mortality for different jobs differed, and it differed also for different grades in the same job. This difference in mortality may reflect the difference in the nature of job contents, and further studies are warranted to elucidate which job characteristics are responsible.

Association between Hypertriglyceridemic-Waist Phenotype and Metabolic Abnormalities in Hypertensive Adults (고혈압자의 고중성지방혈증-허리 표현형과 대사이상 사이의 연관성)

  • The hypertriglyceridemic-waist (HTGW) phenotype can predict the risk of coronary artery disease. This study evaluated the relationship between the HTGW phenotype and metabolic abnormalities in hypertensive adults. The test was conducted on adult hypertension patients aged 20 or older who underwent health checkups from January 2018 to December 2021 at general hospitals in Gyeonggi Province. The HTGW phenotype was triglyceride ≥150 mg/dL and waist circumference ≥90 cm for men and ≥85 cm for women. The prevalence of the HTGW phenotype in the study subjects was 17.9%. After adjusting for age, gender, and BMI, the odds ratio of the HTGW group was 5.09 (95% confidence interval [CI]: 3.545~7.309) for low HDL-cholesterol, 1.68 (95% CI: 1.176~2.411) for high LDL-cholesterol, was 2.92 (95% CI: 2.009~4.235) for high total cholesterol, 3.39 (95% CI: 2.124~5.412) for diabetes and 1.85 (95% CI: 1.286~2.674) for hyperuricemia compared to the normal triglyceride levels with the normal waist (NTNW) group. The area under curve values of the HTGW phenotype for diagnosing metabolic syndrome were 0.849 for all subjects, 0.858 for men, and 0.890 for women. In conclusion, the HTGW phenotype is closely related to metabolic abnormalities and is a useful indicator for monitoring adult hypertensive patients with metabolic syndrome.
