농촌지역 주민들의 구취실태와 유발요인 (Halitosis and Related Factors among Rural Residents)

  • 본 연구는 농촌지역 주민들의 구취실태를 파악하고 구취에 관련된 요인을 알아봄으로써 구취예방 및 효율적인 구취제거 방안을 마련하는데 기초 자료를 제공하고자 일부 농촌지역의 주민 293명을 대상으로 2006년 1월 4일부터 1월 21일까지 면접설문조사(구강위생관리 행태, 구취관련 질병력, 구취실태), 구취측정, 구강검사, 치아우식활성검사(스나이더검사, 타액분비율검사, 타액완충능검사)를 실시한 결과 다음과 같은 결론을 얻었다. 1. 잇솔질 횟수는 1일 2회가 46.1 %로 가장 많았고, 여자가 남자보다 잇솔질 횟수가 많았다. 매일 혀솔질을 하는 군은 25.6%이었고, 보조 구강위생용품을 사용하는 군은 9.2 %이었다. 2. 평상시 구취를 자각하고 있는 사람은 62.5 %이었고, 구취를 가장 심하게 자각하는 시기는 기상 후가 72.7 %, 구취를 자각하는 부위는 잇몸에서 23.0 %, 구취의 유형으로는 구린 냄새가 37.2 %로 높게 나타났다. 3. 구취측정 결과 OG는 50 ppm미만이 54.3 %, $50{\sim}100ppm$ 범위에 41.6 %로 나타났고, $NH_3$$20{\sim}60ppm$ 범위에 52.6 %로 가장 높았다. 4. 구취관련 질병력별 OG는 치아우식증으로 인한 식편압입, 당뇨병과 구취에 대한 가족력군에서 $50{\sim}100ppm$ 범위에 유의하게 높았으며, $NH_3$는 호흡기계 질환군에서 유의한 차이가 있었다. 5 평상시 구취 자각정도별 OG는 '냄새가 나지 않는다'는 군과 '가끔 냄새가 난다'는 군에서 50 ppm 미만에 각각 55.9 %, 57.5 %로 나타났고, '본인이 느낄 정도로 항상 냄새가 난다'는 군과 '항상 심하게 냄새가 난다'는 군에서 $50{\sim}100ppm$ 범위에 각각 52.0 %, 63.6 %로 높게 나타났으며, $NH_3$는 모두 $20{\sim}60ppm$ 범위에 높게 나타났다. 6. 구강검사별 OG는 치수노출치와 식편압입이 많을수록, 설태지수가 높아질수록 $50{\sim}100ppm$ 범위에 OG값이 증가되었고, $NH_3$는 보철치가 많을수록, 설태지수가 높아질수록 유의하게 증가되었으며, 하악 국소의치군에서 60 ppm 이상으로 유의하게 증가되었다. 7. 스나이더검사는 고도활성이 43.0 %로 가장 높았고, 산 생성균의 활성이 높을수록 OG값이 증가되었다. 자극성 타액분비율 검사는 8.0 ml 이하에서 62.5 %로 가장 높았고, 타액분비율이 많을수록 OG값이 감소된 분포를 보였으며, 타액완충능검사는 0.1N 유산용액의 방울 수가 $6{\sim}10$ 방울에서 58.7 %로 가장 높았고, 타액완충능이 증가될수록 OG값이 냄새를 느끼지 못하는 50 ppm 미만에서 증가되었다. 8. 구강환경과 구취와의 상관관계에서 OG는 타액분비율, 보철치와 음의 상관관계를, 치수노출치, 충전치, 현존치, 설태량, 식편압입과 양의 상관관계를 보였으며, $NH_3$는 우식치와 음의 상관관계를, 보철치, 잇솔질 횟수와 양의 상관관계를 보였다. 9. 다중회귀분석 결과에서 OG에 영향을 주는 요인으로는 여자, 치수노출치, 보철치, 식편압입, 타액분비율, 설태지수, 스나이 더검사의 고도활성이 선정되었고 이들의 설명력은 45.1 %이었으며, NH3에 영향을 주는 요인으로는 여자, 치수노출치, 설태지수, 보철치가 선정되었으며 이들의 설명력은 6.6 %이었다. 이상의 결과를 볼 때, 조사대상 농촌지역 주민들의 구취실태는 구강환경 및 구취관련 요인, 치아우식활성검사의 스나이더 검사, 타액분비율검사와 밀접한 관련이 있음을 시사한다. 따라서 이들 주민들의 구취예방을 위해서는 식후에 올바른 잇솔질 방법 및 혀솔질과 더불어 보조 구강위생용품을 사용하여 식편압입과 설태제거를 해야 할 필요성이 강조된다. 구취의 원인과 그 성분은 매우 복잡하고 다양하므로 개인별 구취발생 요인을 정확하게 분석하기 위해서는 추후 계속적이고, 체계적인 연구가 필요하며, 보건(지)소의 치과위생사를 활용하여 지역사회 주민들에게 계속적인 구강보건교육 프로그램이 제공되어야 한다고 생각된다.

주요일간지(主要日刊紙)의 보건의료관계사설(保健醫療關係社說) 내용분석(內容分析) (An Analysis on Contents of Health-Concerned Editorials in Korean Neswspapers)

  • The editorials of the leading newspapers may reflect as well as help formulate the public opinions to a significant degree. Bearing this in mind, this study was carried out to provide information useful in formulating such public health policies that could practically meet the social interests and demands in health appearing in the editorials of the newspapers. This analysis covered the editorials of 5 leading newspapers such as Donga Ilbo, Chungang Ilbo, Hankook Ilto, Chosun Ilbo and Seoul Shinmoon for about 10 years from Jan. 1st, 1970 through Oct. 31th, 1979. The major findings are as follows: 1. The total number of health-concerned editorials in the five daily papers for the period was 1,768 or occupied 6.4% of the total editorials of the same sources. The increasing trend of frequnecy of the health-concerned editorials since 1976 indicates the increment of social interests and demands in health. Analysing the contents, environmental pollution received the greatest attention in those editorials, which was followed by medical affairs, environmental sanitation, and disease control in order in terms of frequency of appearance. However, there was a tendency that the interests in the environmental pollution, medical affairs and social welfare tended to increase year by year, whereas those in environmental hygiene and disease control comparatively decreased. 2. Motives of dealing with the health-concerned editorials were provided by announcement of the governmental policies and implementation for 25.6% and by out-breaks of the relevant events for 23.9%. This tendency coincides with the general characteristics of the editorials that reflect the timely issues. Closely analysing, however, the fact that the motives engendered by the out-breaks of the relevant events or by the season concerned comparatively tended to decrease as years pass by, indicates that the editorials tend more to seek the future-oriented demands in health rather than the current issue-oriented. 3. The editorials appeared to be more concerned with the governmental policies. 95.7% of all the editorials analysed were addressed to the government. This signifies the role of government in the field of public health and medical affairs. Their attitudes toward health-related policies of the government were much more negative than the other editorials that were addressed to the government in other fields. This suggests that the governmental interests in health were neither sufficient nor fair. 4. What the editorials most stressed were; (1) increment of governmental interests in health, (2) improvement of the governmental health administration, (3) enriching the basic statistics, and (4) development of various technologies pertinent to health affairs and disease control, and so forth. However, must of their suggestions were not concrete but rather abstract and conceptual. 5. The editorials also expressed strong interests in research area. The areas of the needed research most suggested by items are; (1) development of effective health care delivery system, (2) establishment of more practical system of calculating medical cost, (3) implementation of effective policies to control degenerative diseases, (4) division of medical care services and pharmaceutical services systems, (5) effective ways to prevent Co poisoning accidents, (6) changing status of environmental pollution and its effects upon health, and (7) status of occupational diseases, and so forth. 6. There were some editorials -not small in quantity -that have risk to mislead the public opinions as well as the health policies due to lack of professional knowledge of the writers. It is desirable to establish some kind of mechanism that screens the erroneous contents of the editorials to help prevent misleading opinions.

뇌졸중 환자의 질병경험에 관한 연구 (A Study of CVA patients에 Experience of the Illness)

  • This work was done for 9 patients having experience of a herb medical treatment after being diagnosed as CVA during a year from January, 1996 to December, 1996 by using an ethnographic research method. The summarized results of this research are following. Ⅰ. THE EXPERIENCE OF THE ILLNESS First, the falling-ill phase is the time that they have the first stroke of paralysis and the decision pattern of medical institution' comes out. The emotional experience in the period is something like 'flustration', 'anxiety', 'despair', and 'expectation'. Second, the active-treatment phase is the time that the patients as well as their family or care giver not only show the positive attitude and actively participate in the illness treatment but also show a lot of interest in medical institutions and activities of health recovery. There is a primary factor of the continuation of treatment as an experience of treatment and being crushed and sensitivity as an experience of the illness. Third, the rehabilitation phase is the time that the patients or their family become tired and insensitive to the treatment and recuperation, and then reduce the treatment activity. There is a primary influence factor of the discontinuance of treatment as an experience of treatment and physical experience and emotional experience as an experience of the illness. The physical experience is divided into 'personal-hygiene care', and 'the sphere of activity' The emotional experiences are 'blaming someone', 'contempt' and 'despair' as a negative experience and 'hope' as a positive experience. Ⅱ. COPING STRATEGY There are a physical coping, an emotional and mental coping, a social coping, and a spiritual coping as a coping strategy used for the patients to overcome their illness and adjust themselves to their altered life. First, the physical coping comes out as 8 categories, 'using an auxiliary tool', 'doing exercise', 'protecting', 'improving their diet', 'taking care of something', 'using subsidiary medicines', 'trying a folk remedy', and 'having interest in their health'. Second, for the emotional and mental coping, there are 'accepting' and 'trying' as a positive coping and a failure of control as a negative coping. Third, the social coping is appeared as 'being supported'. Fourth, the spiritual coping is recognized as' recourse to God' and 'preparation of death'. After all, the elderly CVA patients in an agricultural area choose the act of treatment based on the traditional belief and the relationship with a caretaker. A personal health can be maintained by taking care of themselves and controling their mind, and the overcome of the illness is decided on the basis of traditional concepts and cultural principles in which the patients as well as the family, neigbors and take carers should work out together and cooperate with each other in order to achieve that.

VMD에 대한 소비자태도가 브랜드자산과 브랜드태도에 미치는 영향:베이커리전문점을 중심으로 (A Study on the Effects of the Consumer Attitude toward Visual Merchandising on Brand Equity and Brand Attitude: Focused on Bakery Shops)

  • Purpose - This study aims to establish the relationships among brand equity, brand attitudes, and Visual Merchandising (VMD) in bakery stores. The results will help bakery franchisees and franchisers in devising appropriate strategies for successfully managing their bakery shops. Research design, data, and methodology - The VMD model for bakery shops comprises harmony, attractiveness, suitability, pandemic, functionality, and reliability. Further, brand equity encompasses brand awareness, perceived quality, and brand image. In this study, the VMD factors of bakery shops are used as independent variables, while brand equity and brand attitude are used as dependent variables. This study aims to ascertain the extent of the influence VMD components of bakery shops have on brand equity and brand attitude. Regression analysis was used to verify those effects. The measurement items, deemed reliable and valid in a previous study, were modified for this study. Questionnaires were distributed to 500 consumers nationwide, of whom 340 were used for the sample. The SPSS 19.0 statistical program was used for the analysis. Results - First, among the VMD bakery shop components, harmony, attractiveness, and pandemic have a positive impact on brand equity (brand awareness and brand image). Second, amongst the VMD components of bakery shops, attractiveness, pandemic, and reliability have a positive impact on brand equity (perceived quality). Third, amongst the VMD components of bakery shops, harmony, attractiveness, and reliability have a positive impact on brand attitude. Fourth, brand equity (brand awareness, brand image, and perceived quality) have a positive effect on brand attitude. Conclusions - This study identifies the relationships between the VMD components of a bakery shop and brand equity and attitude in order to propose a new model. The study has several practical implications. First, the development of brand equity and management can be important components in bakery shops' determination of whether their VMD components have an impact on brand equity. Second, among VMD components, harmony and attractiveness have a positive influence on the choice of bakery shop, while reliability and pandemic have a partly positive influence on the choice of bakery shop. Therefore, store atmospheres should be attractively designed, and store menus and interiors should be reviewed periodically to conform to the latest trends. This study suggests marketing strategies for brand equity formation. First, providing collateral product quality, hygiene, and safety benefits as well as product offerings to complement the season and employees' services should be considered. Second, display methods, illumination designs, and new product ideas should be periodically reviewed. Third, these should complement the overall harmony of the interior and exterior and of the store atmosphere in order to be attractive. Funds should thus be dedicated specifically for the facilities and their interiors. Fourth, it is necessary to build a friendly and relaxed image that can be easily called to mind by the customers.

식민지 '미디어 효과론'의 구성 대중 통제 기술로서 미디어 '영향 담론' ('Media Influence' Discourses Articulated for Crowd Control in Colonial Korea)

  • 사진, 환등, 영화는 20세기 전반기의 경이롭고 마술적인 시각 기술, 1910년대 중반 상업적 가능성을 확인시킨 시기를 거쳐, 1920~1930년대 내내 문화적 패권을 향유한 유일한 미디어였다. 처음부터 근대적 오락으로 소비된 시각 미디어들에 대한 담론은 1920년대 초반까지 영화보다는 관객이 집합하는 공간인 극장 통제, 즉 군중 통제에 역점을 두고 구성되었다. 그러다 영화 자체가 현대적 문화와 미학의 표준이 된 1920년대 중반 이후 무성영화시대에는 당대의 지배적 학문 분야들인 심리학, 사회학, 대중 심리학의 개념과 이론을 적용하여 영화가 대중에게 미치는 강력한 도덕, 심리, 정신면의 부정적 영향을 강조하게 된다. 이데올로기 도구이자 동시에 근대 리얼리즘의 재현 매체로서 가능성을 지닌 시각 기술과 그것의 정점에 있는 영화의 영향을 부정적으로 규정해 간 것이다. 이 연구는 이러한 '식민적 미디어 영향 담론'이 처음부터 통치의 문제였으며 상업적 소비대중과 저항적 군중 통제를 위한 담론이었음을 주장하였다. 주목해야 할 사실은 이를 통해 일제의 식민지 통치성이 부단히 세계적, 보편적 사유와 지적흐름을 수용하면서 구성되어 갔지만 식민지 대중의 동의를 얻는 데는 실패한 담론이었다는 점이다.

사전연명의료의향서 작성 동기에 관한 연구 (A Study on the Motivation to Write Signing Advance Medical Directives)

  • 본 연구의 목적은 호스피스 완화의료와 임종과정에 있는 환자에게 최선의 이익을 보장하고 자기결정을 존중하여 인간으로서의 존엄과 가치를 보호하기 위하여 S시 소재의 C 기독교 종교시설을 방문하여 사전연명의료의향서를 작성한 노인을 대상으로 사전연명의료의향서 작성의 동기를 알아보기 위해 시행되었다. 조사 기간은 2019년 7월 1일부터 10일간 진행하였다. 연구 결과 사전 연명의료 의향서 작성 동기에 관해 4개 주제와 8개의 하위주제가 도출되었다. 주제는 '자식들을 위해서', '고통스러울 것 같아서', '내 삶을 잘 정리하고 싶어서', '필요성을 느껴서' 등의 4가지 주제가 도출되었으며, 하부주제로 '자식들 고생시키고 싶지 않아서', '자식들에게 의료비 부담주고 싶지 않아서', '고통에 대한 두려움이 있어서', '고통스러워하는 가족의 임종을 돌본 경험이 있어서', '편안한 마음을 위해서', '자기결정권의 중요성을 느껴서', '그전부터 알고는 있었지만 이번에 결심하게 되어서', '정보를 처음 알게 되었는데 결심하게 되어서'였다. 본 연구결과는 노인들의 좋은 죽음을 위한 웰다잉 교육 프로그램 개선과 사전연명의료의향서 작성을 위한 효과적 제도를 보완하기 위한 기초자료가 될 수 있다는 점에서 의의가 있다.
