  • Nitrogen(N) concentration of preterm(PT) and term (T) milk in various fractions, such as total, protein, nonprotein, whey protein and casein were determined at 2-5 days, 1, 2, 4 and 6 weeks of postpartum. The purpose of this study was to investigate the nitrogen concentration of human milk from mothers delivering at preterm and term, and the propriety of preterm milk for premature infants. The concentration of total N, nonprotein N, protein N, whey protein N and casein N in preterm milk was decreased significantly with time postpartum. Total N was 374mg/이 at colostrum, 232mg/dl at mature milk. Whey protein N was decreased from 42mg/dl at 2-5 days to 32mg/dl at 4-6 weeks. Protein N was 332mg/dl at colostrum, 202mg/dl at mature milk. The proportion of whey protein N and casein N were 39:61 at colostrum, 28:72 at mature milk. No difference were found between T and PT milk for total nitrogen excepted 2 weeks. In this report we show that nitrogen concentration except casetpt casein N is smaller in milk from mothers giving birth prematurely than in milk from mothers giving birth at term, over the first two weeks of lactationl But protein N was higher in preterm milk than term milk, whey protein nitrogen was lower. By comparing predicted nitrogen intakes to estimated requirements of preterm infants fed 150 to 200ml/kg/day of their own mother's milk, we predict that the quantities of protein provided would be adequate to meet the requirements of the prematured infants during the early weeks of life.

  • Protein concentration in human from 39 well-norished American women and its adequacy for growth of exclusively breastfed infants(BF) and breastfed infants fed supplementary foods(BFS)from 1-6 months postpartum were studied. Mean protein concentration of breast milk measured by Lowry et al., using human serum albumin as a standard, over the first 6 months lactation was 1.31$\pm$0.13g/dl. Concentration of protein was singnificantly higher at the first month of lactation (1.55$\pm$0.23g/dl)(P<0.05) than any other month studied. Mean volume of breast milk ranged from 662-848ml/day in the BE group and from 415-661ml/day in the BFS group during the first 6 months of lactation. Mean protein intake of infants ranged from 1.3-2.2g/kg in the BF group and from 1.4-2.1g/kg in the BFS group. Mean protein intake (g/kg body weight) of both BF and BFS groups was less than Recmmended Dietary Allowance(1989, USA) of 2.2g/kg except at 1 month of age. However, mean growth of the infants was normal according to NCHS reference, suggesting that the RDA for protein was unrealistically high for infants during 2-6 months of age. Protein provided by breast milk alone appeared adequate for normal growth during this time.

  • The purpose of this study was to compare dietary behavior and food intake in relation to physical complaints, such as farmer' syndrome(FS) and gastro-intestinal(GI) problem among Korean farmers. The questionnaire was composed of 24 Cornell Medical Index (CMI), 8 farmers' syndrome, and 5 GI problem questions. Food intake data was gathered by the semi-quantitative food frequency method. The subjects(male 226, female 415) who had FS and Gi problem were 12./8% and 8.3%, respectively. The physical complaints were higher in female and the elderly group. In the FS group, lower activity was seen that in normal groups. The lower health status and fatigues were found in the physical complaint groups. Dietary behavior showed low appetite in the physical complaint group, irregular lunch in FS, and irregular breakfast in GI problem group. Changes in dietary behavior were shown in the aspects of lower amount of intake quantity(40.8%), lower consumption in fat(32.8%) and salty(38.8%) foods, and diverse food items(47.8%). Quantity of food consumed was significantly different withing groups with FS. Kinds of food consumed, intake of protein source foods, milk and calcium and total animal foods were lower in the FS group. But milk and calcium source food and all animal food intakes were higher in the GID problem group. The results suggest that dietary behavior and food intake differ within the group of physical complaints.

  • The lactose, total nitrogen and total lipid contents of human breast milk were determined longitudinally after protein and term deliveries. Milk samples were obtained from day 2 to 5, on weeks 1, 2, 4, and 6 after preform (PM : n = 24) and term (TM : n = 22) deliveries. The lactose contents of PM and TM increased from 5.28 g/dl and 5.93 g/dl at days 2 to 5, to 7.95 g/dl,7.87 g/dl at 6 weeks, respectively. The total nitrogen contents of PM and TM decreased significantly during the first 6 weeks after birth (from 374 to 220 mg/dl versus 382 to 220 mg/dl respectively). However the total lipid contents of the PM and TM increased during the same periods. Significant differences in the total lipid contents between the PM and TM were 2.15 versus 3.27 g/dl at 2 weeks, and 3.26 versus 2.52 g/dl at 6 weeks, respectively. The energy intakes of preform infants was 134.8 ㎉/kg/d during the 6 weeks of lactation, which satisfied with energy requirements (85-130 ㎉/kg/d) of the preform infants. • Lactating women have an increased need of folate in the breastfeeding period and, as a consequence, may be in risk of folate deficiency. Folate content of breast milk, furthermore, is important for infants to support exponential growth. However, little is known about the folate content of breast milk from Korean lactating women and their folate nutritional status. In this study, therefore, we investigated the folate status of Korean lactating women and the folate content of their breast milk during extended lactation. A total of 10 subjects who delivered full-term infants participated this study voluntarily. Dietary folate intakes were measured and blood and breast milk were collected at 1, 2, 3, and 6 months postpartum. The women who did not take folic acid supplements failed to meet the recommended intake(RI) of folate for lactating women during all the study periods but those who did met the RI. The unsupplemented women showed lower plasma folate concentrations compared to the supplemented women and all the women were in suboptimal folate status determined by plasma folate concentration throughout the study periods. But the supplemented women showed lower prevalence of suboptimal folate status only at 3 or 6 months postpartum. Plasma folate concentrations of both groups decreased with the progression of lactation. Erythrocyte folate concentrations were not different between the two groups, however, that of the unsupplemented reduced further as time progressed. Plasma homocysteine levels were not different between the two groups. Concentrations of erythrocyte folate and plasma homocysteine were not changed throughout the study periods. Folate contents of their breast milk through the study periods were not different between the two groups and it decreased as lactation progressed in both groups. The results of this study suggest that the folate nutritional status of Korean lactating women might be deteriorated with the progression of lactation without folic acid supplements.

  • Objectives: This study investigated the improvement of food habits and eating behavior of cancer patients during treatment through the continuous nutrition counseling and monitoring. Methods: Thirty cancer patients during treatment were participated in this study, and the first nutrition counseling and the first and second monitoring were conducted after 2-3 week intervals. Results: As a result of the nutrition counseling and monitoring, all patients improved to a great extent to consume 3 meals a day. The frequency of having breakfast was significantly increased with the nutrition counseling and continuous monitoring as well. The meal fixed quantity was stabilized and the eating speed slowed down as more nutrition counseling were done. The snack intakes of patients did not show any significant difference after the nutrition counseling but showed a slight decline after the monitoring. The frequencies of eating out and a late-night meal significantly decreased after the monitoring. The intakes of fish, meat, vegetables, milk and fruits needed for a well-balanced diet significantly increased as more nutrition counseling were done. The intakes of processed food were significantly decreased after the nutrition counseling and the intakes of fast food were significantly decreased as more nutrition counseling were done. The water intake of patients also significantly increased. In the eating behavior related to health, the frequency of drinking alcohol significantly decreased after nutrition counseling consultation and no patients had dietary supplements after the first monitoring. Conclusions: These results suggested that continuous nutrition counseling is effective in improving eating habits of cancer patients.

  • The purpose of this study was to examine the long term effects of breast feeding on growth, bone development and nutrient intakes in preschool children. Subjects were 62 preschool children. Anthropometric characteristics and bone mineral density of carpus were measured using DEXA. The questionnaire was composed of health status, life style, dietary behaviors, and dietary intakes and was completed by the children's mothers. The average age of the boys was 62.4 months and that of the girls was 62.1 months. Average birth height and weight of the subjects was 50.9 cm and 3.4 kg for boys and 50.3 cm and 3.3 kg for girls, respectively. The average height, weight, $\%$ body fat, and obesity index were 111.7 cm, 19.6 kg, $15.0\%$, $-2.1\%$ in boys and 109.4 em, 18.7 kg, $17.5\%$, $0.2\%$ in girls, respectively. Forty children were fed colostrum, 21 were fed breast milk, 29 were fed formula, and 12 were fed mixed milk. There was no significant difference in growth status between children who were fed colostrum and those who were not. Children who had mixed feedings were significantly taller than those who did not (p <0.001), and children who had colostrum had significantly higher bone mineral density than those who did not ($0.25 \pm 0.04 g/ cm^2$ vs. $0.23 \pm 0.04 g/cm^2$, p < 0.05). However, the different feeding methods did not show any difference in bone mineral density. Except vitamin $B_6$, folic acid and vitamin E, vitamin, mineral and calorie intakes did not meet the Korean RDAs. Calcium intake was especially lower than recommendations by as much as $62\%$ and $70\%$ in boys and girls, respectively. There was no significant difference in nutrient intakes between children who had colostrum and those who did not. However, children who were formula consumed significantly more animal fat than those who did not (p < 0.05). Neonatal feeding and breast or formula feeding seems to associate with height, bone mineral density and animal fat intakes in preschool children, based on the results of this study. A longitudinal study is needed to clarify this relationship.

  • To evaluate the effect of feeding methods on growth and zinc nutritional status of infants early in life, we monitored from birth to 36 months in 51 infants who were exclusively fed human milk (HM, n=20), casein-based formula (CBF, n=12), or soy-based formula (SBF, n=19) during the first five months of life. Zinc status was assessed by analyzing serum zinc concentrations and zinc intakes. Zinc contents in HM and formulas were measured. Zinc intake was estimated by weighing infants before and after feeding in the HM group and by collecting formula-intake records in the CBF and SBF groups. After solid foods were introduced, all foods consumed were also included to estimate zinc intake. The growth of infants in all groups was similar to that established for normal Korean infants. Human milk zinc concentrations declined as lactation progressed. Zinc concentrations in all formulas tested in this study were higher than HM and were also higher than those claimed by the manufacturers. During the first twelve months, mean serum zinc concentrations of infants were similar in all groups, although infants in the HM group consistently had the lowest zinc intake among the groups, and the overall zinc intake in infants fed SBF was highest. This finding could be explained by the difference zinc bioavailability of HM and formulas. In conclusion, infants fed HM, CBF or SBF has normal growth up to three years of age, although HM contained the lowest zinc concentration followed by CBF, then SBF.

  • The purpose of this study was to investigate the effects of a nutrition education and personalized lunch service program in a senior welfare center. A total of 30 elderly (14 males, 16 females) aged 62~89 years participated in this study. Nutrition education lessons (2 hour/lesson/week) were provided for 4 weeks. Also, ten weeks from week 3rd to week 12th, personalized lunch providing 1/3 personal needed energy was served 5 times for a week. After the nutrition intervention program, we compared anthropometric characteristics, blood biochemical characteristics, nutrition knowledge, dietary attitude and dietary intake using 24 hr recall with those before the intervention. The body weight (p < 0.001) and body mass index (BMI) (p < 0.001) were decreased. There were significant increases in score of nutrition knowledge (p < 0.01) and consumption of milk & milk products for snacks. There was a positive effect on fasting blood sugar (FBS) showing significant decreases in portion of impaired fasting glucose and diabetes mellitus (p < 0.05). Also, serum triglyceride (TG) was significantly decreased (p < 0.05). In evaluation of nutrient intake by Dietary Reference Intakes for Koreans (KDRIs), riboflavin (p < 0.01), vitamin C (p < 0.001), calcium (p < 0.05) were positively improved. The index of nutritional quality (INQ) and intakes of vitamin C (p < 0.001), riboflavin (p < 0.05), Ca (p < 0.01) and Fe (p < 0.05) were increased. In conclusion, this nutrition education and lunch service program providing 1/3 personal needed energy can be used to develop and implement a tailored nutritional intervention programs in the setting of a community senior welfare center to improve health and nutritional status of Korean elderly.
