  • Compared with other transport means, safety and timeliness are the merits of railways. Unexpectedly when accident happens, much time and human strength are required to cope with the accident. And for swift recovery, systematic rehabilitation is needed. Recently using MTS(Moving picture Transmission System), we can perform accident rehabilitation and recording work efficiently. MTS is the device that transmits continuous picture information from accident field to control center. We are developing the appropriate system to railway situation to make use of the existing information communication technology, processing technology of video-tex, super high speed transmission technology through fiber-optic, copper cable and network description of information Technology, etc. If these communication-based can technologies are applied to railway system, railway managers can control the accident by inspecting the picture of accident field and can contribute to the safe train operation and the improvement of railway management. In this paper, we investigate the connecting methods when optical fiber is used for moving picture data transmission of train accidents, and its problems. And, we validate MTS's performance through about 28km section of field test.

  • Representative systematic accident analysis methods proposed so far include AcciMap, STAMP, and FRAM. This study used these three techniques to analyze a fire accident case that occurred during routine manufacturing work in a domestic chemical plant and compared the results. The methods used different approaches to identify the cause of the accident, but they all highlighted similar causal factors. In addition to technical issues, the three accident analysis methods identified factors related to safety education, risk assessment, and the operation of the process safety management system, as well as management philosophy and company culture as problems. The AcciMap and STAMP models play complementary roles because they use hierarchical structures, while FRAM is more effective in analyses centered on human and organizational functions than in technical analyses.

  • Bacause of increasing the use in Industrial robots, the accident rate has been increasing now a days. The prediction of accident could be very hard as there are so many factors which occured accident. Removing the accident factors in industrial robots can be diagnosed by the human experts who are very familiar with in those area. The purpose of this study is a development of finger fault diagnosis system for industrial robots. We have many problems such as a long time to get the expert knowledge and the number of expert to be limited. To solve these problems lots of investment and time are required, and then the exepert system to finger fault diagnosis for industrial robots can be applied.

기업의 안전문화 수준변화와 재해율과의 상관관계: P사 사례연구 (Correlation between the Change in the Level of Safety Culture of Enterprises and the Accident Rate: Case Study of Company P)

  • Safety has once again emerged as a social issue due to the enforcement of the Serious Disasters Punishment Act and the occurrence of major disasters recently. As such, the safety culture related to unsafe human behavior is drawing attention as various discussions about the importance of safety and accident prevention are currently being undertaken. This study aims to determine whether changes in the level of safety culture in companies affect the accident rate. To this end, we targeted three workplaces in the same company in the domestic manufacturing industry, and 2,790 people were surveyed using DNV's Safety Will(ISRS-C version 4.0) for safety culture level diagnosis. The change in safety culture level and accident rate were compared and analyzed after carrying out safety culture level improvement activities for 5 years. As a result, the level of safety culture increased by 5.8 points, an overall improvement of 9.5%, while the total number of accidents decreased by 20%, from 5 to 4, indicating a general decreasing trend in accident occurrence. Results confirmed that the change in the level of safety culture affects the accident rate, and the improvement in the level of safety culture affects a reduction in accidents. Therefore, this study suggests that efforts to improve the level of safety culture within a company can be associated with accident reduction and accident prevention outcomes.

해외 주요국에서의 최근 가스배관 사고의 원인과 빈도의 비교 분석 (Comparative Analysis on the Causes and Frequency of Recent Gas Pipelines Accidents in Major Overseas Countries)

  • 천연가스는 폭발성 유체로 고압의 매설 배관 파손시 심각한 인적/물적 손해를 야기할 수 있으며, 최근에도 실제 매설 배관의 파손으로 상당한 인명피해가 발생한 사례가 보고되고 있다. 국내의 경우, 급속한 성장 및 보급에도 불구하고 천연가스 배관의 운영 길이와 운영기간이 짧아 가스배관의 사고원인 등에 대한 유효한 사고데이터의 확립이 부족한 실정이다. 국내에서 사고 사례에 대한 데이터베이스를 체계적으로 구축하기 위하여 국내보다 천연가스 배관의 운용 역사가 길고, 긴 배관망을 갖고 있는 해외 주요국에서 발생한 최근의 천연가스 배관관련 사고의 원인과 빈도를 분석하는 것이 필요하다. 따라서 본 연구에서는 해외 주요국에서 가스배관의 사고원인별 발생 빈도의 추이를 파악한 후 이를 비교 분석하여 국내 고압가스 운송 배관망의 안정성 확보의 토대를 구축하고자 하였다.

  • 사고대비물질 취급 사업장에서 사고가 발생하면 일반 화학물질사고보다 더 많은 인명, 재산피해를 야기한다. 사고를 예방하기 위해 여러 시스템들과 제도들을 개발하고 있지만, 사고발생의 주원인인 작업자의 오류를 제거 또는 감소시키기 위한 기술들은 미흡한 실정이다. 본 논문에서는 2012년 9월 구미 불산 누출사고 case study를 통해 국내기술지침들을 기반으로 위험작업 수치화 및 잠재위험확인, 작업평가를 수행하였으며 기존 시스템들을 보완하기 위한 새로운 risk mapping 방법론 개발에 관한 연구를 진행하였다.

학교와 인접한 회전교차로 보행자 사고심각도 영향요인 분석 (Analysis on the Accident Factors of Pedestrian Accident Severity in Roundabout Near School)

  • The purpose of this study is to analyze the factors affecting the roundabout accidents near schools. This study gives particular attentions discussing characteristics by pedestrian accident severity using the ordered logit models. In pursuing the above, 63 roundabouts installed before 2014 are surveyed for modeling. the traffic accident data from 2014 to 2016 are collected from TAAS data set of Road Traffic Authority. Such 35variables explaining the accidents as environment, human, geometries, school and roundabout factor are selected from literature reviews. The main results are as follows. First, the ordered logit models (${\rho}^2$ of 0.272, $x^2$ of 24.723) which is statistically significant have been developed. Second, environment factor variable is analyzed to be day or night ($X_1$ ), human factor variables are evaluated to be driver gender($X_4$), older driver($X_5$), pedestrian gender($X_7$) and children pedestrian($X_8$ ). Third, geometries factor variable are analyzed to be speed limit sign($X_{16}$) and median barrier($X_{21}$), school factor variables are evaluated to be hump-type crosswalk($X_{25}$), CCTV($X_{26}$) and school zone sign($X_{27}$), roundabout factor are analyzed to be roundabout sign($X_{30}$) and number of circulatory roadway lane($X_{32}$). Finally, this study could give some implications to decreasing the accidents severity at roundabout near schools.

KTX 정차역 통과사고 원인분석 및 예방대책 (A Study on the Technique for Preventing Passing-by of High-speed Train)

  • It is commonly recognized that railway is one of the representative transportation and it offers public service based on strategies for being rapid, automation, safety. Since the opening of high speed railway, 3-hundred-million people have used it and acknowledged its efficiency. However, derailed accident at Kwangmyeong station in February, 2011, frequent malfunction of KTX-Sancheon, and accidents by engineer's careless fault damaged on credibility of safety, Especially, spreaded accidents through social networking service by cell phones amplified anxiety of public, being criticized by the press. This study analyzed statistics of past accident and cases of passing-by accident, and surveyed 152 KTX captain engineers about their recognition of the accident by careless fault and experiences of possibility of occurrence for preventing engineer's careless fault and restoring trust According to the analysis, engineers worry about responsibility and disadvantages related to the accidents for the most, and they are nervous about malfunction for the second most. This study presents prevention methods regarding the result. First, it is required to improve mental stability and concentration on their work, secondly, advanced ability to cope with malfunction or error through repetitive education and training are required to increase confidence, and for the last, improvement of operational supporting system such as ATP, GPS to prevent errors by human factors. Improvement of the system is expected to lead engineers to prevent careless fault and regain the reputation of railway.

Exploring the Contributory Factors of Confined Space Accidents Using Accident Investigation Reports and Semistructured Interviews

  • Background: The oil and gas industry is one of the riskiest industries for confined space injuries. This study aimed to understand an overall picture of the causal factors of confined space accidents through analyzing accident reports and the use of a qualitative approach. Methods: Twenty-one fatal occupational accidents were analyzed according to the Human Factors Analysis and Classification System approach. Furthermore, thirty-three semistructured interviews were conducted with employees in different roles to capture their experiences regarding the contributory factors. The content analyses of the interview transcripts were conducted using MAXQDA software. Results: Based on accident reports, the largest proportions of causal factors (77%) were attributed to the organizational and supervisory levels, with the predominant influence of the organizational process. We identified 25 contributory factors in confined space accidents that were causal factors outside of the original Human Factors Analysis and Classification System framework. Therefore, modifications were made to deal with factors outside the organization and newly explored causal factors at the organizational level. External Influences as the fifth level considered contributory factors beyond the organization including Laws, Regulations and Standards, Government Policies, Political Influences, and Economic Status categories. Moreover, Contracting/Contract Management and Emergency Management were two extra categories identified at the organizational level. Conclusions: Preventing confined space accidents requires addressing issues from the organizational to operator level and external influences beyond the organization. The recommended modifications provide a basis for accident investigation and risk analysis, which may be applicable across a broad range of industries and accident types.

재난대응 효율화를 위한 사업장 대규모 인적사고 위기관리 표준매뉴얼 개정방안 연구 (A Study on the Crisis Management Standard Manual for Large-scale Human Accident at Workplace for Efficiency of Disaster Response)

  • 연구목적: 고용노동부에서는 사업장 내 화재·붕괴 등의 대형재난이 발생하면 사업장 대규모 인적사고 위기관리 표준매뉴얼에 근거하여 재난을 관리하고 있다. 현재 운용중인 표준매뉴얼을 점검하고 미흡한 항목에 대해 개선방안을 제시하고자 한다. 연구방법:이에 따라, 재난관리 매뉴얼 내·외부 전문가, 본부·지방관서의 재난업무 담당자 등과 함께 표준매뉴얼을 점검하였고 우선적으로 개선해야 할 항목을 도출하였다. 연구결과: 붕괴사고의 경우에는 재난관리 주관부처 선정의 논란을 제거하기 위해 행정안전부에서 선정기준을 정확히 제시할 필요가 있으며, 인파밀집사고의 경우에는 표준매뉴얼 보다는 행동·실무 매뉴얼에 구체적으로 반영하여 유사시 근로자 대피 안전성을 확보해야 할 것이다. 이 외에도 재난안전 통신망 운영 및 대피요령에 대해 상세한 내용의 추가 보완이 필요해 보인다. 결론: 향후에도 국민의 눈높이에 부응하는 재난관리 제도 개선을 위해서 해당 분야별 전문가, 재난업무 담당자, 그리고 일반시민 등의 의견수렴을 통해 근로자의 생명과 안전을 보장하고 좀 더 현장감 있는 사업장 대규모 인적사고 위기 관리 표준매뉴얼 마련을 기대한다.
