Around 70% of domestic aircraft accidents in helicopter aircraft. The causal factors of the helicopter accidents are identified as human factors. People have focused mostly on unsafe acts to prevent the accident. The accidents should be analysed on human factors to reduce accident. The unsafe acts can be managed effectively if the latent failures were identified through the HFACS. This paper is to introduce about the latent failure classified by the HFACS and provide the analysis regarding the latent failure of the helicopters by the aviation safety advisors through the interviews.

Through so that accident of semiconductor industry deduces unsafe factor of the person center on unsafe behaviour that incident history and questionnaire and I made starting point that extract very important factor. It served as a momentum that make up base that analyzes factors that happen based on factor that extract factor cause classification for the first factor, the second factor and the third factor and presents model of human error. Factor for whole defines factor component for human factor and to cause analysis 1 stage in human factor and step that wish to do access of problem and it do analysis cause of data of 1 step. Also, see significant difference that analyzes interrelation between leading persons about human mistake in semiconductor industry and connect interrelation of mistake by this. Continuously, dictionary road map to human error theoretical background to basis traditional accidental cause model and modern accident cause model and leading persons. I wish to present model and new model in semiconductor industry by backbone that leading persons of existing scholars who present model of existent human error deduce relation. Finally, I wish to deduce backbone of model of pre-suppression about accident leading person of the person center.
Conventionally, all the unsafe acts by human beings in relation to industrial accidents have been regarded as unintentional human errors. Exceptionally, however, in the cases with fatalities, seriously injured workers, and/or losses that evoked social issues, attention was paid to violating related laws and regulations for finding out some people to be prosecuted and given judicial punishments. As Heinrich stated, injury or loss in an accident is quite a random variable, so it can be unfair to utilize it as a criterion for prosecution or punishment. The present study was conducted to comprehend how categorizing intentional violations in unsafe acts might disrupt conventional conclusions about the industrial accident process. It was also intended to seek out the right direction for countermeasures by examining unsafe acts comprehensively rather than limiting the analysis to human errors only. In an analysis of 150 industrial accident cases that caused fatalities and featured relatively clear accident scenarios, the results showed that only 36.0% (54 cases) of the workers recognized the situation they confronted as risky, out of which 29.6% (16 cases) thought of the risk as trivial. In addition, even when the risks were recognized, most workers attempted to solve the hazardous situations in ways that violated rules or regulations. If analyzed with a focus on human errors, accidents can be attributed to personal deviations. However, if considered with an emphasis on safety rules or regulations, the focus will naturally move to the question of whether the workers intentionally violated them or not. As a consequence, failure of managerial efforts may be highlighted. Therefore, it was concluded that management should consider unsafe acts comprehensively, with violations included in principle, during accident investigations and the development of countermeasures to prevent future accidents.
The purpose of this study was parents' perception on young children's safety life, safety accident, and safety education and provided basic data of administrating parent education for young children's safety. Subjects of this study were 620 parents (310 fathers and 310 mothers) of young children attending at four public kindergartens and two day care centers located in C city and D county. The results of the study were as follows: First, regarding parents' perception on young children's safety life, parents thought that their perception and attitude would mostly affect young children' safety life. Second, with regard to parents' perception on safety accident, half of parents experienced such safety accident as accident during play, traffic accident, accident in dangerous places, accident from dangerous matters, accident in sport activities, fire, and electric shock. Third, most parents looked upon safety education as very important one, and fathers were more satisfied with the safety education administered by kindergartens. Fourth, with regard to parents' perception on parent education for young children's safety, most parents thought that parent education for safety would be necessary. They ranked traffic and play accidents as the most important contents of safety education.
This paper describes the radiation doses to human and non-human biota in the Republic of Korea, as a result of the Fukushima nuclear accident. By using the measured airborne activity and ground deposition, the effective and thyroid doses of five human age groups (infant, 5 years, 10 years, 15 years and adult) were estimated by the ECOSYS code, and the whole body absorbed dose rate of the eight Korean reference animals and plants (RAPs) was estimated by the K-BIOTA (the Korean computer code to assess the risk of radioactivity to wildlife). The first-year effective and thyroid human doses ranged from 5.7E-5 mSv in the infant group to 2.0E-4 mSv in the 5 years group, and from 5.0E-4 mSv in the infant group to 3.4E-3 mSv in the 5 years group, respectively. The life-time (70 years) effective and thyroid human doses ranged from 1.5E-4 mSv in the infant group to 3.0E-4 mSv in the 5 years group, and from 6.0E-4 mSv in the infant group to 3.5E-3 mSv in the 5 years group, respectively. The estimated maximum whole body absorbed dose rate to the Korean RAPs was 6.7E-7 mGy/d for a snake living in soil (terrestrial biota), and 2.0E-5 mGy/d for freshwater fish (aquatic biota), both of which were far less than the generic dose criteria to protect biota from ionizing radiation. Also, the screening level assessment for ERICA's (Environmental Risks from Ionizing Contaminants: Assessments and management) limiting organisms showed that the risk quotient (RQ) for the estimated maximum soil and water activity was significantly less than unity for both the terrestrial and freshwater organisms. Conclusively, the radiological risk of the radioactivity released into the environment by the Fukushima nuclear accident to the public and the non-human biota in the republic of Korea is considered negligible.

Expresway traffic accident is fatal accident by high speed, especially human injury is a great social issue. This paper aims to identify characteristic differences of highway accidents that involve human injuries or not. To analysis the elements that affect the two types of accidents used the logistic regression model. The analysis showed that human injury accident rate is increased in case of straight road, flat, or cut-slope areas, barriers, male driver, and older driver. These results provide the ground for actions to counter the problems. By discovering factors for accidents leading to fatality, this study can provide important implications for authorities that establish highway safety measures and policies in preventing human injuries or deaths from car accidents.
Aviation safety is increasingly important to secure the safety of the Republic of Korea Air Force (ROKAF). A critical activity for enhancing aviation safety is to analyze an accident throughly and to identify causes that can explain it reasonably. The results of such a systematic accident investigation can be effectively used for improving information displays, task procedures, and training systems as well as for reorganizing team structure and communication control system. However, the current practice of analyzing aviation accidents in ROKAF is too superficial and simple to diagnose them systematically. Additionally, the current practice does not give a full consideration to human factors that have been identified as main causes of most of the aviation accidents. With this issue in mind, this study aims to suggest a new approach to analyzing aviation accidents related to human factors. The proposed method is developed on the basis of several models and frameworks about system safety, human error, and human-system interaction. Its application to forty-two human factors-related accidents, which have occurred in ROKAF during the last ten years, showed that the proposed method could be a useful tool for analyzing aviation accidents caused by human factors.
Basis frame-work's base in a semiconductor industry have gas, chemical, electricity and various facilities in bring to it. That it is a foundation by fire, power failure, blast, spill of toxicant huge by large size accident human and physical loss and damage because it can bring this efficient, connect with each kind mechanical, physical thing to prevent usefully need that control finding achievement factor of human factor of human action. Large size accident in a semiconductor industry to machine and human and it is involved that present, in system by safety interlock defect of machine is conclusion for error of behaviour. What is not construing in this study, do safety in a semiconductor industry to do improvement. Control human error analyzes in human control with and considers mechanical element and several elements. Also, apply achievement factor using O'conner Model by control method of human error. In analyze by failure mode effect using actuality example.

Results: The analysis reveals that 57% of railway accidents are attributed to human factors, followed by mechanical (28%), environmental (7%), and management (8%) factors. Conclusions: The study underscores the necessity of prioritizing safety and establishing a unified organizational approach to prevent human error accidents. It calls for an alignment of risk perception between headquarters and field operations, advocating for educational and perceptual changes, as well as systematic improvements to achieve safety goals.
This research analyzes blasting accident cause that is happening construction and engineering works spot taking advantage of AHP (Analytic Hierarchy Process) techniques as metrical. Result that apply AHP with blasting accident that is happened the South Korea and Japan, appeared by thing which relative importance by human cause is highest. Specially, it is observance of safety rule that dominate the highest ratio among of human cause, and if observe a little, causes that prevention is possible are much. By result of this research, necessity of safety education is important first of all for prevention of blasting accident. Also, thorough safety control plan of during work and enough on-the-slut probe before work should be established. Because explosives uses gunpowder and explosive high energy, work by qualified person is essential. Ant it may become help to minimize dissipation of important life and property preventing beforehand explosion accident of gunpowder.
