  • Successful emergency pulmonary embolectomy with the cardiopulmonary bypass was performed in a 41 \ulcorneryear old male who suffered massive pulmonary embolism after longterm bed rest due to the injury of left knee. Temporary cardiopulmonary bypass provided 120 minutes of circulatory support while complete removal of bilateral pulmonary emboli accomplished using Forgarty catheter and Gall stone forceps. Also, manual compression of the lungs was necessary to remove distal branching emboli. The patient had smooth and uneventful hospital course without complications and discharged from hospital taking coumadine on the 13th day after the operation.

  • Coronary arterial involvement in Takayasu arteritis (TA) is not uncommon. Herein, we describe a case of TA with celiac trunk and superior mesenteric artery occlusion combined with coronary artery disease. Bilateral huge internal thoracic arteries (ITAs) and the inferior mesenteric artery provided the major visceral collateral circulation. After percutaneous intervention to the right coronary artery, off-pump coronary artery bypass grafting for the left coronary territory was done using a right ITA graft and its large side branch because of its relatively minor contribution to the visceral collateral circulation.

  • Recently, population-based studies of type 2 diabetes patients have provided evidence that metformin treatment is associated with a reduced cancer incidence and mortality, but its mode of action remains unclear. Here we report effects of metformin on hepatocellular carcinoma (HCC) Hep-G2 cells and details of molecular mechanisms of metformin activity. Our research indicates that metformin displays anticancer activity against HCC through inhibition of the mTOR translational pathway in an AMPK-independent manner, leading to G1 arrest in the cell-cycle and subsequent cell apoptosis through the mitochondrion-dependent pathway. Furthermore, we showed that metformin strongly attenuated colony formation and dramatically inhibited Hep-G2 tumor growth in vivo. In conclusion, our studies suggested that metformin might have potential as a cytotoxic drug in the prevention and treatment of HCC.

  • Causaliga is a syndrome of sustained burning pain, allodynia and hyperpathia after a traumatic nerve lesion, often combined with vasomotor and sudomotor dysfunction and later trophic changes. Various treatments of causalgia contain sympathetic blockade, sympathectomy, transcutaneous electrical nerve stimulation, physical therapy, cryotherapy and psychotherapy. Repeated stellate ganglion blocks with 6ml of 0.25% bupivacaine provided good results for 2 patients. We recommand sympathetic blocks for treatment of causalgia.

  • Hemifacial spasm is a distressing condition characterized by involuntary, intermittent, unilateral twitching of, all or parts, of the muscles innervated by facial nerve. This ailment is most common among middle-aged women. Neither causative agent nor reliable treatment has been established because etiology of idiopathic hemifacial spasm has remained undefined to date. This report describes two cases of hemifacial spasm treatment by injection of Botulinum toxin. An injection of Botulinum toxin provided relief of hemi facial spasm. Botulinum toxin therapy is an effective and convenient treatment of hemifacialspasm.

  • Purpose: The purpose of this study was to investigate the contribution score of nursing organizational level for Performance Management Indicators (PMIs) in the nursing department in a hospital. Methods: The process consisted of three stages. Stage I: The strategies based on 4 perspectives of Balanced Scorecard (BSC) were identified by BSC committee in the hospital nursing department. Stage II : PMIs according to the strategies were developed. Stage III : The contribution score according to nursing organizational level (nursing department, nursing unit, nurse) for PMIs were surveyed by questionnaire. Data was analyzed using descriptive statics and ANOVA test. Results: Eight strategies were defined according to 4 perspectives of BSC. Eighteen PMIs were determined according to the strategies. The nurse considered the contribution of nursing department most highly on the 13 among 18 PMIs. Conclusion: This study provided basic data for further study to identify the subject of responsibility on the strategic management of performance.

  • Objective: This case report examines the effects of Jaeumganghwa-tang as a treatment option for Sjögren’s syndrome. Methods: We provided the patient with Jaeumganghwa-tang twice a day, as well as acupuncture and bee venom therapy (both twice a week), for 30 days. Symptoms were charted and evaluated using the European League Against Rheumatism (EULAR) Sjögren’s Syndrome Patient Reported Index (ESSPRI).Results: The patient’s ESSPRI score decreased from 8 to 6 after the 30-day treatment. Conclusions: Jaeumganghwa-tang may be helpful in alleviating Sjögren’s syndrome.

  • A 2.5-kg neonate with coarctation of the aorta and a small left ventricle experienced a severe pulmonary hypertensive crisis. An emergency pulmonary artery-to-systemic artery shunt was placed to break the positive feedback loop caused by pulmonary hypertension and functional mitral stenosis. This shunt provided immediate relief of suprasystemic pulmonary hypertension and the resultant low cardiac output.

  • $^{68}Ga-PET$ is of growing importance in the practice of nuclear medicine diagnostic imaging for neuroendocrine tumors as well as prostate cancers. Following this interests, we herein present the radiosynthesis process of [$^{68}Ga$]edotreotide ([$^{68}Ga$]DOTA-TOC) based on the fully automated procedure for clinical doses that can be provided the reduction of radiation exposure and high reproducibility. The quality controls of clinical doses in compliant with European Pharmacopoeia are also discussed.

  • This study was performed to devise a sizing system for child hospital gown(uniform). The researchers surveyed the usage of child hospital gown and developed a sizing system for the child hospital gown. The suitability of revised size specification of gown was evaluated. 219 child inpatients participated in the survey and 13 child inpatients took part in the wear test. The results of the study were as following. 1) 71.7% of the subjects wore the uniform shirts. however numerous number of subjects did not wear uniform pants that the hospital provided(52.5%). The 68.2% of children who were shorter than 100cm did not wear uniform pants. 2) The most significant reason not wearing the hospital gown was unsuitable sizing system (42.1%). Inconvenience for donning and doffing comes next(26.2%). 3) The least satisfied garment size problems were the pants length and sleeve length. 4) The researchers developed a sizing system for the child hospital gown. The system provides 4 different sizes for children age from 2 months to 12 years. One-piece dress style was suggested for the smallest size(XS). The larger sizes(S. M. L) were designed in pajama style. 5) The sizing system developed in this study was moderately satisfied by the users.
