소부(少府)(HT8) 자침(刺針)이 정신적 스트레스를 가한 성인의 심박변이도에 미치는 영향 (Effect of Acupuncture for Mental Stress on Short-term Analysis of Heart Rate Variability(HRV))

  • Objectives : The purpose of this study was to assess the effect of acupuncture for reducing mental stress using power spectrum analysis of the heart rate variability. Methods : 5 healthy volunteers participated in this study. After instrumentation and 5-minute rest period, mental stress was provided for 5-minute. HRV was recorded before and after the mental stress. Acupuncture was put on the HT8, and needle was removed after 15min. Than 2nd mental stress was given for 5-minute with same method. For the control, same process was repeated to same subject except for acupuncture. Results : After mental stress, LF and LF/HF ratio is significantly increased. After acupuncture treatment, LF/HF ratio is significantly decreased, and LF do not significantly changed despite of mental stress. Conclusions : The result suggest that acupuncture treatment can regulate and prevent the alteration of autonomic nerve system due to mental stress.

  • The purpose of this study is to analyse the job activities of the H/D nurses andto develop their job activities. This study surveyed 222 H/D nurses' reports using a questionnaire which contained 101 questions about the nursing care contents of the H/D nursing and 7 questions about the characteristics of the H/D nurses who were working in the H/D rooms of the 80 hospitals and local clinics. H/D nurses checked for 6 days the frequency of the H/D job activities and the amount of time spent in each individual nursing content by the stop watch which had been provided. The data was gathered from Jan. 21, 1999 to Oct. 30, 1999 and analysed by descriptive statistics, F-test and Pearson's correlation r with SPSS program. The results are as follows:

의료기관 진료비 삭감율 및 이의신청 회수율과 관련되는 관리요인 (The Management Factors of Medical Expenditure Reduction and Recovery of Hospital Health Insurance Department)

  • Main objective of this study is to investigate the factors that affect the reduction and recovery rate of hospital health insurance review departments of Korean tertiary hospitals, general hospitals and hospitals. A nationwide mail survey was conducted through structured questionnaire for chiefs and members of health insurance review departments of 178 hospitals, and data from 1,064 respondents were utilized in the final analysis. Major findings are as follows: 1) Monthly average reduction rate of surveyed hospitals was 1.18% for inpatients and 0.56% for outpatients. Also, average recovery rates from protests were 45.7% and 53.6%, respectively. 2) Average ages of health insurance review department employees were 41.5 for chiefs and 34.9 for members, and their average working period was 6.64 years. 69.1% of the respondents felt stress in their jobs, and they suggested 'communication problems with doctors' along with 'heavy workloads' as major reasons for the stress. 3) Reduction rates decreased and recovery rates increased when separate member in charge was assigned to each medical department, when health insurance review departments periodically provided medical parts with related education, and when the members participated in various committees.

재활전문병원 간호사의 환자안전문화에 대한 인식과 안전간호활동 수행의 관계 (Relationship between Perception of Patient Safety Culture and Performance for Safety Care Activity in Rehabilitation Hospital Nurse)

  • Purpose: This study was to examine the relationships between nurse's perception of patient safety culture and performance for safety nursing activities at rehabilitation hospitals. Methods: This study applied a descriptive research design. Participants were 194 nurses who have provided nursing services for more than 6 months at 4 rehabilitation hospitals located in B metropolitan city. Data were analyzed using descriptive statistics, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficients and multiple regression. Results: There was a positive correlation between the awareness of patient safety culture and safety nursing activity. Multivariate analysis showed that hospital work environment, experience of education, hospital climate, frequency of reported events, and marital status were significantly associated with the safety nursing activity. Overall, approximately 23.1% of total variability in the safety nursing activity could be explained by the 5 variables ($R^2=0.231$, p<.005). Conclusion: Nurses at rehabilitation hospitals are relatively positive about patient safety culture. Therefore, we need to develop safety education programs at the level of organization in order to improve patient safety through performing effective safety nursing activities in addition to increase awareness of patient safety culture among nurses. Furthermore, we need hospital's strategies at the system level for open communication and outcome reports regarding patient safety.

병원행정직원의 간병비 급여화에 대한 인식 (The Awareness on Medical Insurance on the Caregivers Cost for Hospital Administrative Personnel)

  • The purpose of this study is to investigate awareness on medical insurance on the caregivers cost for hospital administrative staff and to provide the basic data for realization of legislation. The subjects were caregivers living in Busan, the survey was conducted from February 18 to March 9, 2013, 283 except for 17 copies of non-response and error response among a total of 300 questionnaires were analyzed. As a result, To improve the quality of care services, there were 51.8% of refresher training needs in refresher training items, 72.7% in favor of premiums increases in health insurance details, as for health insurance coverage subjects, patients' income were 32.0%, copayment for caregivers cost was 20.0%, which was 42.3%. Refresher training item, premiums increases, health insurance applied subjects, variables for copayment for care fee were related to medical insurance on the caregivers cost. On legislation on the medical insurance, systematic and standardized criteria should be provided to provide standardized curriculum for caregivers, to relieve patients and guardians of economic burden for caregivers cost and offer the stability of the cost.

  • Cancer is a highly complex medical problem with ramifications for public health throughout the world. Most studies have mainly focused on change in the nuclei as being aetiologically responsible. Few have examined the relationship between the cytoplasm and cancer, despite the fact that research has indicated that the cytoplasmic environment is an important factor for cellular differentiation and that the genetic information provided by the nucleus is entirely dependent on this environment for its expression. Gene mutations may be the result, rather than the cause of carcinogenesis. We submit a new concept - "short base sequences" (50-500 bps, including DNA or RNA sequences) in the cytoplasm which could play an important role in carcinogenesis. This is a new theory to explain the origin of the cancer.

사용자요구기반에 의한 노인전문병원 아트리움계획 연구 (A Study on Planning for Elderly Hospital's Atrium on the base of Users Demand)

  • As the number of aged people increase, hospitals specializing in elderly cares have also increased very rapidly. Therefore it is necessary to improve the quality of these hospitals for the elderly people in order to enhance the function of these hospitals not only for conventional treatment but also for recovery and convalescence. To study such trend and needs, this study researched on planning for Elderly Hospital's Atrium that provides a space for the elderly people so that they may enjoy the natural environment while being supported from diverse aspects for long term cares and sustained living, and tried to find out design solutions based on users' demand through questionnaire survey and interviews both with the management of the elderly hospital and the inpatients or the users. As a result of this study, it was revealed that each user showed his or her own respective characteristics concerning his or her expectation about the environment of their using spaces, and according to this result the strategic direction and design plan could be drawn. Based on plans for this Elderly Hospital's Atrium specializing in cares for the elderly people, useful information will be provided for caring, treatment and recovering environment of the elderly hospitals in future considering not only the conventional medical and technical services but also the inpatients' or the users' physical, psychological and social factors.

Alloplastic total temporomandibular joint replacement using stock prosthesis: a one-year follow-up report of two cases

  • Alloplastic total replacement of the temporomandibular joint (TMJ) was developed in recent decades. In some conditions, previous studies suggested the rationale behind alloplastic TMJ replacement rather than reconstruction with autogenous grafts. Currently, three prosthetic products are available and approved by the US Food and Drug Administration. Among these products, customized prostheses are manufactured, via computer aided design/computer aided manufacturing (CAD/CAM) system for customized design; stock-type prostheses are provided in various sizes and shapes. In this report, two patients (a 50-year-old female who had undergone condylectomy for the treatment of osteochondroma extending to the cranial base on the left condyle, and a 21-year-old male diagnosed with left temporomandibular ankylosis) were treated using the alloplastic total replacement of TMJ using stock prosthesis. The follow-up results of a favorable one-year, short-term therapeutic outcome were obtained for the alloplastic total TMJ replacement using a stock-type prosthesis.

구강암의 간편 진단 기법 (Diagnostic aids for the detection of oral cancer)

  • Historically, the screening of patients for signs of oral cancer and precancerous lesions has relied upon the conventional oral examination. A variety of commercial diagnostic aids and adjunctive techniques are developed to potentially assist in the screening of healthy patients for evidence of occult cancerous change. This paper is reviewing the literature associated with current oral cancer screening aids such as spectroscopy, chemoiluminescence, exfoliative cytopathology, vital staining and saliva as a diagnostic tool. Despite the increased public awareness of oral cancer, no technique or technology to date has provided definitive evidence to suggest that it improves the sensitivity or specificity of oral cancer screening beyond clinical oral examination alone.

바이오필릭 디자인에 기반한 노인요양병원 로비공간의 다감각적 치유환경에 관한 연구 (A Study on Characteristics of Sensory Richness Towards Healing Environments at the Lobby of Geriatric Hospital Using Biophilic Design Approach)

  • For the elderly with declining sensory due to aging, various sense stimulation factors provided in indoor environment is desirable healing environment. This study aims to propose the direction of sensory healing environment by adapting the richness of natural environment in indoor place based on the biophilic design theory with implementation method of sensory richness environment. The healing environment was limited to the ward in the beginning, but it has widened its range to the whole place, and the lobby is considered quiet important to healing. Therefore, this study selected lobby space of geriatric hospital as study subject. As for the study method, we deducted the analytic matrix focusing on the property and elements of the direct and indirect experience of nature appearing in the biophilic design theory, and analyzed the field investigation of subject space. Also, this study paid attention to point where the hotel lobby leads the change of emotional environment, and conducted analysis and comparison by deciding this is what the hotel and geriatric hospital should direct to. As the result, the biophilic design factor of hotel is actively expressed by complex direction of various factors, but as for the geriatric hospital, it was rather passive and simple in expression. The adaptation ratio between hotel and geriatric hospital was almost more than twice times difference in average.
