There are many studies on the indications and efficacy of splint therapy commonly used in patients with temporomandibular disorders (TMD). However, there have been no studies on the splint weaning in terms of the splint use tapering period in relation to symptom improvement of TMD. This retrospective study aims to analyze a proper splint weaning method in patients with TMD based on symptom improvement. Materials and Methods: The authors examined 130 TMD patients with TMJ disorders, masticatory muscle disorders, and clenching/bruxism who had received splint therapy (occlusion stabilization splint, anterior positioning splint) of patients who visited the Department of Orofacial Pain and Oral Medicine at Yonsei University Dental Hospital from 2015 to 2020. They were evaluated according to the method to wean splints. Results: The mean splint therapy period was 29.0 months, during which patients wore splints 7 days a week for 8.4 months, 3 - 4 days a week for 9.5 months, and finally 1 - 2 days a week for 11.1 months (a total of 29.0 months, about 2.5 years). Conclusion: It seems that TMD symptoms can be alleviated and side effects such as occlusal change can be minimized if patients wear a splint 7 days a week for the first 6 months, followed by 3 - 4 days a week for the next 6 to 18 months, and finally 1 - 2 days a week after 18 months.
Studies have demonstrated that PE_PGRS45 is constitutively expressed under various environmental conditions (such as nutrient depletion, hypoxia, and low pH) of the in vitro growth conditions examined, indicating that PE_PGRS45 protein is critical to the basic functions of Mycobacterium tuberculosis. However, there are few reports about the biochemical function and pathogenic mechanism of PE_PGRS45 protein. The fact that this M. tuberculosis gene is not easily expressed in E. coli may be mainly due to the high content of G+C and the use of unique codons. Fusion tags are indispensable tools used to improve the soluble expression of recombinant proteins and accelerate the characterization of protein structure and function. In the present study, His6, Trx, and His6-MBP were used as fusion tags, but only MBP-PE_PGRS45 was expressed solubly. The purification using His6-MBP tag-specific binding to the Ni column was easy to separate after the tag cleavage. We used the purified PE_PGRS45 to immunize New Zealand rabbits and obtained anti-PE_PGRS45 serum. We found that the titer of polyclonal antibodies against PE_PGR45 was higher than 1:256000. The result shows that purified PE_PGRS45 can induce New Zealand rabbits to produce high-titer antibodies. In conclusion, the recombinant protein PE_PGRS45 was successfully expressed in E. coli and specific antiserum was prepared, which will be followed by further evaluation of these specific antigens to develop highly sensitive and specific diagnostic tests for tuberculosis.

Purpose: At present, there are a variety of serious patient safety incidents related to problems in health information technology (HIT), specifically involving electronic medical records (EMRs). This emphasizes the need for an enhanced electronic medical record system (EMRS). As such, this study analyzed both the nature of and potential to prevent incidents associated with HIT/EMRS based on data from the Korea Patient Safety Reporting and Learning System (KOPS). Methods: This study analyzed patient safety incidents submitted to KOPS between August 2016 and December 2019. HIT keywords were used to extract HIT/EMRS incidents. Each case was reviewed to confirm whether the contributing factors were related to HIT/EMRS (HIT/EMRS-related incidents) and if the incident could have been prevented (HIT/EMRS-preventable incidents). The selected reports were summarized for general clarity (e.g., incident type, and degree of harm). Results: Of the 25,515 obtained reports, 2,664 incidents (10.4%) were HIT-related, while 2,525 (9.9%) were EMRS-related. HIT/EMRS-related incidents were the third largest type of incident followed by 'fall' and 'medication incidents.' More than 80% of HIT/EMRS-related incidents were medication-related, accounting for approximately one-third of the total number of medication incidents. Approximately 10% of HIT/EMRS-related incidents resulted in patient harm, with more than 94% of these deemed as preventable; further, sentinel events were wholly preventable. Conclusion: This study provides basic data for improving EMR use/safety standards based on real-world patient safety incidents. Such improvements entail the establishment of long-term plans, research, and incident analysis, thus ensuring a safe healthcare environment for patients and healthcare providers.

The aim of this study is to evaluate the development of the left mandibular third-molar development with modified Demirjian method and its relation to chronological age. A total of 1653 digital panoramic radiographs of healthy individuals aged between 7 and 23 years who visited Yonsei University Dental hospital were selected. The developmental status of the left mandibular third molars was assessed using dental maturity scoring proposed by Demirjian et al. The mean age of the first appearance of mineralization, complete crown formation, and complete root formation were around 9, 14, 21 years respectively. Statistically significant differences between males and females in the development stage of D and G were revealed that crown formation and root length completion were attained earlier in males than in females. There was significant positive relationship between age and third molar development in both sexes, and new formula was presented to estimate age of children and adolescents based on their developmental stages of third molars. In this study, the use of left mandibular third molar as a developmental marker is appropriate, and age estimation can be attained with dental maturity stage.
The purpose of this study was to insert a pedicle screw into a pig thoracic vertebrae, a general CT scan(Non MAR), and a thoracic axial image obtained with the Metallic Artifact Reduction for Orthopedic Implants (O-MAR) to reduce artifacts. The image obtained by reconstructing the algorithm (Standard, Soft, Bone, Detail) was used using the image J program. Signal to noise ratio(SNR) and contrast to noise ratio(CNR) were compared and analyzed by obtaining measured values based on the given equation. And this study was to investigate tube voltage and algorithm suitable for CT scan for thoracic pedicle screw insertion. As a result, when non-MAR was used, the soft algorithm showed the highest SNR and CNR at 80, 100, 120, and 140 kVp, On the other hand, when MAR was used, the standard algorithm showed the highest at 80 kVp, and the standard and soft algorithms showed similar values at 100 kVp. At 120 kVp, the Soft and Standard algorithms showed similar values, and at 140 kVp, the Soft algorithm showed the highest SNR and CNR. Therefore, when comparing Non-MAR and MAR, even if MAR was used, SNR and CNR did not increase in all algorithms according to the change in tube voltage. In conclusion, it is judged that it is advantageous to use the Soft algorithm at 80, 100, 120, and 140 kVp in Non MAR, the Standard algorithm at 80 and 100 kVp in MAR, and the Soft algorithm at 120 and 140 kVp. This study is expected to serve as an opportunity to further improve the quality of images by using selective tube voltage and algorithms as basic data to help evaluate images of pedicle screw CT scans in the future.

Objective : This study aimed to develop a Korean version of the HLS-EU-Q47 that deals with the concept of health literacy based on the understanding, apply/use, process/appraisal, and access/obtain to health information. The purpose of this study is to confirm the level of comprehensive health literacy of the elderly in Korea using this tool. Methods : After going through the Korean interpretation process of HLS-EU-Q47, the items were confirmed through content validity verification by experts. The completed Korean-HLS-EU-Q47 (K-HLS-EU-Q47) was applied to 254 elderly people in the local community to analyze the degree of internal consistency and reliability. Furthermore, a comparative analysis based on the general variables was conducted. Results : The internal consistency of this tool for the elderly yielded Cronbach's 𝛼 of .81~.91, indicating a high level of reliability. There was a difference in health literacy according to sex and age based on general characteristics. Men had higher health literacy than women, and with increasing age, health literacy decreased. Conclusion : In this study, the K-HLS-EU-Q47 was developed to assess the comprehensive health literacy level of the elderly in Korea. It is expected that the search for ways to maintain a healthier life for the elderly through understanding the health literacy levels of the elderly using the results of this study will become more active.
The Study In order to obtain a sharpness Image from Skull PA axial projection (Haas) in a head axial X-ray Examination, this study changed the posture angle using Skull Phantom and evaluated the image subjectively to 5 radiologists who worked in the Department of Imaging at University Hospital. In the prone position, the head was lowered 4 cm from the back of the head, entered 25° toward the head, and the image evaluation score was high with 20 points, such as the back bone, dorsum sellae projected in the large hole, and posterior clinoid process. In addition, the score significance was verified, and the Cronbach Alpha value was evaluated to have good reliability of 0.789. As a result of calculating the signal-to-noise ratio (SNR) by setting the region of interest (ROI) of the image, it was the highest at 5.957 for 25° incident at the back of the head and 6.430 for 30° incident at the back of the head. As a result of the study, in order to obtain a sharp image of the back of the head bone, dorsum sellae, and posterior clinoid process when shooting in the axial direction after the head, it is filmed by tilting 25° toward the head from 4 cm below the back of the head. In order to obtain a sharp image of rock pyramid symmetry, petrous ridge, sagittal suture, and lambdoid suture, it is thought that it will be helpful for clinical use if you shoot it 8cm down from the back of the head and tilt it 30° toward the head.
One of the typical methods for lowering radiation dose while maintaining image quality of computed tomography (CT) is the use of model-based iterative reconstruction (MBIR). This study is to evaluate the image quality by adjusting the strength of the advanced modeled iterative reconstruction (ADMIRE), which is well known as a representative model of MBIR. The study was conducted using phantom, and CT images were obtained while adjusting the strength of ADMIRE in units of 1 to 5. Quantitative evaluation includes noise levels using coefficient of variation (COV) and contrast to noise ratio (CNR), as well as natural image quality evaluation (NIQE) and blind/referenceless image spatial quality evaluator (BRISQUE). As a result, in both noise level and blind quality evaluation results, the higher the strength of ADMIRE, the better the results were derived. In particular, it was confirmed that COV and CNR were improved 1.89 and 1.75 times at ADMIRE 5 compared to ADMIRE 1, respectively, and NIQE and BRISQUE were proved to be improved 1.35 and 1.22 times at ADMIRE 5 compared to ADMIRE 1, respectively. In conclusion, this study was proved that the reconstruction strength of ADMIRE had a great influence on the noise level and overall image quality evaluation of CT images.
Research on the presence or absence of radiation shielding for FDM-type filaments has recently begun to be studied, but filaments with shielding capabilities are not sold in Korea, and not studies yet. Therefore, in this research, we will use HDPE (High Density Polyethylene) as a base material, select bismuth as a reinforcing material to manufacture a composite filament, evaluate the shielding ability, and provide basic data for the development of a radiation shielding composite material using 3D printing.A filament is produced by mixing Bismuth with an effective atomic number 83 with HDPE of PE series and adjusting the content of Bismuth to 20% wt, 30% wt, 40% wt. Compounded filaments were evaluated for their physical properties and shielding capabilities by ASTM evaluation methods. As the bismuth content increases, the density, weight, and tensile strength increase, and the shielding capacity is confirmed to be excellent. As a result of the radiation shielding capacity evaluation, it was confirmed that HDPE (80%) + Bi (20%) showed a shielding rate of 82% at 60 kV and a shielding rate of up to 94% or more at 40% bismuth content. In this study, we confirmed that it was possible to produce a radiation shield that is lighter than the metal particle-containing filaments. Furthermore, that have been shield radiation by using HDPE + Bi filaments, and radiation in the medical and radiation industries. The possibility of using it as a shielding complex was confirmed.
Medical image segmentation is the most important task in radiation therapy. Especially, when segmenting medical images, the liver is one of the most difficult organs to segment because it has various shapes and is close to other organs. Therefore, automatic segmentation of the liver in computed tomography (CT) images is a difficult task. Since tumors also have low contrast in surrounding tissues, and the shape, location, size, and number of tumors vary from patient to patient, accurate tumor segmentation takes a long time. In this study, we propose a method algorithm for automatically segmenting the liver and tumor for this purpose. As an advantage of setting the boundaries of the tumor, the liver and tumor were automatically segmented from the CT image using the 2D CoordConv DeepLab V3+ model using the CoordConv layer. For tumors, only cropped liver images were used to improve accuracy. Additionally, to increase the segmentation accuracy, augmentation, preprocess, loss function, and hyperparameter were used to find optimal values. We compared the CoordConv DeepLab v3+ model using the CoordConv layer and the DeepLab V3+ model without the CoordConv layer to determine whether they affected the segmentation accuracy. The data sets used included 131 hepatic tumor segmentation (LiTS) challenge data sets (100 train sets, 16 validation sets, and 15 test sets). Additional learned data were tested using 15 clinical data from Seoul St. Mary's Hospital. The evaluation was compared with the study results learned with a two-dimensional deep learning-based model. Dice values without the CoordConv layer achieved 0.965 ± 0.01 for liver segmentation and 0.925 ± 0.04 for tumor segmentation using the LiTS data set. Results from the clinical data set achieved 0.927 ± 0.02 for liver division and 0.903 ± 0.05 for tumor division. The dice values using the CoordConv layer achieved 0.989 ± 0.02 for liver segmentation and 0.937 ± 0.07 for tumor segmentation using the LiTS data set. Results from the clinical data set achieved 0.944 ± 0.02 for liver division and 0.916 ± 0.18 for tumor division. The use of CoordConv layers improves the segmentation accuracy. The highest of the most recently published values were 0.960 and 0.749 for liver and tumor division, respectively. However, better performance was achieved with 0.989 and 0.937 results for liver and tumor, which would have been used with the algorithm proposed in this study. The algorithm proposed in this study can play a useful role in treatment planning by improving contouring accuracy and reducing time when segmentation evaluation of liver and tumor is performed. And accurate identification of liver anatomy in medical imaging applications, such as surgical planning, as well as radiotherapy, which can leverage the findings of this study, can help clinical evaluation of the risks and benefits of liver intervention.
