  • Objective : To investigate the effects of trauma center establishment on the clinical characteristics and outcomes of trauma patients with traumatic brain injury (TBI). Methods : We enrolled 322 patients with severe trauma and TBI from January 2015 to December 2016. Clinical factors, indexes, and outcomes were compared before and after trauma center establishment (September 2015). The outcome was the Glasgow outcome scale classification at 3 months post-trauma. Results : Of the 322 patients, 120 (37.3%) and 202 (62.7%) were admitted before and after trauma center establishment, respectively. The two groups were significantly different in age (p=0.038), the trauma location within the city (p=0.010), the proportion of intensive care unit (ICU) admissions (p=0.001), and the emergency room stay time (p<0.001). Mortality occurred in 37 patients (11.5%). Although the preventable death rate decreased from before to after center establishment (23.1% vs. 12.5%), the difference was not significant. None of the clinical factors, indexes, or outcomes were different from before to after center establishment for patients with severe TBI (Glasgow coma scale score ${\leq}8$). However, the proportion of inter-hospital transfers increased and the time to emergency room arrival was longer in both the entire cohort and patients with severe TBI after versus before trauma center establishment. Conclusion : We confirmed that for patients with severe trauma and TBI, establishing a trauma center increased the proportion of ICU admissions and decreased the emergency room stay time and preventable death rate. However, management strategies for handling the high proportion of inter-hospital transfers and long times to emergency room arrival will be necessary.

Population variable in 2009 was taken from the website of Statistics Korea. SAS 9.2 was used for the statistical analysis. In regression analysis, year of establishment (2008) was statistically significant in explaining the number of hospital departments. Hospitals established in 2008 showed significant positive number of department than other years. Study hospitals showed increasing number of hospital departments during four years. • The purpose of this study is to analyze the effects of the health center hospital on the health service utilization pattern of the rural population in a county. Two field studies had been conducted in Yonchon County, Kyunggi Province, on February 1989 and on August 1991 before and after the establishment of the Yonchon health center hospital. This study revealed that Yonchon health center hospital occupied 7.3% of total outpatient visits and 16.8% of hospitalization of the county population and the self-sufficient rate of the outpatient visit and hospitalization of Yonchon County between two field studies increased by 1.7% and 20.9% each. Yonchon health center hospital contributed to the growth of the public health sector but it weakened the role of health sub-centers. For the efficient health service utilization of the population in that County, more investment to health center hospital would be needed and the primary health activities of the health subcenter should be enforced.

Questionnaire investigation was conducted in April 2014 throughout five different medium-size hospitals in Seoul, Gyeonggi and Chungcheongbuk provinces. 213 sets of survey were used to conduct Frequency Analysis, T-test, Dispersion Analysis(ANOVA) and Structural Equation Modeling Analysis. The research concluded that employees with higher level of education, working position and longer term of working experience were more aware about Eco-friendly hospital. In addition, male, doctor, and employees with higher level of education and working position were more satisfied with their working environment. And male, employees with higher level of education and working position were more aware about necessity of establishment of eco-friendly hospital. According to the Structural Equation Modeling Analysis, the awareness of Eco-friendly hospital and the satisfaction level of working environment were the varying factors that influence the necessity of establishment of Eco-friendly hospital. Therefore, it is crucial for hospitals that are preparing for establishing Eco-friendly hospital to raise employees' level of understanding of Eco-friendly hospital and their working environment satisfaction level.

  • Objectives : To look at the medical law reformation of Korean Medicine hospitals through the establishment and management of the Jaehan Oriental Medicine Hospital, which was the first Korean Medicine Hospital in South Korea. Methods : Revisions of the medical law since the establishment of the National Medical Act in 1951 up until 1973 when the 'Korean Medicine hospital' first entered the medical institution category were examined. Based on the revised contents, the establishment and management of the Jaehan hospital were examined. Results & Conclusions : The first mentioning of 'Korean Medicine hospital' in the medical law took place on Feb 16, 1973 when the medical law was completely revised. After law regulations on Korean Medicine hospitals were established, the fist Korean Medicine hospital was founded on Nov. 24th, 1973 according to act 2533 of the medical law. This is the Jaehan Oriental Medicine Hospital, which is the predecessor of what we now know as the Daegu Korean Medicine University Hospital. Although the Jaehan hospital was registered as a legitimate Korean Medicine hospital in November of 1973, it had already started medical practice in December of 1970. While it was established according to the standards of medical law, it changed its institution category from 'Korean Medicine hospital' to 'affiliated Korean Medicine clinic' based on another clause within the same law. The decade from 1960 to 1970 was a time when national economy was developing, and the field of medicine and medical institutions were also booming. As such, revisions in the medical law seems to not have been able to keep up with what was happening in reality. To meet the patients' right to move or to manage diseases which Korean Medicine was taking responsibility for, a medical institution with inpatient capacity was required. Therefore it is possible that the Jaehan hospital which had already been providing such a role could have been a sample case for reference in the medical law revision process.

Only one hospital establishes independent office for dietary consultation for in-and-out patients and in 3 hospitals among 32 hospital dietitians practice regular rounds to visits patients. 3. Consistant patterns and methods to counsel patients were established in 53% of hospitals. In remaining hospitals, teaching methods depend entirely on the individual dietitians. 4. A few hospitals have teaching aids and follow-up systems. 5. Most dietitians want strongly the establishment of independent office for dietary consultation, but its establishment was hindered by the poor system in the hospital administration and lack of preparation in the dietetics.

  • The development of a general hospital has a close connection with the trend of the society in general and it flows to the ways that satisfies the demanded value of the time. Therefore, various internal changes have occurred since a hospital has built. The changes is appearing through the time not only in the quantitative changes, but also in functional changes and correlations between each department. This study focused on these changes, and observed the St. Mary's Hospital - one of the major hospitals - which is constructed in 1980's. And then, it was divided into the D/T, O.P.D and ward as function that focused on the time of hospital's establishment (1980), the present (2006), and new hospital (2009) to observe the changing aspect of the function and department. As a result, according as age passes, this study shows function of each department is mixed. Among the D/T, O.P.D and ward, mixing functions of O.P.D is most notable. The mixing function brings changes in hospital's arrangement type. In the past, verticality style hospital was evaded, but recently verticality style hospital is appearing. Through this study, it is expected to forecast the new aspects of hospital and apply it to the new project.
