This research was performed to investigate the characteristics and determination factors on tertiary hospital inpatients. The used data was the four waves of Korea Health Panel(2008, 2009, 2010, 2011), and the number of subjects was 4,430 cases of tertiary and general hospital admission. The statistical methodology used in the study is the logistic regression model. The significant affecting factors in utilizing tertiary hospital admission were gender, marital status, education, household income, residence region and ICD-10 classification. Man, graduating college/university, married, high-income were socio-economic affecting factors in tertiary hospital admission. Medical need factor of ICD-10 classification and residence region of inpatients was also significant affecting factors in tertiary hospital admission. The 81.4% of inpatients at tertiary hospital had chronic disease and the 12.9% of inpatients readmitted, the 68.2% had a selecting doctor and the only 26.7% of inpatients reinforced by private medical insurance. This study recommended the Korean government to provide proper rule for tertiary hospital admission in order to improve the equity and efficiency of health care system.
Purpose: The purpose of this study was to determine factors that affect satisfaction of the hospital services among injured workers. Methods: The subjects of this study were 532 injured workers. The questionnaires were selected as literature suggested for explaining satisfaction of the hospital services, general characteristics of injured workers, characteristics of the hospital services. The data were analyzed with descriptive statistics, t-test, ANOVA, multiple regression analysis by SPSS 19.0. Results: There were significant differences in satisfaction scores, depending on the types of disease, health status and possibility to return to work among the injured workers. Also, there were significant differences in satisfaction scores, in terms of medical facilities, food service, specialization of doctors and nurses, cleanliness of hospital, convenience of hospital teaching and counselling of doctors and nurses, supporting workers' compensation process, counselling of workers' compensation and unfair treatment. In the results of the standard multiple regression analysis, food service, medical facilities, specialization of doctors and nurses, health status and hospital cleanliness were significant factors for satisfaction scores of the hospital services. Conclusion: It is necessary to promote food service, medical facilities, specialization of doctors and nurses, hospital cleanliness, health status of injured workers for developing hospital services.
It is very important to estimate the patients satisfaction level with medical services, to classify the objectvies according to the patients characteristics and sub-satisfaction factors. The purpose of this study is to determine the factors affecting satisfaction in oriental hospital. The 549 patients' hospitalized in five oriental hospital in Taegu city and one oriental hospital in Kyungbuk province were selected for this study. The results summarized are as follows. l. The general characteristics of 549 objectvies were included gender, age, education, occupation, income level, length of stay, health status of hospitalized, and expectation for medical care. 2. Patients characteristics affecting patients total satisfaction, as for age(b=0.05), health status of patients(b=-0.052), and expectation for medical care(b=0.117) were significant, while gender, education, job, income level, and length of stay were not. As the factors according to patients satisfaction, accessibility(b=0.09l), doctor's kindness(b=0.357), staff kindness(b=0.137), nurse's skills(b=0.111), hospital facilities(b=0.211), and medical fee(b=-0.160) were significant. In total patients' satisfaction, Doctor's kindness was the most significant of prediction variables. In general the factors affecting In-patient satisfaction of oriental hospital was highly associated with doctor's kindness.
As healthcare environment being more complex and turbulent, strategic approach of hospitals became more important. This study was to investigate strategic orientation of Korean hospitals and their related characteristics. We surveyed managers of 360 hospitals randomly selected from all hospitals in Korea. For typology of strategic orientation, we used that developed by Miles and Snow, and results are as follows. Firstly, major types of organizational strategic orientation of Korean hospitals were analyzer(42.0%) and prospector(34.0%). Secondly, characteristics affecting to hospitals' strategic orientation significantly were hospital ownership and sex of managers. Medical corporation had a high tendency of prospector strategic orientation by 2.7 times compared to personal ownership. Female managers had a low tendency of prospector strategic orientation by 0.2, which was statistically significant. Though 60+ age group (compared to twenties and thirties) and middle managers (compared to CEO) had a tendency of being more prospector strategic orientation, but insignificant. The study result that majority of strategic orientation were prospector and analyzer reflects Korean hospital environment are complex and unstable. Hospital managers need to more focus on environment and boundary spanning function for maintenance and survival of their organizations.
This study is to analyze the reason of Breakdown of collective bargaining caused by the difference of main issue about bargaining and other factors influencing collective bargaining from the viewpoint of both labor and capital in Korean medical system. Korean medical system is facing the huge change of medical policy, so it should pursue institutional change following the change of medical system, On the contrary, the activity of labor union in medical system is more organized, activated, so understanding and cooperating on the related policy between labor and management are necessary. As the method of administration participation is collective bargaining, main issues between labor and management may lead Brwakdown of bargaining, so the author will analyze it from three viewpoints. First, the difference of recognition between labor and management due to the characteristics of hospital Secondly, the difference of recognition between labor unions due to the characteristics of hospital Thirdly, the difference of recognition between the characteristics of hospital and labor union.
Purpose: The purpose of this study was to explore and describe the factors related to new nurses' organizational socialization (OS) process and to find out the strategic information for successful organizational socialization. Methods: The qualitative data were gathered from three focus group discussions with 13 new nurses and 5 experienced nurses. All interviews were recorded and transcribed. The transcript analyzed according to qualitative content analysis processes. Results: The factors related to new nurses' OS derived from new nurses' focus groups were 'burdens related to the characteristics of nursing duties', 'emotional and psychological characteristics' and 'facilitating factors to OS'. Two factors at the forepart were interfering factors to new nurses' OS. The factors related to new nurses' OS derived from experienced nurses' focus group classified to 'facilitating factors to OS', 'interfering factors to OS' and 'supports to facilitate new nurses' OS process'. Conclusion: The findings of the study revealed the facilitating and interfering factors to organizational socialization of new nurses. The main factors to interfere new nurses' OS were individual factors such as professional incompetency and psychological characteristics. On the other hand, Interactional factors within the organization were important to facilitate new nurses' OS. The findings can be applied for developing a special program to help new nurses adjust to the nursing organization successfully.
Background: This study aimed to identify the characteristics of the referral and return of patients to clinics in the endocrinology and cardiology departments at the National Health Insurance Service Ilsan Hospital to evaluate the "referral and return of patients to clinics" program and reduce the rate of returning patients. Methods: From May 2018 to December 2020, we identified the number of visits to referral hospitals and hospital usage status at Ilsan Hospital after returning to clinics. We also identified the patients who returned to Ilsan Hospital within 6 months, defined as "failure to transport," among those recommended to be transported to clinics of the Medical Cooperation Center. Additionally, we evaluated the characteristics of the "failure to transport" patients. Results: Among the returning patients, the rate of visiting Ilsan Hospital within 6 months was higher in cardiology than in endocrinology (25.1% vs. 16.7%). Older age, more severe disease, and more number of visits to the department were associated with a high rate of failure to transport. The rate of failure to return was low in cases diagnosed with hyperlipidemia/lipoprotein metabolism disorder. With respect to diabetes, the rate of failure to transport differed according to each type of diagnosis of diabetes. Conclusion: The success rate of the "referral and return of patient to clinics" program differed based on each patient's characteristics, department of visit, and diagnosis. Individualizing according to the visit department and diagnosis is required to ensure successful transfers, and infrastructure expansion and institutional arrangements must be facilitated.

Purpose: In this study, we aimed to compare the characteristics of patients with trauma by age group in a single center in Korea to identify the clinical characteristics and analyze the risk factors affecting mortality. Methods: Patients aged ≥18 years who visited the Chungbuk National University Hospital Regional Trauma Center between January 2016 and December 2022 were included. The accident mechanism, severity of the injury, and outcomes were compared by classifying the patients into group A (18-64 years), group B (65-79 years), and group C (≥80 years). In addition, logistic regression analysis was performed to identify factors affecting death. Results: The most common injury mechanism was traffic accidents in group A (40.9%) and slipping in group B (37.0%) and group C (56.2%). Although group A had the highest intensive care unit admission rate (38.0%), group C had the highest mortality rate (9.5%). In the regression analysis, 3 to 8 points on the Glasgow Coma Scale had the highest odds ratio for mortality, and red blood cell transfusion within 24 hours, intensive care unit admission, age, and Injury Severity Score were the predictors of death. Conclusions: For patients with trauma, the mechanism, injured body region, and severity of injury differed among the age groups. The high mortality rate of elderly patients suggests the need for different treatment approaches for trauma patients according to age. Identifying factors affecting clinical patterns and mortality according to age groups can help improve the prognosis of trauma patients in the future.
The purpose of this study was to identify the effect of job characteristics, job performance and organizational commitment on job satisfaction of hospital dietitians. The first survey was carried out on 74 hospital dietitians to determine their demographic characteristics, job characteristics, organizational commitment, and job satisfaction. The second survey was carried out on 47 hospital foodservice dietitians to determine their job performance. Thirty-two percent of the subjects were aged between 26 and 30, and 62.2% had Bachelor's degrees. The dietitians felt most satisfied by their co-workers, followed by the work-itself, supervision, pay, and promotion possibilities. Dietitians who had a higher education were satisfied with pay and supervision. The longer the length of employment, the more the satisfaction with the work-itself and pay, plus the higher the monthly wage the more the satisfaction with the work-itself, pay, supervision, and co-workers. lob satisfaction was high in the group that scored high on autonomy, feedback, task identity, and friendship of in the job characteristic inventory. The job performances of nutrition counseling(r= 0.469, p < 0.01), nutrition education(r= 0.446, p < 0.01) and management of therapeutic diet(r= 0.394, p < 0.01) were positively correlated with job satisfaction. The $R^2$for the multiple regression model was 0.677, indicating that 67.7% of the variance in job satisfaction could be accounted for by feedback, organizational commitment, nutrition counseling performance, and nutrition education performance. In conclusion, the level of job satisfaction for hospital dietitians would appear to improve with increased feedback, organizational commitment, and opportunity for nutrition counseling and nutrition education.
Objectives : Gender differences in demographic characteristics, symptom presentation, and severity may influence the selection of target behaviors as well as the course and outcome of therapeutic interventions for patients with social phobia. The purpose of this study was to explore the gender differences in the symptomatic characteristics of Korean patients with social phobia. Methods : A total of 256 outpatients with social phobia at Kangbuk Samsung Hospital were included in this study. All subjects were evaluated using the Korean version of the MINI International Neuropsychiatric Interview Plus. Potential differences in demographic characteristics, severity of anxiety, and situational fear and avoidance were examined. Self-report questionnaires and interviewer-administered instruments, including the Social Phobia Scale (SPS), Social Phobia subscale of the Fear Questionnaire (FQ-social), Appraisal of Social Concerns (ASC), and Liebowitz Social Anxiety Scale (LSAS), were used to evaluate the severity of the patients' symptoms. Results : There were no gender differences in demographic characteristics, age, age at onset of social phobia, total SPS score, FQ-Social score, ASC score, and LSAS score. In addition, there was no gender difference in the severity of fear in specific social situations. However, women reported significantly higher total scores on the social helplessness category of the ASC than men (p=0.009). Conclusion : Previous studies reported that the severity of social phobia symptoms is greater in women than in men, but our results suggest that there are no significant gender differences in the severity of social phobia symptoms. Thus, gender differences among patients with social phobia are discussed in the context of traditional sex-role expectations.
