알코올과 식이엽산수준이 혈장 Homocysteine, 간기능, 간 조직검사에 미치는 영향 (Effects of Alcohol Administration and Dietary Folate on Plasma Homocysteine and Liver Histopathology)

  • The critical role of folate vitamin in the remethylation pathway for methionine synthesis from homocysteine has been well documented. Hyperhomocysteinemia resulting from inadequate folate nutrition has been implicated in increased incidence of macrovascular diseases, colorectal cancer, neural tube defects, etc. Chronic exposure to ethanol impairs folate nutrition and one-carbon metabolism in the liver, which often results in fatty liver due to a defective remethylation process. This study was carried out to investigate the chronic effects of moderate levels of alcohol and dietary 131ate on plasma homocysteine levels, and on histopathology and biochemical functions of the liver Rats were raised on experimental diets with three levels of folate(0, 2, 8mg/kg diet), and 50% ethanol(1.8m1/kg body weight) was administered intragastrically by intubation tubes three times a week for 10 weeks. Plasma homocysteine concentrations were found to be significantly influenced by dietary folate intake and alcohol administration. Among all treatment groups, Plasma homocysteine levels were highest in the animals receiving a combined treatment of folate deficient diet and alcohol administration. Plasma homocysteine concentration was negatively correlated with folate concentration in the plasma(p<0.01) and liver(p<0.05). Among alcohol treated rats, increase in plasma homocysteine values due to ethanol was prevented by 131ate supplementation. When liver histological tests were performed, macrovascular and microvascular fatty changes and spotted necrosis were observed more frequently in folate-deficient animals diet than those on folate-adequate and folate-supplemented diets in alcohol-treated rats. These results indicate that folate supplementation above the recommended level might be beneficial in the prevention of alcohol-related hyperhomocystei-nemia and abnormal histologic changes in the liver due.   • Elevation of homocysteine levels is a risk factor for cardiovascular diseases and liver diseases. It has been reported that both streptozotocin-induced type I diabetic rats and obese type II diabetic rats have plasma total homocysteine lower than each control rats. We determined the effects of lean type II diabetes on homocysteine levels using type 2 diabetic Goto-Kakizaki rats. The concentrations of serum glucose were increased to ${\sim}two-fold$ of control levels and the total cholesterol levels were also increased in GK rats. Hepatic aspartate, histidine, threonine, alanine and methionine levels were significantly increased in GK rats. Plasma aspartate and glutamate levels were elevated, but threonine and arginine levels were decreased in GK rats. Plasma total homocysteine levels were not changed in GK rats, but hepatic total homocysteine levels were increased to ${\sim}three-fold$ of control levels. These results suggest that hepatic metabolism of sulfur-amino acid may be altered in diabetic condition.

Homocysteine and Total Antioxidant Status in Acute Myocardial Infarction Patients Among Tamil Population

  • Oxidative stress is prerequisite for the development of atherosclerosis. Apart from the traditional risk factors that contribute to this devastating condition, in the past few decades, much attention has been focused on plasma total homocysteine mainly because of its strong association with coronary artery disease. It has been suggested that homocysteine induces oxidative stress and hence the present work was undertaken to assess the total homocysteine status and plasma total antioxidant capacity in the acute myocardial infarction (AMI) patients among Tamil population. The study subjects included only the Tamil population. Blood samples were collected from 100 AMI patients and 100 controls. Plasma was separated and the total antioxidant status was assessed as a measure of ferric reducing power of antioxidants using spectrophotometric method. Plasma total homocysteine concentrations were assessed by automated chemiluminescence method. While Total antioxidant status was significantly decreased, the plasma homocysteine concentrations were elevated in AMI patients compared to the controls. However, there was no correlation between the homocysteine levels and total antioxidant status. The findings of this study may have therapeutic implications, including food sources rich in antioxidants for all AMI patients to minimize the effect of free radicals formed during oxidative stress among Tamil population.

  • Hyperhomocysteinemia, resulted from an interaction between the mutation of MTHFR gene and B vitamin deficiency, is suggested as a possible cause for complications and adverse outcomes of pregnancy. The purpose of the present study was to investigate the relationship between the intakes of B vitamins and serum homocysteine concentrations with the C677T mutation in the MTHFR genotypes in 135 normal pregnant women of 24-28 weeks of gestation. Dietary intake of B vitamins did not differ among the three genotypes, but the negative correlation between dietary folate intake and the serum homocysteine level was the strongest in the T/T type (r = -0.249) than in other genotypes (C/T: r= -0.040, C/C:r= 0.126, p<0.05). Among the subject with the T/T type, the pregnant women who consumed folate less than 50% of the RDA had higher serum homocysteine levels than those who consumed folate greater than 125% of the RDA (10.4$\pm$5.9 vs 7.0$\pm$1.5 $\mu$mol/L, p<0.05). Serum homocysteine levels were higher in the women with micronutrient supplements than those with no supplements in the T/T type, but such relation was not present in the C/C or the C/T type. In conclusion, serum homocysteine concentrations were influenced by the interrelationship between the MTHFR polymorphisms and dietary folate intake or micronutrient supplementation.

혈장 호모시스테인 농도가 상완-발목 맥파 속도에 미치는 영향 (The Effects of Plasma Homocysteine Concentration on Upper Arm-Ankle Pulse Wave Velocity)

  • 본 연구는 2016년 4월 1일부터 2016년 10월 31일까지 종합병원 건강검진센터에 내원한 435명을 대상으로 혈장 호모시스테인 농도가 정상 호모시스테인 군과 무증상 고호모시스테인 군 간에 발목-상완 pulse wave velocity에 유의한 차이가 있는지를 비교해 심혈관 질환 발생의 향후 예측인자로서 호모시스테인 검사와 동맥의 경직도를 간접적으로 반영하는 상완-발목 pulse wave velocity의 직접적인 상호 연관성을 비교해보고자 시도되었다. 연구결과 고 호모시스테인혈증 군에서 정상 호모시스테인 군에 비해 연령, 허리둘레, BUN, 혈장 크레아티닌이 유의하게 높았고, HDL은 유의하게 낮았다. 또한, 고 호모시스테인혈증은 흡연 및 음주 여부와 관련이 있었으며, 남성에서 유의하게 많았다. 관련 변수를 보정하기 전에는 고 호모시스테인 군에서 우측 및 좌측 상완-발목 맥파 속도가 유의하게 높았으나 관련 변수를 보정한 후에는 두 군의 상완-발목 맥파 속도는 유의한 차이를 보이지 않았다. 따라서, 향후 국내 수검자를 대상으로 호모시스테인 농도를 낮추는 중재적 치료 후 심혈관질환 예방에 대한 독립적 연관성 및 호모시스테인과 신장 기능의 관련성에 대한 추가 연구가 필요하겠다.

정신분열병 환자에서의 혈중 Homocysteine 농도와 MTHFR C677T, A1298C의 유전자 다형성 (Serum Homocysteine Level and MTHFR C677T, A1298C Gene Polymorphism in Korean Schizophrenics)

  • Objectives:Recently in schizophrenia high incidence of MTHFR(methylenetetrahydrofolate reductase), which is a main relating enzyme that reduce homocysteine level, genetic variations were reported. So we examined serum homocysteine level and MTHFR gene polymorphism in Korean schizophrenics. Method:We compared serum homocysteine level and MTHFR polymorphism between 235 schizophrenics (100male, 135female) and 235 normal controls(100male, 135female). C677T and A1298C polymorphism of MTHFR gene were analyzed. Results:1) C677T genetic mutation(CT and TT) were more frequent in schizophrenia group than normal control group(p<0.01). But the difference of A1298C mutation frequency was not found between two groups. 2) In schizophrenia patients, TT genotype of C677T mutation showed significantly higher homocysteine level (29.99uM/L) than other group(CT:13.34uM/L, CC:9.34uM/L p<0.01). 3) MTHFR 677 TT homogeneous mutation genotype showed two times more risk(odds ratio=2.15) than 677CC normal genotype in schizophrenia. Conclusion:Some schizophrenia patients with high homocysteine serum level may have C677T TT genotype. In that case, folate ingestion could be a good management for clinical improvement.

Plasma total homocysteine and macrovascular complications are associated with food and nutrient intake in patients with Type II diabetes mellitus

  • The present study was conducted to document the association between plasma homocysteine levels and the presence of macrovascular angiopathy with food and nutrient intake patterns among patients with Type II diabetes mellitus in Korea. Plasma total homocysteine concentration was analyzed by HPLC-fluorescence detector method in 127 patients with non-insulin dependent diabetes mellitus. Logistic regression analyses were performed respectively to study the association of plasma homocysteine levels with clinical and dietary characteristics and macroangiopathy (MA). The average plasma homocysteine level of patients with MA was $14.2{\mu}mol/l$, which was significantly higher than that of patients without MA ($11.4{\mu}mol/l$). The proportions of patients with MA showed a significant difference, being 32.3% in hyperhomocysteinemic ($>14.0{\mu}mol/l$) patients and 13.5% in others with homocysteine levels lower than $14.0{\mu}mol/l$. Odds ratios for macroangiopathy by tertile increase of plasma homocysteine concentration were 1.633 ($Q_2$) and 4.831 ($Q_3$), when adjusted for age, sex, and cigarette smoking. Patients with MA consumed reduced amounts of vitamin $B_1,\;B_2$, and folate. The results indicate that the plasma homocysteine levels are significantly increased in NIDDM patients who have macroangiopathy. Dietary management such as increased fruits and vegetables and decreased potatoes and starches might be beneficial for the prevention of macroangiopathy in diabetic patients.

  • This study was performed to investigate effects of dietary folic acid supplementation on plasma homocysteine levels, thiobarbituric acid reactive substance s (TBARS) level s and liver SAM/SAH ratio in hyperhomocysteinaemia-induced pregnant rats. Forty-two female Sprague-Dawley rats were divided three groups (C: control diet, HFD: $0.3\%$ homocystine and 0 mg folic acid diet, HFS: $0.3\%$ homocystine and 8 mg/kg folic acid diet) according to homocystine and folic acid levels in the diet. They were fed experimental diets for 5 weeks prior to the mating and also during the entire period of pregnancy till gestational day 20. Dietary folic acid supplementation caused a significant decrease in plasma homocysteine levels which had been increased by a homocystine-diet, with a concomitant increase in plasma and liver folate levels. Liver TBARS levels in homocysteine-folic acid-deficient group (HFD) were higher than those in control group. Dietary folic acid supplementation increased hepatic SAM/SAM ratio in homocysteine-folic acid- sopplemetantion group (HFS) when compared to the HFD (p < 0.05). These data suggest that folate depletion and elevated plasma homocysteine may promote oxidative stress in rat livers and influence the remethylation cycle of the homocysteine metabolism detrimentally. In conclusion, dietary folic acid supplementation was found to be effective for lowering plasma homocysteine levels, relieving oxidative stress, and improving the methylation status in the body.

Poor nutrition and alcohol consumption are related to high serum homocysteine level at post-stroke

  • BACKGROUND/OBJECTIVES: Increased serum homocysteine (Hcy) levels have been reported to be related to the occurrence of cardio- and cerebrovascular diseases. High serum Hcy levels are also related to the development of secondary stroke and all-cause mortality. The purpose of this study was to investigate the prevalence of high serum homocysteine level and relating factors, and the change over the 10 month period post-stroke. SUBJECTS/METHODS: Consecutive stroke patients who were admitted to the Asan Medical Center were enrolled. Ten months after the onset of stroke, an interview with a structured questionnaire was performed and blood samples were obtained for the biochemical parameters. Nutritional status was determined using the mini nutritional assessment (MNA) score and dietary nutrient intakes were also obtained using a 24 hour recall method. RESULTS: Out of 203 patients, 84% were malnourished or at risk of malnutrition, and 26% had high homocysteine levels at 10 months post-stroke. Using logistic regression, the factors related with high homocysteine levels at 10 months post-stroke included heavy alcohol consumption (P = 0.020), low MNA scores (P = 0.026), low serum vitamin $B_{12}$ (P = 0.021) and low serum folate levels (P = 0.003). Of the 156 patients who had normal homocysteine levels at admission, 36 patients developed hyperhomocysteinemia 10 months post-stroke, which was related to heavy alcohol consumption (P = 0.013). Persistent hyperhomocysteinemia, observed in 22 patients (11%), was related to male sex (P = 0.031), old age (P = 0.042), low vitamin $B_6$ intake (P = 0.029), and heavy alcohol consumption (P = 0.013). CONCLUSION: Hyperhomocysteinemia is common in post-stroke, and is related to malnutrition, heavy alcohol drinking and low serum level of folate and vitamin $B_{12}$. Strategies to prevent or manage high homocysteine levels should consider these factors.

The Levels of Vitamin D, Vitamin D Receptor, Homocysteine and Complex B Vitamin in Children with Autism Spectrum Disorders

  • Objective: Autism spectrum disorder (ASD) is a complex neurodevelopmental syndrome with an increasingly prevalent etiology, yet not fully understood. It has been thought that vitamin D, complex B vitamin levels and homocysteine are associated with environmental factors and are important in ASD. The aim of this study was to examine serum vitamin D, vitamin D receptor (VDR), homocysteine, vitamin B6, vitamin B12 and folate levels in ASD. Methods: In this study, serum vitamin D and VDR, homocysteine, vitamins B6, B12 and folate levels were determined in 60 patients with ASD (aged 3 to 12 years) and in 45 age-gender matched healthy controls. In addition, calcium, phosphorus and alkaline phosphatase, which are associated with vitamin D metabolism, were measured from serum in both groups. ASD severity was evaluted by the Childhood Autism Rating Scale (CARS). Results: Serum vitamin D and VDR were substantially reduced in patients with ASD in comparision to control group. However, homocysteine level was significantly higher and vitamin B6, vitamin B12 and folate were also reduced in patients with ASD. Total CARS score showed a positive association with homocysteine and a negative correlation with vitamins D,B6, B12, folate and VDR. Conclusion: This comprehensive study, which examines many parameters has shown that low serum levels of vitamins D, B6, B12, folate and VDR as well as high homocysteine are important in the etiopathogenesis of ASD. However, further studies are required to define the precise mechanism(s) of these parameters and their contributions to the etiology and treatment of ASD.
