This study aimed to investigate predictors of turnover intention of home visiting nurses at 16 public health centers in Busan. There are two groups of independent factors: non-work related(i. e., age, educational level, working duration as nurses in hospitals, and certificate), and work related factors(i. e., working duration as nurses in public health centers, working duration as home visiting nurse, position, number of households visited per week, workload, cooperation among staff, support by supervisors, supply of vehicles, and supply of materials). Data were collected with self-administrated questionnaires which consisted of 4 items of non-work related factors, 9 items of work related factors, and 1 item of turnover intention to other department. Data were analyzed using an univariate logistic regression and multiple logistic regression. According to the results, heavy workload (Yes vs No, OR=4.31, $95\%$ CI=1.16-16.04) was the most powerful predictor on the turnover intention. In conclusion. this result was similar to those of other studies on the predictors of turnover intention of clinical nurses at hospitals. To decrease the turnover intention among home visiting nurses at public health centers in Busan, alleviating the workload is needed.
Purpose: This study was conducted in order to present the effects of case management of a home visiting health service for clients with hypertension. Method: One-group pretest-posttest design was used. The subjects were 280 clients who received case management among the first and second registered group. The data were eight-week-case management results from January to December, 2010. In order to evaluate the effect of the visiting nursing service, biological indexes(blood pressure, total cholesterol, triglyceride, high density lipoprotein, low density lipoprotein, BMI) and self-care abilities(knowledge/attitude of hypertension, management of medication, nutritional care, confidence of self care) were measured. Result: Biological indexes and self-care abilities were improved, except total cholesterol, low density lipoprotein. Conclusion: The case management of home visiting health service by public health centers is considered to be an effective nursing service. Therefore, greater effort is needed for better maintenance of case management, and more research is needed in order to examine a variety of biological indexes.
Background: This article reviewed the current status of home-based physical therapy infrastructure in long term care insurance and then solved the problem. Method: We used two forms of data that were acquired from ⅰ) the Ministry of Health & Welfare and the Family and National Health Insurance Corporation, ⅱ) a home-visiting health care program, and ⅲ) evaluation data from the Korea Health Industry Development Institute. The home-based physical therapy program was then analyzed. Results: The role and concept of home-based physical therapy was not clearly established. There were few home-based physical therapy programs in the community. The manpower of home-based physical therapists in the home-visiting health care program was very low. The role between home-visiting nurses (caregivers) and home-based physical therapists was mixed. Research and promotion regarding home-based physical therapy was poor. Conclusion: To establish a system of legal, long-term care insurance, we must increase the manpower of home-based physical therapists and the amount of research pertaining to the demand for home-based physical therapy.
Purpose: This study aims to reveal violence victimization of visiting nurses and investigate the coping and prevention strategies adopted by public institutions. Method: The data were collected over three weeks in 2017 using self-report questionnaires from 237 visiting nurses. Data were collected between August 23 and September 15, 2017. Result: Among the respondents, 74.7% had experienced some types of work-related violence during the past year. Verbal abuse (65.4%) was most common, followed by physical threat (46.8%), sexual abuse (43.9%), and physical violence (5.5%). The nurses perceived the most common cause of violation as socio-economic frustration of the offenders vented out on the nurses. Besides, most reported offenders had mental health problems. The two most effective measures to prevent violence faced by the nurses are terminating visiting services for the offenders and implementing the buddy system, which has rarely been used in practice. Conclusion: These findings show that visiting nurses are exposed to a risk of serious abuse in everyday work life, and the nurses themselves or institutions have failed to handle the violence, showing shaky responses at best. Therefore, it is necessary to establish internal regulations and systems at the institutional level that can help prevent violence against visiting nurses.
Purpose: This study aimed to identify the concrete educational needs of visiting nurses working in a community health setting in Korea. Methods: We conducted four focus group interviews from October 7 to October 18, 2021. Twenty-five visiting nurses who worked in public health centers were recruited through purposive sampling. A qualitative content analysis was used to analyze the interview data. Results: The demands of educational contents for visiting nurse safety management practical training were: (1) coping with physical and verbal violence, (2) coping with sexual violence, (3) infection control for infectious diseases with a high prevalence in the community, and (4) preventing and coping with animal bites during home visits. In addition, visiting nurses suggested training programs that comprised: (1) case-based learning, (2) short video clips, and (3) recurrent integrated education. Conclusion: Safety management training programs for visiting nurses should be implemented to the extent that they add no burden on their workload and are easily accessible at any time. In addition, training programs should be based on actual cases and be focused on contents that can be applied in home visit situations. A practical safety management training program should be developed based on the educational needs of visiting nurses, as identified through this study.

However, there was not a statistical difference in accordance with age and work experience (p>.05). Japan has been prepared for aging society from 1970. In 1970, the percentage of distribution of population of 65 years old and over was 7.1%. It is similar to present percentile of the elderly in Korea. Therefore, it will be needed to study about home care and home visiting nursing in Japan at present. This study was aimed to prepare the fundamental documents for home care nursing in Korea and to know the background of new health care system of Long-term Care Insurance in Japan, by studying home care and home visiting nursing in Japan. With the continuing aging of the population, especially the increase in the number of latter stage elderly, it is predicted that there will be an increase in the number of the elderly who are bedridden and suffering senile dementia. To ensure that these people will be able to continue living in the communities and homes they are accustomed to, surrounded by their families and neighbors, Japan substantially improve and expand in-home services. There were also long-term effort to reach the level of services outlined in the Gold Plan and the New Gold Plan within the decade between FY 1991 and FY 1999 in the field of health care and welfare. Under this plan, the most noticeable change was occurred in home care, home was permitted as the field of care and visiting nursing was established in law. Through this 10- Year Strategy for Promotion of Health and Welfare Services for the Aged, many problems have been improved and solved, but some problems remained such as inadequate service supply and consumption of medical insurance for the elderly. Japan will be a society composed 25% of elderly people of total population in 2020, and it will be soon faced with a shortage of welfare and medical facilities and manpower. As for equalizing the benefits and cost burdens, and other future arrangements for health care and welfare, Long-term Care insurance system was established in 1994. This system will be enforced from April 2000 and use present facilities and services. To know home care and home visiting nursing in Japan, we need to consider present conditions well and to take notice of changes and measures to cope with an aging society continuously.
