Purpose: To analyze the effects of customized home visiting health services on the health and health behaviors of clients with hypertension (n=107) and diabetes mellitus (DM: n=67). Methods: A one group pre and post-test research design was used. The subjects were registered in a customized 8-week, interventional, home visiting health services available in Daegu. Data was collected from November 17, 2008 to January 23, 2009. Analyses involved descriptive statistics, $x^2$ test and paired t-test. Result: Hypertension control rate was improved 25.2% and DM control rate was improved 3.0%. There were significantly beneficial hypertension-related differences in BP, health belief, health knowledge and health behaviors including performance of 10 min of moderate exercise, diet, BP monitoring and medication. Significantly beneficial DM-related changes included glucose, health belief, health knowledge and health behaviors including performance of 10 min of moderate exercise and glucose monitoring. But there were no significant hypertension-related differences in health belief (barrier) and health behavior including drinking and exercise length/frequency. Also, no significant DM-related differences were evident in health belief (barrier) and health behaviors including drinking, smoking, exercise length/frequency, diet and medication. Purpose: Competency is a key foundation of the nursing education curriculum and a baseline element of nursing practice. The purpose of this study was to explore the competencies of community health nurses in a maternal early childhood sustained home-visiting program based on nurses' field experiences. Methods: The participants were 21 nurses who had more than 1 year of experience in this program. Reflective interviews were performed; lasting 1 hour per nurse, on August 18, 2016, and the collected data were analyzed using qualitative directed content analysis. Results: Twenty-four themes were extracted for 9 competencies. The major themes included implementing a long-term relationship-based approach, providing client-led service rather than expert-led service, helping mothers with psychosocial difficulties, and applying skills in a practical manner in the home environment. Conclusion: In order to develop a home-visiting program as part of maternal-early childhood nursing practice, nurses should develop competencies that help them make significant and positive interventions. Results indicate that the curriculum for community health nurses should be changed to improve competencies in building relationships with clients and to focus on the application of skills in specific cases and in clinical practice.

Purpose: This study aims to provide baseline data on children's health care and household safety management in one region and to support implementation of customized visiting health care services for children. Methods: 51 children and their caregivers from socially vulnerable group and 69 children and their caregivers from general group are selected as research participants. Data were collected through home visiting survey by 10 professional nurses. Results: Children from socially vulnerable group are at higher risks of poor health care and safety accidents in households than general group. Rates of taking developmental screening tests, medical check-up remain considerably low in vulnerable social group. Regarding home safety, vulnerable children are more likely to be exposed to unsafe conditions. Conclusion: These results indicate that in order to promote health care and safety conditions for vulnerable children, it is necessary to implement customized visiting health care programs which actually can reach vulnerable households and meet their demands. These findings can be used as baseline data to develop customized visiting health care programs for children.
Purpose: The purpose of this study was to estimate the demand and supply of visiting nursing services provided by health centers in urban area, aiming at strengthening infrastructure, which may improved the quality of life and health status of vulnerable population in the community. Methods: This study was conducted through nominal group discussion, focus group study. The demand and supply of visiting nursing were estimated by health economists based on the secondary analysis data from 25 health centers in Seoul. Result: Primary targets for the visiting nursing must be people who are homebound in the community. They can be classified into: a group of Level I: chronic patients who need visiting nursing care at least once a week: and a group of Level II: vulnerable families that need management periodically e. g. twice a month. Based on the estimation of demand for visiting nursing services in the community, the estimated supply required was $651{\sim}770$ visiting nurses including home health nurses in visiting nursing programs based on health centers in Seoul. Conclusions: The estimated demand and supply of visiting nursing are expected to provide basic data for establishing alternative policies on visiting nursing infrastructure that might be accomplished through demand-based visiting nursing programs by districts.

This is a descriptive research study that aims to identify the effects of professionalism, job satisfaction, and resilience on the retention intention of home visiting nurses from the long-term care insurance system of Korea. Data were collected from self-administered surveys completed by 141 home visiting nurses of registered visiting-nursing institutions in the long-term care insurance database (April 1-May 31, 2019). Analyses were independent t-tests, one-way analysis of variance (ANOVA), the Scheffé test, Pearson's correlation analysis using SPSS/WIN 22.0, and multiple hierarchical regression analyses. Results showed that the mean professionalism score was 3.51/5, 3.27/5 for job satisfaction, 3.79/5 for resilience, and 4.04/5 for retention intention. A positive correlation existed between retention intention and professionalism (r = .272), job satisfaction (r = .201), and resilience (r = .530). The final regression model showed that resilience (β = .455, p < .001) and job satisfaction (β = .175, p = .016) significantly affect retention intention. The variables' model explanation power was 32.0% (F = 11.968, p < .001). The results show the need for strategies to improve resilience, followed by strategies to improve job satisfaction to increase the retention intentions of these home visiting nurses.

Purpose: The aim of this study was to evaluate the efficiency of long-term care visiting nursing centers in communities using data envelopment analysis (DEA). Methods: Data were collected using a self-reported questionnaire. The average number of staff per 6 months and total space of center were used as input variables. The average number of clients per 6 months and the average profits per 6 months were used as output variables. EMS Window version 3.1 was used to measure the efficiency scores. Descriptive statistics and tobit regression were applied to analyze the general characteristics of the variables and the factors affecting efficiency scores. Results: The average efficiency of 30 long-term care visiting nursing centers in communities was approximately 66.9% on technical efficiency analysis, and 79.1% on scale efficiency analysis. Eight nursing centers on technical efficiency analysis and 12 centers on scale efficiency analysis had 100.0% efficiency. Conclusion: Our findings reveal that long-term care visiting nursing centers in communities have low operational efficiency. Therefore, it is essential to institute policies and regulations to improve the efficiency of visiting nursing centers and to strengthen the business competencies of center officers.
