가정전문간호사의 환자·가족교육 수행정도와 저해요인 (Performance and Impediments to Patient and Family Education among Home Health Care Nurses)

  • Purpose: This study aimed to provide basic data necessary to develop education programs and educational services for home care by investigating the degree of patient and family education among home health care nurses. Methods: Data collection was carried out with 145 people from 47 institutions that agreed to participate in the research. A total of 128 questionnaires were received, of which 122 were analyzed. Data were analyzed using SPSS 12.0. Results: The item on which education was most frequently delivered was intravenous injection speed control (66.4%), whereas the item requiring the longest teaching period was pressure ulcer care. The average degree of impediment perceived by home care nurses was 2.82 out of 5. Conclusion: Medical institutions should develop educational materials and programs that reflect the characteristics and degree of home health care needed. Repeated research, including that by medical institutions, on the degree and impeding factors related to patient and family education performance of home health care nurses should be conducted. Moreover, medical institutions should investigate the nursing and educational needs of patients and families who received home health care service.

가정간호대상자의 간호기록 분석을 통한 가정간호중재 목록구축 (Development of a Home Health Care Nursing Intervention List through Analysis of Home Health Care Nursing Records)

  • The purpose of this study was to identify nursing diagnoses and nursing interventions that are found in the home health care patients, and to establish a basis for a standardized Nursing Intervention List that would help nurses doing home health care nursing. For this study, the records of 150 home health care clients who were discharged, from the Home Health Care Center at Yonsei Medical Center, between January to July. 2001 were analyzed. Of the 43 nursing diagnoses recorded for these clients are 43, the most frequent diagnoses were in the area of Exchanging. There were 2.814 nursing interventions which is a mean of 4.73 nursing interventions Per diagnosis. We confirmed that most of the interventions were related to 'education' and 'advice'. We present a Home Health Care Nursing Intervention List that was developed based on the results of this study. It has the five 5 criteria of the ICNP classification, Observing, Management, Performance, Caring, and Informing.

가정간호: 위기와 전망 (Home care services: crisis and prospects)

  • The Korean government introduced Home Care Services System to cut medical cost and make efficient use of limited medical resources because of increasing chronic diseases and the growing population of the elderly. The Korean government established measures to control the use of insurance services by restricting the number of nurse's visits to patient's home and by asking the patients to shoulder the transportation fee of nurses during the visit. Factors such as oversupply of hospital facilities, low price of home care services, high insurance coverage for hospital services and increased nuclear family set up resulted in the limited use of home care nursing services. The introduction of long-term care insurance in 2007 brought the decrease in the number of home care agencies and these agencies are facing a crisis today. The increase in chronic diseases and growing population of the elderly recently resulted in the need to control the high medical cost. Home care services for early discharge patients and chronic-severe disease patients will contribute in the reduction of medical cost at the same time improves the quality of patient's life. To catch up with the demands of the nation, accessibility to home care services should be improved and policies such as the expansion of home care services insurance coverage and promotion of establishing home care agencies should be considered.

  • The subject of this study is to review the practical approaches of Home Care Services. Included is a brief overview of its nature, providers of Home Care Services, recent history of Home Care Services, and the impact of the national movement toward cost containment in health care. The data used in this study are obtained from the Elderly Program of the Medical Services and other data on the Home Care Services in Japan. With the growing elderly population in Japan, it is to be expected that the medical care expenditure for this sector will continue to increase. With the aim of keeping expenditure for medical care within reasonable bounds, it is essential that this increasing expenditure on the elderly be used effectively. With the Health and Medical Services Law for the Aged was enforced, therefore, remuneration for medical treatment of the elderly and what is known as the staff placement standard at hospital for the elderly were rationalized. In addition to rationalization from the point of view of medical care supply, it is necessary to guarantee the appropriate treatment within the community and at home for those elderly who are bedridden but not in need of hospital care. For this it is required that Home Care Services, such as health services like visiting guidance by public health nurse in hospital of Health Center. So that the elderly can feel secure in receiving treatment within the community and at home, allowances for guidance on leaving hospital and for intermittent nursing and guidance thereafter are to be newly introduced. Home care Services in one aspect of comprehensive health care, it is comprised of health services provided to individuals and families in their homes. Its purposes include promoting, maintaining and restoring health, specifically maximazing independent functioning and minimizing the disabling effects of illness, including terminal illness. Services appropriate to the needs of clients and their families are planned, coordinated, and delivered by providers organized for the delivery of home health care through the use of contractual arrangement, employed staff, or a combination of the two.

당뇨병성 족부궤양 환자의 치료형태 및 가정간호 연계 (An Analysis of Treatment Types and Home Care Services Referral for Patients with Diabetic Foot)

  • Purpose: The purpose of this study was to investigate treatment types and the status of referral to home care services for patients with diabetic foot. Methods: A retrospective survey was conducted by reviewing medical records from January to December in 2008 at a university hospital. The subjects were 76 patients at the age of 20 years or older who were admitted, had home care services, or received outpatient care for diabetic foot. The data were analyzed using descriptive statistics. Results: Among the total of 9,317 patients diagnosed with diabetes, 5.03% (n=469) had diabetic foot. Admission (81.6%) was the most frequently used treatment type followed by outpatient care only (7.9%) and hemodialysis only (10.5%). Of the 76 admission cases, 44.9% received post-discharge care at outpatient clinics, 20.5% had both outpatient and home care services, and 16.7% were transferred to other hospitals. Readmission rate after discharge was 15.6% for one year. Conclusion: This study suggests referral to home care services should be encouraged to provide effective follow-up care to patients with diabetic foot after discharge from a hospital.

  • Puerperants need postpartum care for the recovery to the condition before pregnancy. The postpartum care used to be done at home however, nuclear families and the industrialization of society brought postpartum care home, postpartum care home has two major problems; first, there is no rules and regulations for postpartum care home. Second, no reasonable guidelines of postpartum care home exist. This study inquires into those problems and directions of legalization and reasonable guidelines of postpartum care home. In sum, the legal system for the postpartum care home has to be built and the supervision has to be done by the specialist, in particular, by the oriental medical doctor. The postpartum care home should be supervised for the healthy rehabilitation of puerperants mental, social health as well as their bodies. Also the neonate that has wick immune system requires special care for the prevention of infection and for the adaptation to the environment.

가정간호대상자 주돌봄자의 삶의 질에 영향을 미치는 요인 (Factors Influencing Quality of Life of Home Care Patient's Primary Caregiver)

  • Purpose: This study investigated the factors affecting the quality of life (QOL) of the primary caregivers of home health care patients. Method: The subjects were 110 primary caregivers of patients who were receiving home health care from two home health care centers affiliated with general hospitals in Seoul. Data collection was conducted using five questionnaires. Results: Positive relationships were evident between QOL and social support and perceived health status of the primary caregiver. Negative relationships were evident between QOL and burden and depression. Multiple linear regression analysis for QOL revealed that the most powerful influencing factor was social support. Social support, burden, and depression explained 34.3% of the variance. Conclusion: Burden, depression, and social support are related with QOL of primary caregivers of home health care patients. Nursing intervention strategies directed at this caregiver population are needed.

의료기관 가정간호의 현황(2007-2012): 가정간호 급여청구자료 분석 (The Current State of Hospital-based Home Care Services in Korea: Analysis of Data on Insurance Claims for Home Care from 2007 to 2012)

  • Purpose: The objectives of this study were to analyze the state of hospital-based home care (HC) services annually and to provide basic information for research and policy regarding home care. Methods: This study is a secondary analysis of the yearly state of HC services from the Health Insurance Review & Assessment Services from 2007 to 2012. Results: The decreased by 34.6%, from 214 agencies in 2007 to 140 in 2012. The annual average number of active home care nurses was 440, which included 6.7% of the licensed home care nurses until 2012. The annual average number of HC patients were 32,000, and this number decreased by 21% in 2012, compared to that of 2008. Of the HC patients, about 70% were over 60 years of age. The chronic diseases among HC patients have been decreasing steadily since 2007. Seventy to eighty percent of the home visits were made in general hospitals or higher level hospitals. The total medical cost for HC services was 21 billion won in 2007, which consisted of 0.06% of the national medical costs, and it was 22 billion won and 0.03% in 2012. Conclusion: Based on the results of this study, further research on HC services is necessary to frame policies for the expansion of HC agencies.

병원중심 가정전문간호사의 역할인식, 역할갈등 및 직무만족 (A Study of Role Awareness, Role Conflict and Job Satisfaction for Hospital-based Home Care Nurses)

  • Purpose: This study is a descriptive research conducted for looking into the role awareness level. role conflicts, and job satisfaction among professional home care nurses in the field of hospital based home care nursing service. Method: 154 specialized home care nurses in 63 hospitals and clinics that are implementing home care programs were subject to the study. Result: 1. The level of role awareness among professional home care nurses was $4.47\pm0.41$ on average out of 5. 2. The average score for role conflicts was $3.87\pm0.61$ out of 5. 3. In terms of job satisfaction level. $3.33\pm0.71$ was scored out of 5. 4. It has been discovered that there are statistically significant positive correlations among role awareness level, role conflicts(r= .224, P= .005), and job satisfaction level(r= .166, P=.040), while a negative correlation was detected between role conflicts and job satisfaction level(r=-.210, P= .009). Conclusion: The level of role awareness among professional home care nurses in the field of hospital based home care nursing service was very high and job satisfaction level was high when the level of home care nursing antecedents was high and the organization structure of working place was independent. But the level of role conflicts coming out of the weakness of job-supporting system was high. So the realistic alternative such as the improvement of working environment by administrative, institutional support, is required to make up for this problem in the future.

건강보험 자료를 이용한 전국 의료기관 가정간호 실시 및 이용 현황 분석: 2008년-2017년 (Analysis of Hospital-based Home Care Service Utilization Using National Health Insurance Claim Data from 2008 to 2017)

  • Purpose: The purpose of this study is to analyze the status quo of hospital-based home care utilization in Korea from 2008 to 2017. Methods: Data from a total of 1,396 medical institutions, 350,390 patients, and 3,563 home care nurses were analyzed using claim data from the Health Insurance Review & Assessment Service. Results: The number of hospital-based home care agencies decreased from 177 in 2008 to 115 in 2014. This number started to increase in 2015 and reached 179 in 2017. The number of hospital-based home care patients declined from 35,056 in 2008 to a low of 26,848 in 2013. This number started to increase in 2014 and reached 67,863 in 2017. Essential hypertension was the most common disease among hospital-based home care patients from 2008 to 2015. The number of hospital-based home care visits declined from about 500,000 in 2008 to a low of 362,000 in 2013. This number started to increase in 2014 and reached 658,000 in 2017. Conclusion: It is necessary to vigilantly monitor hospital-based home care agencies, patients, and the utilization of services. This may help establish platforms for providing community and home-based nursing services for the super-aged society in Korea.
