조선 인조(仁祖)의 질병기록에 대한 고찰 - 승정원일기 기록을 중심으로 - (A review on disease records of King-Injo of Chosun Dynasty - based on the records from The Daily Records of Royal Secretariat of Chosun Dynasty -)

  • 'The Daily Records of Royal Secretariat of Chosun Dynasty' is a record created in Seung-jeong-won, a secretariat for kings of Chosun, and is a government record which holds conversations between kings and their vassals as it is. General affairs in terms of the royal family and national administration are recorded, but what is more important is the records on diseases of kings and how they were treated. This study is to look into diseases from which King Injo(1959-1649) had suffered based on the records written during the time of his reign, which was from 1623 to 1649. Also, the "curse incident" and the death of prince Sohyeon, son of King Injo, both of which had significant influence on the health of the king, were reviewed in relation to the disease records.

  • Through studies on "Dong Eui Bo Gam", I have looked into what kind of books are referred to and its comcompositive characteristics of Tai-yang disease of SangHan. This research is expected to be a new try for further "Dong Eui Bo Gam" researches.

"금궤요략심전" 백합병(百合病) 조문에 대한 연구 (A Study on Baihe Disease(百合病) Texts in "The Esseence of the Synopsis of the Golden Chamber")

  • "The Essence of the Synopsis of the Golden Chamber" is an annotated book on the "Synopsis of the Golden Chamber" written by You-Yi(尤怡) of the Qing Dynasty (1729). Chapter 3 of this annotated book contains explanations of BaiheDisease(百合病). You-Yi(尤怡) maintained that the cause of Baihe-Disease(百合病) is a deficient-type fever(虛熱) induced by lung-fluid deficiency(肺陰不足). Generally, a higher fever led to a worse prognosis, and this disease was mainly treated by supplement methods(補法). In his pharmacological explanations, You-Yi(尤怡) often used the analogical inference of the five evolutive phases(五行歸類), and he frequently quoted "The Yellow Emperor's Canon of Internal Medicine(黃帝內經)" and "The Medical Secret of an Official(外臺秘要)" to explain the texts.

Diagnosis and treatment of teeth with primary endodontic lesions mimicking periodontal disease: three cases with long-term follow ups

  • A tooth with primary endodontic disease that demonstrates a periodontal defect might be extracted because of misdiagnosis as severe periodontal disease or a vertical root fracture. The aim of this case report was to demonstrate the long-term survival of endodontically treated teeth, which had been initially considered unsavable. With meticulous evaluation including the patient's dental history, clinical and radiographic examinations, teeth with primary endodontic lesions could be differentiated and saved after proper root canal treatment. Pain history, vitality test, and radiographic examinations, as well as a general periodontal condition check with periodontal probing on an affected tooth, might be the key methods to differentiate endodontic pathosis from that of periodontal disease.

  • Appearance of a new disease can be seen as a raising a question to the established medicine system. If the problem can not be solved, the established medicine system becomes a degenerating program. So, the process of discovering the nature of a disease and giving a medical explanation was studied focused on smallpox. When smallpox was infected in the begining, many were killed especially mortality of young people were high. It is a general process of contagious diseases known as an epidemic. When the plague is prevalent continuously, it comes to stay and becomes a pediatric disease called smallpox. Time of discovering the nature of smallpox and giving a medical explanation and the time that came to stay are correspondant. This process of medical explanation is a dialectical process.

유창(喩昌)의 "추조론(秋燥論)"의 조병(燥病)에 대한 내용 분석 (Analysis of the Dry disease of Yuchang's "Autumn dry theory")

  • In "EuiMoonBubRyool" chapter 4, Yuchang criticized previous doctors since "damaged by wet in autumn" written in "Somun" must be corrected as "damaged by dry in autumn". The reason why the sentence must be corrected is that dry and damp are different atmosphere. He tried to explain with changes of season and the rapid pulse in autumn is said to be a contraindiction in "Naekyung" and it supports Autumn dry theory. He criticized previous doctors about many incorrect points of dry disease throu호 various aspects. Criticism on Dongwon's treatment, Dangye's medical theory and not having a treatment, and Mujungsoon's treatment and etc. are the examples. Symtoms and treatment of Dry disease that Yuchang commented helped in deepening the contents and widening the general understanding of the disease.

중년여성의 심뇌혈관질환 관련요인 및 취약군 규명 (Factors related to the Identification of Middle-Aged Women Who are Disadvantaged by Cardio-cerebrovascular Disease)

  • Purpose: To investigate factors related to cardio-cerebrovascular disease and groups disadvantaged by it in Korean middle-aged women, using the Korea National Health and Nutrition Examination Survey (KNHANES). Methods: The present study was conducted with 1,627 middle-aged women, aged 40 to 64 years, who participated in the 7th (2016) Korea National Health and Nutrition Examination Survey conducted by the Korea Centers for Disease Control and Prevention. Results: Cardio-cerebrovascular disease among middle-aged women was associated with quality of life, menopause, diabetes mellitus, body mass index, and family history of hypertension. The incidence of cardio-cerebrovascular disease in middle-aged women was found to be the most prevalent in women who have entered menopause, have a family history of hypertension, and have a body mass index greater than $30.0kg/m^2$. Conclusion: This study classifies the subjects according to the risk level of each disadvantaged group for cardio-cerebrovascular disease prevention and management in middle-aged women. The results provide evidence to support a tailored cardio-cerebrovascular disease prevention and management program based on the related factors of disadvantaged groups and to establish strategies in educational and practical aspects.

Relationship between smoking history and periodontal disease among the elderly in Korea

  • Objectives: The objective of this study was to investigate the relationship between smoking history and periodontal disease among the elderly in South Korea. Methods: The study subjects comprised 2,703 elderly people who underwent oral health examination as part of the 7th South Korea National Health and Nutrition Examination Survey (KNHANES; 2016-2018). Data were analyzed using frequency analysis, Rao-Scott chi-square test, t-test, and binary logistic regression analysis. Results: A complex sample logistic regression analysis showed that the odds for periodontal disease development were higher in past smokers (odds ratio [OR]=1.461; 95% CI=1.070-1.994) and current smokers (OR=1.601; 95% CI=1.011-2.536) than in lifetime non-smokers. Conclusions: Smokers must actively participate in smoking cessation programs and interventions starting from middle age.

  • Background: Chronic obstructive pulmonary disease (COPD) is characterized by airflow limitation and results from environmental factors and genetic factors. Although cigarette smoking is a major risk factor, other environmental exposures can influence COPD. The purpose of this study is to investigate the clinical characteristics of COPD according to the history of environmental exposure. Methods: The study population comprised of 347 subjects with COPD who were recruited from the pulmonary clinics of 14 hospitals within the Korean Obstructive Lung Disease Study Group. We classified environmental exposures according to history of living near factory, and direct exposure history to firewood or briquette. According to living environmental exposures, we compared the frequency of respiratory symptoms, pulmonary function, quality of life, exercise capacity, and computed tomography phenotypes. Results: Thirty-one subjects (8.9%) had history of living near factory, 271 (78.3%) had exposure history to briquette, and 184 (53.3%) had exposure history to firewood. Patients with history of living near a factory had a significantly longer duration of sputum, while patients with exposure to firewood tended to have lower forced expiratory volume in one second, and patients with exposure to briquette tended to have lower six minute walk distance. Conclusion: COPD subjects with the history of living near factory had more frequent respiratory symptoms such as sputum. Our data suggest that environmental exposure may influence clinical phenotype of COPD.

Hereditary protein S deficiency presenting acute pulmonary embolism

  • Protein S deficiency is one of the several risk factors for thrombophilia and can cause blood clotting disorders such as deep vein thrombosis and pulmonary embolism. A 54-year-old man was admitted with the complaint of dyspnea and was diagnosed with pulmonary embolism. The patient had very low level of free protein S, total protein S antigen, and protein S activity (type I protein S deficiency). In history taking, we found that his mother, 78 year old, had a history of same disease 10 years ago, and confirmed the pronounced low level of protein S. The patient's son also had very low level of protein S, however there had not been any history of pulmonary embolism yet. This case study suggests that asymptomatic persons with a family history of protein S deficiency and pulmonary embolism should be checked regularly for early detection of the disease, as protein S deficiency can be suspected.
