A Study on the Contact Stress Analysis of the Artificial Hip Joint Using Finite Element Method (유한요소법을 이용한 인공 고관절의 접촉응력 해석에 관한 연구)

  • The modern orthopaedics frequently uses the total hip replacement in the artificial hip joint. The wear in this joint requires the re-replacement of the hip joint beacuse it is under the severe load and friction conditions. To solve these problems the previous studies have been mainly focused on the development of the new materials. The research of new material, however, needs much time and effort since it should be experimented for its bio-compatibility, friction, and wear characteristics. To reduce the work, in this study, the finite element analysis is applied to find the new combination of the materials in the total hip replacement which has the excellent contact characteristics. The finite element uses MARC and the 5-node aximetric element. The results show that in case of acetabulum UHMWP has good characteristics, in femoral head, the $Al_2O_3$, and in stem, Ti6Al4V.

To reduce the work, in this study, the finite element analysis is applied to find new combinations of bio-materials in the total hip replacement which has the excellent contact characteristics. A non-linear FEM program MARC with 5-node axisymmetric element was used for analyzing the contact stresses between the hip joints. The computed results show that in case of acetabulum UHMWP has good characteristics, in femoral head, $Al_2O_3$, and in stem, Ti6Al4V.

Kinematic Characteristics during of Gait in Parkinson's Disease Patients (파킨슨 질환 환자의 보행시 운동형상학 특성)

  • The aim of this study is to present the basic reference data of age and specific gait parameters for Parkinson's Disease Patients. The basic gait parameters were extracted from 5 patients, 5 men and 65 years of age using VICON 512 Motion Analyzer. The temporal gait parameters and kinematic parameters is data of Parkinson's Disease Patients. The results were as follows; 1. The cadence, velocity, stride length decreased and single limb support period, double limb support period increased than normal adult in the temporal parameters. 2. The mean angles of joint pelvic tilt and hip, knee, ankle joint decreased than normal adult at kinematic characteristics on sagittal plane. 3. The mean angles of joint pelvic tilt and hip, knee joint has no difference than normal adult at kinematic characteristics on coronal plane. 4. The mean angles of joint pelvic tilt, hip joint no difference and internal, external rotation in ankle joint significantly decreased than normal adult at kinematic characteristics on transverse plane.

Clinical Characteristics of Hip Joint Rotations and Knee Adduction Moment through 3D Gait Analysis (3차원 보행분석을 통한 무릎 모음 모멘트와 고관절 내외회전의 임상적 특성)

  • Purpose : The purpose of this study was to verify the relationships among the knee adduction moment, hip rotation range, strength of hip rotators, and Foot Posture Index of healthy young adults. Method : Thirty-two healthy adults(24 male, 8 females) participated in this study. Subjects performed 5 walking trials to evaluate the knee adduction moments using a three-dimensional motion analysis system. Hip rotation ranges and hip rotator strengths were measured using a standard goniometer and a handheld dynamometer, respectively. The mean of three trials of clinical tests was used for data analysis. Results : The first peak knee adduction moment was significantly correlated with the hip rotation ranges and hip rotator strengths (P<.05). The second peak knee adduction moment was showed significant correlations with hip external rotation and rotation ratio. There were no correlations between Foot Posture Index and all knee adduction moments (P>.05). Conclusion : This study suggests that imbalances of the range of motion and strength of the internal and external rotation of the hip joint can affect knee adduction moments. The impact may exacerbate musculoskeletal disorders such as osteoarthritis of the knee. Therefore, further studies should be conducted to evaluate the effects of clinical interventions to correct these imbalances on the reduction of the knee adduction moments in patients with knee osteoarthritis.

Immediate Effect of Hip Joint Exercise Program on Low Back Pain and Performance Level in Amateur Golfers with Chronic Low Back Pain (고관절 운동 프로그램이 만성 요통이 있는 아마추어 골퍼의 통증과 골프수행 수준에 미치는 즉각적인 효과)

  • PURPOSE: This study investigated the influence of a specific hip joint exercise program on the characteristics of chronic low back pain (LBP), hip joint range of motion (ROM), and performance level in amateur golfers with LBP. METHODS: The study included 28 adult male amateur golfers with LBP and reduced internal rotation (IR) of the lead side hip joint. Subjects were randomly assigned to an experimental (EG) (n=14) or control group (CG) (n=14). The Numerical Rating Scale score for pain, ROM of the hip joint and trunk rotation, performance parameters for golfing were evaluated. A specific exercise program designed to improve IR of the lead side hip joint was performed by the EG, but not the CG. Post-intervention evaluation was performed and the collected data were analyzed. RESULTS: After the intervention, the severity of LBP was significantly reduced (p<.05) and the IR ROM of the lead side hip joint was significantly increased (p<.01) in the EG. Moreover, the trunk rotation range was significantly increased in the EG (p<.01). The drive distance was significantly difference between the EG and CG (p<.05). CONCLUSION: A specific hip joint exercise program can help to reduce pain level and improve performance level in amateur golfers with LBP and increased IR ROM of the lead side hip joint. A significant increase in ROM of the lead side hip joint reduce LBP and was related to driving distance in the EG.

A Biomechanical Study on the Hip Joint Flexibility Based on the Proprioceptive Neuromuscular Facilitation (고관절 가동성 증진을 위한 고유수용성 신경근 촉진 스트레칭의 생체역학적 고찰)

  • Joint flexibility is an important factor which affect the process and duration of the therapeutic methods in the filed of occupational therapy. Hip joint flexibility and electromyography (EMG) of major flexor and extensor for the hip joint were examined to understand the biomechanical characteristics of Proprioceptive Neuromuscular Facilitation (PNF). Hip joint flexibility increased $18.9^{\circ}$ on average after PNF was performed by a designated assistant on 10 college students. EMGs of quadriceps femoris muscle and hamstring muscles agreed with biomechanical characteristics of proprioceptive organs in muscles.

A Study for Hip Joint in Children with Cerebral Palsy (뇌성마비 아동의 엉덩관절에 관한 연구)

  • Purpose The purpose of this study is to identify the prevalence, risk factors, characteristics, and interventions of hip joint problems in children with cerebral palsy, and to be able to serve as leverage for early detection, prevention, and function recovery. Method The electronic journal site was searched by the search terms "cerebral palsy", "hip joint", "hip joint dislocation", we analyzed and descript the cited articles from domestic and foreign papers in Pubmed 9, Science Direct 7, and K RISS analysis and analysis. Results Children with cerebral palsy showed the different prevalence according to their disability type, severity, and functional level, and we knew that abnormal neuromuscular control, stiffness, and biomechanical changes could be risk factors. Migration Index, Acetabolum Index, Neck shaft angle using by radiography and passive ROM test, special tests were available for the diagnosis and evaluation of the hip joint. Combination of physical therapy and orthopedic surgery was very important intervention, and complementary alternative therapy, orthosis, and postural assistant are effective. Conclusion We suggested that early detection and prevention is the most important periodic examination and that a multidisciplinary approach is a major factor in intervention.

Identifications of Reflex Muscle Activities and Joint Moments Triggered by Electrical Stimulation to Sole of the Foot during Lokomat Treadmill Walking

  • The aim of this study was to investigate the characteristics of the flexion withdrawal reflex modulated during Lokomat treadmill walking in people with spinal cord injury. The influence of the limb position and movement were tested in 5 subjects with chronic spinal cord injury. EMG activities from tibialis anterior and moments of the hip joint elicited by the foot stimulation were examined during Lokomat treadmill walking. To trigger the flexion withdrawal reflex during Lokomat treadmill walking, a train of 10 stimulus pulses was applied at the skin of the medial arch. The TA EMG activity was modulated during gait phase and the largest TA reflex was obtained after heel-off and initial swing phase. During swing phase, TA EMG was 40.9% greater for the extended hip position (phase 6), compared with flexed hip position (phase 8). The measured reflex moment of the hip joint was also modulated during gait phase. In order to characterize the neural contribution of flexion reflex at the hip joint, we compared estimated moments consisted of the static and dynamic components with measured moment of the hip joint. The mean static gains of reflex hip moments for swing and stance phase are -0.1, -0.8, respectively. The mean dynamic gains of reflex hip moments are 0.25 for swing, 0.75 for stance phase. From this study, we postulate that the joint moment and muscle response of flexion withdrawal reflex have the phase-dependent modulation and linear relationship with hip angle and angular velocity for swing phase during Lokomat treadmill walking.

A Statistical Model for Marker Position in Biomechanics

  • Objective: The purpose of this study was to apply a general linear model in statistics to marker position vectors used to study human joint rotational motion in biomechanics. Method: For this purpose, a linear model that represents the effect of the center of hip joint rotation and the rotation of the marker position on the response was formulated. Five male subjects performed hip joint functional motions, and the positions of nine markers attached on the thigh with respect to the pelvic coordinate system were acquired at the same time. With the nine marker positions, the center of hip joint rotation and marker positions on the thigh were estimated as parameters in the general linear model. Results: After examining the fitted model, this model did not fit the data appropriately. Conclusion: A refined model is required to take into account specific characteristics of longitudinal data and other covariates such as soft tissue artefacts.

A Clinical Study of Hip Joint Pain Improved with Gihwangbaekho-tang (지황백호탕(地黃白虎湯)으로 호전된 고관절 통증 환자 치험례)

Objectives The purpose of this study is to evaluate the effects of Gihwangbaekho-tang which is based on the Sasang Constitutional Medicine for the patient who has suffered from hip joint pain. 2. Methods The subject is about 86-year-old patient suffering from hip joint pain who was diagnosed as Soyangin interior over heated disease based on his Nature & Emotion, physical characteristics, symptoms, and we have prescribed Gihwangbaekho-tang. We used Visual analogue scale(VAS) for the assessment of Hip joint pain. 3. Results and Conclusions The patient was treated with Gihwangbaekho-tang and hip joint pain, thirst, redness of the face, constipation and general condition were improved. This study describes tile improvement of Hip joint pain and the change of symptoms by using Gihwangbaekho-tang. In this study, we applied Gihwangbaekho-tang to 86-year-old patient for about forty days, and Gihwangbaekho-tang didn't cause any serious side effect on the patient.
