신경 차단 마취를 이용한 족부 및 족근 관절 수술 (Nerve Block Anesthesia for Foot and Ankle Surgery)

  • We present our experience with the use of nerve block anesthesia in 212 of 484(43.8% ) surgical procedures of the foot and ankle between 1995 and 1997. Nerve block anesthesia was used for surgical procedures of the forefoot, midfoot, hindfoot, and ankle in the setting of elective surgery and trauma. From the viewpoint of the surgeon, nerve block anesthesia was completely successful in 99.5% of the procedures performed. We confirmed that 80% of patients were satisfied with the use of nerve block anesthesia for their operative procedure. There were only one(0.5%) miner complication being toxic neuritis of posterior tibial nerve. With increasing experience, the expanded indications and uses of nerve block anesthesia for foot and ankle surgery are proving to be simple, safe, reliable, and well tolerated by the patient.

Rear Foot Wedge 각도가 보행시 전족저 최대압력에 미치는 영향 (The Effect of Rear Foot Wedge Angle on Peak Plantar Pressures on the Forefoot During Walking)

  • The purpose of this study was to find the effect of rear foot wedge angle on peak plantar pressures on the forefoot during walking. Twenty normal healthy subjects (10 female, 10 male) were recruited. Peak plantar pressure was measured using pressure distribution platforms (MatScan system) in medial forefoot (under the first, second metatarsal head) and lateral forefoot (under the third, fourth, fifth metatarsal head). The subjects walked at the comfortable velocity under seven conditions; bare footed, $5^{\circ}$, $10^{\circ}$ and $15^{\circ}$ wedges under the medial and lateral sides of the hindfoot. The three averaged peak plantar pressures were collected at each condition at stance and toe off phases. The results showed that a significant increase in lateral forefoot plantar peak pressure investigated in the medial wedge and a significant decrease in lateral forefoot plantar peak pressure investigated in lateral wedge at stance phase (p<.05). These results suggest that rear foot wedge may be useful to modify the peak plantar pressure on the forefoot.

개방적 내고정술을 시행한 발목 골절환자 2례에 대한 수술 후 한의 재활치료: 증례보고 및 5개월 추적관찰 (Postoperative Rehabilitation of Korean Medicine for Ankle Fractures Treated by Open Reduction with Internal Fixation: 2 Case Reports with 5 Months Follow-Up)

  • The purpose of this study is to report the effect of Korean medicine rehabilitation therapies in two patients with ankle fractures who underwent Open Reduction with Internal Fixation (ORIF). Two patients with fractures who received ORIF received acupuncture, electroacupuncture, herbal medicine, and physical therapy during hospitalization. Patients were evaluated for AOFAS score, NRS, ROM, and ankle circumference. Case 1 was improved from 30 points to 62 points on the AOFAS score, from 8 to 2 on the NRS, and from 33 cm to 30 cm on the ankle perimeter. ROM was improved in all directions. Case 2 showed an AOFAS score of 64 to 90 points, frome 5 to 2 on the NRS, and from 25 cm to 23.5 cm on the ankle perimeter. ROM was improved in all directions. The results of this study suggest that the treatment of Korean medicine has a meaningful effect on improvement and rehabilitation of ankle fracture patients who have received ORIF.

족근관절 전치환술의 중장기 추시 (Mid-term follow up of Total Ankle Replacement Arthroplasty)

  • Purpose: To show the results in term of pain and functional recovery in the mid-term follow up of total ankle replacement. Materials and Methods: We followed up 7 patients who had undergone total ankle arthroplasty during the periods between April 1990 and May 1997. They were evaluated after mean follow up of 3.6 years. Results: We reviewed these cases with regand to (in terms of) pain, function and alignment according to the Ankle-Hindfoot Scale designed by American Orthopaedic Foot and Ankle Society. The average point was 78. Conclusion: Total ankle replacement especially unconstrained type seems good alternative to arthrodesis in selected cases of ankle arthrosis.

전위된 거골 경부 골절의 수술적 치료 (Operative Treatment of Displaced Talar Neck Fracture)

  • Purpose: To evaluate the results of open reduction and internal fixation for displaced talar neck fracture. Materials and Methods: Fourteen patients were followed for more than 1 year after open reduction and internal fixation of displaced talar neck fracture. The mean age was 41.5 years, and the mean follow-up period was 3.8 years. There were 10 type II, 3 type III and 1 type IV fractures per Hawkins. There were 3 open fractures. Clinically AOFAS ankle-hindfoot scale and Hawkins criteria were utilized. Radiologically quality of reduction, duration of bony union, avascular necrosis and posttraumatic arthritis were evaluated. Results: At last follow-up, AOFAS scale was mean 89.1 points. There were 8 excellent, 4 good, and 2 fair results according to Hawkins criteria. Radiologically anatomical reduction was obtained in 13 cases. Mean duration of bony union was 11.8 weeks. There were 3 avascular necrosis and 2 post-traumatic arthritis. There was no significant difference in the incidence of avascular necrosis between early operation group and surgically delayed group. Conclusion: Accurate open reduction and rigid internal fixation seem to be prerequisites for satisfactory treatment of a displaced talar neck fracture.

족부에 발생한 건막 거대 세포종 (Giant Cell Tumor of Tendon sheath in the Foot)

  • Purpose: We evaluate the clinical, radiologic and pathologic features of giant cell tumor of tendon sheath (GCTTS) in the foot Materials and Methods: Twelve cases of GCTTS excised from foot region, at our hospital from 1999 to 2002, were analyzed. The mean duration of follow up was 19 months. The age and sex of the patient, location and size of the lesion, symptom as well as radiologic findings were evaluated. Results: The most common symptom was painless mass in 7 patients. Three patients had pain sympton and 2 patients had tenderness. The mean duration from identification of the mass to excision was 14 months. Seven cases were located in the forefoot, most commonly in the big toe with 4 cases, 2 cases in the midfoot and 3 cases in the hindfoot. The average diameter along the long axis was 2.8cm. Conclusion: GCTTS in the foot was more common in the big toe and also plantar side same as in the hand. But bony erosion and pain were more frequent than in the hand.

중증 족관절 관절염: 족관절 전치환술 (Severe Ankle Osteoarthritis: Treatment with Total Ankle Arthroplasty)

  • Ankle osteoarthritis is a debilitating condition that causes severe pain associated with functional impairment and decreased activity. Ankle osteoarthritis, unlike that of the knee or hip joint, is rare in primary arthritis. Most cases are traumatic arthritis that occur after ankle sprain or fractures or chronic ankle instability. Although ankle fusion has been regarded as the standard treatment of ankle osteoarthritis in the past, total ankle arthroplasty (TAA) is increasing due to the development of the implant design and surgical techniques. TAA is biomechanically superior to ankle fusion by preserving the movement of the ankle joint. In particular, it is functionally superior to ankle fusion because it enables normal joint motion during gait. In addition, there is an advantage of preserving the movement of the hindfoot and reducing the abnormal stress applied to the adjacent joints after ankle fusion to prevent the occurrence of long-term adjacent joint arthritis. Although the short-term and mid-term results of TAA have been reported to be excellent, long-term follow-up has a relatively low survival rate and high complication rate compared to total knee or hip arthroplasty. Therefore, continuous and further research is needed.

만성 족관절 염좌에 자하거 약침을 병행한 한방 치료로 호전된 2례 보고 (The Case Report of Chronic Ankle Sprain Improved with Hominis Placenta Pharmacopuncture Treatment)

  • Objectives The purpose of this study is to investigate and report the effectiveness of Hominis Placenta pharmacopuncture treatment for chronic ankle sprain. Methods 2 patients are treated at Dept. of Korean Medicine, the Armed Forces Busan Hospital, diagnosed as thinning of anterior talofibular ligament (ATFL), suggestive of partial tear and anterior tibiofibular ligament partial tear. They are treated with Hominis Placenta pharmacopuncture. Each cases are measured and assessed by ankle hindfoot scale (AHS), visual analgue scale (VAS), cumberland ankle instability tool (CAIT) scores. Results 2 patients of chronic disease of ankle sprain have a different kind of diagnoses they have. After treatment of Hominis placenta pharmacopuncture, spontaneous pain is decreased and ankle instability, ankle functions are increased significantly. Conclusions Hominis placenta Pharmacopuncture has a effect on chronic ankle sprain.

족관절 염좌의 한의학적 치료에 대한 논문 고찰 -국내 학술지를 중심으로- (Review on the Study of Ankle Sprain Published in Korea)

  • Objectives The aim of this review is to analyze the study tendency in papers related with ankle sprain which are published in Korea. Methods I searched the OASIS and checked relevant Korean journals. I classified the papers by field of study, publish date, main treatment, evaluation methods and analysed the study tendency. Results 1. There are 9 case studies, 4 comparative effectiveness studies, 11 controlled studies. 2. The paper was published almost every year for more than one. 3. Treatment tool such as acupuncture, pharmacopuncture, acupotomy, heating-conduction acupuncture, etc. was used. 4. The most commonly used evaluation method is VAS (Visual Analog Scale) and AHS (Ankle-Hindfoot scale). Conclusions These results suggest that Korean medical treatment on the ankle sprain can be useful as cure medicine, but there are not sufficient evidence based papers, so there should be further studies.

  • The objective of this study is to propose postoperative rehabilitation for foot fracture in Korean Medicine and to report its effectiveness. There were three patients who were got foot fracture surgery received korean medicine with acupuncture, herbal medicine, cupping and exercise treatment. Patients were evaluated numeric rating scale (NRS), range of motion (ROM), American Orthopaedic Foot and Ankle Society (AOFAS) ankle hindfoot score, foot function index (FFI) and EuroQol-5 dimension (EQ-5D) index. Case 1 and 2 were improved NRS from 6 to 3. Case 3 was improved NRS from 5 to 2. These cases were improved ROM in all directions, AOFAS score, FFI, EQ-5D and walking status. This study suggest that Korean medicine rehabilitation could be effective for patients who have received foot fracture operation.
