Difference in the Incidences of the Most Prevalent Urologic Cancers from 2003 to 2009 in Iran

  • Background: Urological cancers represent a major public problem associated with high mortality and morbidity. The pattern of these cancers varies markedly according to era, region and ethnic groups, but increasing incidence trends overall makes focused epidemiological studies important. The aim of the present study was to assess the incidence of most prevalent urological cancers in Iran from 2003 to 2009. Materials and Methods: The data for this study were obtained from the population-based Cancer Registry Center of the Iran Ministry of Health and Medical Education. Differences of mean age and age distributions of each cancer were compared between 2003 and 2009 in men and women. Results: Bladder cancer was the most common urologic cancer in both genders. The rate difference of age standardized ratio (ASR) of bladder and renal cell carcinoma in women were 1.54 and 2.01 percent per 100,000 population from 2003 to the 2009, respectively. In men, the rate difference of age standardized ratio of prostate, testis, kidney and bladder cancer was also 2.23, 1.2, 1.8 and 1.5 percent per 100,000 population from 2003 to 2009, respectively. The mean ages of patients in all cancers in both genders did not differ significantly through time (p value>0.05) but the distribution of ages of patients with bladder and prostate cancer changed significantly from 2003 to 2009 (p value<0.001). Conclusions: The results of present study suggest the general pattern and incidence of urological cancers in Iran are changing, the observed increase pointing to a need for urological cancer screening programs.

성장기의 편측 하악 과두 골절이 하악골 성장 및 기능에 미치는 영향에 대한 연구 (The study of the effect of mandibular growth and function in pediatric unilateral condyle fractures)

  • Introduction: Condylar fractures are common in the maxillofacial region, comprising 29-40 percent of all mandibular fractures, accounting for about 20-62 percent). Previous studies reported that pediatric condylar fractures can cause disorders in facial growth and function, and the treatment methods have been controversial. Recently, conservative treatment has shown good results in skeletal growth and functional recovery but the conservative treatment of pediatric condylar fractures has shown unpredictable and undesirable results in some cases, such as facial asymmetry and temporomandibular joint disorder. This study examined the specific age groups and specific mandibular condylar fracture type in growing children treated conservatively in the past. Materials and Methods: Eighteen patients (10 men and 8 women) who received conservative treatment for unilateral condylar fractures in Dankook University Dental Hospital between 2000 to 2007 were followed up for a mean period of 7.2 years. Results: In the survey of 18 pediatric patients who received conservative treatment for condylar fractures, the incidence of temporomandibular dysfunction and growth disturbance was 45% and 35%, respectively. Conclusion: In all complications, the symptoms observed most frequently was mouth opening displacement of the mandible exceeding 2 mm. The other complications of functional and growth disturbance included facial asymmetry concentrated along specific condylar types. Complications including facial asymmetry and functional and growth disturbances showed an increasing tendency according to the specific fracture types. Functional and growth disturbances in the undisplaced condylar fracture type showed a lower incidence(P <0.05). Functional and growth disturbances differed according to the fracture type, which has poor relationship with articular fossa and condyle(P <0.05). Functional and growth disturbance in the cases of the high-level condylar fracture type showed a higher incidence(P <0.05). The functional and growth disturbances of the fracture types were similar in the fragment-contact and non-contact groups(P >0.05).

Trends and Spatial Pattern Analysis of Dengue Cases in Northeast Malaysia

  • Objectives: Dengue remains hyperendemic in Malaysia despite extensive vector control activities. With dynamic changes in land use, urbanisation and population movement, periodic updates on dengue transmission patterns are crucial to ensure the implementation of effective control strategies. We sought to assess shifts in the trends and spatial patterns of dengue in Kelantan, a north-eastern state of Malaysia (5°15'N 102°0'E). Methods: This study incorporated data from the national dengue monitoring system (eDengue system). Confirmed dengue cases registered in Kelantan with disease onset between January 1, 2016 and December 31, 2018 were included in the study. Yearly changes in dengue incidence were mapped by using ArcGIS. Hotspot analysis was performed using Getis-Ord Gi to track changes in the trends of dengue spatial clustering. Results: A total of 10 645 dengue cases were recorded in Kelantan between 2016 and 2018, with an average of 10 dengue cases reported daily (standard deviation, 11.02). Areas with persistently high dengue incidence were seen mainly in the coastal region for the 3-year period. However, the hotspots shifted over time with a gradual dispersion of hotspots to their adjacent districts. Conclusions: A notable shift in the spatial patterns of dengue was observed. We were able to glimpse the shift of dengue from an urban to peri-urban disease with the possible effect of a state-wide population movement that affects dengue transmission.

정형외과 수술 후 발생한 섬망의 발생 빈도와 관련 인자 (Incidence and Associated Factors of Delirium after Orthopedic Surgery)

  • 목적: 정형외과 수술 후 환자의 섬망 발생 빈도 및 섬망에 미치는 영향 인자를 분석하고자 하였다. 대상 및 방법: 1년간 단일 상급 종합병원에서 정형외과 수술을 시행 받은 성인 환자 2,122예를 대상으로 하였다. 이들 중 정신건강의학과 협의 진료하에 Diagnostic and Statistical Manual of Mental Disorders-V 진단 기준에 부합하며 섬망 관련 투약력이 확인된 환자 132예를 선별하였고 수술 원인 및 수술 부위에 따른 섬망 발생률의 차이와 입원 후 섬망 발생까지의 시간 및 지속 시간에 영향을 미치는 인자를 분석하였다. 결과: 전체 수술 환자 중 6.2% (132/2,122예)에서 섬망이 발생하였다. 섬망 환자군의 평균 나이는 77.4세(범위, 54-92세)로 비섬망 환자군의 평균 나이(58.1세)보다 많았고, 섬망 환자군의 여성 비율이 63.6% (84/132예)로 비섬망 환자군의 여성 비율(49.0%)보다 높았다. 외상에 의한 수술 후 9.3% (85/916예), 질환에 의한 수술 후 3.9% (47/1,206예)의 섬망 발생률을 보였다. 수술 부위별 섬망 증상의 발생률은 두 부위 이상 수술을 시행한 경우 29.2% (7/24예), 고관절 13.7% (72/526예), 척추 9.6% (14/146예), 슬관절 및 하퇴 3.5% (20/577예), 족부 및 족관절 2.5% (5/199예), 견주관절 2.4% (11/457예), 전완 및 수부 1.6% (3/189예)였다. 여성인 경우와 질환으로 수술하는 경우 입원 후 섬망이 빨리 발생하였고, 치매와 주요 우울 장애가 있는 경우 섬망 지속기간이 길게 나타났다. 결론: 외상으로 인해 수술한 경우, 두 부위 이상 수술을 받은 경우 섬망 발생 빈도가 높았고, 단일 부위에서 고관절, 척추, 슬관절 순서로 높았다. 본 연구를 통해 술 후 섬망 발생의 위험도를 예측하고 교정 가능한 인자에 대한 적극적 개입이 필요할 것으로 생각된다.

Evaluation of Risk Factors for Nasopharyngeal Carcinoma in a High-risk Area of India, the Northeastern Region

  • Northeastern India is a major nasopharyngeal carcinoma (NPC) high risk-area although the rest of the country has very low incidence. A case-control study of 105 NPC cases and 115 controls was conducted to identify the potential risk factors for NPC development in this region. Information was collected by interviewer about socio-demographic characteristics, cigarette smoking, alcohol consumption, dietary history, occupational history, and a family history of cancer. Epstein-Barr viral load was assayed from the blood DNA by real time PCR. Associations between GSTs genotypes, cytochrome P450 family including CYP1A1, CYP2E1 and CYP2A6 polymorphisms and susceptibility to relationship between the diseases were studied using PCR-RFLP assay. Results indicate that Epstein-Barr virus load was significantly higher in patients compared to controls (p<0.0001). Furthermore, concentration of blood EBV-DNA was significantly higher in advanced stage disease (Stage III and IV) than in early stage disease (Stage I and II) (p<0.05). Presence of CYP2A6 variants that reduced the enzyme activity was significantly less frequent in cases than controls. Smoked meat consumption, exposure to smoke, living in poorly ventilated house and alcohol consumption were associated with NPC development among the population of Northeastern India. Thus, overall our study revealed that EBV viral load and genetic polymorphism of CYP2A6 along with living practices which include smoked meat consumption, exposure to smoke, living in poorly ventilated houses and alcohol consumption are the potential risk factors of NPC in north eastern region of India. Understanding of the risk factors and their role in the etiology of NPC are helpful forpreventive measures and screening.

Predictions of non-uniform tip clearance effects on the flow field in an axial compressor

  • Asymmetric tip clearance in an axial compressor induces pressure and velocity redistributions along the circumferential direction in an axial compressor. This paper presents the mechanism of the flow redistribution due to the asymmetric tip clearance with a simple numerical modeling. The flow field of a rotor of an axial compressor is predicted when an asymmetric tip clearance occurs along the circumferential direction. The modeling results are supported by CFD results not only to validate the present modeling but also to investigate more detailed flow fields. Asymmetric tip clearance makes local flow area and resultant axial velocity vary along the circumferential direction. This flow redistribution 'seed' results in a different flow patterns according to the flow coefficient. Flow field redistribution patterns are largely dependent on the local tip clearance performance at low flow coefficients. However, the contribution of the main flow region becomes dominant while the tip clearance effect becomes weak as the flow coefficient increases. The flow field redistribution pattern becomes noticeably strong if a blockage effect is involved when the flow coefficient increases. The relative flow angle at the small clearance region decreases which result in a negative incidence angle at the high flow coefficient. It causes a recirculation region at the blade pressure surface which results in the flow blockage. It promotes the strength of the flow field redistribution at the rotor outlet. These flow pattern changes have an effect on the blade loading perturbations. The integration of blade loading perturbation from control volume analysis of the circumferential momentum leads to well-known Alford's force. Alford's force is always negative when the flow blockage effects are excluded. However when the flow blockage effect is incorporated into the modeling, main flow effects on the flow redistribution is also reflected on the Alford's force at the high flow coefficient. Alford's force steeply increases as the flow coefficient increases, because of the tip leakage suppression and strong flow redistribution. The predicted results are well agreed to CFD results by Kang and Kang(2006).

Measuring the Burden of Disease in Korea, 2008-2018

  • The study aims to examine the current status and differences in the burden of disease in Korea during 2008-2018. We calculated the burden of disease for Koreans from 2008 to 2018 using an incidence-based approach. Disability adjusted life years (DALYs) were expressed in units per 100 000 population by adding years of life lost (YLLs) and years lived with disability (YLDs). DALY calculation results were presented by gender, age group, disease, region, and income level. To explore differences in DALYs by region and income level, we used administrative district and insurance premium information from the National Health Insurance Service claims data. The burden of disease among Koreans showed an increasing trend from 2008 to 2018. By 2017, the burden of disease among men was higher than that among women. Diabetes mellitus, low back pain, and chronic lower respiratory disease were ranked high in the burden of disease; the sum of DALY rates for these diseases accounted for 18.4% of the total burden of disease among Koreans in 2018. The top leading causes associated with a high burden of disease differed slightly according to gender, age group, and income level. In this study, we measured the health status of Koreans and differences in the population health level according to gender, age group, region, and income level. This data can be used as an indicator of health equity, and the results derived from this study can be used to guide community-centered (or customized) health promotion policies and projects, and for setting national health policy goals.

Novel Mutations in the Displacement Loop of Mitochondrial DNA are Associated with Acute Lymphoblastic Leukemia: A Genetic Sequencing Study

  • Background: Acute lymphoblastic leukemia (ALL) is the most common cancer diagnosed in children and represents approximately 25% of cancer diagnoses among those younger than 15 years of age. Materials and Methods: This study investigated alterations in the displacement loop (d-loop) region of mitochondrial DNA (mtDNA) as a risk factor and diagnostic biomarker for early detection and diagnosis of acute lymphoblastic leukemia. Using mtDNA from 23 subjects diagnosed with acute lymphoblastic leukemia, the first 450 bp of the d-loop region were amplified and successfully sequenced. Results: This revealed 132 mutations at 25 positions in this region, with a mean of 6 alterations per subject. The d-loop alterations in mtDNA in subjects were all identified as single nucleotide polymorphisms in a homoplasmic distribution pattern. Mutant alleles were observed in all subjects with individual frequency rates of up to 95%. Thirteen mutant alleles in the d-loop region of mtDNA occurred with a high frequency. Novel alleles and locations were also identified in the d-loop of mtDNA as follows: 89 G insertions (40%), 95 G insertions (13%), 182 C/T substitutions (5%), 308 C insertions (19%), and 311 C insertions (80%). The findings of this study need to be replicated to be confirmed. Conclusions: Further investigation of the relationship between mutations in mitochondrial d-loop genes and incidence of acute lymphoblastic leukemia is recommended.

투과성 이중 반원통 구조물에 의한 파 차단성능 (Wave Deformation and Blocking Performance by a Porous Dual Semi-Cylindrical Structure)

  • 투과성 이중 반원통 구조물과 경사 입사파간의 상호작용문제를 선형포텐셜 이론을 사용하여 살펴보았다. 투과성 이중 반원통 구조물은 바닥에 고정된 동심원상으로 배열된 2개의 원통 구조물로 이루어지며 각 원통 구조물의 전면은 일정한 공극율을 갖는 투과벽으로 후면은 투명한 벽으로 구성된다. 전면 투과벽의 공극율과 간격 그리고 파랑특성(주파수, 입사각)을 변화시키면서 파랑하중과 처올림 파형 그리고 파 차단성능을 살펴보았다. 파 차단 성능의 척도로서 차단영역내의 평균제곱 변위의 제곱근(R.M.S.)을 사용하였다. 투과성 반원통 구조물은 불투과성 구조물과 비교하여 차단영역내의 파도응답을 감소시키며 구조물에 작용하는 파랑하중을 크게 줄여준다. 특히 이중 구조물은 고주파주 영역에서 단일 구조물보다 파를 차단하는데 효과적이다.

The Effects of Spinopelvic Parameters and Paraspinal Muscle Degeneration on S1 Screw Loosening

  • Objective : To investigate risk factors for S1 screw loosening after lumbosacral fusion, including spinopelvic parameters and paraspinal muscles. Methods : We studied with 156 patients with degenerative lumbar disease who underwent lumbosacral interbody fusion and pedicle screw fixation including the level of L5-S1 between 2005 and 2012. The patients were divided into loosening and non-loosening groups. Screw loosening was defined as a halo sign larger than 1 mm around a screw. We checked cross sectional area of paraspinal muscles, mean signal intensity of the muscles on T2 weight MRI as a degree of fatty degeneration, spinopelvic parameters, bone mineral density, number of fusion level, and the characteristic of S1 screw. Results : Twenty seven patients showed S1 screw loosening, which is 24.4% of total. The mean duration for S1 screw loosening was $7.3{\pm}4.1$ months after surgery. Statistically significant risk factors were increased age, poor BMD, 3 or more fusion levels (p<0.05). Among spinopelvic parameters, a high pelvic incidence (p<0.01), a greater difference between pelvic incidence and lumbar lordotic angle preoperatively (p<0.01) and postoperatively (p<0.05). Smaller cross-sectional area and high T2 signal intensity in both multifidus and erector spinae muscles were also significant muscular risk factors (p<0.05). Small converging angle (p<0.001) and short intraosseous length (p<0.05) of S1 screw were significant screw related risk factors (p<0.05). Conclusion : In addition to well known risk factors, spinopelvic parameters and the degeneration of paraspinal muscles also showed significant effects on the S1 screw loosening.
