Significance of Clopidogrel Resistance Related to the Stent-Assisted Angioplasty in Patients with Atherosclerotic Cerebrovascular Disease

  • Objective : To evaluate the prevalence and risk factors of clopidogrel resistance, and association between thromboembolic complications and clopidogrel resistance in patient with stent-assisted angioplasty for atherosclerotic cerebrovascular disease. Methods : Between September 2006 and June 2008, clopidogrel resistance test was performed on 41 patients who underwent stent-assisted angioplasty for atherosclerotic cerebrovascular disease. It was performed before drug administration and about 12 hours after drug administration (loading dose : 300 mg, maintain dose : 75 mg). Two patients were excluded, and 41 patients were included (mean : $67.59{\pm}7.10$ years, age range : 41-79). Among 41 patients, 18 patients had intracranial lesions, and 23 had extracranial lesions. We evaluated the prevalence, risk factors and complications related to clopidogrel resistance. Results : Twenty-one patients (51.2%) showed clopidogrel resistance [intracranial : 10 patients (55.6%), extracranial : 11 patients (47.8%)] and no clopidogrel resistance was seen in 20 patients. Hypercholesterolemia was an indepedent risk factor of clopidogrel resistance. Stent-assisted angioplasty was technically successful in all patients, but acute in-stent thrombosis occurred in 5 patients with intracranial lesions (4 patients with clopidogrel resistance and 1 without clopidogrel resistance). Acute thrombi were completely lysed after intra-arterial infusion of abciximab. Conclusion : There was relatively high prevalence of clopidogrel resistance in patients with atherosclerotic cerebrovascular disease. Hypercholesterolemia was an independent predictive factor of clopidogrel resistance. Acute in-stent thrombosis was more frequently seen in the clopidogrel resistant group. Therefore, clopidogrel resistance test should be performed to avoid thromboembolic complications related to stent-assisted angioplasty for atherosclerotic cerebrovascular disease, especially patients with hypercholeterolemia and intracranial lesion.

HPV Genotyping Linear Assay Test Comparison in Cervical Cancer Patients: Implications for HPV Prevalence and Molecular Epidemiology in a Limited-resource Area in Bandung, Indonesia

  • Background: Persistent infection with high risk human papillomavirus (hrHPV) is strongly associated with cervical cancer. Normal cervical cells may also harbor hrHPV, and detection of early hrHPV infection may minimize risk of cervical cancer development. This study aimed to compare two commercial HPV genotyping assays that may affordable for early screening in a limited-resource setting in Bandung, Indonesia. Materials and Methods: DNA from cervical biopsies with histologically confirmed as squamous cell cervical cacinoma were HPV genotyped by Linear Assay 1 (Roche Diagnostics, Mannheim, Germany) or Linear Assay 2 (Digene HPV Genotyping RH Test, Qiagen Gaithersburg, MD). In a subset of samples of each group, HPV genotype results were then compared. Results: Of 28 samples genotyped by linear assay 1, 22 (78.6%) demonstrated multiple infections with HPV-16 and other hrHPV types 18, 45 and/or 52. In another set of 38 samples genotyped by linear assay 2, 28 (68.4%) were mostly single infections by hrHPV type 16 or 18. Interestingly, 4 samples that had been tested by both kits showed discordant results. Conclusions: In a limited-resource area such as in Indonesia, country with a high prevalence of HPV infection a reliable cervical screening test in general population for early hrHPV detection is needed. Geographical variation in HPV genotyping result might have impacts for HPV prevalence and molecular epidemiology as the distribution in HPV genotypes should give clear information to assess the impact of HPV prophylactic vaccines.

A Study on the Development of Low Back Pain and the Risk Factors of Manufacturing Workers (일부 제조업 근로자들의 요통유병률과 요인에 관한 조사)

  • This study was conducted to investigate the prevalence rate of low back pain during past one year and its risk factors. The data were collected from 1,384 manufacturing factory workers from March 1, 1992 to August 30, 1992. The results obtained were as follows: 1. The one year prevalence rate of low back pain by manufacturing company was 38.4% in textile manufacturing, 35.2% in concrete reenforcement, 31.0% in cigarette and 26.1% in metal part(P<0.01). 2. In textile manufacturing, age groups with high prevalence rate of low back pain were 30's(36.8%) and 40's(36.4%) (P<0.05), and the one year prevelence rates of low back pain by marital state were 34.9% in married workers and 28.5% in unmarried(P=0.0511). 3. The one year prevelence rates of low back pain by job part are 35.0% in labor workers and 26.1% in clerical(P<0.05), and by work hour per day it was 34.9% in 9 or more and 28.9% in 8 or less(P<0.05). 4. The highest group of low back pain by work posture was 43.5% in 'lifting and transfering materials', and it by fitness of chair was 56.0% in uncomfortable one, and by height of working board was 33.6% in low working one. 5. In logistic regression analysis, significant determinants with low back pain were marital state(p<0.05), work hour per day(P<0.05), height of working board(P<0.01), and work posture(P<0.01).

A Study of Health Related Factors and Food Habits During Pregnancy of Full-term and Preterm Delivery (만기분만과 조기분만 산모의 임신 중 건강관련요인과 식습관에 관한 연구)

  • In Korea, delivery of preterm has increased gradually, preterm delivery rate was 6.4% in 1995 and 9.8% in 2002. The purpose of this study is to provide guidelines for preventing preterm delivery in aspect of health factors and food habits. The health factors and dietary habits were compared between preterm delivery group and full-term delivery group on to recognize risk factor of delivering premature. The results obtained are summarized as follows. The full-term delivery group showed a high rate of professionals and the preterm delivery group showed a high rate of the service industry, showing differences in kinds of occupation(p<0.05). Heights were higher in full-term delivery group(p<0.05). Among the nutrition supplements, iron supplement consumption was the most, period of the intake of iron was significantly longer for the full-term delivery group than for the preterm delivery group(p<0.05). Also, prevalence of coffee was higher in preterm delivery group(p<0.001). The activity level was higher in full-term delivery group(p<0.005).

Risk Factors Associated with Cataract by in Middle-aged and Older Korean Adults (한국인의 장년층과 노년층의 백내장 위험 요인)

  • Purpose: This study was found the risk factors of cataract by gender in elder and older Korean adults. Methods: We investigated the data for 5,024 (men 2,163 people, women 2,861 people) people, aged 40-95 years, from the Korean National Health and Nutrition Examination Survey. We divided into two groups (group 1: 40-64 years, group 2: 65-94 years). We divided into two groups (group 1: 40-64 years, group 2: 65-94 years). Cataract was decided in case of at least one eye with lens opacity. We presented significant odds ratio (OR) increase of cataract according to the obesity and socioeconomic information including house income and education level after adjusting for diabetes, hypertension, high triglyceride, outside active, smoking and drinking habits in two groups with distinction of sex. Results: House income (or education) was strongly related to the prevalence of agerelated cataract in all groups. In models adjusting for all other related factors, cataract patients with lower income and education had a higher prevalence of cataract (group1: OR, 1.84[1.17-2.91], OR, 3.00[1.90-4.74], group 2: OR, 3.47[2.53-4.74], OR, 7.44[5.41-10.23] for men. In comparison, for women, cataract patients with lower income, lower education and obesity had a higher prevalence of cataract (group1: OR, 1.72[1.14-2.60], OR, 4.48[2.72-7.38], OR, 1.40[1.10-1.82], group 2: OR, 2.32[1.70-3.16], OR, 29.99[20.31-44.28], OR, 1.27[1.04-1.54]). Conclusions: Factors of low socioeconomic status were associated with age-related cataract in Korean. Obesity-cataract association was stronger in women.

Health and risk taking behaviors of freshmen in college (대학교 신입생들의 건강위험행태)

  • Purpose : This study was conducted to survey the preliminary data on risk behaviors and to identify the factors that prevent risk-behaviors in late adolescence. Methods : Freshmen(n=1,297) beginning the first semester in Korea University, Seoul, Korea completed self-administered risk behavior questionnaires, comprising 5 domains : demographics, smoking, drinking, drug abuse and sexual behavior. Results : The rate of smoking experience was higher in people having friends who smoke and the predictors of transition to current smoking were male gender, urban residence, friends' smoking, and nicotine dependence. The rate of high risk drinking was higher among students who are male and who had experience of heavy episodic drinking. The study group showed a low prevalence of narcotic users, but two-thirds of students could get medicine easily without prescriptions. The prevalence of sexual experience was 6.5 percent, and the sexual education was not a predictor of contraceptive behavior. The prevalence of homosexuality was 1.6 percent, and the rate of mostly heterosexuality was higher in female students. Conclusion : The main targets of youth health education should be campaigns aimed at atcessation of reinforcing risk behaviors and the development of a surveillance system for the prevention of chronic disease. These results can be used to find risk factors of health-risk behaviors among late adolescents.

Interreationship between the Subclinical Hypothyroidism and Serum Total Cholesterol, LDL-Cholesterol (불현성 갑상선 기능저하증과 혈청 총 콜레스테롤, 저밀도 콜레스테롤 농도와의 상관성)

  • This study was carried out in 14,443 subjects(8,706 men, 5,737 women, mean age $41.8{\pm}9.16$) who visited the health promotion center of one university hospital. Serum $T_3$, $T_4$ and TSH concentrations were measured with CIA using commercial kits on ADVIA Centaur Assay and Serum total cholesterol, triglyceride, and HDL-cholesterol were measured by auto-analyzer Hitach 7600 in enzyme method. 1. The prevalence of subclinical hypothyroidism was 12.6/1,000 population(men 6.0/1,000, women 22.7/1,000) and as high as about 4 times in women than in men. 2. In subclinical hypothyroidism, the values of total cholesterol and LDL-cholesterol were significantly high by 5.5 mg/dL, 4.6 mg/dL compared with normal group(p<0.05). 3. In all the tested hypercholesterolemia and hyperLDL-cholesterolemia, were correlated with TSH. There is higher correlation in women group than men group. These results indicate that the subclinical hypothyroidism and dyslipemia significantly correlated. Especially, it is very important to find that the hypercholesterolemia and hyperLDL-cholesterolemia are the high risk factors of atherosclerosis. Therefore, it is essential for the group of subclinical hypothyroidism to take the cholesterol, LDL-cholesterol and serum lipid test as well as thyroid function test.

School Safety Education Factors Predicting Injury Prevalence Among Korean Adolescence (학교의 안전교육 관련 특성이 청소년의 사고발생 예측에 미치는 영향)

  • Injury is a leading cause of death in the children and adolescent populations. In particular, more than 80% of unintentional injury was related to risk-taking behaviors involved in diverse accidents around school and home. Therefore, educational approaches should be provided for children and adolescent populations, and schools are the essential and appropriate sites to conduct safety education. This study was conducted to identify injury prevalence and safety education at schools among middle and high school students in Korea. About 1,034 middle and high students in 28 schools participated in a self-administered survey. The target schools were selected from the stratified random sampling method throughout schools of seven metropolitan cities in Korea. The questionnaires were delivered to the vice-principals by ground mailing service and the vice-principals administered survey data collection. The questionnaire asked about safety education provided in schools, injury experience in the last year, needs for injury prevention class in school, and demographics. All survey responses were entered into SPSS worksheet. Multivariate analysis of variance (MANOVA) and descriptive discriminant analysis (DDA) were used in statistical analysis with SPSS software 11.1. Multivariate analysis of variance was conducted as a preliminary analysis of DDA. According to the result of multivariate analysis of variance, gender (man), grade (poor), living with both parents, and displaying injury prevention messages on school news board were significantly different between the injured student group and the uninjured student group (p= .00). These four factors also had significant effects on students' injury experience in DDA, although correlation of the four factors with injury experience was weak overall based on their canonical function coefficients. All structure coefficients of the four factors were greater than .30, which means the four factors have discriminant effects on injury prevalence. The sizes of the discriminant effects, in order, were largly from gender, grade, living with both parents, and safety message display on school news boards.

  • Cancer of the cervix is the third most common cancer in women worldwide, more than 85% of the cases occurring in developing countries such as China. In China, since a national cancer registry is already set up but with geographically limited data generated, the burden of cervical cancer is believed to be underestimated. High-risk human papillomavirus (HR-HPV) prevalence among women attending routine cervical cancer screening programs has been shown to correlate well with cervical cancer incidence rates based on independently obtained HPV prevalence data as well as findings for the worldwide cervical cancer burden. Therefore, reviewing data on HR-HPV prevalence in population-based screening studies and hospital-based case studies will be important in the context of better understanding the cervical cancer burden and for the evaluation of the potential impact of HPV vaccination in the country. With the advent of prophylactic vaccines, significant progress is likely to be made in cervical cancer prevention. This article reviews available data on the HPV epidemiology over a 12-year time period (2001-2012) in mainland China under different epidemiological aspects: by age group of study population, by ethnicity, by geographic area, as well as time period. The authors also review the potential acceptability of HPV vaccination among Chinese women.

Seasonal Prevalence of Mosquitoes, Including Vectors of Brugian Filariasis, in Southern Islands of the Republic of Korea

  • A survey of mosquitoes, including the vector status of Brugia malayi filariasis and their relative larval density, was conducted from 2002 to 2005 at several southern remote islands of Jeollanam-do (province), Gyeongsangnam-do, and Jeju-do, Korea, where filariasis was previously endemic. Overall, a total of 9 species belonging to 7 genera were collected. Ochlerotatus togoi (formerly known as Aedes togoi), Anopheles (Hyrcanus) group, and Culex pipiens were the predominant species captured at all areas. Oc. togoi larvae were most frequently collected at salinity levels < 0.5% during June and July, with densities decreasing sharply during the rainy season in August. The most likely explanation for the eradication of filariasis in these areas is suggested to be an aggressive treatment program executed during the 1970s and the 1990s. However, high prevalence of the vector mosquitoes may constitute a potential risk for reemerging of brugian filariasis in these areas.
