Stictodoyu sp. (Trematoda; Heterophyidae)의 인체기생 1례 (Stictodora sp. (Trematoda: Heterophyidae) Recovered from a Man in Korea)

  • 이형흡충류의 하나인 Stictodora속 흡충 2마리가 구충제 및 하제를 투여한 24세 남자환자의 설 사변에서 검출되었다. 총체는 길이 0.90∼0.98 mm, 폭 0.38 mm로 소형이며 특이한 복흡반-생식만(ventrogenital) 장치가 있고 생식공과 별개인 gonotyl(12∼15개의 미소한 갈고리 보유)을 가지고 있었다. 충체는 형태학적으로 Stictodora fuscatum(Onji and Nishio, 1916)과 매우 비슷하였으나 다수의 충체를 획득할 때까지 확정적인 종 동정은 보류하였다. 환자는 숭어, 농어 등 반염수산 어류를 즐겨 생식한다고 하였다. 이들이 Stirtodora sp.는 물론 따로 보고한 3종류 이형홉충의 감염원이었을 것으로 추측된다. 이 중례는 문헌상 Stictodora속 홉충의 최초 인체기생례에 해당된다.

남해만 숭어(崇漁)에서 검출된 Stellantchusmus fulcutus 피낭유충의 동정 (Identification of Snellantchasmus falcatus Metacercariae Encysted in Mullets in Korea)

  • 경상남도 남해 지방산 숭어에서 Stellantchasmus falcatus의 길낭유충을 발견하고 횐쥐에 감염시킨 후 성충을 얻어 동정하였다. 검사한 숭어는 모두 10마리로서 크기 20~30cm이었고 그중 3마리에서 S. falcatus로 생각되는 퍼낭유충이 470개 검출되었다. 피낭유충은 원형 또는 타원형으로 크기 $0.15~0.20{\times}0.13~0.19mm$이었고 대부분이 숭어의 근육에서 발견되었다. 총 280개의 피낭유충을 횐쥐 5마리에 각각 30~100개씩 나누어 감염시키고 7~10일후에 흰쥐를 희생시켜 소장(소장) 내용물을 검경한 바 총 83마리의 성충이 회수되었다. 회수된 충체는 길이 0.41~0.70mm, 폭 0.32~0.39mm이었고, 장낭형(장낭형 ; elongated sac-like)의 expulsor를 가진 저정낭(seminal vesicle) 등 몇가지 형태학적 특징을 근거로 S. ialcatus Onji et Nishio, 1915로 동정되었다. 이 연구로 우리나라에서도 숭어가 S. falcatus의 제2중간숙주역할을 하고 있음이 확인되었고 이 홉충의 인체감염원이 되고 있을 것으로 추측되었다.

Echinostoma hortense and Heterophyid Metacercariae Encysted in Yellowfin Goby, Acanthogobius flavimanus, from Shinan-gun and Muan-gun (Jeollanam-do), Korea

  • Fishborne trematode (FBT) metacercariae were investigated in yellowfin goby, Acanthogobius flavimanus, collected from Shinan-gun and Muan-gun, Jeollanam-do (province), Korea. All collected fishes were examined using the artificial digestion method. In all of 15 gobies from Aphae-myeon in Shinan-gun, metacercariae of Stictodora spp. (334 metacercariae/fish), Heterophyes nocens (153/fish), and Heterophyopsis continua (20/fish) were detected. In 2 of 14 gobies from Jido-myeon in Shinan-gun, 8 Echinostoma hortense metacercariae in total were detected. In 15 gobies from Haeje-myeon in Muan-gun, the metacercariae of H. continua were found in 100%, Stictodora spp. in 86.7%, and H. nocens in 6.7% of fish examined. The average numbers of metacercariae per infected fish were 23.3 (H. continua), 416.0 (Stictodora spp.), and 2.0(H. nocens), respectively. The metacercariae of E. hortense found in gobies were elliptical, with 150 ${\times}$ 138 ${\mu}m$, in average size, and had 27 collar spines on the head crown. The above results suggest that yellowfin gobies from 2 localities may be the potential infection sources of FBT. Moreover, it is proved for the first time that the yellowfin goby, A. flavimanus, acts as a second intermediate host for E. hortense.

Surface ultrastructure of Pygidiopsis summa (Digenea: Heterophyidae) adult flukes

  • A scanning electron microscopic study was performed on the surface ultrastructure of Pygidiopsis summa (Digenea: Heterophyidae) adults. Metacercariae were collected from gills and muscles of mullets (Mugil cephalus) caught in a known endemic area, and adult flukes were harvested from dogs after 8 weeks of experimental infection. The worm was calabash form with its posterior part broader than the anterior part. Tegumental spines were densely distributed over the body surface, except on the suckers and genital apparatus, and around the excretory pore. Well differentiated spines were observed on the anterior half of the body, with 14-16 tips ventrally, and 19-20 tips dorsally. On the oral sucker. three pairs of type I sensory papillae (uni-ciliated knob-like swellings) and one pair of type II sensory papillae (aciliated round-swellings) were observed on the anterior and posterior parts of the lip, respectively. On the lip of the ventral sucker, one pair of type II sensory papillae was distributed only on its posterior part. Sperms were seen emerging from or entering into the genital apparatus. The results showed that the surface ultrastructure of P. summa was unique among the heterophyid trematodes, especially in digitation of tegumental spines and in distribution of sensory papillae on oral and ventral suckers.

한국의 간흡충에 관한 연구 XI. Heterophyes heterphyes nocens의 인체감염 2례 (Studies on Intestinal Trematodes in Korea XI. Two Cases of Human Infection by Heterphyes heterphyes nocens)

  • 이형흡충류의 하나인 Heterophyes heterophkes nocens에 의한 인체감염 2례가 치료후 충체를 얻세(제1예) 및 24세(제2예) 남자로 전남 고흥군 해안마을이 고향이거나 현재 거주지이며 제1예는 상복부 통증 및 소화불량 등의 증상을, 제2예는 EKG검사상 불정맥 및 ventricular premature be각 등 심장 기능이상과 설사, 복통 등 소화기 증상을 호소하였다. 1983연 4월 및 8월에 bithionol 30mg/kg 또는 prasiquantel 15mg/kg을 투여하고 설사편을 입체해부현미경하에서 검사한 바 각각 35및 67마리의 H. heterophyes noces가 수집되었다. 대편검사 또는 충체검사상 제 1예는 간흡충이, 제2예는 3종의 다른 이형흡충류와 광절렬두조충이 동시 감염되어 있었다. 환자들은 H. heterophyes nocens의 중간숙주인 숭어 (Mugil cephalus), 농어 (Lateolalabrax japonicus) 및 문절망둑(Acanthogobius navimanus) 등 반염수산 어류를 회로 먹은 정력이 있었다.

Centrocestus formosanus (Digenea: Heterophyidae) Encysted in the Freshwater Fish, Puntius brevis, from Lao PDR

  • The metacercariae of Centrocestus formosanus, a minute intestinal trematode of mammals and birds, were detected in the freshwater fish, Puntius brevis, from Vientiane Municipality, Lao PDR. The metacercariae were experimentally fed to mice, and adult flukes were recovered in their small intestines 7 days later. The adult flukes were morphologically characterized by having 32 (rarely 34) circumoral spines arranged in 2 alternative rows, a large bipartite seminal vesicle, an oval-shaped ovary, and an X-shaped excretory bladder. Based on these characters, the adults were identified as Centrocestus formosanus (Nishigori, 1924). The taxonomic significance of C. formosanus, in relation to a closely related species, C. caninus (Leiper, 1913), is briefly discussed. It has been first verified by adult worm recovery that C. formosanus is prevalent in Vientiane areas of Lao PDR, taking the freshwater fish, P. brevis, as a second intermediate host.

Intestinal Parasite Infections among Inhabitants in Yanbian Prefecture, Jilin Province, China

  • To investigate the prevalence of intestinal parasite infections in Yanbian Prefecture, Jilin Province, China, epidemiological surveys were conducted on a collaboration basis between the Korean Centers for Disease Control and Prevention and the Yanbian Center for Disease Control and Prevention. A total of 8,396 (males 3,737 and females 4,659) stool samples were collected from 8 localities and examined with the formalin-ether sedimentation technique, and additionally examined with the cellotape anal swab to detect Enterobius vermicularis eggs. The overall rate of intestinal parasites was 1.57%. The prevalence of Ascaris lumbricoides was the highest (0.80%), followed by Entamoeba spp. (0.23%), heterophyid flukes (0.15%), Clonorchis sinensis (0.08%), Enterobius vermicularis (0.07%), hookworms (0.06%), Trichostrongylus spp. (0.06%), Giardia lamblia (0.04%), Paragonimus spp. (0.02%), Diphyllobothrium spp. (0.02%), Trichuris trichiura (0.02%). The prevalence by sex was similar, 1.58% (n=59) in males and 1.57% (n=73) in females. By the present study, it is partly revealed that the prevalences of intestinal parasite infections are relatively low among the inhabitants of Yanbian Prefecture, Jilin Province, China.

Infection Status of Estuarine Fish and Oysters with Intestinal Fluke Metacercariae in Muan-gun, Jeollanam-do, Korea

  • The source of human infection with intestinal flukes was surveyed in estuarine fishes, including the dotted gizzard shad, common sea bass, common blackish goby, redlip mullet, black sea bream, and oyster collected from Muan-gun, Jeollanam-do, Korea during August and September 2007. Collected fishes and oysters were artificially digested in pepsin-HCl solution and examined under a stereomicroscope. In 36 shads (Konosirus punctatus) and 20 basses (Lateolabrax japonicus) examined, Heterophyopsis continua metacercariae were found in 58.3% and 100%, and their average numbers were 12.0 and 6.3 per infected fish, respectively. In 34 gobies (Acanthogobius flavimanus) examined, metacercariae of H. continua were detected in 79.4%, Stictodora lari in 97.1%, and Acanthotrema felis in 92.1%, and their average numbers were 45.8, 189.3, and 235.3 per infected fish, respectively. In 37 redlip mullets (Chelon haematocheilus), Heterophyes nocens metacercariae were found in 56.8%, Pygidiopsis summa in 94.6%, and Stictodora fuscata in 45.9%, and the average metacercarial densities were 17.4, 31.3, and 35.1 per infected fish, respectively. In 30 black sea breams (Acanthopagrus schlegeli) and 45 oysters (Crassostrea gigas) examined, no metacercariae were detected. From the above results, it has been confirmed that the dotted gizzard shad, common sea bass, common blackish goby, and redlip mullet from Muan-gun, Jeollanam-do, Korea are infected with the metacercariae of heterophyid flukes.

Potential Zoonotic Trematodes Recovered in Stray Cats from Kuwait Municipality, Kuwait

  • Stray cats are a common feature roaming the streets and alleys of Kuwait; they could be a source of parasites, including trematodes, that affect humans. A survey was conducted to identify feline trematodes and throw the light on their public health significance in Kuwait. Out of 240 stray cats trapped from different localities of Kuwait from June 2011 to May 2012, 59 (24.6%) were found to be infected with 14 species of trematodes. The most common were trematodes of the genus Heterophyes, particularly H. heterophyes and H. dispar that were found in respectively 15.8% and 10.8% of the cats examined. Other trematodes recorded, with lower prevalences, were Heterophyes nocens (2.9%), Haplorchis taichui (3.8%), Stictodora sawakinensis (2.1%), Stellantchasmus falcatus (1.6%), Echinochasmus japonicus (1.6%), and Mesostephanus dottrensi (1.3%). Centrocestus cuspidatus, Galactosomum fregatae, Ascocotyle sp., Mesostephanus appendiculatus, Haplorchis yokogawai, and Pygidiopsis genata showed the lowest prevalence (0.4%) and intensity. The majority of the trematodes are recorded for the first time in Kuwait and even in the Gulf region. The study reveals that stray cats are good indicators of fish-borne trematodes in the environment. As all trematodes recovered are zoonotic, their significance to public health should be considred.

Prevalence of Gymnophalloides seoi infection in coastal villages of Haenam-gun and Yeongam-gun, Republic of Korea

  • One coastal village in Haenam-gun and two in Yeongam-gun, Jeollanam-do were surveyed for intestinal parasite infections by fecal examination. The egg positive rates of Gymnophalloides seoi were high, $24.1\%$ (14/58) in Haenam-gun and $9.3\%$ (11/118) in Yeongam-gun. The egg positive rates of heterophyids, including Heterophyes nocens, and of Clonorchis sinensis were $10.3\%\;and\;6.9\%$ in Haenam-gun, and $14.4\%\;and\;8.5\%$ in Yeongam-gun, respectively. After praziquantel treatment and purgation, a total of 37,761 fluke specimens were recovered from 17 patients; 11 in Haenam-gun and 6 in Yeongam-gun. Gymnophalloides seoi was the most commonly recovered species, with 37,489 specimens in total (2,205 per person). Other recovered flukes included Heterophyes nocens, Stictodora fuscata, Heterophyopsis continua, Pygidiopsis sUmma, and undetermined species. These results indicate that the areas surveyed are new endemic foci of G. seoi.
