현곡(玄谷) 사간탕(瀉肝湯)의 구성한약과 그 기미배오(氣味配伍) 분석 (Hyeongok's Sagan-tang was Combined according to the Theory for Properties and Tastes of Herbal Medicines)

  • There are four kinds of formulas for purging the liver to cure its sthenic syndrome based on the types of preparation formulas : Sagan-tang, Saganhwan, Saseem-san and Saganeum. Another formula called Sacheonghwan, Sacheong-tang and Sacheong-san is to purge the green colour of liver. There are 38 kinds of Sagan-tang, 2 kinds of Saganhwan, 29 kinds of Sagan-san, 5 kinds of Saganeum, 4 kinds of Sacheonghwan, 3 kinds of Sacheong-tang and 1 kind of Sacheong-san. Combination of herbal medicines, carried out in formulas for purging the liver, consists of various kinds depending on medical scientists' personal experience in medical treatment without any general principles, which makes it difficult to apply it to clinical use. The objectives of this study lie in theoretical establishment of Sagan-tang for curing the sthenic syndrome of liver through analyzing the component medicines and combination principles of Hyeongok's Sagan-tang, and furthermore, maximizing the clinical use of Sagan-tang. This study analyzed the component medicines and combination principles of Hyeongok's Sagan-tang based on the theory for properties and tastes of herbal medicines from the ${\ulcorner}$Yellow Emperor's Canon of Internal Medicine${\lrcorner}$, the theory for principal herbal medicine, assistant herbal medicine, adjuvant herbal medicine, dispatcher herbal medicine, and the five elements doctrine. Hyeongok's Sagan-tang is an odd prescription, composed of 7 kinds of ingredients : No.1 Radix Paeoniae (2don;7.5 g), No.2 Fructus Chaenomelis (1don;3.75 g), No.3 Radix Rehmanniae (1don), No.4 Folium Phyllostachydis Henonis (1don), No.5 Radix Bupleuri (1don), No.6 Radix Scutellariae (1don), and No.7 Radix Glycyrrhizae (1don). There are three methods for curing the sthenic syndrome of liver according to the five elements doctrine : purging the liver, purging the heart and invigorating the lung. In the case of taste purgation, two herbal medicines with sour taste, Radix Paeoniae and Fructus Chaenomelis, are combined into the principal and assistant herbal medicine, respectively. For property purgation, two herbal medicines with the cool property, Radix Bupleuri and Radix Scutellariae, are combined into adjuvant herbal medicines. Both sweet and cold herbal medicines, Radix Rehmanniae and Folium Phyllostachydis Henonis, are combined into adjuvant herbal medicines. Sour herbal medicines, Radix Paeoniae and Fructus Chaenomelis, were combined to invigorate the lung. Cool herbal medicines, Radix Bupleuri and Radix Scutellariae, were combined to invigorate the lung and to purge the liver. In addition, Radix Glycyrrhizae are combined as dispatcher herbal medicine, harmonizing all the herbal medicines composing the formula. First, to cure the sthenic syndrome of the liver, the methods of purging the liver and the heart, and invigorating the lung should be used according to the five elements doctrine. Secondly, herbal medicines appropriate for those treatment methods should be chosen according to the theory for properties and tastes of herbal medicine and thirdly, the combination of those herbal medicines should be carried out according to the theory for principal herbal medicine, assistant herbal medicine, adjuvant herbal medicine, dispatcher herbal medicine. As a good example, Hyeongok's Sagan-tang is combined according to the above theories.

The importance of assessing heavy metals in medicinal herbs: a quantitative study

  • Consumption of herbal products from the medicinal plants contaminated with heavy metals can cause serious consequences on human health. This is a major concern for traditional and herbal medicine. The present study was carried out to analyze and quantify the levels of six potentially toxic heavy metals namely arsenic, lead, cadmium, mercury, chromium and nickel in ten important Indian medicinal herbs. The air dried raw herbs were processed by microwave assisted wet digestion and analyzed by using atomic absorption spectrophotometer equipped with graphite tube atomizer. Except the chromium content in three plants, all the levels of six heavy metals analyzed were found to be quite below the permissible limits in all the ten raw medicinal herbs analyzed. The present work implies that, regular and systematic screening of raw medicinal herbs is necessary to check the levels of the heavy metal contaminants before using them for consumption or preparation of herbal formulations so that the possible contamination cannot cumulate up to the finished products.

생약제제의 등록규정 차별화에 관한 연구 (A Study on the Distinction of Registration Regulations for Herbal Medicines)

  • Herbal medicines have been used since ancient times as medicines to treat and relieve the symptoms of many different human diseases. However, so far, relatively few herbal medicines have been evaluated scientifically to prove their safety, potential benefits and effectiveness. This study was conducted to provide the groundwork for improving the current registration regulations for herbal medicines in distinction from synthetic medicines. The study was performed based on the literature research and individual interviews with 5 experts who had extensive experience in registration of herbal medicines. When compared with synthetic drugs, herbal medicines exhibit some marked differences, namely the active principles are frequently unknown, standardization, stability and quality control are not easy, they are usually mixtures of complex compounds. Second, the current regulations for herbal medicines are reviewed by comparison of foreign regulation systems like the one in China. The regulation requirements of herbal medicine in China are in distinction from synthetic drugs. The authors conclude that new registration requirements for the herbal medicines should be changed as follows; the toxicity and efficacy data should be submitted as mixed herbal preparation and the documents and other research on the reproduction and generation toxicity need to be shown for the proof of reproduction and generation toxicity, if needed.

만성기침에 대한 보조제제로써 청인(淸咽)트로키 복합투여의 임상적 유효성 평가 (Clinical Effects of Chungin-troche As an Adjuctive Medicine with Chronic Cough Patients)

  • Objectives : We designed this study to evaluate the effectiveness of Chungin-troche(CIT) preparation as an adjunctive medicine to a herbal extract of Chungsangboha-tanggagambang(CSBH). Methods : A paralled, comparative study of CSBH+CIT and CSBH was conducted on 42 patients of chronic cough who attended Division of Allergy, Immune & Respiratory System, Department of Internal Medicine, College of Oriental Medicine, Kyung Hee University between December, 2007 and June, 2008. The treatment in each group lasted for two weeks. Patients were asked to fill in the Leicester Cough Questionnaire (LCQ) before and after administration of each treatment legimen. Results : 42 patients completed the trial and no adverse effects were reported. Both CSBH +CIT group and CSBH group showed statistical significant improvement in total LCQ score after 2 weeks from baseline. Social domain score and total score in patients taking CSBH+CIT showed a statistical significant increase compared to those scores in CSBH patients. Conclusion : CSBH+CIT compositive preparation and CSBH separate preparation appeared to have some benefits in the treatment of chronic cough. However, additional CIT preparation to CSBH can improve social domain in LCQ of chronic cough patients.

곤충 유래 한약재를 활용한 피부질환 개선 기능성 소재에 대한 특허 동향 분석 (Analysis of trends in patents on insect-derived medicinal materials for skin diseases)

  • Objective: In this study, we analyzed the trend of patents registered prior to October 4, 2019, to understand the industrial trends in insect-derived medicinal materials used for the treatment of skin diseases. Methods: Using the WIPSON database, we collected information regarding the patents related to insect-derived materials for treating various skin diseases. Results: The patents registered prior to October 4, 2019, from Korea, Japan, the USA, China, and the EU, along with those registered under PCT were selected. There were 195 patents related to the use of insect-derived medicinal materials in treating various skin diseases such as psoriasis, inflammatory skin diseases, eczema, pruritus, and atopic dermatitis. China is mostly superior in total number of registered patents compared with the other countries. Korea was the major patent technology-holder for atopic dermatitis, but China dominated in the remaining categories of skin diseases. Upon first patent registration in 1992, there had been a continual increase in the number of patents. Especially, patents related to eczema, psoriasis, inflammatory skin disease were markedly increased. Most frequently used insect-derived medicinal materials was Scolopendra, Cicadidae Periostracum, Scorpions, Cantharides, and Batryticatus Bombyx. The insect-derived medicinal materials were generally used as a combined preparation with other medicinal materials in patents. Conclusion: This study could help to establish the basis for future research and development related treating skin diseases using insect-derived medicinal materials. In order to provide sufficient data, further study including analysis of rejected patents is needed.

  • Herbal medicines have an accurate effect on atopic dermatitis, and at the same time, they hardly have adverse reactions. However, herbal medicines are difficult to be quantitatively analyzed due to low-quality preparation and types. In addition, herbal medicines have raised many problems as they have not been standardized and their active components have not been analyzed. Patients with atopic dermatitis began to recognize the effectivity and safety of herbal medicines. Accordingly, standardization, biological analyses, animal experiments and clinical trials should be generally performed in order that herbal medicines may be recognized all over the world. A standard, which is to objectively judge the curative effect of atopic dermatitis, should be established as soon as possible. Case studies and RCTs (Randomized Controlled Trials) should be actively performed on the basis of rigid clinical trial design to the end that the curative effect of herbal medicines is recognized all the world over.

한의학 외치법과 관련된 국내 실험 연구 고찰 (Trend of Domestic Research about Experimental Studies of External Treatment in Korean Medicine)

  • Objectives : The purpose of this study is to analyze the trends of domestic experimental research of external treatment in Korean medicine and use it as a basic data to expand the therapeutic field of Korean medicine. Methods : In December 2016, two authors searched for experimental studies of external treatment in Korean medicine researches in 6 Korean databases. Results : Among the 379 researches that were initially searched, two authors reviewed 190 researches, excluding duplicate researches, and finally 11 researches were selected for the study. The target disease was confined to the skin and mucosa, and the most of selected Korean medicine was based on the literature. Conclusions : Considering the range of application of the external treatment described in the literature and the domestic market for external medicine, it is expected that the development of various external medicine using herbal medicine will be expected in the future.

바이칼린 함유 생약의 제제화 및 생체이용률 (제3보) : 황금 및 황련 공침물의 포접화합물 제조 및 생체이용률에 관한 연구 (Preparation and Bioavailability of Oriental Medicine Containing Baicalin (III) : Preparation of Inclusion Complex and Bioavailability of Coprecipitated Product of Scutellariae Radix and Coptidis Rhizoma)

  • Precipitation was formed during the preparation of decoction from a mixture of Scutellariae Radix and Coptidis Rhizoma. Baicalin and berberine were identified in this coprecipitated product (CPP) and these components were the active ingredients of two herbal medicine. We extracted respectively crude baicalin and berberine in Scutellariae Radix and Coptidis Rhizoma and prepared coprecipitate of crude baicalin-berberine. To increase the stability and bioavailability of coprecipitate of crude baicalin-berberine(CBB), which is slightly soluble drug, its inclusion complex was prepared and studied in this experiment. Inclusion complex of CBB with ${\beta}-cyclodextrin(CBB-{\beta}-CD)$ was prepared by freeze drying method and its characteristics were ascertained by means of solubility test, differential thermal analysis(DTA) and scanning electron microscope(SEM). The type of $CBB-{\beta}-CD$ is classified as $A_L-type$ on phase solubility diagram, and the stoichiometric ratio of CBB(baicalin in CBB) : ${\beta}-CD$ complex is 1:1 and formation constant is 151 $M^-1$. The solubility, dissolution, in situ absorption and serum concentration of $CBB-{\beta}-CD$ were significantly increased when compared to CBB. Therefore enhanced bioavailability of CBB by inclusion complexation with ${\beta}-cyclodextrin$ might be useful for dosage form design of active ingredients of two herbal medicine.

Trinitrobenzene sulfonic acid에 의해 유발된 랫드의 대장염에서 HemoHIM의 항염증 효과 (Anti-inflammatory activities of a herbal preparation (HemoHIM) in colitis induced by trinitrobenzene sulfonic acid in rats)

  • The cause and pathogenesis of inflammatory bowel disease remain unknown and no definitetherapy exists until now. The present study was conducted to investigate the anti-inflammatory effectsof a herbal preparation (HemoHIM) in colitis induced by 30 mg of trinitrobenzene sulfonic acid (TNBS)in rats. Sprague-Dawley rats were divided into 5 groups. Each group was treated with 1 mg ofHemoHIM/ml of drinking water, 4 mg of HemoHIM/ml of drinking water, 50 mg of HemoHIM/kgof body weight (i.p. once every other day) or 10 mg/kg of HemoHIMof body weight (i.p. onceevery other day) from the next day. After 2 weeks, rats were sacrificed and morphologic featuresof colons were examined. Ulceration, adhesion, thickening and dilatation were noticed in the colonicmucosa after TNBS instillation. Intraperitoneal injection of HemoHIM (50 and 100 mg/kg of bodyweight) showed the anti-inflammatory effect on adhesion, thickening, dilatation, ulceration, and theinhibition effect on damage score by 72.7% and 90.9%, respectively. Histologically, the colon of TNBS-treated rat showed inflammatory cell infiltration by polymorphonuclear cells, multiple erosive lesionsignificant improvement in these symptoms. The results obtained suggest marked anti-inflamatoryactivity of the HemoHIM at the dose levels examined.

HemoHIM, A herbal preparation, alleviates airway inflammation caused by cigarette smoke and lipopolysaccharide

  • HemoHIM, herbal preparation has designed for immune system recovery. We investigated the anti-inflammatory effect of HemoHIM on cigarette smoke (CS) and lipopolysaccharide (LPS) induced chronic obstructive pulmonary disease (COPD) mouse model. To induce COPD, C57BL/6 mice were exposed to CS for 1 h per day (eight cigarettes per day) for 4 weeks and intranasally received LPS on day 26. HemoHIM was administrated to mice at a dose of 50 or 100 mg/kg 1h before CS exposure. HemoHIM reduced the inflammatory cell count and levels of tumor necrosis factor receptor (TNF)-${\alpha}$, interleukin (IL)-6 and IL-$1{\beta}$ in the broncho-alveolar lavage fluid (BALF) induced by CS+LPS exposure. HemoHIM decreased the inflammatory cell infiltration in the airway and inhibited the expression of iNOS and MMP-9 and phosphorylation of Erk in lung tissue exposed to CS+LPS. In summary, our results indicate that HemoHIM inhibited a reduction in the lung inflammatory response on CS and LPS induced lung inflammation via the Erk pathway. Therefore, we suggest that HemoHIM has the potential to treat pulmonary inflammatory disease such as COPD.
