  • Objectives: The purpose of this clinical case is to evaluate the efficacy of traditional Korean medicine as a treatment for fibromyalgia. Methods: The patient was treated with Korean medicine therapy (herbal medicine, acupuncture, pharmacopuncture, and Chu-na therapy). We measured the status and progress of this patient using a numerical rating scale (NRS), EuroQol-5D, range of movement (ROM), and a special test. Results: After the treatments, the patient's pain was controlled and the NRS score decreased. The overall symptoms of the patient and the EQ-5D score were both improved. The ROM and special test score were also improved. Conclusions: Traditional Korean medicine may have positive effects as a treatment for fibromyalgia.

Single Dose Toxicity Test of Bojungikkeehapdaechilkitang water extract in Male and Female ICR Mice (보중익기합대칠기탕(補中益氣合大七氣湯) 추출물의 ICR마우스에서 경구 단회투여독성 평가)

  • An herbal water extract of Bojungikkeehapdaechilkitang(BDT) was prepared to test it for single-dose and repeated-dose toxicity, genotoxicity and reproductive toxicity, and to obtain a 50% lethal dose$(LD_{50})$, approximated lethal dose(ALD), and approximated target organs for BDT. The extract was tested on female and male ICR mice according to KFDA Guideline 1999-61 at doasge level of 2000, 1000, 500, 250 and 125mg/kg/10mL In this study, clinical signs, mortalities and gross findings of principal organs were observed for 14 days of single dosing, and afterwards in some cases. The ALD and $LD_{50}$ of BDT extract obtained in this study was>2000mg/kg for both male and female ICR mice. Also, any possible digestive toxicity of BDT extract was found to be above 1000mg/kg in both male and female ICR mice. The results of this study strongly suggest that BDT extract has no toxic effect at dosage level below 500mg/kg.

  • Junggi-haham (中氣下陷) is a term referring to lethargic symptoms such as digestive system disorder originating from energy deficiency. The authors applied therapy including Bojungikgi-tang(or Seungyangikgi-tang) and Bi-jeonggyuk method to two patients, one having ptosis and the other stress urinary incontinence, respectively, as Junggi-haham. Bojungikgi-tang is an herbal medication preparation and Bi-jeonggyuk is an acupuncture therapy method, and both are used to treat Junggi-haham symptoms. The combined treatment showed positive outcome, and the result suggests that Bojungikgi-tang and Bi-jeonggyuk are effective treatments for patients diagnosed as Junggi-haham.

Effects of an Herbal Medicine, Gagam-daewhang mangcho-tang, and its Components on Cerulein-induced Acute Pancreatitis in Mice (加減大黃芒硝湯및 그 구성약재가 Cerulein으로 유도된 생쥐의 急性 膵臟炎에 미치는 影響)

  • Objectives: This study investigated the effects of Gagam-daewhang mangcho-tang (GDM) and its components on cerulein-induced acute pancreatitis (AP) in mice Methods: The AP mouse model was induced by intra-peritoneal injection of cerulein (50 μg/kg) at hourly intervals for 6 times. The experimental drug was administrated intraperitoneally 1 hour prior to the first injection of cerulein. Mice were sacrificed at 6 hours after the last injection of cerulein. Blood samples were taken to determine serum amylase level. The pancreas and lungs were rapidly removed for histochemical examination and myeloperoxidase (MPO) assays. Results: Administration of modified GDM significantly reduced the ratio of pancreas/body weight, level of serum amylase, neutrophil infiltration, and histological damage of the pancreas and lung. In a test of the components of GDM, the Salvia miltiorrhiza (SM) group showed a significant suppression of the severity of AP. In an experiment testing the concentration of SM, the 150 mg/kg SM group showed significant attenuation of the severity of AP. Conclusions: Modified GDM and a SM water extract could attenuate AP and AP-associated lung injury via suppression of digestive enzyme secretion and MPO activity.

Evaluation for Therapeutic Effectiveness of Bowhatang in Functional Dyspepsia (기능성 소화불량증 환자에 대한 보화탕(保和湯)의 임상효과)

  • Objective: Functional dyspepsia refers to sustainable or repetitive pain at upper stomach related with food intake. This research was intended to observe the improvement of the symptom by administering Bowhatang to a number of functional dyspepsia patients and to clarify its clinical effects. Method: The research was performed upon the appropriate 36 patients conforming to the criteria after being diagnosed with functional dyspepsia among the 87 patients who visited the department of internal medicine at Gumi Oriental Hospital of Daegu Haany University for digestive diseases and were administered with Bowhatang. Result: 91.8% of the patients had upper stomach globus symptoms. which is of the largest percentage. and 13.5% had hunger pain. which is of the least percentage. After administering Bowhatang. by and large Likert scale was significantly reduced. According to the subjective improvement evaluation by the patients. 32 patients(86%) were improved. and 3 patients(8%) had neither improvement nor relapse. while 2 patients ( 6%) were relapsed. Conclusion: Bowhatang has shown significant improvement about the symptoms for functional dyspepsia patients. It is predicted when more outcomes become available from researches on side effects or toxicity of Bowhatang later on. it will be more often used in the clinic.

Yukgunja-tang for Irritable Bowel Syndrome: A Protocol for a Systematic Review and Meta-Analysis

  • Background: Irritable bowel syndrome (IBS) is a digestive disorder characterized by abdominal discomfort or pain accompanied by a change in stool condition. Owing to its complicated mechanisms, a standard treatment for IBS has not yet been established. Yukgunja-tang (YGT) is a Korean herbal medicine known in Asia to be effective in the treatment of gastrointestinal symptoms. In this study, we will conduct a systematic review of randomized controlled trials (RCTs) to assess the efficacy and safety of YGT in IBS treatment. Methods and analysis: English databases, such as Embase, Medline (via PubMed), Allied and Complementary Medicine Database, and Cochrane Central Register of Controlled Trials, will be searched for articles published up to April 2023. Additional databases, such as five Korean, one Chinese, and one Japanese database, will be included. RCTs and quasi-RCTs will also be included in the assessment of the efficacy of YGT. The overall efficacy rate will be the primary outcome, and data such as IBS quality-of-life measurements, global symptom scores, and adverse events will be the secondary outcomes. Review Manager Version 5.3 will be used for evaluation, and the risk of bias (RoB) will be evaluated using Cochrane Collaboration's RoB tool. The Grading of Recommendations Assessment, Development, and Evaluation approach will be used to score the quality of evidence. Conclusion: This study will demonstrate the efficacy and safety of YGT for treating patients with IBS.

The study of Achyranthis Bidentatae Radix & Dipsaci Radix in Bang-Yak-Hap-Pyun ("방약합편(方藥合編)"중(中) 생강(生薑) 및 대조(大棗)를 포함한 처방(處方)에 대한 고찰(考察))

  • In oriental medicine, Zingiberis Rhizoma Recens & Jujubae Fructus are frequently used materials for the digestive system, outer harmful qi(外感) and dam-yeum(痰飮). Bang-Yak-Hap-Pyun(方藥合編) is widely used for clinical herbal prescription book. In Bang-Yak-Hap-Pyun(方藥合編), Zingiberis Rhizoma Recens is used 3pyun(片)(4g) and Jujubae Fructus is used 2mae(4g). The most important fact is that, in contrast to most people know that chyranthis Bidentatae Radix & Jujubae Fructus are used most prescription at the same time, the prescription used chyranthis Bidentatae Radix & Jujubae Fructus at the same time is merely 77. (Bang-Yak-Hap-Pyun percentage of Zingiberis Rhizoma Recens & Jujubae Fructus is 16.48%). The number of prescription Zingiberis Rhizoma Recens & Jujubae Fructus in Bang-Yak-Hap-Pyun is total 173. 42 in sang-tong(上統), 86 in jung-tong(中統), 45 in ha-tong(下統). The group of mostly used disease is about outer harmful qi(外感) and dam-yeum(痰飮). The ratio of Zingiberis Rhizoma Recens in Bang-Yak-Hap-Pyun is 3.70%~100%. And the ratio of Jujubae Fructus is 0.45%~29.14%. The mostly quoted medical literature what prescription included Zingiberis Rhizoma Recens & Jujubae Fructus in Bang-Yak-Hap-Pyun is Tae-Pypng-Ha-Jae-Guk-Bang.

Usage of Korean Herbal Medicine Banhabaekchulcheonmatang-Analysis of Korean Clinical Studies (반하백출천마탕을 사용한 국내 임상연구 분석)

  • Objectives : To investigate the clinical usage of Banhabaekchulcheonmatang (BBCT), this study reviewed clinical studies conducted in Korea. Method : Literature search was performed on four Korean database; OASIS, RISS, KTKP and NDSL. Studies published before July 2023 were categorized and analyzed according to the target disease. Results : Fifty-four studies including 36 single case reports and 18 case series were included in this review and the total number of case patients was 79. There were 24 patients with peripheral vertigo such as benign paraoxysmal positional vertigo and Meniere's disease, 17 with central vertigo such as cerebellar infarction, 15 with vertigo accompanied by digestive symptoms, 13 with headache, and 10 with other diseases. Vertigo treatment accounted for more than 70% of the total case and there was no cases of hypertension and dementia. The BBCT formulation used was decoction in 69 patients (87.3%), powder in 7 patients (8.9%), and combination of decoction and powder in 3 patients (3.8%). Conclusion : Clinical research should be accumulated to expand the use of BBCT in the treatment of hypertension and dementia. In order to increase the use of insurance-covered extract preparation, it is necessary to make an effort to include the various diseases actually treated with BBCT in the indications.

Effects of BanhabackchulChunma-tang on the Vascular Relaxation and its mechanism (혈관 긴장성 조절에 미치는 반하백출천마탕)

  • Hypertension is not only a well-established cardiovascular risk factor but also increase the risk of atherosclerosis. Most studies conducted to investigate the effectiveness of treatment for cardiovascular disease such as hypertension have focused primarily on conventional drug and physiotherapeutic treatments. BanhabackchulChunma-tang(半夏白朮天麻湯:BCT) is popular herbal medicine used in clinic for the treatment of various symptoms of drulatory disorders and weakness of digestive system, including anorexa and nausea with vertigo, severe headache, vomiting and so on. However, the mechanisms underlying its efficacy are unknown. This study investigated the effects of BCT as an alternative medication on the contraction induced by phenylephrine and KCI in rat thoratic aorta. BCT revealed siginificant relaxation on phenylephrine-induced arterial contraction, but revealed noncompetitive effect on concentration responses of phenylephrine-induced contraction. Treatment of N-L/sup ω/ -argine methyl ester(L-NAME) and methylene blue(MB)(10/sup -5/M) reduced the relaxation of BCT. BCT also increased in vitro NO production. It suggest that the relaxation effect of BBT is related with NO pathway, becausse the effect of L-NAME and MB are due to inhibition of NO synthesis from endothelial cells. These results indicate that BCT would be effective in hypertension treatment and its mechanism of relaxtion on arterial contraction is likely to be related with NO production, blocking of α-receptor and signal transduction after receptor activation.

  • Objectives : The Aim of this study was to investigate the clinical characteristics of male and female patients who have been suffered from headaches. The investigation was undertaken based on Qi-section(Methods of prescribing oriental herbal medicine) from DongEuiBoGam(東醫寶鑑). Methods: A statistical survey was conducted to compare the difference in clinical characteristics between male and female patients experiencing headaches. Oriental medical Diagnosis was used to classify all parients under the following categories Qi-deficiency (氣虛), Jing-dificiency (精虛), Food-stagnation (食積), Damp-accumulation (痰飮), Qi-stagnation (氣滯). Patients were treated using acupuncture therapy and herbal medicines. Results : 1. The ratio between male and female patients was about 1:3. 2. It showed that periods of illness were longer in female patients than in male patients. 3. The total number of treatments received was higher in female patients than in male patients. 4. Statistically, most female patients had Metopodynia and Migraines(Rt.), while male patients suffered mostly from Laryngalgia. 5. Male patients had feelings of strain in the head and female patients had splitting headaches. 6. Musculo-skeletal System and General Symptoms were frequently observed in male patients. Digestive System and Nervous System were frequently observed in female patients. 7. Results from oriental medical Diagnosis showed that male patients mostly had Qi-deficiency, ling-deficiency while the majority of female patients had Food-stagnation, Damp-accumulation, Qi-stagnation. 8. The recovery time for male patients was shorter than it was for female patients. The recurrence rate of the headaches were higher for female patients. Prognosis was better than in female patients in comparison to male patients. Conclusions : The statistical survey conducted was based on Qi-section of DongEuiBoGam. It had significant differences in clinical characteristics between male & femal patients.
