Contrast-enhanced ultrasonography (CEUS) has been applied to evaluate parenchymal organs in human and veterinary medicine. However, to our knowledge, there is no report on the identification of active bleeding and the bleeding site in veterinary clinical patients. Herein, we describe the use of CEUS in two cases of abdominal bleeding caused by ruptured lesions with malignant abdominal tumors. One dog had a splenic hemangiosarcoma, which had metastasized to the liver; the other dog had hepatic cell carcinomas in the left hepatic lobe, which were lobectomized, and another nodule was identified in the right hepatic lobe. Immediately after the rupture of these oncogenic lesions was suspected, CEUS was performed to identify the bleeding sites. The active bleeding sites were confirmed by hyperechoic pooling signs in the arterial phase, and extravasation could be observed within the defects showing hypoechoic perfusions in the delayed phase of the CEUS. Microbubbles were also observed in the ascites; thus, CEUS could detect the presence of hemorrhage and accurately identify the bleeding site. Collectively, the study findings suggest the usefulness of CEUS in emergent situations as it enables rapid and noninvasive evaluation of bleeding points in case of active bleeding in dogs.
7년령의 불임 암컷 소말리 교잡종 고양이가 4주 동안 지속된 식욕부진, 구토, 체중감소, 무기력을 주증으로 내원하였다. 환자는 지방간으로 진단 되었으며, 3주 동안 집중치료와 더불어 비위 영양관을 통해 유동식을 공급하였다. 치료기간 중, 지방간으로 인한 임상증상은 개선 되었지만 고양이 피부유약증으로 인한 것으로 의심되는 피부병변이 왼쪽과 오른쪽 어깨 부위에서 발견되었다. 과립당치료, 레이저치료 및 외과적 피판술을 통한 집중적인 상처 치유를 실시하였으며, 그 결과 환자는 다른 임상 증상이나 피부 병변의 재발 없이 성공적으로 치료되었다. 보고된 바에 따르면, 본 증례는 간질환과 고양이 피부유약증이 동반한 환자에서 좋은 예후를 보인 첫 번째 증례이다.
The role of scintigraphic imaging has moved from the detection of lesions to the tissue-specific characterization of lesions over the past 2 decades. Major advances in nuclear medicine imaging include: 1) positron imaging, 2) improved instrumentation, such as the use of multidetector (dual or triple head) gamma cameras for single photon emission computed tomography, and 3) development of numerous new radiopharmaceuticals for positron or single photon imaging (labeled glucose analogue, amino acids, fatty acids, hormones, drugs, receptor ligands, monoclonal antibodies, etc). These advances have resulted in a significantly improved efficacy of radionuclide techniques for the evaluation of various tumors, including those within the liver. The current role of nuclear medicine in the evaluation of focal hepatic tumors is reviewed in this article with an emphasis on the clinical applications of various tracer studies and imaging findings.
Alveolar echinococcosis (AE) is considered as a fatal zoonosis caused by the larvae of Echinococcus multilocularis. The lungs and brain are the most common metastatic organs. We report a human case of hepatic alveolar echinococcosis accompanied by lung and brain metastasis. In particular, the patient had a history of tuberculosis and the lung lesions were easily misdiagnosed as lung abscesses. The lesions of liver and lung underwent radical resection and confirmed as alveolar echinococcosis by pathological examination. The patient had no surgical complications after operation and was discharged after symptomatic treatment. Unfortunately, the patient later developed multiple intracerebral AE metastases. We required the patient to take albendazole orally for life and follow up.
In order to know the effect of dexamethasone on the induction of the rabbit viral hepatitis, the pathological changes were observed in the native rabbits, 2 to 6 months old in age, that were injected by dexamethasone and liver emulsion of Angora rabbit naturally infected with viral hepatitis. The results were summarized as follows: The native rabbits injected by dexamethasone and liver emulsion were infected with viral hepatitis and died between 2 and 7 days after inoculation. Clinical signs and gross lesions were very similar to those of Angora rabbit naturally occurred, In microscopical findings, the hepatic lesions were characterized by peripheral necrosis of the lobules, and peripheral necrosis of the lobules with fatty changes of hepatic cells was occurred in a few cases. Perivascular lymphocytic infiltration in the central nervous system was observed in some cases, The lesions of the other organs were very similar to those of Angora rabbit naturally occurred. On the other hand, the native rabbits that were injected by only liver emulsion of Angora rabbit naturally infected with viral hepatitis were not infected with the disease except very few cases.
Background: To evaluate whether DM affects the SUVmax of metastatic lesions on 18F-FDG PET/CT and whether the SUVmax can influence the prognosis of metastatic PDAC patients. Methods: We conducted a retrospective study of 86 patients with metastatic PDAC who underwent PET/CT before treatment. The SUVmax of primary and metastatic lesions and the ratios of the SUVmax were measured. Long-term survival was evaluated using clinical parameters. Results: The mean SUVmax of primary lesion was lower in the DM group than in the non-DM group (4.74 vs. 5.96, p=0.009). The SUVmax for all metastatic lesions, except those in the lung, were lower in the DM group than in the non-DM group, and these differences were statistically significant in the lymph nodes and peritoneum. In the 35 patients with hepatic metastasis, higher ratios of the liver SUVmax significantly correlated with shorter OS (HR, 2.625; p=0.013). Conclusion: DM can influence the lower SUVmax of metastatic lesions as well as primary lesions. The SUVmax ratio of hepatic metastasis could influence on prognosis in metastatic PDAC patients.

Objective: To assess the diagnostic performance of the Liver Imaging Reporting and Data System (LI-RADS) version 2018 treatment response algorithm (TRA) for the evaluation of hepatocellular carcinoma (HCC) treated with transarterial radioembolization. Materials and Methods: This retrospective study included patients who underwent transarterial radioembolization for HCC followed by hepatic surgery between January 2011 and December 2019. The resected lesions were determined to have either complete (100%) or incomplete (< 100%) necrosis based on histopathology. Three radiologists independently reviewed the CT or MR images of pre- and post-treatment lesions and assigned categories based on the LI-RADS version 2018 and the TRA, respectively. Diagnostic performances of LI-RADS treatment response (LR-TR) viable and nonviable categories were assessed for each reader, using histopathology from hepatic surgeries as a reference standard. Inter-reader agreements were evaluated using Fleiss κ. Results: A total of 27 patients (mean age ± standard deviation, 55.9 ± 9.1 years; 24 male) with 34 lesions (15 with complete necrosis and 19 with incomplete necrosis on histopathology) were included. To predict complete necrosis, the LR-TR nonviable category had a sensitivity of 73.3-80.0% and a specificity of 78.9-89.5%. For predicting incomplete necrosis, the LR-TR viable category had a sensitivity of 73.7-79.0% and a specificity of 93.3-100%. Five (14.7%) of 34 treated lesions were categorized as LR-TR equivocal by consensus, with two of the five lesions demonstrating incomplete necrosis. Interreader agreement for the LR-TR category was 0.81 (95% confidence interval: 0.66-0.96). Conclusion: The LI-RADS version 2018 TRA can be used to predict the histopathologic viability of HCCs treated with transarterial radioembolization.
Four water monitors at Zoological Garden, Chang Gyeong Won, Seoul, died within a week after signs of anorexia, lethargy, and discharge from eyes, nasal and oral cavities. The autopsy findings of the four animals were similar. As a main lesion, the liver was congested and diffuse necrosis was observed. The terminal portions of the rectum were studded with numerous small ulcers causing rectal stenosis. Histopathologically, massive hepatic necrosis preceded by fatty changes were evident. The rectal lesions manifested coagulative necrosis and thrombosis in the mucosa and submucosa.
Majority of hepatocellular carcinoma is evolved from a well differentiated cancerous condition such as hyperplastic lesions eg. adenomatous hyperplasia in cirrhotic liver or de no vo carcinogenesis and prolifenation along with dedifferentiation. Adenomatous hyperplasia is may be seen in severe acute hepatic injury, like submassive hepatic necrosis, or in chronic liver diseases, particularly liver cirrhosis and it has recently attracted much interest from both clinicians and pathologists because it is regarded as a precursor lesion of hepatocellular carcinoma. Hepatic adenomatous hyperplasia resembling focal nodular hyperplasia might have developed from localized vascular changes associated with chronic liver disease, pre-existing arterial malformation and early stage of angiogenesis in hepatocarcinogenesis. We present a patient who developed hepatocellular carcinoma after hepatic artery ligation.
