  • The general local cause of gingival bleeding is the vessel engorgement and erosion by odontogenic infection. Abnormal gingival bleeding is also associated with systemic causes. Bleeding disorders in which continuous gingival bleeding is encountered include the followings : vascular abnormalities, platelet disorders, hypoprothrombinemia and other coagulation defects. There are classic methods for gingival bleeding control, such as, direct pressure, electrocoagulation, suture, crushing and application of hemostatic agents. If the continuous gingival bleeding is not stopped in spite of the conventional methods, the life of patient is threatened owing to upper airway obstruction, syncope, vomiting and hypovolemic shock. Therefore, the rapid and correct hemostatic method is very important in the emergency condition. This is a case report of continuous gingival bleeding control by primary endodontic drainage & suture in a disabled patient with systemic bleeding disorders.

Alternative Mechanism of Aspirin in Anti-Thrombotic Therapy: Inhibition of Thrombin Activatable Fibrinolysis Inhibitor

  • The use of aspirin is widely recommended for the prevention of heart attacks owing to its ability to inhibit platelet activation by irreversibly blocking cyclooxygenase 1. However, aspirin also affects the fibrinolytic and hemostatic pathways by mechanisms that are not well understood, causing severe hemorrhagic complications. Here, we investigated the ability of aspirin and aspirin metabolites to inhibit thrombin-activatable fibrinolysis inhibitor (TAFI), the major inhibitor of plasma fibrinolysis. TAFI is activated via proteolytic cleavage by the thrombin-thrombomodulin complex to TAFIa, a carboxypeptidase B-like enzyme. TAFIa modulates fibrinolysis by removing the C-terminal arginine and lysine residues from partially degraded fibrin, which in turn inhibits the binding of plasminogen to fibrin clots. Aspirin and its major metabolites, salicylic acid, gentisic acid, and salicyluric acid, inhibit TAFIa carboxypeptidase activity. Salicyluric acid effectively blocks activation of TAFI by thrombin-thrombomodulin; however, salicylates do not inhibit carboxypeptidase N or pancreatic carboxypeptidase B. Aspirin and other salicylates accelerated the dissolution of fibrin clots and reduced thrombus formation in an in vitro model of fibrinolysis. Inhibition of TAFI represents a novel hemostatic mechanism that contributes to aspirin's therapy-associated antithrombotic activity and hemorrhagic complications.

Iatrogenic Vertebral Artery Injury During Anterior Cervical Spine Surgery : Report of Two Cases

  • The incidence of vertebral artery injury during the anterior approach to the cervical spine is rare, but potentially lethal. The authors describe two cases of vertebral artery injury during anterior cervical decompression surgery. In the first case, infection was the cause of the vertebral artery injury. During aggressive irrigation and pus drainage, massive bleeding was encountered, and intraoperative direct packing with hemostatic agents provided effective control of hemorrhage. Ten days after surgery, sudden neck swelling and mental deterioration occurred because of rebleeding from a pseudoaneurysm. In the second case, the vertebral artery was injured during decompression of cervical spondylosis while drilling the neural foramen. After intraoperative control of bleeding, the patient was referred to our hospital, and a pseudoaneurysm was detected by angiography four days after surgery. Both pseudoaneurysms were successfully occluded by an endovascular technique without any neurological sequelae. Urgent vertebral angiography, following intraoperative control of bleeding by hemostatic compression in cases of vertebral artery injury during anterior cervical decompression, should be performed to avoid life-threatening complications. Prompt recognition of pseudoaneurysm is mandatory, and endovascular treatment can be life saving.

흉골 출혈에 대한 fibrin glue의 지혈 효과 (Hemostatic effect of fibrin glue for sternal marrow bleeding.)

  • Bleeding from bone marrow after sternotomy for open cardiac surgery can be sometimes difficult to control and even lead to reoperation for hemostasis. A clinical comparative study was carried out to demonstrate the hemostatic effect of fibrin glue [Beriplast] for sternal marrow bleeding after sternotomy for open heart surgery. Postoperative blood loss was measured in two patient groups, group A included 19 patients operated upon from June to October 1987 and the fibrin glue was applied to the sternal marrow together with collagen fleece and group B consisted of 22 patients from January to May 1987 and only collagen fleece was applied without fibrin glue. There was no difference between two groups in age and sex distributions, coagulation state, method of extracorporeal circulation and operative management. The blood loss one hour after operation was 2.04 ml/hr/kg in group A and 3.55 ml/hr/kg in group B [P<0.001]. The most significant difference was observed during the first 4 hours after surgery with 1.34 ml/hr/kg versus 2.05 ml/hr/kg. over the following 20 hours the amount of drainage from the chest tubes was identical in both groups. Fibrin glue reduces blood loss after open heart surgery by local hemostasis at sternum. Our study has shown that local application of fibrin glue to sternal marrow is an effective method of controlling the sternal bleedings. No side effect or complication of fibrin glue was noted.

  • In surgery the hemostatic control is very important for the parenchymatous organs. These organs consist of the fragile and the blood content tissues such as liver, spleen and kidney, etc.. One of the control methods to solve this problem is to insert the mono polar typed needle electrode, which gathers the thermal effect of microwave, directly into tissues so as to coagulate and stop the hemorrhage. This method has same advantages: First, the range of the heat energy is limited. Second, the coagulation, the hemostatic characteristic, and stability are excellent. Third, more convenient operation is possible. This paper is aimed to manufacture the microwave tissue coagulation system and to suggest the new direction for development, hereafter.

龍膽瀉肝湯加減方의 止血效果에 關한 實驗的 硏究 (Experimental Studies on Hemostatic Effect of Yongdamsagantang Subtracted Bupleuri radix and Yongdamsagantang Subtracted Bupleuri Radix Added Biotae Cacumen, Imperatae Rhizoma, Typhae Pollen)

  • Based upon our laboratory experiments of Yongdamsagantang subtracted Bupleuri Radix and Yongdamsagantang subtracted Bupleuri Radix added Biotac Cacumen, Imperatae Rhizoma, Typhae Pollen on male rats to examine the medicines' hemostatic effect, we draw conclusions as follows: 1. Compared to the control group, Yongdamsagantang subtracted Bupleuri Radix and Yongdamsagantang subtracted Bupleuri Radix added Biotae Cacumen, Imperatae Rhizoma, Typhae Pollen increased the number of platelet counts meaningfully. 2. Compared to the control group, Yongdamsagantang subtracted Bupleuri Radix and Yongdamsagantang subtracted Bupleuri Radix added Biotae Cacumen, Imperatae Rhizoma, Typhae Pollen suppressed prothrombin time without any meaning. 3. Compared to the control group, Yongdamsagantang subtracted Bupleuri Radix and Yongdamsagantang subtracted Bupleuri Radix added Biotae Cacumen, Imperatae Rhizoma, Typhae Pollen, all of these medicines elevated the fibrinogen level, but only Yongdamsagantang subtracte Bupleuri Radix is recognized to raise the fibrinogen level meaningfully. 4. Compared to the control group, Yongdamsagantang subtracted Bupleuri Radix and Yongdamsagantang subtracted Bupleuri Radix added Biotae Cacumen. Imperatae Rhizoma, Typhae Pollen repressed partial thromboplastin time meaningfully. In conclusion, Yongdamsagantang subtracted Bupleuri Radix and Yongdamsagantang subtracted Bupleuri Radix added Biotae Cacumen, Imperatae Rhizoma, Typhae Pollen can be used for stopping blood effectively in terms of platelet count increase, prothrombin time control, fibrinogen level rise, and partial thromboplastin time suppression.

歸脾湯 및 歸脾湯加味方의 止血效果에 관한 實驗的 硏究 (A Study on the Homostatic Effects of Guipitang and Guipitangjiaweifang Extracts in Experimental Animals)

  • This Experiment was conducted to investigate the hemostatic effects of Guipitang and Guipitangjiaweifang. For this purpose, the effects of the crude extracts on the platelet count. prothrombin time, fibrinogen level and partial prothromboplastin time were estimated. The rats were divided into normal group, control group and experimental group. Control group was administrated normal saline and experimental group was divided into Sample 1 (Guipitang) and Sample 2 (Guipitangjiaweifang). The results obtained were as follows : 1. The platelet count was significantly increased in both of two samples as compared with the control group. 2. The prothrombin time was significantly shortened in case of Sample 2. however there were no significant changes in case of Sample 1 as compared with the control group. 3. The fibrinogen level was significantly increased in case of Sample 1. however there were no meaningful changes in case of Sample 2 as compared with the control group. 4. The partial prothromboplastin time was significantly shortened in both of two samples as compared with the control group. According to above results. it is supposed that Guipitang and Guipitangjiaweifang have the hemostatic effects in experimental animals.

Hemostatic Dysfunction in a Dog with Mammary Gland Carcinoma

  • Hypercoagulability is the most common coagulopathy seen in dogs with neoplasia, whereas a hypocoagulable state is relatively rare. A 16-year-old spayed female miniature Schnauzer presented with bilateral epistaxis, bilateral hindlimb swelling, and lameness. Previously, the dog was diagnosed with intermediate grade solid carcinoma after mastectomy, and then showed generalized ecchymoses on the abdomen and bilateral hindlimbs on presentation. Laboratory tests revealed a hypocoagulable state, including thrombocytopenia, hypofibrinogenemia, and delayed prothrombin and activated partial thromboplastin time. Thromboelastography demonstrated a prolonged K time with a decreased alpha angle and low maximal amplitude, reflecting a decrease in clot strength. The hypocoagulable state of disseminated intravascular coagulopathy was speculated based on the presence of an underlying tumor, clinical symptoms, and laboratory results of the hypocoagulable state. This case shows the incidence of hemostatic dysfunction as a paraneoplastic syndrome in a dog with mammary gland carcinoma.

  • Congenital afibrinogenemia is a rare disorder characterized by a congenital lack of fibrinogen, a key component of the hemostatic system. Bleeding manifestations of congenital afibrinogenemia vary in severity from mild to catastrophic. This is a case report of splenic rupture occurred in an eight-year-old boy with congenital afibrinogenemia. Nonoperative treatment with cryoprecipitate and virally inactivated, purified fibrinogen concentrates successfully avoided splenectomy.
