Familial Hemifacial Spasm - Case Report - (가족성 편측안면경련 - 증 례 보 고 -)

  • The authors report two cases of familial hemifacial spasm according to the clinical and three dimensional shortrange magnetic resonance angiography(3D-SRMRA) findings. In the family of the first case, there were five patient's with acquired hemifacial spasm in successive generations. Three male and two female patients in successive generations of the same family developed acquired hemifacial spasm. Four patients were on the left side and one was on the right side. In the family of the second case, there were three patient's with acquired hemifacial spasm in successive generations. Two brothers developed left-sided hemifacial spasm. 3D-SRMRA finding of the probands demonstrated that both anterior inferior cerebellar artery in first case and anterior inferior cerebellar artery & vertebral artery in second case offend the 7th cranial nerve respectively. The presence of familial clustering of these rare disorders suggest an underlying genetic predisposition. All family pedigrees suggest that a pattern of autosomal-dominant inheritance with partial penetrance.

  • Objectives : This study was performed to report two cases of improved hemifacial spasm by herbal remedy and moxa-treatment. Methods : This study calendered the improvements of symptoms of two patients with hemifacial spasm who visited the author's clinic. Ukgansangamibang(抑肝散加味方) and moxa-treatment on chung-wan(CV12) were prescribed to the patients concerned due to their antipathy against acupuncture treatment. Results : Significant improvements were observered from both patients not only for hemifacial spasm but neuropsychosis-related symptoms Conclusion : The study shows remarkable results of the efficacy of herb remedy and moxa-treatment, for the treatment of hemifacial spasm , however the cases investigated in this study were only two(2). Therefore, more studies should be followed to support the efficacy of Oriental Medicine for the treatment of hemifacial spasm.

Is there any specificity in gender, triggering factors and HRV according to the left and right lesions of hemifacial spasm? : A retrospective chart review (반측성 안면경련의 좌우병소에 따라 성별, 유발요인, HRV 특이성이 있는가 : 후향적 차트 리뷰)

  • Objectives: This study is designed to investigate the difference of gender, triggering factors and Heart rate variability(HRV) between left and right hemifacial spasm patients. Methods: Among hemifacial spasm patients who visited the Acupuncture and Moxibustion medicine clinic of Kyung Hee University Korean Medicine Hospital from January 1, 2017, to April 18, 2022, we reviewed the clinical records of subjects with right hemifacial spasm(n=55) and left hemifacial spasm(n=76). We investigated the difference of gender, triggering factors and HRV between left and right hemifacial spasm patients groups by student's t-test, chi-square test and fisher's exact test using SPSS for windows(version 28.0.0.0.). Results: There was no difference in gender, triggering factors and HRV between left and right hemifacial spasm groups. Conclusion: According to this study, there were few specificity of lesions of left and right in hemifacial spasms.

  • In the treatment of hemifacial spasm, the operative methods, such as neurovascular decompression, rhizotomy etc. are the most efficacious therapy on its control. But, due to some side effects and complication, pharmacotherapy has been the first attempted by first intention of hemifacial spasm. The cabamazepine is the most commonly used agent, however it is often inefficacious in some cases, and attenuates as the time passes, engages in some symptoms, such as dizziness, nausea, vomiting, and causes an aplastic anemia, it has many deficiencies in becoming the first intention. Acupuncture is a simple, effective and conservative spasm control modality. It is so simple to use in hemifacial spasm. We had observed 5 cases of hemifacial spasm which were treated by acupuncture in our hospital recently. We reported these cases and reviewed related literatures briefly. The results of treatment are as follows : The average grade of spasm intensity classified by Scott's description was 3 before acu-therapy and it became 2 after the therapy. These results suggest that acupuncture and ear-acupuncture were effective as the first intention in healing hemifacial spasm.

Diagnostic Usefulness of CISS Image in Preoperative Evaluation of Trigeminal Neuralgia and Hemifacial Spasm (삼차신경통과 반측안면경련에서 CISS 영상의 진단적 유용성)

  • Objectives : Trigeminal neuralgia and hemifacial spasm are caused by vascular compression of the REZ(root entry or exit zone) of the 5th and the 7th cranial nerve. Preoperative detection of neurovascular compression is essential for accurate diagnosis, appropriate treatment, and the good operative results. Three dimensional Fourier Transformation-Constructive Interference in Steady State(3DFT-CISS) images are known to give good contrast between CSF, nerve, and vessels. We applied a 3DFT-CISS imaging technique for the preoperative evaluation of patients with these diseases and estimated the diagnostic accuracy and usefulness of this study. Methods : A series of 71 patients with trigeminal neuralgia and hemifacial spasm were treated by microvascular decompression. Among them 34 patients with trigeminal neuralgia and 24 patients with hemifacial spasm had preoperative CISS images. We compared the radiologic finding with the operative finding, and analysed the diagnostic usefulness of 3DFT-CISS imaging. Results : The sensitivity of CISS images of detecting the neurovascular compression was 90.3% in trigeminal neuralgia and 100% in hemifacial spasm. There were one false-positive and three false-negative cases in trigeminal neuralgia, and one false-positive case in hemifacial spasm. The accuracy in diagnosing the causative vessel was 73.5% in trigeminal neuralgia and 83.3% in hemifacial spasm. Conclusion : CISS image is very useful diagnostic tool for preoperative evaluation of neurovascular compression in patients with trigeminal neuralgia and hemifacial spasm. No additional neuroradiologic examination other than CISS image and MRA is needed for preoperative evaluation of patients with trigeminal neuralgia and hemifacial spasm.

  • Hemifacial spasm is painless uncommon disorder characterized by involuntary paroxysmal movement of one side face. In the treatment of hemifacial spasm, the Western operation or non-operation therapy has many side effect such as hearing loss, exudative otitis media, facial paralysis etc. So we studied mainly the effect of cause, treatment frequency, diseased part of 29 cases with hemifacial spasm for effective clicical application of oriental medicine. The results of study are as follow; 1. It shows more female patients than male patients, and mostly patients in their forties. 2. The diseased part is shown more on left side than right side, and the left side hemifacial spasm is cured more effective. 3. The causes of hemifacial spasm are mainly stress, ignorance, overwork, and the stress, a cold are more effective causes than ignorance in. 4. The disease period is mainly within six months, and that period is the most effective time. 5. The palpebral site is the most frequent site, ineffective site. 6. The effect go with the treatment frequency is not available. 7. There is a 55 percent improvement of 29 cases.

The Effect of Needle-embedding Therapy Combined with Oriental Medical Treatment on Hemifacial Spasm: Report of 2 Case (편측안면경련에 매선침법을 포함한 한방치료 치험 2례)

  • Objectives : The aim of this study was to observe the effect of needle-embedding combined with oriental medical treatment therapy on hemifacial spasm. Methods : 2 patients were treated with needle-embedding therapy with oriental medical treatment. After treatment, we evaluated the effect by Scott's scale. And we checked the satisfaction degree and side effect. Results : After treatment, the grade of spasm classified by Scott's scale was decreased and hemifacial spasm was visibly improved. Conclusions : This study showed that needle-embedding therapy with oriental medical treatment was effective to improve hemifacial spasm.

Efficacy of Intraoperative Facial Electromyographic Monitoring in Patients with Hemifacial Spasm

  • Objective : Hemifacial spasm has characteristic and specific electrophysiological finding, lateral spread response[LSR]. We study the correlation between change of lateral spread response during microvascular decompression[MVD] and clinical outcome after MVD. Methods : Sixty two patients with hemifacial spasm who were treated with microvascular decompression from March 2000 to February 2003 were included in this study. The monitoring of intraoperative facial electromyography[EMG] and brain stem auditory evoked potential were performed. Results : In 28 [44.7%] patients, there was persistence of lateral spread response after vascular decompression in root exit zone of facial nerve. Among these 28 patients, 9 had mild hemifacial spasm at discharge. Three out of 34 patients who had intraoperative disappearance of lateral spread response after MVD had mild hemifacial spasm. But Both groups, disappearance of LSR [Group I], and persistence [Group II] had only 2 patients with mild hemifacial spasm, and 5 patients at 3 months, respectively. Conclusion : Although intraoperative EMG monitoring is very useful in assessing the efficacy of MVD, the clinical outcome of MVD in patient with hemifacial spasm does not always correlate with EMG finding. The prognostic value of intraoperative LSR monitoring in the long-term results is questionable.

Botulinum Toxin Injection Treatment for Facial Spasm -Two Cases Report- (Botulinum Toxin을 이용한 안면 경련의 치험 -증례 보고-)

  • Hemifacial spasm is a distressing condition characterized by involuntary, intermittent, unilateral twitching of, all or parts, of the muscles innervated by facial nerve. This ailment is most common among middle-aged women. Neither causative agent nor reliable treatment has been established because etiology of idiopathic hemifacial spasm has remained undefined to date. This report describes two cases of hemifacial spasm treatment by injection of Botulinum toxin. An injection of Botulinum toxin provided relief of hemi facial spasm. Botulinum toxin therapy is an effective and convenient treatment of hemifacialspasm.
