Mesenteric torsion in a Miniature Schnauzer (Miniature Schnauzer에서 발생한 장간막 염전 1증례)

  • Mesenteric torsion was diagnosed in a 2-year-old, spayed female Miniature Schnauzer. The patient was presented with acute depression, vomiting, lethargy and hematochezia. On physical examination, severe dehydration, tachycardia, tachypnea, weak femoral pulse, delayed capillary refill time and pale mucous membrane were found and the dog was in shock. Radiography and ultrasonography revealed intestines distended with gas, ascites and the "C" shaped distended intestine. Medical treatments including fluid therapy, analgesics, antibiotics and lidocaine for reducing reperfusion injury were applied. And then, the mesenteric torsion was definitively diagnosed through exploratory laparotomy and intestinal resection and anastomosis were performed. The dog made an uneventful recovery and was free of clinical sign one week after surgery. Mesenteric torsion is an unusual and life-threatening disease in dogs. It has usually been described in the middle and large breed dogs, especially German Shepherds. However, the mesenteric torsion should be included in the differential diagnostic lists for acute abdomen even in small breed dog. The mortality rate of mesenteric torsion can be reduced through prompt diagnosis, proper preventive therapy for shock and reperfusion injury and emergency surgery.

Perivascular epithelioid cell tumor (PEComa) of the ascending colon: the implication of IFN-${\alpha}$ 2b treatment

  • A 7-year-old boy presented with hematochezia and abdominal pain. A 3.7-cm-sized mass was identified in the ascending colon by abdominal computed tomography and colonoscopy. The patient underwent surgical resection. Pathological examination revealed a low-grade perivascular epithelioid cell tumor (PEComa). PEComa in the colon is very rare. Only a few cases have been reported so far. An effective treatment method for this rare tumor has not been established yet. The patient received adjuvant interferon-${\alpha}$ immunotherapy for 1 year. He has been tumor-free for 26 months since the initial diagnosis. This report is the first documented case of the use of interferon-${\alpha}$ for pediatric PEComa of the colon.

Surgical Treatment of Intussusception: the 1970s vs. the 1990s (소아 장중첩증의 수술적 치료: 20년 간의 변화)

  • The surgical treatment of intussusception during two periods, 1975-1978 and 1995-1998 (Group B) were compared. There were 48 patients in Group A and 75 cases in Group B. Male were predominant in both group (2.7:1 vs 1.6:1). The mean age at operation was $6.7{\pm}5.0$ months (Group A) and $8.1{\pm}7.0$ months (Group B). The major signs and symptoms in both Groups included vomiting, hematochezia and irritability. There was a significantly higher bowel resection rate for group B (31.3 % vs 14.7 %, p=0.041). There were two operative deaths in group B but no deaths in group A. Hospitalization was significantly shorter in group B($7.5{\pm}2.7$ days vs $5.4{\pm}2.1$ days, p<0.001). We conclude that there were no differences in patient characteristics but surgical treatment in the 1990s results in more rapid recovery and reduced hospital stay.

A Rare Presentation of Metastasis of Prostate Adenocarcinoma to the Stomach and Rectum

  • Prostate cancer is the second most common cause of cancer death in men in the United States. The most common sites of metastasis include the bone, lymph nodes, lung, liver, pleura, and adrenal glands, whereas metastatic prostate cancer involving the gastrointestinal tract has been rarely reported. A 64-year-old African-American man with a history of prostate cancer presented with anemia. He reported the passing of dark colored stools but denied hematemesis or hematochezia. Colonoscopy revealed circumferential nodularity, and histology demonstrated metastatic carcinoma of the prostate. Esophagogastroduodenoscopy showed hypertrophic folds in the gastric fundus, and microscopic examination revealed tumor cells positive for prostate-specific antigen. Bone scanning and computed tomography of the abdomen and pelvis did not show metastasis. It is crucial to distinguish primary gastrointestinal cancer from metastatic lesions, especially in patients with a history of cancer at another site, for appropriate management.

  • The major complications of Meckel's diverticulum (MD) are bleeding, intestinal obstruction, infection, umbilical fistula and perforation. Although the relative incidences vary between authors, bleeding is the most common complication in children. The aim of our study is to show the symptomatic guideline for the diagnosis of the bleeding MD. Eight cases with bleeding MD which were operated upon at the department of Pediatric Surgery, Yeungnam University Hospital from April 1985 to April 2001 were reviewed. Half of the patients were under 2 years of age and all patient s we re male. All patients were preoperatively diagnosed by previous history of intestinal bleeding (melena, hematochezia) and 99mTc pertechnetate MD scan. Segmental resection and end-to-end anastomosis was performed in 6 patients and diverticulectomy in 2 patients. Heterotropic gastric mucosa was found in 6 patients. Postoperative complication was not observed in any cases. In conclusion, in any male children with obscure intestinal bleeding, especially less than 2 years of age, bleeding MD must be suspected. It seems to us that 99mTc pertechnetate MD scan is a useful tool to diagnose bleeding MD.

Colorectal Cancer in Children - 2 Cases Report - (소아에서 발생한 결장암)

  • A 12-year-old boy with severe periumbilical pain visited the emergency room. Physical examination, abdominal ultrasonography, colonoscopy and CT, identified a lesion of sigmoid colon. Endoscopic biopsy showed a signet ring cell carcinoma of the sigmoid colon. On explorative laparotomy, cancer invasions of the adjacent structures and metastases on peritoneal wall were noticed. We performed palliative loop-ileostomy. He underwent chemotherapy and radiotherapy for 3 months. The second case was a 16-year-old boy with abdominal pain and hematochezia, transferred to our hospital with the diagnosis of acute appendicitis with periappendiceal abscess. Although he underwent appendectomy, the abdominal pain persisted. Digital rectal examination revealed a lumen-obstructing fungating mass in the rectum. Endoscopic biopsy revealed a adenocarcinoma. Cancer invasion of the adjacent structures and metastases involving the mesentery of the small intestine were found at laparotomy. A palliative procedure, a Hartmann's operation and end-colostomy at the sigmoid colon were performed. The patient died 8 month later due to pneumonia and sepsis. Chemotherapy was not applied.

A Case of Hepatic Vein Thrombosis Associated with Ulcerative Colitis (궤양성 대장염에 동반된 간정맥 혈전증 1예)

  • Patients with inflammatory bowel disease are known to have hypercoagulability and an increased risk for venous thromboembolism. The deep veins of the lower extremities and the pulmonary veins are the most common sites of thrombosis in ulcerative colitis. However, hepatic vein thrombosis (Budd-Chiari syndrome) is a very rare extra-intestinal complication of ulcerative colitis in children. We describe a case of hepatic vein thrombosis in a 15-year-old girl with ulcerative colitis who presented with abdominal pain and hematochezia. Doppler ultrasonography and an abdominal CT scan revealed the characteristic filling defects caused by large thrombi in both hepatic veins. These lesions were successfully treated with conventional management for ulcerative colitis and anticoagulation therapy.

  • Hibiscus syriacus L. (Malvaceae) is widely distributed over Korean, China, India and Siberia. The dried flower of Hibiscus syriacus is used as a folk medicine for curing of hematochezia, dysentery, obstruction due to wind-phlegm, regurgitation, and vomiting of food, and the dried root bark is used antipyretic, anthelmintic and antifungal agents. From a chloroform extract of root bark of this plant, compound I, II, and III were isolated and the structures were elucidated by various spectroscopic analyses. These compounds were identified as syringaresinol. E-N-feruloyltyramine, and Z-N-feruloyltyramine, respectively and were isolated from this plant for the first time. Compound II and III exhibited lipid peroxidation inhibitory activities with $IC_{50}$ of 15.5 and 28.6 ${\mu}g/ml$, respectively.

  • Hemolytic uremic syndrome(HUS) is characterized by acute renal failure, microangiopathic hemolytic anemia, and thrombocytopenia and the most common pathogen is Escherichia coli (E. coli) O157 : H7. Ischemic colitis, which rarely occurs in children, is due to the reduced local blood flow to the intestine, tissue necrosis and secondary bacterial infection. We describe a patient who was admitted with abdominal pain, vomiting and hematochezia, and diagnosed as ischemic colitis by barium enema. This patient showed hemolytic anemia, thrombocytopenia and progressive renal failure and was subsequently diagnosed as hemolytic uremic syndrome. After hemodialysis, the patient showed improvement of symptoms and resolution of renal failure and ischemic colitis.

Colorectal Adenocarcinoma in a Dog (개에서 발생한 결장직장 선암종 1례)

  • An 8-year-old, female Pointer dog was presented with weight loss, hematochezia, and dyschezia. For the diagnosis, physical examination, complete blood counts, serum chemistry, radiography, ultrasonography, computed tomography, colonoscopy, cytology and histopathological examination were performed. The complete blood counts and serum biochemistry results were within the reference range. Ultrasonographic findings were presented with the thickened wall of the colon and rectum. Colonoscopy revealed irregular and ulcerated mucosal surface, mass and luminal narrowing of colorectal lesions. There were thickened wall and contrast enhancement of the lesion in colorectal region on the computed tomography. The cytologic examination suggested the adenocarcinoma. Postmortem histopathologic examination revealed adenocarcinoma. Based on these findings, the dog was diagnosed with annular colorectal adenocarcinoma.
