Molecular survey of Dirofilaria immitis in mosquitoes collected from parks the in the Incheon metropolitan city in Korea (인천지역 공원 채집 모기에서 심장사상충 감염도 조사)

  • Unlike previous studies that reported the infection rates of Dirofilaria immitis in companion dogs, D. immitis genes were tested directly on mosquitoes to see how many mosquitoes actually carry larvae. Mosquito samples were collected from six parks in Incheon from March to August 2018. A total of 18,589 mosquitoes was collected, classified by type and then made 575 sample pools based on 50 mosquitoes. The PCR method was used to detect Dirofilaria immitis genes of sample pools. Dirofilaria immitis genes were detected in 7 of the 575 pools, and these positive mosquito samples were collected in 4 out of the 6 parks. The types of detected mosquitoes were Aedes dorsalis and Ochlerotatus koreanus which have been identified to act as a vector for heart worm propagation in Incheon. Based on these results, it was concluded that D. immitis genes were found in mosquitoes in the neighborhood park.

Suspected cytosine arabinoside induced interstitial lung disease in a dog with meningoencephalitis

  • An 11-year-old neutered male Dachshund dog weighing 7 kg presented with acute onset of respiratory distress after subcutaneous administration of cytosine arabinoside (CA). The patient previously diagnosed with meningoencephalitis of unknown origin and was being treated with oral prednisolone, levetiracetam, potassium bromide, gabapentin, and periodic subcutaneous CA administration (50 mg/m2, q 12 h, subcutaneous, 4 times, every 3 weeks). The patient developed tachypnea with labored respiratory effort after 9th CA administration. Thoracic radiograph revealed bilateral diffuse interstitial to alveolar pulmonary opacities, and echocardiogram indicated no evidence of left-sided heart failure. Based on the onset coinciding with the administration of CA, low possibility of other pulmonary disease, remission of symptom showed after discontinuation of CA, we suspected CA-induced interstitial lung disease. The patient's pulmonary opacities on the radiograph improved to a similar degree as before the adverse event over time, but respiratory symptoms were not fully resolved. Sildenafil (2 mg/kg, per oral, q 12 h) was given as therapeutic trial to manage possible pulmonary hypertension, suspected a sequela of the lung disease, based on an echocardiographic evidence and clinical signs. The patient's respiratory symptom was well managed since, and achieved discontinuation of sildenafil.

  • Anger cause heart disease and domestic violence and recently there have been an increasing number of media reports of crimes committed because of their inability to control their anger. Anger has been reported to cause massive financial losses due to heart disease and domestic violence. Therefore, since anger raises serious social problems and social costs, it is necessary to take active intervention methods to alleviate inappropriate anger coping. A variety of methods such as cognitive and emotional approach, psycho-mechanical approach, psycho-educational approach, relaxation therapy, cognitive behavior-art therapy, and stress immunization are being explored and utilized for presenting effective anger-coping intervention alternatives. In this manner, information and communication technology is likely to be used as an effective means for this, and various researches are being conducted. The purpose of this study is to develop an anger coping service technology using ICT (Information & Communication Technology) technology as an effort to improve anger coping ability. In other words, the goal is to develop a technology that delays anger and evokes the surroundings by providing location-based services to recognize anger situations and facilitate anger. This research is about the technology which intervene directly the anger situation to resolve it using location information and introduce the base technology to realize it.

Pricing Policy of Music Service in Digital Music Market-focused on the Regulations for the Digital Music Service (디지털 음악시장에서 음원사용료 징수 개정안의 고찰-2013년 징수개정안을 중심으로)

  • As a result of growth of digital music market in Korea, Creator's rights and interests have been at the heart of the debate in terms of legal system. In the music industry, digital music revenue has now overtaken records sales and compared to download service it is now clear that music streaming and subscription is mainstream model in the rise of the worldwide market of digital music. This trend is also indentical to the domestic and withholding regulations for music service and creator's rights and interests have become an ever growing issue. Ministry of Culture, Sports ad Tourism decided the revision of withholding regulations for the digital music service in 2013. The amendment is to change its policy of charging music service from the flat rate pricing to a usage-based system. This paper brought forward some disputable points such as fair division of profit, reasonable pricing for consumers etc. about the revision. Therefore, improvement of system and change in the perception of such copyrights are still required for both the encouragement of creator's activities and the high consumer satisfaction.

Association between defoliant exposure and survival to discharge after out-of-hospital cardiac arrest (고엽제 노출과 병원 밖 심정지 후 생존 퇴원과의 연관성)

  • Purpose: We investigated the association between defoliant exposure and survival to discharge after out-of-hospital cardiac arrest. Methods: This is a retrospective case-control study based on the cardiopulmonary resuscitation (CPR) registry. The electronic medical records of out-of-hospital cardiac arrest victims from 6/9/2008 to 12/31/2016 were analyzed statistically. The case patients group had a history of defoliant exposure while the control group did not. Among the 401 victims studied, a total of 110 patients were male out-of-hospital cardiac arrest patients. Baseline characteristics and the parameters involved in cardiac arrest were analyzed and compared between the two groups after propensity score matching. The primary outcome was survival to discharge, and secondary outcomes were sustained return of spontaneous circulation (ROSC) and survival to admission. Results: After propensity score matching a total of 50 patients (case=25, control=25) were analyzed. Primary outcome (survival to discharge) was not significantly different between case and control groups [(OR, 1.759; 95% C.I., 0.491-6.309) and (OR, 1.842; 95% C.I., 0.515-6.593), respectively]. In the subgroup analysis, there were also no significant differences between the control group and subgroups in primary and secondary outcomes according to defoliant exposure severity. Conclusion: There is no statistically significant association between defoliant exposure and survival of out-of-hospital cardiac arrest.

The Association of Hospital Volume of Percutaneous Coronary Intervention with Cardiac Mortality

  • Background: This study investigates the potential volume and outcome association of coronary heart disease (CHD) patients who have undergone percutaneous coronary intervention (PCI) using a large and representative sample. Methods: We used a National Health Insurance Service-Cohort Sample Database from 2002 to 2013 released by the Korean National Health Insurance Service. A total of 8,908 subjects were analyzed. The primary analysis was based on Cox proportional hazards models to examine our hypothesis. Results: After adjusting for confounders, the hazard ratio of thirty-day and 1-year mortality in hospitals with a low volume of CHD patients with PCI was 2.8 and 2.2 times higher (p=0.00) compared to hospitals with a high volume of CHD patients with PCI, respectively. Thirty-day and 1-year mortality of CHD patients with PCI in low-volume hospitals admitted through the emergency room were 3.101 (p=0.00) and 2.8 times higher (p=0.01) than those in high-volume hospitals, respectively. Only 30-day mortality in low-volume hospitals of angina pectoris and myocardial infarction patients with PCI was 5.3 and 2.4 times those in high-volume hospitals with PCI, respectively. Conclusion: Mortality was significantly lower when PCI was performed in a high-volume hospital than in a low-volume hospital. Among patients admitted through the emergency room and diagnosed with angina pectoris, total PCI volume (low vs. high) was associated with significantly greater cardiac mortality risk of CHD patients. Thus, There is a need for better strategic approaches from both clinical and health policy standpoints for treatment of CHD patients.

Study on Heart Rate Variability and PSD Analysis of PPG Data for Emotion Recognition (감정 인식을 위한 PPG 데이터의 심박변이도 및 PSD 분석)

  • In this paper, we propose a method of recognizing emotions using PPG sensor which measures blood flow according to emotion. From the existing PPG signal, we use a method of determining positive emotions and negative emotions in the frequency domain through PSD (Power Spectrum Density). Based on James R. Russell's two-dimensional prototype model, we classify emotions as joy, sadness, irritability, and calmness and examine their association with the magnitude of energy in the frequency domain. It is significant that this study used the same PPG sensor used in wearable devices to measure the top four kinds of emotions in the frequency domain through image experiments. Through the questionnaire, the accuracy, the immersion level according to the individual, the emotional change, and the biofeedback for the image were collected. The proposed method is expected to be various development such as commercial application service using PPG and mobile application prediction service by merging with context information of existing smart phone.

A Study on Model Development for Community based Health Life Enhancement Centers (일 지역 건강생활실천센터 모형개발을 위한 연구)

  • Purpose: The purpose of this study was to identify the health status and request of community residents. This study is part of development of efficient health life enhancement programs. Methods: The subjects of this study were 586 households in a rural community and well-trained interviewers visited every household in the designated area and individually interviewed heads of households for general information and health service utilization. The data were collected using a questionnaire for seven months from March 25 to September 24, 2002. The data were analyzed using frequency. percentage by SPSSWIN (v 10.0). Results: 1. 58.9% of subjects were above sixty, and 60.8% of them were women. 2. The most serious problems they identified were health problems (45.6%), and the second was(##-was+were) economic problems(22.0%). 3. Almost half of the study population regarded themselves as healthy (46.2%). The types of illness they had were neuralgia, arthritis, hypertension, diabetes, and heart disease. 4. Most of the study population answered that they didn't do anything special for health enhancement. 5. Among medical facilities, hospitals or medical clinics (55.3%) were most commonly utilized when they were sick. 6. The highest request of the subjects was 'Home visiting service (45.4%)', and 'free treatment connected to the service of hospitals' was the second. What they needed most regarding health enhancement programs in health life enhancement centers was 'free health examination (58.2%), and heath education programs (57.7%). 7. As a result of investigating the subjects' demand for nursing services in health life enhancement center, 'the clinical examination' was the most requested, and next was 'blood pressure check', 'emergency treatment', and 'rehabilitation service', in that order.

A Statistical Analysis on Forensic Autopsies Performed in Korea in 2017 (2017년도 법의부검에 대한 통계적 고찰)

  • Statistical analysis was performed on national forensic autopsy data collected in the Republic of Korea, with the exception of Ulsan, during 2017. A total of 8,777 cases were categorized based on the region; information was provided by the Police Agency and the Coast Guard regarding sex, age, manner of death, and cause of death. Analysis of the manner of death revealed that 3,971 cases (45.2%) were unnatural deaths, 3,679 cases (41.9%) were natural deaths, and 1,127 cases (12.8%) were unknown deaths. Among the unnatural deaths, the majority of the cases (1,740 cases, 43.8%) were accidents, 1,316 cases (33.1%) were suicide, 385 cases (9.7%) were homicide, and 530 cases (13.3%) were undetermined deaths. Among the unnatural deaths, the majority of the cases (1,575 cases, 39.7%) were trauma, followed by 793 cases (20.0%) of poisoning and 689 cases (17.4%) of asphyxia. Falling down was the major cause of death by trauma (737 cases, 46.8%). As a result of the classification of asphyxia based on previous study, strangulation was the major cause, with 538 cases (78.1%). Among the natural deaths, heart disease was the major cause (1,790 cases, 48.7%), followed by vascular disease (697 cases, 18.9%).

Prehospital care after return of spontaneous circulation in out-of-hospital cardiac arrest patients: Based on Heart Saver laureate (병원 전 심정지 환자의 자발순환 회복에 관한 병원전 처치 - 하트세이버 수상자를 중심으로 -)

  • Purpose: We aimed to improve the survival rates of out-of-hospital cardiac arrest patients. Methods: We analyzed data regarding cardiopulmonary resuscitation (CPR) outcomes and clinical characteristics of out-of-hospital cardiac arrest patients. The data included prehospital emergency medical service reports of 207 patients, 135 patients of Heart Saver, who survived over 72 hours after return of spontaneous circulation (ROSC) in Gyeonggi-do from January, 2012 to December, 2013. Data were analyzed using SPSS 18.0 descriptive statistics. Results: Among patients who achieved ROSC, 87.6% were men and 73.6% were aged 41-70 years; 86.7% were cases of witnessed cardiac arrest, and cardiopulmonary resuscitation was performed by bystanders in 65.9% of cases. The initial electrocardiogram showed ventricular fibrillation or pulseless ventricular tachycardia in 96.3% of patients. The call time was 1.0 minutes, arrival time was 6.3 minutes, time spent at the scene was 8.0 minutes, hospital arrival time was 10.0 minutes, and total CPR duration was 9.6 minutes. The certificate of them was paramedics in 89.6%. Conclusion: To improve the survival rates of out-of-hospital cardiac arrest patients, standard prehospital care for these patients and educational programs regarding CPR for lay rescues should be developed.
