Surgical Reconstruction of old Calcaneal Fracture (진구성 종골 골절의 재건술)

  • Treatment of calcaneal fraclure is difficult and full of controversy still and choice of treatment of the displaced intracalcaneal fracture is not available yet. Furthermore, the treatment of old calcaneal fracture with displaced subtalar joint or malunited calcaneal fracture is really difficult and painful to solve the problem other than subtalar arthrodesis, ignoring conservative treatment, excision of bone mass and/or adhesiolysis, which is/are a kind of palliative or salvage treatment in stead of definitive treatment that restores smooth articular surface of the subtalar joint as far as we can. Authors had some experiences treating this difficult old and displaced calcaneal fractures. Some of them were malunited already. Hereby we report our favorable results to treat the fractures with surgical reduction (reconstruction) and internal fixation without bone graft. We recommend reconstruction of the displaced subtalar joint even though it is not congruent and partly gone to get subtalar motion insead of palliative operation such as subtalar fusion, which can be done later and long term potential cause of mid tarsal arthrosis of the foot.

Limited Posterior Approach for the Surgical Treatment of Intraarticular Fracture of Calcaneus (제한적 후방 도달법을 이용한 관절내 종골 골절의 치료)

  • Purpose: To evaluate the clinical efficacy of the limited posterior approach for the surgical treatment of intraarticular fracture of calcaneus. Materials and Methods: From March 2000 to February 2006, we studied retrospectively 186 patients, 203 cases who were treated with open reduction and internal fixation through limited posterior approach and were followed up for more than 1 year. The clinical results were evaluated with Creighton-Nebraska score and circle draw test after 1 year. We checked simple AP, lateral, axial and Broden's view preoperatively and 1 year after surgery, and compared $B{\ddot{o}}hler$ angle and Gissane angle. Results: By Creighton-Nebraska score, Sanders type 2 was 86.4, type 3 was 74.3, type 4 was 62.4. And by circle draw test, type 2 was 8.9 cm, type 3 was 7.2 cm, type 4 was 5.9 cm. $B{\ddot{o}}hler$ angle and Gissane angle were $7.6^{\circ}$, $102.4^{\circ}$, and it increased to $23.5^{\circ}$, $128.6^{\circ}$ after postoperative 1 year. Conclusion: Limited posterior approach for the surgical treatment of intraarticular fracture of calcaneus was considered to an effective treatment modality.

Sural nerve grafts in subacute facial nerve injuries: a report of two cases

  • Because facial nerve injuries affect the quality of life, leaving them untreated can have devastating effects. The number of patients with traumatic and iatrogenic facial nerve paralysis is considerably high. Early detection and prompt treatment during the acute injury phase are crucial, and immediate surgical treatment should be considered when complete facial nerve injury is suspected. Symptom underestimation by patients and clinical misdiagnosis may delay surgical intervention, which may negatively affect outcomes and in some cases, impair the recovery of the injured facial nerve. Here, we report two cases of facial nerve injury that were treated with nerve grafts during the subacute phase. In both cases, subacute facial nerve grafting achieved significant improvements. These cases highlight surgical intervention in the subacute phase using nerve grafts as an appropriate treatment for facial nerve injuries.

  • A cyst is a pathologic lesion characterized by a cavity filled with fluid, celluar products, air, or a combination of these. Dentigerous cysts were formed around the crown of unerupted teeth. The reduced enamel epithelium persists around the crown after it has formed. Proliferation of the epithelium in a fluid-filled sac may be induced by osmotic pressure. In the first decade the most frequent location is the premolar site. In each subsequent decade the largest number of cysts are in the mandibular third molar site, with the second most frequent site being the maxillary canine. The treatment of odontogenic cyst can be mostly classified into three types of cyst enucleation, marsupialization and decompression. We should consider age of patient, anatomic structure, location and size for choosing a treatment method. Advantage of cyst enucleation is fast healing, but a injury of a surrounding structure is highly. Marsupialization is conservative treatment that can reduce the damage of a adjacent structure, but it is only limited at superficial lesion. Decompression also is conservative treatment, but it has the difficulty of the oral hygiene and the troublesome of the lavage. We present the possibility that reduces the defect of decompression and cures the lesion efficiently. We report a male patient with the dentigerous cyst developed at left mandibular third molar in this study. We used the decompression for a treatment and created special appliance to treat the lesion efficiently. We report a case of the cyst treatment that is association with efficiency of decompression appliance.

  • Iatrogenic femoral artery pseudoaneurysm is a complication in patients undergoing catheterization. The risk increased when large-bore sheaths, concomitant anticoagulation therapy, and antiplatelet therapy are used during the intervention. Ultrasound-guided thrombin injection has become the treatment of choice. Rapid expansion, rupture, infection, and mass effect resulting in distal or cutaneous ischaemia or peripheral neuropathy, as well as failure of other treatment options are all indications for surgery. We report a 48-year-old man who developed hemorrhagic shock due to femoral pseudoaneurysm rupture after coronary angiography, and successfully treated by ultrasound-guided thrombin injection.

Outcome of Video-assisted Thoracoscopic Surgery for Spontaneous Secondary Pneumothorax

  • Background: Conventional treatment (i.e. chest tube insertion and chemical pleurodesis) still remains standard for patients with secondary spontaneous pneumothorax because the risk of surgical bullectomy is deemed high in this subset. However, it has been suggested that surgical treatment using thoracoscopy may expedite postoperative recovery and, thus, may reduce hospital stay. Materials and Methods: Retrospective review of 61 patients with secondary spontaneous pneumothorax, who underwent conventional treatment (n=39) or video-assisted thoracoscopic surgery (VATS) (n=22) between January 2007 and December 2009, was performed. Talc was used for chemical pleurodesis in both groups. Results: Hospital stay of conventional treatment group and VATS group was $14.2{\pm}14.2$ days (4~58 days) and $10.6{\pm}5.8$ days (5~32 days), respectively, with statistically significant difference (p=0.033). Recurrence rate of conventional treatment group was also significantly higher (12/39, 30%) compared to VATS group (1/22, 4.5%) (p=0.016). Conclusion: In selected patients with secondary spontaneous pneumothorax with continuous air leak or inadequate lung expansion, thoracoscopic surgery with chemical pleurodesis using talc results in shorter hospital stay and lower recurrence rate compared to conventional approach.

Study on the Radial Pulse Wave Variables and Heart Rate Variability after Acupuncture Stimulation (침자극이 좌관맥상과 심박수 미세변화에 미치는 영향)

  • We investigated the effects of acupuncture stimulation on pulse wave variables and heart rate variability (HRV) in healthy adults. To evaluate twenty healthy volunteers (10 men, 10 women) had acupuncture into both Hapkok (Ll4) and Taechung (Liv3) for 15 minutes. Radial pulse, Heart Rate Variability, body temperature and blood pressure were checked before and after acupuncture to evaluate Pulse Wave Variability and Autonomous Function. The results were as follows; Heart rate was significantly increased while systolic blood pressure (mmHg) and diastolic blood pressure (mmHg) were significantly increased after acupuncture treatment. Sixteen subjects didn't change representative pulse wave variables after acupuncture treatment. Energy, height of main peak (H1), height of pre-dicrotic valley (H2) and height of descending valley (H4) were decreased while height of dicrotic peak (H5) was significantly decreased after acupuncture treatment. Time to main peak (T1), time to pre-dicrotic valley (T2), time to dicrotic peak (T5), total time (T) and T-T4 were decreased while time to descending valley (T4) was increased after acupuncture treatment. Total area (At), area of main peak (Aw) and ratio of diastolic period area (Ad) were decreased while ratio of systolic period area (As) and angle of main peak (MPA) were increased after acupuncture treatment. The standard deviation of all normal RR intervals (SDNN) was increased while the root mean square of successive differences between the normal heart beats (RMSSD) was significantly increased after acupuncture treatment by time domain analysis. Low frequency power (LF) and LF/HF ratio were decreased while high frequency power (HF) was significantly increased after acupuncture treatment by frequency domain analysis. This study suggests that acupuncture treatment changes pulse wave variability and heart rate variability. Further study on various acupuncture treatment for pulse wave variability and heart rate variability is required.

Protective Effect of Lonicerae Flos Aqueous Extracts on a Pressure Overload-induced Heart Failure Model

  • Objectives: Lonicerae flos (LF), a dried flower part of Lonicera japonica Thunb., has been widely used in Korean medicine as anti-inflammatory and antioxidative agent. The purpose of this study was to determine the cardioprotective effects of LF, through potential antioxidant effects, on the pressure overload (PO)-induced heart failure (HF) in C57BL/6 mice after transverse aortic constriction (TAC) surgery. Methods: Resveratrol (10 mg/kg body weight) or LF (125, 250 or 500 mg/kg body weight) was orally administered, once daily for 14 days, starting 14 days after TAC surgery. Changes in the mortality, body weights, heart weights, histopathology of the heart, and antioxidant defense systems of the heart were analyzed. Results: Marked and noticeable increases of heart weights, mortalities, and hypertrophic, focal, and lytic fibrotic histological changes in the LVs were observed, with destruction of heart antioxidant defense systems after surgery. However, HF signs, induced by TAC surgery through PO, and destruction of heart antioxidant defense systems were significantly and dose-dependently inhibited by 14 days of maintained oral treatment with LF 500, 250 or 125 mg/kg. Treatment with 250 mg/kg LF was comparable to treatment with 10 mg/kg resveratrol. Conclusions: The results in this study suggest that oral administration of LF favorably relieves PO-induced HF following TAC, through increase of heart antioxidant defense systems. The overall effects of 250 mg/kg LF were similar to those of 10 mg/kg resveratrol. More detailed mechanistic studies should be conducted in the future, with screening of the biologically active compounds in LF.

Case Study of Patient with Pleural Effusion Due to Congestive Heart Failure (울혈성 심부전으로 인한 흉막삼출에 대한 한방치험 1례)

  • The purpose of this case was to report the effect of Korean medical treatment for patient with pleural effusion due to congestive heart failure. The patient was treated with herbal medicine(Cheongsingeonbi-tang) and acupuncture. The effect of treatment was evaluated by chest X-ray, New York Heart Association(NYHA) functional classification, and Hugh-Jones classification. After 3 weeks of treatment, the amount of pleural effusion was decreased and NYHA class, Hugh-Jones grade were improved. NYHA functional classification improved class III to II and Hugh-Jones classification changed grade IV to II. This result suggests that herbal medicine(Cheongsingeonbi-tang) and acupuncture treatment might have an effect on patient with pleural effusion due to congestive heart failure.

  • Patients who have repaired cleft lip and palate generally undergo restriction of maxillary growth. Concave facial profile is often exhibited with relatively normalized mandible. Horizontal and sagittal deficiency of the maxilla could cause anterior and posterior crossbites. In growing patients, ortho-dontic and orthopedic treatment is acceptable with maxillary expansion and protraction. However, surgical approach has to be accompanied with orthodontic treatment in skeletally matured patients. We used SARPE and BSSRO to expand the constricted maxilla and retract the mandible in a patient who had cleft palate repaired in infancy. Through SARPE, orthodontic treatment and BSSRO, we sufficiently expanded the maxillla and improved facial profile.
