중등도 이상의 좌심실 기능 부전 환자에서의 관상동 우회술의 임상 분석 (Coronary Artery Bypass Graft in Patient with Advanced Left Ventricular Dysfunction)

  • 배경 : 관상동맥 질환 환자에게 시행되는 관상동맥 우회술의 수술 성적이 많이 향상되었으나, 아직도 좌심실기능부전이 중등도 이상으로 심한 환자에서의 관상동맥 우회술은 합병증과 사망률이 비교적 높다. 최근 수술기법 및 심근보호법의 발달로 이러한 고위험군의 환자에게 내과적 보존 치료보다는 외과적 재혈관화가 환자의 증상 개선과 장기 생존율을 향상시킨다고 한다. 대상 및 방법 : 이에 저자는 1995년 1월부터 1999년 3월까지 시행한 관상동맥 우회술 843예 가운데 수술 전 좌심실 박출계수가 30% 이하인 환자 31예(4.1%)의 임상자료를 후향적으로 조사하고 수술 전후 측정한 심장 초음파상의 심박출 계수의 변화를 비교 분석하였다. 환자의 연령은 41세에서 72세 사이로 평균 60.7$\pm$2.2세였고, 남자 26예, 여자 5예였다. 수술전 위험인자로 심근경색의 과거력이 있었던 경우가 30예로 대부분이었으며, Thallium heart scan 검사에서 불가역인 심근 손상이 7예였다. 관상동맥 조영술에서 3개 혈관 병변이 26, Rentrop 분류 1도가 16례로 가장 많았다. 관상동맥 우회술 동안에 이식된 혈관의 수는 평균 4.88$\pm$0.8 개/명이었고, 전 예에서 복재정맥을 사용하였으며 내흉동맥을 동시에 사용한 경우는 20예였다 대동맥 차단 및 심실세동 시간은 평균 77.9$\pm$1.6분 이었고 인공 심폐기 가동시간은 평균 244.7$\pm$3.7분 이였으며, 관상동맥 우회술과 동시에 시행된 술식으로는 좌심실류 제거술 2예, 승모판막 성형술 2예, 대동맥 판막 치환술 1예였다. 술후 합병증은 부정맥 3예, 출혈 2예, 흉골 지연 봉합 1예였고 수술 전후의 대동맥내 풍선펌프의 사용이 11예였으며, 2예에서 사망하여 수술 사망률은 6.5%이었다. 수술 후 흉통 및 증상의 개선을 보인 경우는 29예였고 수술 후 시행한 심초음파 검사상 좌심실 박출 계수는 평균 38.5$\pm$11.6%로 술전 평균 측정치 25.3$\pm$2.3%에 비해 유의하게 증가되었다(p 0.001). 환자들의 평균 추적기간은 25.3$\pm$5.6개월이었다. 결론 : 좌심실 기능이 저하된 관상동맥 질환 환자에서 관상동맥 우회술을 시행하여 비교적 만족할 만한 결과를 얻을 수 있었으며, 추후 장기 추적 조사가 필요할 것으로 생각된다.

승모판막수술 시 히스티딘를 함유한 결정성 심정지액(Histidine-tryptophan-ketoglutarate Solution)과 저온 혈성 심정지액이 심근기능 보존에 미치는 영향 비교 (A Comparison of the Effects of Histidine-tryptophan-ketoglutarate Solution versus Cold Blood Cardioplegic Solution on Myocardial Protection in Mitral Valve Surgery)

  • 배경: 개심술 시 불충분한 심근 보호로 인한 허혈과 재관류 손상은 술 후 심실 기능과 예후에 영향을 미친다. 본 저자들은 승모판막 수술에서 HTK 용액과 냉혈 혈성 심정지액이 심근 보호와 임상 결과에 미치는 영향을 전향적으로 비교 분석하였다. 대상 뜻 방법: 승모판막폐쇄부전으로 승모판막수술을 받는 70명의 환자 중 8명의 환자를 제외한 31명에서 HTK 용액을(HTK군), 31명에서 냉혈 혈성 심정지액을(CBC군) 사용하였다. 수술 중과 후에 환자의 혈역학, 심혈관계 약물과 심장박동조율기의 사용, 임상경과 및 합병증을 관찰하였다. 모든 환자에서 최소 6개월 이상 사망률과 이환율을 추적 관찰하였다. 결과: 혈역학적 변수는 평균폐동맥압, 중심정맥압과 폐모세혈관쐐기압이 체외순환으로부터 이탈 후 시기에 HTK군에서 유의하게 낮았던 점을 제외하고는 모든 시기에서 군 간 차이가 없었다. 심근수축제와 심장박동조율기의 사용은 모든 시기에서 군 간 차이가 없었다. 수술 후 1일과 2일째 CK-MB 수치는 HTK군에서 $77{\pm}54,\;41{\pm}23$ (ng/mL)로 CBC군의 $70{\pm}69,\;44{\pm}34$ (ng/mL)와 유의한 차이가 없었다. 6개월 이상의 추적관찰 중 임상경과는 비슷하였다. 결론: 이상의 결과에서 HTK 용액은 냉혈 혈성 심정지액과 비슷한 심근 보호효과를 가짐을 확인할 수 있었다.

비글견에서 Propofol/Remifentanil과 Ketamine/Remifentanil을 사용한 완전 정맥 내 마취법의 비교 (Comparison between Propofol/Remifentanil and Ketamine/Remifentanil for TIVA in Beagle Dogs)

  • 비글견에서 remifentanil/ketamine 점적 투여 병용마취법과 remifentanil/propofol 점적 투여 병용마취법이 심폐기능에 미치는 영향에 대하여 비교평가하였다. 14 마리의 비글견을 이용하였다. 실험견은 acepromazine (0.1 mg/kg, 피하)과 medetomidine (20 ${\mu}g$/kg, 정맥내)으로 전처치하고, Group P는 정맥 내 propofol 1 mg/kg, Group K는 정맥 내 ketamine 5 mg/kg으로 마취 유도 하고, 이 후 실험군별로 고정된 용량의 remifentanil (0.5 ${\mu}g$/kg/min)과 ketamine 0.1 mg/kg/min 또는 propofol 0.3 mg/kg/min을 3 시간 동안 투여하였다 (Group K와 Group P). 동맥혈압, 심박수, 호흡 수, 혈액가스분석과 마취회복기 동안의 행동변화를 측정하였다. 또한 toe-web clamping 검사를 통해 마취 깊이를 평가하였다. 외과적 마취기는 두 군 모두에서 전 시간 동안 유지가 되었다. Group K의 수축기 동맥혈압, 평균 동맥혈압, 동맥산소 분압, 동맥 산소 포화도는 Group P에 비해 정상 범위 내에서 현저히 높았으며 Group K의 이산화탄소 분압은 Group P에 비해 현저히 낮았다. 그러나 이완기 동맥혈압, 심박수, 호흡수에서는 현저한 차이가 없었다. 점적투여 중단시점부터 발관까지의 평균시간은 Group K에서 현저히 감소되었지만, 평균 sitting time은 Group P에서 현저히 감소되었다. 평균 head-up time과 평균 walking time은 현저한 차이가 없었다. Group K에서는 약간의 근강직, 머리 흔듬, 혀로 핥는 동작이 회복기에 관찰되었다. 결론적으로, Group K가 Group P보다 심폐 기능에서 더 좋았다. 즉, remifentanil/ketamine을 이용한 점적투여 병용마취법이 remifentanil/propofol을 이용한 점적투여 병용마취법 보다 3 시간의 마취 유지에서 보다 나은 방법으로 판단되었다.

Treat-to-Target Strategy for Asian Patients with Early Rheumatoid Arthritis: Result of a Multicenter Trial in Korea

  • Background: To evaluate the therapeutic benefits of the treat-to-target (T2T) strategy for Asian patients with early rheumatoid arthritis (RA) in Korea. Methods: In a 1-year, multicenter, open-label strategy trial, 346 patients with early RA were recruited from 20 institutions across Korea and stratified into 2 groups, depending on whether they were recruited by rheumatologists who have adopted the T2T strategy (T2T group) or by rheumatologists who provided usual care (non-T2T group). Data regarding demographics, rheumatoid factor titer, anti-cyclic citrullinated peptide antibody titer, disease activity score of 28 joints (DAS28), and Korean Health Assessment Questionnaire (KHAQ) score were obtained at baseline and after 1 year of treatment. In the T2T group, the prescription for disease-modifying antirheumatic drugs was tailored to the predefined treatment target in each patient, namely remission (DAS28 < 2.6) or low disease activity (LDA) ($2.6{\leq}DAS28$ < 3.2). Results: Data were available for 163 T2T patients and 162 non-T2T patients. At the end of the study period, clinical outcomes were better in the T2T group than in the non-T2T group (LDA or remission, 59.5% vs. 35.8%; P < 0.001; remission, 43.6% vs. 19.8%; P < 0.001). Compared with non-T2T, T2T was also associated with higher rate of good European League Against Rheumatism response (63.0% vs. 39.8%; P < 0.001), improved KHAQ scores (-0.38 vs. -0.13; P = 0.008), and higher frequency of follow-up visits (5.0 vs. 2.0 visits/year; P < 0.001). Conclusion: In Asian patients with early RA, T2T improves disease activity and physical function. Setting a pre-defined treatment target in terms of DAS28 is recommended.

  • Objectives : Ten heavenly stems(10天干) and Twelve earthly branches(12地支) are symbols exposing change order in heaven and earth, and are a very important sign in studying oriental philosophy and oriental medicine. Especially, 10 heavenly stems(10天干) and 12 earthly branches(12地支) are indispensable for the study of Five Circuits And Six Qi(오운육기), and a deep study is needed. Methods : I have examined Yin Yang combination(음양배합), Five elements combination(오행배합), Six Qi 3Yin 3Yang combination(육기삼음삼양배합), viscera combination(장부배합), Mutual collision(상충), Six combination(육합), Three combination(삼합), etc. of 12 earthly branches(12지지) by referring to books such as "Yellow Emperor Internal Classic" ("黃帝內經") and "Principle of universe change" ("우주변화의 원리"). Results & Conclusions : Zi Yin Chen Wu Shen Xu(子 寅 辰 午 申 戌) become Yang(陽), Chou Mao Si Wei You Hai(丑 卯 巳 未 酉 亥) become Yin(陰), Zi Si Yin Mao Chen Si(子 丑 寅 卯 辰 巳) become Yang, and Wu Wei Shen You Xu Hai(午 未 申 酉 戌 亥) become Yin. Twelve earthly branches can be divided into five movements by its original meaning, where YinMao(인묘) is tree, SiWu(사오) is a fire, ShenYou(신유) is a gold, HaiZi(해자) is water, and ChenXuChouWei(진술축미) mediate in the middle of four movements So they become soil(土). SiHai(巳亥) is JueYin Wind Tree(궐음 풍목), ZiWu(子午) is ShaoYin Monarch Fire(소음 군화), ChouWei(丑未) is TaiYin Humid Soil(태음 습토), YinShen(寅申) is ShaoYang Ministerial Fire(소양 상화), MaoYou(卯酉) is YangMing Dry Gold(양명 조금), and ChenXu(辰戌) is TaiYang Cold Water(태양 한수). Viscera combination(장부배합) combines Zi(子) and Bile(膽), Chou(丑) and Liver(肝), Yin(寅) and Lung(肺), Mao(卯) and Large intestine(大腸), Chen(辰) and Stomach(胃), Si(巳) and Spleen(脾), Wu(午) and Heart(心), Wei(未) and Small intestine(小腸), Shen(申) and Bladder(膀胱), You(酉) and Kidney(腎), Xu(戌) and Pericardium(心包), Hai(亥) and Tri-energizer(三焦), Which means that the function of the viscera and channels is the most active at that time. Twelve earthly branches mutual collisions collide with Zi(子) and Wu(午), Chou(丑) and Wei(未), Yin(寅) and Shen(申), Mao(卯) and You(酉), Chen(辰) and Xu(戌), and Si(巳) and Hai(亥). The two colliding earthly branches are on opposite sides, facing each other and restricting each other by the relation of Yin-Yin and Yang-Yang it rejects each other so a collision occurs. Six Correspondences(六合) coincide with Zi(子) and Chou(丑), Yin(寅) and Hai (亥), Mao(卯) and Xu(戌), Chen(辰) and You(酉) and Si(巳) and Shen(申) Wu(午) and Wei(未). Three combination(三合) is composed of ShenZiChen(申子辰), SiYouChou(巳酉丑), YinWuXu(寅午戌), and HaiMaoWei(亥卯未). Three combination(三合) is composed of ShenZiChen(申子辰), SiYouChou(巳酉丑), YinWuXu(寅午戌), and HaiMaoWei(亥卯未). This is because the time Six Qi(六氣) shifts in these three years are the same.

서울 진오기굿의 재차구성과 의미 (The Composition and Principles of Seoul Jinogigut (Shamanistic Ritual))

  • 이 글은 서울지역에서 행해지는 진오기굿의 구성 원리를 도출하고 이를 바탕으로 의미를 규명한 글이다. 진오기굿은 신과 인간이 복합적으로 연결되어 있으면서, 이승과 저승이 혼재되어 있는 세계이고, 인간의 세계에 있던 망자가 신의 세계로 들어가는 과정을 보여주는 의례이다. 진오기굿은 망자를 저승으로 인도하는 과정을 차례로 보여준다. 먼저 신 중심의 거리가 연행되는데, 신이 어떤 과정을 통해 망자를 저승으로 인도할지를 보여준다. 다음으로는 신과 인간이 중심이 되는 거리로 사자를 따라 저승으로 가는 망자의 모습을 보다 구체적으로 보여준다. 마지막으로 인간 중심의 구조로 인간이 어떻게 저승으로 들어가는지를 보여준다. 이러한 반복을 통해 망자가 마침내 저승에 좌정했음을 드러낸다. 신 중심의 굿거리가 신과 인간의 만남을 거쳐 인간 중심으로 연행되는 것이 진오기굿 후반부 저승굿의 짜임이다. <도령돌기> 이후에 연행되는 <베가르기>, <뒷영실>은 망자가 온전하게 저승에 들어갔음을 확인하는 의례이다. 베를 갈라 망자가 저승으로 들어가는 것이 완벽하게 이루어졌음을 보여주고, <뒷영실>에서 망자는 다시 한 번 굿청에 들어와 굿을 해 준 것에 대한 감사함과 함께 죽음의 세계에 안착했음을 알려준다. 저승에 좌정한 조상신에게 비로소 제사를 올리는 <상식거리>는 진오기굿을 통해 인간이었던 망자가 조상신으로 변화했음을 알려준다. 진오기굿은 갑작스러운 죽음을 겪은 재가집을 위로해주는 기능도 수행한다. 재가집의 위로를 위해 가장 필요한 것은 망자가 무사히 저승으로 갔음을 보여주는 것이다. 진오기굿은 그러한 과정을 삼중구조를 통해 보여준다. 신들의 세계에서 망자가 어떻게 천도될 것인가를 보여주고, 저승에서 이승으로 나온 사재를 따라 망자가 무사히 저승으로 들어감을 보여준다. 다시 바리공주 뒤를 따라 저승으로 들어가는 망자의 모습을 보여주어 남은 가족의 마음을 달래준다.

Cefoperazone(T-1551)의 약리학적 연구 (Pharmacological Studies of Cefoperazone(T-1551))

  • The pharmacological and microbiological studies of Cefoperazone (T-1551, Toyama Chemical Co., Japan) were conducted in vitro and in vivo. The studies included stability and physicochemical characteristics, antimicrobial activity, animal and human pharmacokinetics, animal pharmacodynamics and safety evaluation of Cefoperazone sodium for injection. 1) Stability and physicochemical characteristics. Sodium salt of cefoperazone for injection had a general appearance of white crystalline powder which contained 0.5% water, and of which melting point was $187.2^{\circ}C$. The pH's of 10% and 25% aqueous solutions were 5.03 ana 5.16 at $25^{\circ}C$. The preparations of cefoperazone did not contain any pyrogenic substances and did not liberate histamine in cats. The drug was highly compatible with common infusion solutions including 5% Dextrose solution and no significant potency decrease was observed in 5 hours after mixing. Powdered cefoperazone sodium contained in hermetically sealed and ligt-shielded container was highly stable at $4^circ}C{\sim}37^{\circ}C$ for 12 weeks. When stored at $4^{\circ}C$ the potency was retained almost completely for up to one year. 2) Antimicrobial activity against clinical isolates. Among the 230 clinical isolates included, Salmonella typhi was the most susceptible to cefoperazone, with 100% inhibition at MIC of ${\leq}0.5{\mu}g/ml$. Cefoperazone was also highly active against Streptococcus pyogenes(group A), Kletsiella pneumoniae, Staphylococcus aureus and Shigella flexneri, with 100% inhibition at $16{\mu}g/ml$ or less. More than 80% of Escherichia coli, Enterobacter aerogenes and Salmonella paratyphi was inhibited at ${\leq}16{\mu}/ml$, while Enterobacter cloaceae, Serratia marcescens and Pseudomonas aerogenosa were somewhat less sensitive to cefoperagone, with inhibitions of 60%, 55% and 35% respectively at the same MIC. 3) Animal pharmacokinetics Serum concentration, organ distritution and excretion of cefoperazone in rats were observed after single intramuscular injections at doses of 20 mg/kg and 50 mg/kg. The extent of protein binding to human plasma protein was also measured in vitro br equilibrium dialysis method. The mean Peak serum concentrations of $7.4{\mu}g/ml$ and $16.4{\mu}/ml$ were obtained at 30 min. after administration of cefoperazone at doses of 20 mg/kg and 50 mg/kg respectively. The tissue concentrations of cefoperazone measured at 30 and 60 min. were highest in kidney. And the concentrations of the drug in kidney, liver and small intestine were much higher than in blood. Urinary and fecal excretion over 24 hours after injetcion ranged form 12.5% to 15.0% in urine and from 19.6% to 25.0% in feces, indicating that the gastrointestinal system is more important than renal system for the excretion of cefoperazone. The extent of binding to human plasma protein measured by equilibrium dialysis was $76.3%{\sim}76.9%$, which was somewhat lower than the others utilizing centrifugal ultrafiltration method. 4) Animal pharmacodynamics Central nervous system : Effects of cefoperazone on the spontaneous movement and general behavioral patterns of rats, the pentobarbital sleeping time in mice and the body temperature in rabbits were observed. Single intraperitoneal injections at doses of $500{\sim}2,000mg/kg$ in rats did not affect the spontaneous movement ana the general behavioral patterns of the animal. Doses of $125{\sim}500mg/kg$ of cefoperazone injected intraperitonealy in mice neither increased nor decreased the pentobarbital-induced sleeping time. In rabbits the normal body temperature was maintained following the single intravenous injections of $125{\sim}2,000mg/kg$ dose. Respiratory and circulatory system: Respiration rate, blood pressure, heart rate and ECG of anesthetized rabbits were monitored for 3 hours following single intravenous injections of cefoperazone at doses of $125{\sim}2,000mg/kg$. The respiration rate decreased by $3{\sim}l7%$ at all the doses of cefoperazone administered. Blood pressure did not show any changes but slight decrease from 130/113 to 125/107 by the highest dose(2,000 mg/kg) injected in this experiment. The dosages of 1,000 and 2,000 mg/kg seemed to slightly decrease the heart rate, but it was not significantly different from the normal control. All the doses of cefoperazone injected were not associated with any abnormal changes in ECG findings throughout the monitering period. Autonomic nervous system and smooth muscle: Effects of cefoperazone on the automatic movement of rabbit isolated small intestine, large intestine, stomach and uterus were observed in vitro. The autonomic movement and tonus of intestinal smooth muscle increased at dose of $40{\mu}g/ml$ in small intestine and at 0.4 mg/ml in large intestine. However, in stomach and uterine smooth muscle the autonomic movement was slightly increased by the much higher doses of 5-10 mg/ml. Blood: In vitro osmotic fragility of rabbit RBC suspension was not affected by cefoperazone of $1{\sim}10mg/ml$. Doses of 7.5 and 10 mg/ml were associated with 11.8% and 15.3% prolongation of whole blood coagulation time. Liver and kidney function: When measured at 3 hours after single intravenous injections of cefoperaonze in rabbits, the values of serum GOT, GPT, Bilirubin, TTT, BUN and creatine were not significantly different from the normal control. 5) Safety evaluation Acute toxicity: The acute toxicity of cefoperazone was studied following intraperitoneal and intravenous injections to mice(A strain, 4 week old) and rats(Sprague-Dawler, 6 week old). The LD_(50)'s of intraperitonealy injected cefoperazone were 9.7g/kg in male mice, 9.6g/kg in female mice and over 15g/kg in both male and female rats. And when administered intravenously in rats, LD_(50)'s were 5.1g/kg in male and 5.0g/kg in female. Administrations of the high doses of the drug were associated with slight inhibition of spontaneous movement and convulsion. Atdominal transudate and intestinal hyperemia were observed in animals administered intraperitonealy. In rats receiving high doses of the drug intravenously rhinorrhea and pulmonary congestion and edema were also observed. Renal proximal tubular epithelial degeneration was found in animals dosing in high concentrations of cefoperazone. Subacute toxicity: Rats(Sprague-Dawley, 6 week old) dosing 0.5, 1.0 and 2.0 g/kg/day of cefoperazone intraperitonealy were observed for one month and sacrificed at 24 hours after the last dose. In animals with a high dose, slight inhibition of spontaneous movement was observed during the experimental period. Soft stool or diarrhea appeared at first or second week of the administration in rats receiving 2.0g/kg. Daily food consumption and weekly weight gain were similar to control during the administration. Urinalysis, blood chemistry and hematology after one month administration were not different from control either. Cecal enlargement, which is an expected effect of broad spectrum antibiotic altering the normal intestinal microbial flora, was observed. Intestinal or peritoneal congestion and peritonitis were found. These findings seemed to be attributed to the local irritation following prolonged intraperitoneal injections of hypertonic and acidic cefoperazone solution. Among the histopathologic findings renal proximal tubular epithelial degeneration was characteristic in rats receiving 1 and 2g/kg/day, which were 10 and 20 times higher than the maximal clinical dose (100 mg/kg) of the drug. 6) Human pharmacokinetics Serum concentrations and urinary excretion were determined following a single intravenous injection of 1g cefoperazone in eight healthy, male volunteers. Mean serum concentrations of 89.3, 61.3, 26.6, 12.3, 2.3, and $1.8{\mu}g/ml$ occured at 1,2,4,6,8 and 12 hours after injection respectively, and the biological half-life was 108 minutes. Urinary excretion over 24 hours after injection was up to 43.5% of administered dose.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury.   • 경제력집중(經濟力集中)은 경제적(經濟的) 자원(資源)과 수단(手段)의 상당부분이 소수의 경독주체(經瀆主體)에 집중되어 이들이 자원배분(資源配分)의 흐름에 큰 영향을 미칠 수 있는 상태를 말한다. 그러므로 경제력집중(經濟力集中)은 본질상 자유시장기구(自由市場機構)의 생리와는 부합하지 않지만 자본주의(資本主義)의 역사로 볼 때 그것이 바로 자유경쟁(自由競爭)의 소산이라는 측면도 있다는 점이 문제가 된다. 구미(歐美)와 일본(日本) 등에 있어서 자본주의체제(資本主義體制)의 진화궤적(進化軌跡)은 이 문제가 어떻게 전개되는가에 따라 결정되어 왔다. 우리나라에서의 경제역집중(經濟力集中)은 다수의 독(獨) 과점적(寡占的) 대기업(大企業)들이 소유관계(所有關係)로 결합되어 있는 기업집단(企業集團) 즉 소위 재벌(財閥)의 문제로 집약될 수 있다. 우리나라의 기업집단(企業集團)의 성장은 시장기구(市場機構)의 작동결과에 기인한 면도 있지만 고도경제성장기(高度經濟成長期)의 정부정책(政府政策)에 의하여 촉진된 것도 부인할 수 없다. 기업집단(企業集團)에 의한 경제력집중(經濟力集中)은 과거 우리나라의 정치(政治) 경제(經濟) 사회(社會)가 거쳐온 진화과정(進化過程)을 집약적으로 나타내는 것 이다. 그러므로 우리나라에서 민주주의(民主主義)와 자본주의(資本主義)의 이념(理念)과 질서(秩序)에 대한 국민적(國民的) 합의(合意)를 모색하려는 현시점에서 경제력집중(經濟力集中)을 객관적으로 인식하여 효율(效率)과 형평(衡平)을 조화하는 적절한 대응방향을 모색 하는 것은 매우 긴요한 과제이다.
