저온 요법은 심장수술중 심근 보호에 있어서 기본적인 요소로 널리 인식되어 왔다. 비록 저온 요법이 산소 요구량을 감소시켜 허혈성 심정지의 기간을 연장시키지만 저온 요법은 효소 기능과 에너지 생성, 세포 보존에 해를 끼치는 등의 단점을 나타낸다. 심장의 이상적인 보호 상태는 전기 기계적으로 정지하는 상태이며 혈액에 의해서 관류되는 상태 즉 호기의 심정지라고 생각된다. 전기 기계적 정지, 연속적인 온혈 심정지액의 관류에 의한 정온 호기 상태의 심정지에 기초를 둔 새로운 방법의 심근 보호법이 개발되었다. 1994년 1월부터 1996년 6월까지 간헐적 냉각 심정지액으로 심근 보호를 시행한 31명의 판막 수술을 받은 환자와 연속 온혈 심정지액을 사용하여 심근 보호를 실시한 30명의 판막 수술을 받은 환자를 임상적으로 비교 분석 하였다. 저자들의 결과는 연속 온혈 심정지액을 사용한 판막 수술을 받은 환자는 적어도 냉각 심정지액을 사용한 환자보다 대동맥 차단 해제후 자연 심박 재개율이 높으며, 술후 심부종이 발생하지 않아 심막 절개 부위의 봉합율이 높고 술후 12시간 동안의 배설되는 소변량이 증가된다는 점 및 좌측 횡격막 상승 소견이 없었으며 술후 1시간 및 12시간의 심근 효소 검사상 연속 온혈 심정지액을 사용한 군에서 그 수치가 낮은점 등으로 안전하고 효과적인 심근 보호가 이루어졌으며, 개념적으로 이것은 심장수술중에 우수한 심근 보호를 유지하는 새로운 방법임을 알 수 있었다.
Total anomalous pulmonary venous connection [TAPVC] defines a group of congenital heart disease which have in common the entire pulmonary venous drainage returning directly or indirectly to the right atrium instead of to the left atrium. Although this disorder represents only 1.3% of cases of congenital heart disease, if untreated the resultant mortality is greater than 80% in the 1st year of life. And since there is no satisfactory palliative treatment, correction of TAPVC Is high on the list of indications for open heart surgery in the 1st year of life. This paper describes 10 patients who underwent surgical correction of this disorder at SNUH between 1978 and 1983. 1. 7 were males and 3 females, with ages ranging from 5 months to 24 years. 2. 7 were supracardiac type, 2 cardiac type, and 1 mixed type TAPVC. We didnt experience infracardiac type. 3. All showed some degree of pulmonary hypertension preoperatively. 4. In 4 cases of supracardiac type, total circulatory arrest was used in brief period during anastomosis between common pulmonary venous trunk and left atrium. In the other cases, usual cardiopulmonary bypass with moderate to deep hyperthermia was used. 5. There were 2 cases of mortality; 1 died at operation, and the other at 4 months due to congestive heart failure. 6. Mortality seemed not closely related to age, body weight, or severity of pulmonary hypertension.
Paraquat, is a widely used for its great effect as a herbicide. But the mortality rate by paraquat intoxication is known to be very high. It is thought to act by changing form to superoxide and peroxide free radical. Almost paraquat intoxication is through ingestion. A few intoxication of paraquat is through skin absorption. But there was no known death case through skin absorption. We experienced a case of a expired patient by paraquat intoxication through skin abrasion and scratching wound. A 75-year-old man was visited emergency room after motorcycle accident during transporting paraquat. He has multiple abrasion and scratching wound on extremities, and pelvic bone fracture. There was no evidence of ingestion of paraquat. But serum/urine gramoxone level was all positive. In spite of wound irrigation and hemoperfusion, his condition was been gone form bed to worse. 2 days after, multiple organ failure and the respiratory arrest were developed and he was expired. Background: Minimal infusion of cardioplegic solution (CPS) during aortic surgery using total circulatory arrest (TCA) may reduce several potential side effects: clamping on a diseased aorta, insult of coronary ostia, and edema. Materials and Methods: From 2006 to 2009, 72 patients underwent aortic surgery without infusion of cardioplegic solution at the initiation of circulatory arrest. The diagnoses were acute aortic dissection (44), aneurysm (22), and intramural hematoma (6). Results: The duration of TCA, the lowest nasopharyngeal temperature, bypass time, and aortic clamp time was 45 minutes, $16.4^{\circ}C$, 162 minutes, and 100 minutes, respectively. The amount of CPS was 1,050 mL, and 15 patients underwent surgery without CPS. The average inotrope score was 113 points (range, 6.25 to 5,048.5 points) corresponding to the dopamine infusion of 5 mcg/kg/min for 1 day. Seven patients showed a level of creatine kinase-MB above 50 ng/mL, postoperatively, compared with the average of 12.75 ng/mL. The ischemic change was found on electrocardiogram in 5 patients, postoperatively. There was no cardiac morbidity requiring mechanical assist. The average of intensive care unit stay and postoperative hospital stay was 40 hours (range, 15 to 482 hours) and 11 days, respectively. Conclusion: Minimal infusion of only retrograde CPS during rewarming without initial infusion at TCA in aortic surgery is feasible and can be used with acceptable results.

Background: Survival of children experiencing cardiac arrest refractory to conventional cardiopulmonary resuscitation (CPR) is very poor. We sought to examine current era outcomes of extracorporeal CPR (ECPR) support for refractory arrest. Methods: Patients who were <18 years and underwent ECPR between November 2013 and January 2016 were including in this study. We retrospectively investigated patient medical records. Results: Twelve children, median age 6.6 months (range, 1 day to 11.7 years), required ECPR. patients' diseases spanned several categories: congenital heart disease (n=5), myocarditis (n=2), respiratory failure (n=2), septic shock (n=1), trauma (n=1), and post-cardiotomy arrest (n=1). Cannulation sites included the neck (n=8), chest (n=3), and neck to chest conversion (n=1). Median duration of extracorporeal membrane oxygenation was five days (range, 0 to 14 days). Extracorporeal membrane oxygenation was successfully discontinued in 10 (83.3%) patients. Nine patients (75%) survived more than seven days after support discontinuation and four patients (33.3%) survived and were discharged. Causes of death included ischemic brain injury (n=4), sepsis (n=3), and gastrointestinal bleeding (n=1). Conclusion: ECPR plays a valuable role in children experiencing refractory cardiac arrest. The weaning rate is acceptable; however, survival is related to other organ dysfunction and the severity of ischemic brain injury. ECPR prior to the emergence of end-organ injury and prevention of neurologic injury might enhance survival.
Objective: This study examined the initial partial pressure of carbon dioxide ($PCO_2$) as a possible indicator of prehospital ventilation and its association with prehospital i-gel in out-of-hospital cardiac arrest (OHCA) patients. Methods: The demographics and arrest parameters, including i-gel insertion and initial arterial blood gas analysis, of OHCA patients who visited the emergency department were analyzed retrospectively. Linear regression analysis was performed to examine the association between i-gel insertion and the initial $PCO_2$. Results: A total of 106 patients were investigated. Fifty-six patients had prehospital i-gel insertion and 50 patients did not have a prehospital advanced airway. The initial $PCO_2$ was higher in the i-gel group than the no advanced airway group (105.2 mmHg [77.5-134.9] vs. 87.5 mmHg [56.8-115.3], P=0.03). Prehospital i-gel insertion was associated with a higher initial $PCO_2$ level (${\beta}$ coefficient, 20.3; 95% confidence interval, 2.6-37.9; P=0.03). Conclusion: Prehospital insertion of i-gel was associated with higher initial $PCO_2$ values in OHCA patients compared to no advanced airway.
One case of surgical correction of a large aortopulmonary window in a 4 months old female is reported in detail. Clinical symptoms consisted of fatigue when feeding and recurrent upper respiratory tract infection. Under cardiopulmonary bypass with moderate hypothermia and cardioplegic arrest of the heart, the aortopulmonary window was approached through a vertical incision at the anterior wall of window itself. The defect was closed with pericardial patch using continuous suture posteriorly and sandwitch technique anteriorly. The patient was weaned from the cardiopulmoanry bypas without hemodynamic problem and postoperative course was uneventful. This surgical technique is recommandable as it provides good exposure of the defect and can be performed easily and safely.
생후 30일 된 형성부전성 좌심증후군 환자에 대하여 무명 동맥과 관상동맥의 국소 순환 하에 완전순환 정지 없이 심박동을 유지한 상태에서 Norwood수술을 시행하였다. 환자는 4개월 후 이차 수술전까지 특별한 문제없이 성장하였고 이차 수술 후 폰탄 수술을 기다리고 있다. 상행 대동맥 근위부 직경이 5 mm 이상 정도로 캐뉼라 삽관이 가능한 경우 완전 순환 정지 없이 Norwood 술식이 가능하며 이를 통하여 좋은 결과를 얻었기에 보고하는 바이다.
The purpose of this study was to development CPR training course to promote wide spread training. The course objectives was based on international Guidelines 2000. The course presents information about the AHA adult Chain of survival and signs of heart attack, cardiac arrest, stroke, and chocking in adult, and includes information about healty heart living and actions to reduce the risk of cardiovascular disease. The course also presents information about the AHA infant and chila Chain of survival, signs of breathing emergencies and chocking in infants and children, prevention of sudden infant death syndrome, and prevention of the most common fatal injuies in infants and children. Vedio-mediated instruction is the primary approach to teaching psycomotor skills.
We experienced a case of interrupted aortic arch[Type A] associated with PDA, VSD, mitral regurgitation and single coronary artery. The patient was 7 years old boy, who showed congestive heart failure[NYHA functional class III]. One stage total correction was performed under profound hypothermia with total circulatory arrest. Aortic continuity was established using PDA with anterior wall of main pulmonary artery flap. VSD was closed with Dacron patch and mitral regurgitation repaired by Reed`s annuloplasty method. The postoperative cardiac catheterization revealed no pressure gradient between ascending aorta and descending aorta, decreased pulmonary artery pressure and trivial residual shunt[Qp/Qs: 1.28]. The aortogram showed good continuity of the aorta without narrowing of the anastomotic site. During the period of 1 year follow up, heart failure symptoms were nearly subsided.
