일차보건의료 역량 개발을 위한 지역사회 기반 참여형 국제보건사업 모델 개발: 에콰도르 일개 지역을 중심으로 한 사례연구 (Development of a Community-based Participatory Global Health Project Model for Primary Health Care Capacity Development: A Case Study from a Rural Community in Ecuador)

  • Purpose: The aims of this study were to identify successful strategies and propose a community-based participatory global health project model for primary health care capacity development. Methods: The study used case study methodology. A The unit of analysis was an international cooperation health project entitled "Community-based Primary Health Care Improvement in San Lorenzo, Ecuador" using community-based participatory research conducted in 2007~2008. Data were collected through windshield surveys, focus group discussion, key informant interviews, and provider surveys. Results: Identified successful strategies for the international cooperation health project were reciprocal partnership between researchers and community, partners' capacity building, south-to-south cooperation, and continuous monitoring and feedback. Community participation was found to be an essential tenet to guarantee the improvement of primary health care in the underserved rural community. Evidence from the activities of community health practitioners in Korea was applicable to the development of training programs for primary health care providers in Ecuador. Conclusion: Strategies for strengthening primary health capacity may be tailored depending on socio-cultural, political, and economical situations of each country. The model, however, would be applicable to the entire process of community-based global health projects in underserved rural communities of other countries.

근로자의 직무스트레스와 우울과의 관계에서 자기효능감이 미치는 영향 (Self-efficacy is an Effect Modifier on the Association Between Job-Stress and Depression Scores)

  • Purpose: We were to identify the differences of "Job stress" and "Depression scores" in the divided groups by self-efficacy. And the factors affecting Depression scores were analyzed. Method: This study was conducted from July 2006 to September 2006. Collected 295 surveys were used in this study among 311 surveys since 16 surveys offered insufficient data. SPSS for Windows 10.0 was used to analyse the data. Result: We identified the factors of "Occupational climate", "Job control", "Job demand" as affecting the depression scores in the lower self-efficacy group by the multi-variables statistical analysis. And this statistical model had 12.5% explainable power. Also, the factors of "Occupational climate" were identified as affecting the depression scores in the higher self-efficacy group. And the statistical model had 9.0% explainable power. Conclusion: In the lower self-efficacy group, the scores of the job stress and depression were significantly higher. Therefore, in the lower self-efficacy group, the self-efficacy promotion programs should be needed for prevention of the related occupational diseases. The factors related job stress were identified as affecting the depression scores in both lower and higher self-efficacy groups. Therefore, job stress management program should be prepared for stress loading workers.

보험급여 한약제제 기준처방 개선방안 설문조사 연구 (A survey on additional needs for herbal preparations in national health insurance service)

  • Objectives : In order to increase the utilization rate of insurance covered herbal medicines, it is necessary to improve the standard prescription lists of current national health insurance service. Methods : Previous research and discussions have been summarized, the cases of overseas insurance prescription were investigated, and compared with the insurance prescription of Korea. We conducted two questionnaire surveys. From the first email questionnaire survey, response rate was very low. So once again, from december 4, 2014 to december 17, we emailed a questionnaire survey to 17,275 members of the Korean medicine doctors and 890 peoples(5.1%) responded for two weeks. We analyzed prescriptions from previous studies, overseas cases, and surveys. Results : With overseas herbal medicine health insurance lists, prescriptions recommended by the experts committee, the prescriptions results of the first and second surveys, and the prescriptions presented in previous studies, we compared and analyzed all the results and we derived 202 prescriptions for NHIS. Conclusion: We suggest 127 Single extracts and 202 prescriptions to be added for NHI.

진폐환자의 우울에 영향을 미치는 요인 (The Factors Affecting the Depression in Pneumoconiosis Patients)

  • Objectives: The objective of this study was to analyze factors affecting depression in pneumoconiosis patients. Methods: The subjects were 200 pneumoconiosis patients hospitalized in Pneumoconiosis Hospitals in An-san and Tae-beck. Collected 114 surveys were used in this study among 200 surveys since 86 surveys offered insufficient data. SAS for Windows 8.01 was used to analyse the data Results: Percentage of normal and mild depression is 2.6%, Percentage of moderate depression is 14.9%, Percentage of severe depression is 79.8%. In order to analyze the factors affecting depression rate, the factors with p-values less than 0.1, such as age, education, religion, work department, hospitalized period, physical pain existence, daily living ability, family supports and internal locus of control, were selected as independent factors and analyzed using a stepwise multiple regression. As results, the factor that affected the rate of depression were of the age, education, ADL(Activities of Daily Living), the internal locus of control, family support. Conclusion: It is necessary to develop health program which can improve the patient's ADL(Activities of Daily Living), enforce internal locus of control to minimize the depression rate in pneumoconiosis patients.

Haemoglobin Levels in Mid-Western Nigeria

  • (1) Haemoglobin levels were estimated by cyanmethaemoglobin method in Unoghovo (56 cases) and Tallqvist method in Ibusa 138 cases. (2) The results presented in Table 1, and 2 indicated that the average haemoglobin levels of these two surveys were low. (3) Percentage incidence of liver and spleen enlargements were 20% in Ibusa and 9% in Unoghovo (Table 6). (4) The household survey of average consumption and requirement of these two village, indicated that daily intake of iron per caput was adequate. (5) The results of these two surveys were compared with ether reported studies (Table 4 and 5).

한국인들의 항우울제에 대한 인식 및 태도에 관한 연구 (Perception and Attitude Towards Antidepressants in Koreans)

  • Antidepressants are frequently prescribed for a wide range of symptoms. Surveys in several countries have suggested that negative attitudes towards antidepressants are common. However, there has been limited research associated with the attitudes in Korea. Therefore, this quantitative descriptive study describing the perception and attitudes toward antidepressants was performed using healthy people and outpatients who are not taking antidepressants. The surveys by means of self-reporting questionnaires showed negative attitudes towards antidepressants (74.2%). But, 74.2% of above participants gave positive answers to take antidepressants when they are counseled by medical professionals. Higher rates of negative attitudes were found in younger age group ("it causes addiction") and healthy volunteers and urban area groups ("it causes physical harm"). Study participants without experiences in taking antidepressants showed more negative attitudes although the difference was not statistically significant. Based on the results, accurate and objective information about antidepressants needs to be provided by well-trained health professionals.

Secular trends of body sizes in Korean children and adolescents: from 1965 to 2010

  • An anthropometric survey is one of the most important approaches to use when evaluating the health status of children. Secular trends in body sizes, such as height, weight, head circumference, chest circumference, and body mass index showed significant changes over 40 years in Korea. A series of periodic surveys were conducted in 1967, 1975, 1985, 1997, and 2005 by the Korean Pediatric Society and Ministry of Health and Welfare. The quality of data from school health examinations and the Korea National Health and Nutrition Examination Survey has improved, so we can use them now as resources for anthropometric analysis. The final height differences between 1965 and 1997 were 4.5 cm both in boys (168.9 cm in 1965; 173.4 cm in 1997) and girls (155.9 cm in 1965; 160.4 cm in 1997). The differences between 1997 and 2005 were 0.9 cm in boys (174.3cm in 2005) and 0.8 cm in girls (161.2 cm in 2005). There was no difference in final height measurements between 2005 and 2010. An increase in body size at earlier teen ages was pronounced during these decades compared to the previous generation; however, little change has been identified more recently. Body size has been increasing, and obesity has become more prevalent. Systems that gather data should be updated in order to cope with these secular trends. In an upcoming era of secular trends that would be in a slow transition, several surveys that include body measurements should be prepared to meet future needs.

Feasibility to Expand Complex Wards for Efficient Hospital Management and Quality Improvement

  • Purpose: This study aims to explore the feasibility of expanding complex wards to provide efficient hospital management and high-quality medical services to local residents of Gangneung Medical Center (GMC). Research Design, Data and Methodology: There are four research designs to achieve the research objectives. We analyzed Big Data for 3 months on Social Network Services (SNS). A questionnaire survey conducted on 219 patients visiting the GMC. Surveys of 20 employees of the GMC applied. The feasibility to expand the GMC ward measured through Focus Group Interview by 12 internal and external experts. Data analysis methods derived from various surveys applied with data mining technique, frequency analysis, and Importance-Performance Analysis methods, and IBM SPSS statistical package program applied for data processing. Results: In the result of the big data analysis, the GMC's recognition on SNS is high. 95.9% of the residents and 100.0% of the employees required the need for the complex ward extension. In the analysis of expert opinion, in the future functions of GMC, specialized care (△3.3) and public medicine (△1.4) increased significantly. Conclusion: GMC's complex ward extension is an urgent and indispensable project to provide efficient hospital management and service quality.

성인 구강검진과 미충족 치과의료와의 관련성 (Relationship between dental checkups and unmet dental care needs in Korean adults)

  • Objectives: Oral diseases can be prevented, and early treatment through dental checkups is important. This study was investigated the relationship between dental checkups and unmet dental care needs in Korean adults. Methods: From the data of the 7th Korean national health and nutrition survey (2016-2017), the final 9,300 of the 16,277 participants selected as the method for extracting stratified colonies by complex sample design. A structured questionnaire interview was used, and chi-square test and multiple logistic regression analysis were performed. Results: The relationship between dental checkups and unmet dental care needs for the last one year was analyzed by controlling demographic characteristics, variables related to health conditions, and "use of dental clinics for the last year." The unmet dental care needs were 7.57 times higher (CI: 6.49-8.83) for non-users of dental clinics for 1 year, and 1.32 times (CI: 1.13-1.54) for dental checkups for 1 year compared to non-dental checkups patients. Conclusions: As described above, there was a close relationship between adult dental checkups and unmet dental care needs. Therefore, it is suggested that it is necessary to expand educational publicity and prepare policy strategies such as visiting dental checkups to improve the adult dental checkups.

Proposal to Revise the Screening Test for Latent Tuberculosis Infection in Close Contacts at Elementary Schools in Korea

  • The 2018 National Guideline for Tuberculosis Control, which was published by the Korea Centers for Diseases Prevention and Control (KCDC), mandates conducting an epidemiological survey among close contacts of active tuberculosis patients at public institutions such as schools. In the procedure for these surveys, the tuberculin skin test (TST) is mandated as the screening test for latent tuberculosis infection in elementary school students. However, several guidelines recommend using the interferon-gamma releasing assay (IGRA) for contacts aged over 5 years with a Bacillus Calmette-$Gu{\acute{e}}rin$ vaccination history. The main reason for this is that IGRA has a higher specificity and lower false positive rate than TST. In addition, IGRA requires only a single visit to draw blood and the results are available within 24 hours. These advantages could promote cooperation from both parents and students in conducting these surveys. Thus, these findings regarding the benefits of IGRA for surveys of close contacts at elementary schools should be incorporated into the KCDC guideline.
