주요 선진국의 군 건강관련 조사 실태 및 특성: 미국, 호주, 캐나다를 중심으로 (Current State and Characteristics of Military Survey in Developed Countries: Focused on the Surveys of United States, Australia, and Canada)

  • 본 연구는 주요 선진국의 군인 건강조사 실태 및 특징을 살펴보고, 우리나라 군인 건강조사의 나아갈 방향을 제시하는 것이다. 이를 위해 군인 건강조사를 실시하고 있는 미국, 호주, 캐나다의 군인 건강조사 현황을 파악하였다. 구체적으로 건강조사의 배경, 목적, 대상, 참가인원 수, 방법, 내용, 관련기관, 주기 등을 심층 고찰하였다. 고찰결과 주요 선진국의 군 건강 설문조사의 특징은 다음과 같다. 첫째, 군인 건강조사는 건강 위험요인을 줄이는 방향으로 단계적으로 발전해왔다. 둘째, 정신건강에 대한 관심과 중요성이 강조되고 있다. 셋째, 외국의 군 건강조사는 정부의 협력뿐만 아니라 전문조사기관이나 대학과의 협력을 통해 데이터 수집 및 분석에서 익명성과 전문성을 보장하고 있다. 마지막으로, 제도화를 통해 건강 설문조사를 정기적으로 시행하고 있다는 것이다. 앞으로 군 보건의료 관계자뿐만 아니라 국가적 차원에서 군 건강조사에 대한 필요성과 중요성을 인식하고 법적, 제도적 환경을 구축하는 것이 중요하다 하겠다.

우리나라 청소년 건강위험행위 실태조사의 문제점과 개선방안 (Problems and Improvements in Surveys on Health Risk Behaviors among Adolescents in Korea)

  • Purpose : This study was done for reviewing problems in surveys on youth health risk behaviors in Korea and for looking for strategies to improve surveys of youth health risk behaviors through literature review. Method : This study reviewed foreign and domestic literatures. Results : The main problems were as follows; 1) lack of health risk behavior surveys focused on health behaviors, 2) differences in health risk behaviors surveyed, 3) inconsistency in the measurement on health risk behaviors, 4) lack of representativeness of the sample, using convenient sampling. Conclusions: Several suggestions were made for the future research, including establishment of systems for youth health risk behavior survey nationwide, introduction of internet survey, maintenance of consistency in health risk behaviors surveyed, investigation of relating factors relevant to health risk behaviors, and uses of survey results.

한국 공공의료 구강건강조사 체계 및 아동의 구강건강 현황 (Korea National Oral Health Survey Systems and recent oral health status in children)

  • This study aimed to introduce what Korea national oral health surveys are and examine recent oral health status in Korean children. Korea Government has implemented national oral health-related surveys during the past two decades. The surveys with oral health examination are Korean Children's National Oral Health Survey (KCNOHS) since 2000 in 5-year-old and 12-year-old children and Korea National Hhealth and Nutrition Examination Survey (KNHANES) since 2007 aged 1 year and more. KCNOHS provides the information about the distribution of prevalence of dental caries, gingival health, behavioral factors, and dental clinic demands etc. KNHANES has huge amount of health data for representative Korean population including oral health and general conditions. Based on KCNOHS, the prevalence of dental caries and its experience has decreased steadily, and the status of oral hygiene and preventive treatment called sealents has been also improved after the year of 2000. However, there is still a gap to reach to those of Western developed countries. Therefore, more effective oral health policy plans and strategies for Korean children and adolescents are needed to prevent and manage for dental caries in private and clinical field of dentistry with public sector.

혈액투석 환자의 건강상태 예측요인: 2년 종단연구 (Predictors of Health Status in Patients with Hemodialysis: A Two-year Longitudinal Study)

  • Purpose: The purpose of this study was to examine the effects of physical, psychological, and social factors on the health status of patients undergoing hemodialysis using longitudinal data. Methods: Two surveys were conducted within a period of two-year. A total of 75 patients participated in both surveys. Eight variables including symptoms, treatment belief, hope, meaning-focused coping, family support, friend/peer support, healthcare provider support, and health status were measured. Data were analyzed by paired t-test, Pearson's correlation coefficients, and hierarchical multiple regressions. Results: The mean age of the participants was 47.31 and the average duration of dialysis was 8.27 years. There was no difference in scores on perceived health status between the two year-interval surveys. The two-year follow-up on perceived health status was correlated with baseline perceived health status, friend/peer support, meaning-focused coping, hope, and symptoms. As the result of the regression analysis, friends/peer support (${\beta}=.31$, p=.013) and baseline perceived health status (${\beta}=.30$, p=.020) were found to be significant predictors of subsequent health status which explained 39.0% of the variance. Conclusion: This study suggests the importance of psychosocial resources for positive health outcomes in patients with a chronic illness. Nursing interventions aimed to provide renal peer support need to be developed and tested.

Trends of Smokeless Tobacco use among Adults (Aged 15-49 Years) in Bangladesh, India and Nepal

  • Background: Smokeless tobacco (SLT) has long been realized as an important component of the fight for global tobacco control. It still remains a major problem in countries like India, Bangladesh and Nepal. The objective of this study was to estimate the trends of SLT use in three countries of the SEARO WHO office. Materials and Methods: We used data from national surveys in three countries (Bangladesh, India and Nepal) to estimate trends in prevalence of current SLT use. All available nationally representative data sources were used. Estimates were weighted, age standardized and given along with 95% confidence intervals. Significance of linear trend in prevalence over time was tested using the Cochrane-Armitage test for trend. A p value of less than 0.05 was considered statistically significant. Results: We identified three surveys for Bangladesh, three for India and four for Nepal that met the selection criteria (such as Demographic and Health Surveys, WHO-STEPwise approach to Surveillance and Global Adult Tobacco Surveys). A significantly increasing trend was noticed in the prevalence of current SLT use among Bangladeshi men (20.2% to 23%, p=0.03). In India, a similar significantly increasing trend was seen among men (27.1% to 33.4%, p<0.001) and women (10.1% to 15.7%, p<0.001). In Nepal, there was a no significant trend among both men (39.1% to 31.6%, p=0.11) and women (5.6% to 4.7%, p=0.49). Conclusions: In the study countries SLT use has remained at alarmingly high levels. Usage trends do not show any signs of decline in spite of control efforts. Tobacco control measures should focus more on controlling SLT use.

Anxiety, Depression, and Quality of Life in Parents of Adolescents with Inflammatory Bowel Disease: A Longitudinal Study

  • Purpose: The parents of adolescents with inflammatory bowel disease may experience impaired mental health and quality of life. This longitudinal study aimed to verify whether the mental health and quality of life of the parents of adolescents with inflammatory bowel disease declined when their children had active disease. Methods: Sociodemographic data, parental anxiety, depression, and quality of life were analyzed using validated questionnaires for each variable. After the baseline survey, the second and follow-up surveys were conducted at 3 and 12 months, respectively. The active disease group comprised eight parents whose children had active disease during the baseline and second surveys. The remission group comprised 14 parents whose children remained in remission during both surveys. The improved group comprised nine parents whose children experienced active disease at baseline and remission during the second survey. Parental mental health and quality of life were compared among the groups. Results: Significantly higher levels of anxiety were observed in the active disease group in all surveys (p<0.050). Although depression levels and quality of life did not differ significantly among the three groups, pairing the active disease group with other groups showed some large effect sizes. Conclusion: Parents tended to experience decreased mental health and quality of life when their adolescents experienced active inflammatory bowel disease. Consequently, our hypothesis was partially verified. Therefore, parents need support when their children have active disease; this finding highlights the need for parental support systems.

Current Systems of National and Regional Nutrition Surveys and Future Direction

  • The National Nutrition Survey in Japan (NNS-J) started in 1945 and has provided information on dietary intake and health status of Japanese citizens to the public and policymakers for more than half a century. We summarized several relevant issues on the survey in this report : the current framework of the NNS-J in accordance with the Nutrition Improvement Law, utilization of the survey for nutrition and health policy in Japan, the Health Promotion Law recently enacted in 2003, the national plan for health promotion and disease prevention (Health Japan 21), and possible measures to improve the survey systems under the new law. We also mentioned implementation structures of regional health and nutrition surveys, because the Health Promotion Law designates an active role of local governments on promoting health for their citizens, which will enhance the needs for appropriate assessment of health and nutrition conditions in each community as well as the monitoring at the national level. 의료기관 대상 평가통합 및 효율화 방안 (Integration and Efficiency Strategies of Evaluations for Medical Institutions)

  • Objectives: In Korea, there are many kinds of evaluations for medical institutions. However, evaluations are increasingly burdensome for medical institutions because evaluation agencies, evaluation timing, and evaluation methods are different. The purpose of this study is to improve the efficiency of evaluation for medical institutions and ultimately to provide quality medical services to patients. Methods: In this study, 2,310 indicators of 19 kinds of evaluation for medical institutions were analyzed. Results: 1,424 indicators were available for on-site surveys and 886 indicators were not available for on-site surveys. There were 4 kinds of evaluation that can be integrated in total, 12 kinds of evaluation that can be integrated partially, and 3 kinds of evaluation that need to maintain the current evaluation system. Conclusion: In order to provide patient-centered quality medical services through reduction of burden due to the evaluation for medical institutions, it is necessary to deeply discuss the efficiency of evaluation integration and result utilization.

정신요양시설 거주인의 건강 실태 및 정책 대안 (Health Condition of Residents of Mental Health Sanatoriums and Policy Options)

  • Purpose : The purpose of this study is to directly understand the health condition of residents of mental health sanatoriums nationwide, which has been difficult to ascertain in surveys conducted to date. The study presents specific measures for improving the health of these residents. Methods : A "physical examination questionnaire for residents of mental health sanatoriums" was developed to check the basic physical condition of residents, and 20 out of 59 mental health sanatoriums nationwide were randomly selected. Medical personnel visited the sanatoriums, interviewing and examining the residents in person. A total of 396 health surveys were completed. Results : Many of the residents were underweight but had abdominal obesity. It was confirmed that chronic diseases among the residents were not diagnosed early or were not properly managed. Among the subjective symptoms complained of by the residents, musculoskeletal symptoms were the most common. Oral examinations revealed a serious level of oral health problems among the residents, including dental caries and missing teeth. Basic physical examinations found health problems that required additional examination or medical treatment. Blood pressure abnormalities made up the highest percentage of the health problems. Conclusion : Regular health surveys are needed to determine the health condition of residents of mental health sanatoriums. Access to and quality of primary medical services within the sanatoriums need to be dramatically improved. A delivery system for severe diseases and emergency medical care in the sanatoriums should also be specifically presented. The residents should be notified upon admission and during their stay that they have the right to the enjoyment of the highest attainable standard of mental and physical health. The issue of health rights should be addressed within a larger framework of reorganizing management plans for people in the community - not only residents - with chronic mental illness.

Assessment of Muscle Fatigue Associated with Prolonged Standing in the Workplace

  • Objectives: The objectives of this study were to determine the psychological fatigue and analyze muscle activity of production workers who are performing processes jobs while standing for prolonged time periods. Methods: The psychological fatigue experienced by the workers was obtained through questionnaire surveys. Meanwhile, muscle activity has been analyzed using surface electromyography (sEMG) measurement. Lower extremities muscles include: erector spinae, tibialis anterior, and gastrocnemius were concurrently measured for more than five hours of standing. Twenty male production workers in a metal stamping company participated as subjects in this study. The subjects were required to undergo questionnaire surveys and sEMG measurement. Results: Results of the questionnaire surveys found that all subjects experienced psychological fatigue due to prolonged standing jobs. Similarly, muscle fatigue has been identified through sEMG measurement. Based on the non-parametric statistical test using the Spearman's rank order correlation, the left erector spinae obtained a moderate positive correlation and statistically significant ($r_s$ = 0.552, p < 0.05) between the results of questionnaire surveys and sEMG measurement. Conclusion: Based on this study, the authors concluded that prolonged standing was contributed to psychological fatigue and to muscle fatigue among the production workers.
