Background: Although screening is necessary where gastric cancer is particularly common in Asia, the performance outcomes of mass screening programs have remained unclear. This study was conducted to evaluate cost-effectiveness outcomes of the national cancer screening program (NCSP) for gastric cancer in South Korea. Materials and Methods: People aged 40 years or over during 2002-2003 (baseline) were the target population. Screening recipients and patients diagnosed with gastric cancers were identified using the NCSP and Korea Central Cancer Registry databases. Clinical outcomes were measured in terms of mortality and life-years saved (LYS) of gastric cancer patients during 7 years based on merged data from the Korean National Health Insurance Corporation and National Statistical Office. We considered direct, indirect, and productivity-loss costs associated with screening attendance. Incremental cost-effectiveness ratio (ICER) estimates were produced according to screening method, sex, and age group compared to non-screening. Results: The age-adjusted ICER for survival was 260,201,000-371,011,000 Korean Won (KW; 1USD=1,088 KW) for the upper-gastrointestinal (UGI) tract over non-screening. Endoscopy ICERs were lower (119,099,000-178,700,000 KW/survival) than UGI. To increase 1 life-year, additional costs of approximately 14,466,000-15,014,000 KW and 8,817,000-9,755,000 KW were required for UGI and endoscopy, respectively. Endoscopy was the most cost-effective strategy for males and females. With regard to sensitivity analyses varying based on the upper age limit, endoscopy NCSP was dominant for both males and females. For males, an upper limit of age 75 or 80 years could be considered. ICER estimates for LYS indicate that the gastric cancer screening program in Korea is cost-effective. Conclusion: Endoscopy should be recommended as a first-line method in Korea because it is beneficial among the Korean population.
Objective : The purpose of this study was to analyze the effects of multisensory exercise on foot pressure sensitivity and balance for the elderly. Method : The subjects were 17 elderly women (11 for the experimental group and 6 for the control group) with a mean age of 83. The subjects all lived in senior residence centers in Seoul. Multisensory exercise was done twice a week for 40 minutes during a 12 week period. Exercise programs were changed every 3 weeks according to the principal of gradual progress of the exercise. In order to train the vision system subjects were asked to open and close their eyes during exercise. When it came to training the vestibular system, subjects stood and walked on high elastic mats with their bare feet. For the somatasensory system subjects always stood and walked with their bare feet. The sub-Metatarsal Pad Elasticity Acquisition Instrument (MPEAI) was used to measure foot pressure sensitivity. MFT Balance test (V1.7) was used to measure anterior / posterior and medial / lateral directional balance. For the statistical analysis the IBM SPSS 21.0 was used to perform Repeatde measured ANOVA and Wilcoxon ranked test. Results : For the multisensory exercise group Hallux (after 6 weeks, 12 weeks), heel (after 6 weeks) and 2nd Metartarsal $40^{\circ}$ (after 6 weeks) pressure sensitivity increased statistically, but the control group didn't change. Also, balance didn't change for the experimental and control group statistically. Conclusion : Exercise with bare feet on a high elastic mat had a partially positive effect on foot sensitivity.
Objectives: This study was performed to develop a smoking prevention program for Korean high school students, and to evaluate the effects of the program. Methods: A smoking prevention program, composed of five-session curriculum, was developed by modifying several smoking prevention and cessation programs based on the Social Influence Model. The program was applied to the freshmen of a technical high school. We surveyed with a questionnaire one week before education, one week after education, and two months after education. The number of participants for data analysis were 282(181 males, 101 females). Among those, 162(97 males, 65 females) students were allocated to the education group, and the other 120(84 males, 36 females) students were allocated to the control group. The effect of education was analyzed by multiple logistic regression analysis adjusted by potential confounders. Results: Among smokers, those who had education were more likely to quit smoking (OR=2.99, 95% confidence interval(CI) 0.84-10.64), and to decrease smoking frequency(OR=2.29, 95% CI 0.95-5.53) in borderline significance one week after education. However, the effect of education disappeared two months after education. The effect of education was significant(OR=9.11, 95% CI 3.22-25.76) for the increase of smoking cessation intention one week after education, and it persisted until two months after education(OR=2.94, 95% CI 1.18-7.35). Education was also a significant predictor(OR=1.97, 95% CI 0.89-4.37) for the increase of smoking cessation stage one week after education and it persisted(OR=6.39, 95% CI 2.42-16.86) after two months. Among non-smokers, those who had education were more likely to decrease smoking intention one week after education(OR=4.71, 95% CI 1.63-13.58). However, the statistical significance of education disappeared two months after education. Conclusions: The results showed that the smoking prevention program developed in this study changed smoking behaviors immdeiately after education even though the effect did not persist. However, this program was successful in increasing smoking cessation intention and stage of smoking cessation among smokers.
Objectives: The purpose of this study is to find the influences of the perceived economical difficulty, gender equality consciousness and sexual subjectivity on the adolescents' attitude to prostitution to examine the moderating effects of gender equality consciousness and sexual subjectivity between the perceived economical difficulty and the adolescents' attitude to prostitution, and to provide some recommendations for intervention to improve more appropriate attitude of adolescents on prostitution. Methods: The data used for this study is from the survey with a sample of 1,151 students from 14 middle and high schools in Chungbuk province. Results: The main results of this study are as follows: First, there are statistical differences in the adolescents' attitude to prostitution according to gender, grade, and the type of school. Second, there is a positive effect of the perceived economical difficulty, gender equality consciousness and sexual subjectivity on the adolescents' attitude to prostitution. Third, there are positive moderating effects of sexual subjectivity between the perceived economical difficulty and the adolescents' attitude to prostitution. Conclusions: Intervening in the sex education at an early stage, advertising prostitution's harmful effect at the level of a social concern and developing diverse programs to enhance the sexual subjectivity of students are recommended to promote appropriate attitude of adolescents about prostitution.
Background and purpose : Stroke is a leading cause of death in Korea. Early measures to prevent stroke are extremely important since it has no cure. Korean might have different risk factors since their dietary habits and socio-economical status differ from most western countries. However, the risk factors for stroke in Korea have not yet been identified. Moreover, the lifestyle of healthy Korean adults has not been investigated. In this study we investigate the lifestyle of healthy adults living in Seoul and rural areas and compare the lifestyles of the two. Methods : One hundred seventy one subjects were studied. Among the subjects studied, 128 were from Seoul, the other 43 were from the country area. The age of the subjects was limited to over 40 years. Blood pressure, fast blood sugar, and cholesterol were measured. The subjects' height, weight, body mass index, total body fat, skinfolds thickness of triceps, subscapular and abdomen were measured to determine obesity. Using a structured interview, we assessed : sodium intake, physical activity and exercise, consumption of vegetables, fat, fish and fruits. The results of the two groups were compared. Results : There were no statistical differences in age and education between the two groups of subjects. The mean age of the subjects were 66 years old. The subjects residing in rural areas had a higher intake of sodium(p<0.05), lower physical activity(P<0.05), and higher BMI and body fat (p<0.05) as compared to the subjects in Seoul. Subjects with hypertension were between 24% and 33% and the prevalence of hypertension was the highest when compared to the prevalence of DM or hypercholesterolemia. However, the prevalence of hypertension, DM, hypercholesterolemia, were not significantly different in these areas. Conclusion : Our results show that subjects living in rural areas eat more salty food, exercise less, and tend to be obese. The finding of this study lead to speculation that Korean living in rural areas have less information about the effects of diet on health than city dwellers do. General health and nutrition education programs aimed at the prevention of stroke and other such conditions for rural area Koreans may close the risk factor gap between rural and urban dwellers.
This study explores the effects of rural migrant characteristics and conjugal communication on life satisfaction of baby boomer rural migrants. A total of 205 Korean baby boomers, who are living in rural areas in the year of 2013 that had migrated from cities back in 2003 with their families, were analyzed. Their age ranged from 49 to 57 years. The statistical methods used for data analysis were descriptive statistics, t-test, and hierarchical regression analysis, with SPSS Win 20.0 program. The results of this study are summarized as follows: the reasons these baby boomers chose a rural lifestyle were 'for life after retirement,' 'because rural life is good,' 'for health reasons,' and 'for chosen profession.' The level of conjugal communication (M=3.69) and life satisfaction (M=3.23) were higher than average (M=3.0). The variables influencing life satisfaction of baby boomer rural migrants were 'increased property size,' 'better conjugal communication,' 'better health condition,' 'lower levels of money,' and 'length of time spent farming.' Furthermore, in terms of life satisfaction, the relative contributions of these variables are as follows: general characteristics 20%, rural migrant characteristics 10%, and the level of conjugal communication 6%. These results show that conjugal communication is a very important variable, as well as a rural migrant characteristic when it comes to life satisfaction of these baby boomers. Therefore, to improve life satisfaction of baby boomer rural migrants, support programs need to focus on both farming and conjugal relationships.

This study was surveyed to assess the status of safety accidents occurred in work-places and prepare the fundamental data and prevent the safety accidents. The authors reviewed and analysed the charts of accident cases treated at a primary care clinic in A city from January 1991 to December 2006. The data were classified according to the USA Standards Institute and International Labour Organization method. We analyzed the data using SPSS program. The results were as follows : 1. The total cases of accidents were 455 for 8 years. 2. Accidents were mostly common in the workers who are in thirties and forties age(84.4%). 3. As season variation, spring and summer were common than others, but there was no statistical significance on season, month and weekday. 4. The most frequent injured part of the body were hand and finger, which was 36.0% among total cases. 5. According to the accidents type, cases of caught in, under or between were most frequently observed as 53.9% of the total cases. 6. The most common source of injuries was power machine(50.5%). 7. According to the unsafe acts, cases of carelessness and unsafe information were most frequently observed as 71.2% of the total cases. 8. Admission rate(5.5%) and official report rate(2.2%) were very low rate. As above results, the authors recommend to prepare the systemic control programs on environmental and human factors of safety accidents such as improving the working conditions, working facilities, working methods and safety education, and control of working time for working day.
The purpose of this study was to examine the correlation between self-efficacy and self-care of hemodialysis patients. The subjects consisted of 140 hemodialysis patients who underwent hemodialysis at 2 university hospitals. The data were collected by questionnaires and chart review. The questinnaire used the self-efficacy tool developed by Kim Ju Hyune(1995) and the self-care tool developed by literature review and indepth open openended questions to 10 patients. Also, the questionnaire included physiologic data which collected through review of the patients' charts. The statistical analysis was used the SPSS program for frequency, mean, t-test, ANOVA and Pearson correlation. The results were as follows : 1. The mean score for general self-efficacy of hemodialysis patients was 3.103(1-4point) and there were significant differences according to percieved health. The mean score for specific self-efficacy of hemodialysis patients was 3.113 (1-4point) and there were significant differencies according to percieved health, side effects and complications which related hemodialysis. 2. The mean score for self-care of hemodialysis patients was 3.822(1-5point) and there were significant differencies according to marital status and economic level. 3. The relationship between general self-efficacy and self-care was a positive correlation(P=.000). The relationship between specific self-efficacy and self-care was a positive correlation(P=.000). In conclusion, this study revealed the level of self-efficacy and self-care, and the positive correlation between self-efficacy and self-care in hemodialysis patients. Therefore, intervention is needed to promote self-efficacy for self-care of hemodialysis patients. Considering the vulnerable self-care area same as checking blood pressure and weight, fluid restriction, social adjustment, exercise and rest, luther studies should develop self-efficacy promoting programs for self-care of hemodialysis patients.
This study lays its purpose on providing basic material for managing dental hygiene care programs by grasping images of the elderly perceived by dental hygienists and their behaviors toward the elderly, who have an important role in improving the elderly's dental health in the aging society. For these purposes, the researcher conducted a self-administrative questionnaire survey of 370 dental hygienists working in dental clinics or offices located in the Seoul and Daejeon areas from June 2 to 21 in 2008 and collected data, statistical analysis of the $x^2$-test, t-test, one-way ANOVA, Pearson Correlation used. resulting in the following findings. 1. In perceived images of the elderly according to their general characteristics, a subject group with under 8-year career showed 85.38 points, indicating the statistically significant difference (p=0.001). 2. When we look at perceived images of the elderly and behaviors toward them according their characteristics, in images of the elderly, a subject group with positive perceptions of the elderly showed 85.51 points (p=0.000), a subject group with meaningful old person(s), 84.66 points (p=0.000), a subject group living with old person(s), 86.04 points (p=0.001), and a subject group with experience in volunteer service activities, 84.11 points (p=0.001), and in behaviors toward the elderly, a subject group with positive perceptions of the elderly and with meaningful old person(s) showed 56.46 (p=0.036) and 55.91 points respectively, indicating the statistically significant difference (p=0.040). 3. Images of the elderly and behaviors toward them showed a positive correlation, indicating that the more positive images of the elderly, the more positive behaviors toward them (r=0.317). Putting these results together, it is thought that it is time to need continuous education and efforts regarding the formation of desirable relationships with and images of the elderly and behaviors toward them, for dental hygienists who have an important role in managing and enhancing the elderly's dental health in the aging society.
This study was designed to grasp the knowledge and practice the level about visual impairment protective care of the primary school students and also analyse the state of visual impairment. The subjects of this study ware 347 students composed of 456 boys and 391 girls in primary school students. The data were collected by questionaire developed for the purpose of this study which has been made through studying references. These were carried out from the 10th to 30th of June 1992. Analysis of the data was done utilizing SPSS for percentage, mean, S.D., T-test., ANOVA and Pearson's Correlation Coefficient. The major results obtained from this study were as follows: 1. The level of the knowledge about visual impairment protective care was in intermediate range (mean on 20 point scale of 12.34, with O=definitely no), but low in aspects of preventive care for overwork of eye (mean on 5 point scale of 2.67). 2. The level of the practice about visual impairment protective care was intermediate range (mean on 20 point scale of 12.34, with O=definitely no), but low in aspects of preventive care for overwork of eye (mean on 5 point scale of 2.58). 3. Significant variables influencing knowledge of visual impairment protective care were sex (p<0.01) and grade (p<0.001). 4. Significant variables influencing practice of visual impairment protective care was living area (p<0.01). 5. Correlation between knowledge and practice: Those who have more knowledge score have more practice score (p<0.001). 6. vision impairment state 1) Student with subnormal in 0.9 or less were 26.3% in left eye and 28.4% in right eye. 2) Student with subnormal in 0.6 or less were 11.9% in left eye and 10.9% in right eye. 7. Vision impairment programs 1) Males are slightly higher in the degree of visual impairment than females. 2) Visual impairment were increased in accordance with the increase of grade. 3) Urban students are higher in the degree of visual impairment than rural students, but these seem to have no statistical meaning.
