중국고객 해외의료관광국가 선택의도에 영향을 미치는 요인에 관한 연구: 구전 조절효과를 중심으로 (Factors Influencing Chinese Customers' Selection of Health Care Service Countries: Focusing on Word-of-Mouth Moderating Effects)

  • Purpose - Given globalization, the new niche market of medical tourism is likely to experience sustainable growth for various reasons, such as aging populations and a shift in the medical consumerism paradigm toward prevention. Importantly, understanding medical customers' behavior is necessary to benefit from a competitive advantage in this industry. The existing research primarily accessed the key factors of medical quality and costs to explain health customers' behavior but is limited in terms of enabling an understanding of the decision process. This limitation exists because, given the intangibility and greater associated risks in the highly professional industry of international medical tourism, most customers lack the knowledge and experience needed to evaluate the central factors-such as the medical competence of health care countries-before purchases. Therefore, they actively search for useful information through various distributions to reduce uncertainty and to make better choices. Interestingly, most of these information channels are associated with word-of-mouth (WOM). However, no evidence is found in the literature to estimate the effect of WOM in the medical tourism field. Thus, this study focuses on WOM to explore its interaction with key medical characteristic factors and the attractiveness of destinations referred to by sources. This study also affects customers' evaluations and, in turn, influences their intention to seek health care services abroad. Research design, data, and methodology - The literature review addressed an interesting research model for estimating the relations among WOM, medical characteristics, attractiveness, and customers' choice intention regarding international health care. In the key economic regions in China, such as Beijing, Shanghai, Jiangsu, Shandong, and Guangdong, 2,500 survey questionnaires were distributed to potential customers of different ages, education, and income levels. A resulting 1,717 (68.68 percent of the original 2,500) usable surveys were obtained for analysis. Moderated regression analysis was used to determine the effects of WOM in the decision process regarding international health care destinations. Results - The results indicate that WOM is a good moderator of the relationships between the factors evaluated by sources and customers. More importantly, the WOM effects reflect the factors of tie strength, credibility, and vividness. The results also reveal that, given the moderating role of WOM, the intention of potential Chinese customers to seek the referred health care country varies according to the medical characteristics of medical competency and reputation as evaluated by customers. In contrast, the travel attractiveness of the attractions, facilities, accessibility, and social environment are critical determinants of destination choice intention. Conclusions - The moderating role of WOM has been confirmed through the international healthcare destination selection process. Medical tourism managers should user WOM as an effective marketing tool for industry development. Specially, marketers should consider the effects of WOM determinants, such as tie strength, credibility, and vividness, to develop an effective strategy. Furthermore, this study estimates the factors that affect customers' selection of medical tourism destinations. Health care managers or policy makers should consider a broad variety of variables that may attract more Chinese customers to international health care.

Thoracic and Cardiovascular Surgeons' Perception of the Concentration of Cardiovascular Operations in Seoul Metropolitan Area's Hospitals

  • Background: The purpose of this study is to evaluate the concentration of cardiovascular surgical procedures in a metropolitan area and investigate the perception of specialists regarding governmental policies to resolve this imbalance. Methods: From March to May 2015, surveys were distributed to members of the Thoracic and Cardiovascular Surgery Association. The final pool of research subjects consisted of 75 respondents. Subjects were queried regarding the concentration of cardiovascular operations in metropolitan areas, alternatives to the imbalance, and governmental policies to resolve the inequalities. Results: Survey participants responded that South Korea needs governmental policies to alleviate the concentration of cardiovascular surgery patients in large metropolitan hospitals. Participants agreed that the freedom to choose medical institutions and improved accessibility to metropolitan hospitals due to advanced transportation systems were some of the causes for the concentration. A majority (98.7%) of respondents thought establishing thoracic and cardiovascular surgery centers in provinces was an appropriate solution to alleviate the concentration. Thoracic and cardiovascular surgery specialists were ranked as the number one group on which to focus development. Conclusion: Developing and carrying out policies to establish thoracic and cardiovascular surgery centers in provinces will alleviate the regional imbalance in available heart surgery services and an overall improvement in cardiovascular disease treatment in South Korea.

대학치과병원 예방치과운영 전·후의 환자진료행태와 진료비용 변화 (Changes in treatment behavior and cost according to the operation of preventive dentistry in university dental hospital)

  • Objectives: This study aims to provide basic data for high-quality dental services. In addition, we will promote the operation of preventive dentistry that implements preventive measures. It was conducted to study the change of patient's treatment behavior and treatment cost due to the discontinuation of preventive dentistry in university dental hospitals. Methods: This study collected data using the integrated medical information system of the C University Dental Hospital. From September 1, 2017 to August 31, 2019, data were analyzed using frequency, percentage, mean, standard deviation, chi-square test using SPSS version 24.0 statistical program, and T-test. Results: There was a significant difference in the number of preventive dental treatment cases from 58.3% of preventive dental operation periods to 41.7% of preventive dental operation periods. As a result of comparing the medical expenses, the total medical expenses during the preventive dental operation period decreased from 521,308,872 won to 379,724,995 won during the discontinuation period, 141,583,877 won. The number of medical treatments by treatment behavior decreased 3,835 (28.4%) from a total of 13,520 preventive dental operation periods to 9,685. Conclusions: This study is meaningful as the first study to confirm the change in the treatment behavior and the change in the cost of treatment due to the discontinuation of the operation of preventive dentistry at university dental hospitals. In conclusion, it is thought that there is a possibility of the lack of accessibility and the limitation of professional preventive care due to the discontinuation of preventive dentistry.

공공기관의 공공성 이행 검토: 의료분쟁조정중재원 사례를 중심으로 (The Publicness of Public Institutions: Case Study on the Korea Medical Dispute Mediation and Arbitration Agency)

  • Background: Based on the fact that the Korea Medical Dispute Mediation and Arbitration Agency is a public institution established by social demands for medical disputes, this study reviews the publicness of public organization and discusses its policy implications. Methods: Through Moore's strategic triangle, which consists of legitimacy and support, public value and operational capacity, the process of creating public value is examined. For the analysis, case studies were conducted using related literature data from 2012, when the agency was established, to the present. Results: As a result of the analysis, first, the related law examined in the operational capability has been revised dozens of times, but the revised law has its own contradictions and limitations. The human resource system is also being improved, but there is a problem with the fairness and reliability of the arbitration process, especially due to the limitations of the appraiser system. Second, in terms of legitimacy and support, a regional gap occurred despite efforts to improve accessibility through the expansion of the organization. And the arbitration agency failed to reconcile conflicts caused by stakeholders' perception of each other as a trade-off relationship. Third, the public value result shows that, despite many explicit (statistical) achievements, citizens' use of the past dispute resolution means (litigation) has not decreased. Likewise, the perception of value makers (citizens) is important for creating public value as an invisible result, but it has not yet been formally investigated, so the performance can not be recognized. Conclusion: While the organization's efforts for continuous change and improvement are encouraging, it is not perceived as a better means of resolving disputes and improving quality of services. Therefore, it is necessary to reconsider the institutional design centered on value creators.

한국의 장애인 환자 치과 진료를 위한 국민 건강 보험 가산제도의 종류 및 청구 현황 (THE ADDITIONAL POINT SYSTEM OF NATIONAL HEALTH INSURANCE FOR DENTAL TREATMENT IN PATIENTS WITH A SPECIAL HEALTH CARE NEED IN KOREA)

  • In order to increase the accessibility of dental care for people with disabilities, National Health Insurance Service has implemented an additional point system of National Dental Insurance for dental treatment of patients with a special health care need (AID). The purpose of this study is to investigate the types and status of AID in Korea using data of the Health Insurance Review and Assessment Service from 2011 to 2017. The basic consultation fee is increased by 9.03 points (713 won) for brain disorder, intellectual disability, mental disability, or autistic disorder. From 2011 to 2015, the number of claims with a basic consultation fee increased from 90,456 to 141,179. Dental treatment and surgical treatment fee is increased by 100% of the defined insurance score for each of the 15 items. During the five years from 2012 to 2016, the number and amount of claims for each item increased steadily. Of the total claims for 5 years, endodontic treatment was highest, with 107,477 cases, followed by 51,641 cases of scaling. There are two types of dental safety observation fee, simple and complex. The simple safety observation fee is 10,370 won per day, and the complex safety observation fee is 20,750 won per day. Dental safety observation fees were charged 34 times in 2015, 14 times in 2016, and 41 times through May 2017. From 2011 to 2017, the number and amount of claims using AID for dental care for people with disabilities increased. However, considering that the number of registered dental users with disability was about 560,000 in 2016, the number of claims using AID is 1-20,000, which is less than 2% of registered dental users with disability. Therefore, it is necessary to expand dental services for people with disabilities including AID.

노인의 건강증진행위 영향요인에 관한 연구: 서울지역 거주노인을 중심으로 (A Study of Influential Factors on Health Promoting Behaviors of the Elderly: Focusing on Senior Citizens Living in Seoul)

  • 본 연구의 목적은 지역사회거주 노인들의 건강증진행위(Health Promoting Behavior : HPB)에 영향을 미치는 요인들을 규명하고자 하는 것이다. 본 연구는 생태체계적 관점(Ecological Perspective)에서 Pender (1996)의 건강증진모델을 활용하여 연구모형을 설정하고 이를 분석하고자 하였다. 본 연구의 자료수집을 위해 2009년 1월 18일부터 2월 28일 까지 서울지역에 거주하는 만 60세 이상 노인을 대상으로 면접조사를 하였으며 총 495부를 최종 분석에 사용하였다. 연구결과 노인의 건강증진행위 평균값은 3.11(SD=0.41)로 나타났다. 위계적 다중회귀분석 결과, 본 연구에서 모델I의 설명력은 5%이었으나 행위관련인지요인을 투입한 모델II에서 48.3%로 증가하였고, 환경요인을 투입한 모델III에서는 56.1%로 증가하였다. 모든 변수를 투입한 모델IV의 총설명력은 55.7%로 나타났다. 연구대상자들의 건강증진행위에 영향을 미치는 변수들의 상대적 영향력은 지각된 유익성(β=.475)이 가장 높았으며, 그리고 사회적 지지, 자기효능감, 지역사회환경, 주관적 건강상태, 교육 및 연령 등의 순으로 나타났다. 본 연구결과를 바탕으로 첫째, 노인의 건강증진행위를 증진시키기 위한 교육적이고 지지적이며, 다양한 수준의 프로그램 개발, 둘째, 노인복지기관의 접근성 증진을 위한 환경의 조성, 셋째, 프로그램 참여자들을 대상으로 한 자조그룹의 조직, 넷째, 노인 친화적인 환경의 구축과 건강증진관련 시설 및 서비스들의 통합과 조정을 위한 지역단위 해결방안의 모색과 끝으로 노인복지 담당 사회복지사의 건강관련 전문성 증진을 위한 교육 및 훈련의 필요성이 제시되었다.

고령층 1인 가구 여부와 미충족의료의 연관성 (Association between single-person households in the elderly and unmet medical need)

  • Objective: This study was conducted to provide basic data for the establishment of effective health policies for the unmet medical experience that may occur among the elderly depending on whether they live in a singleperson household or not. Methodology: This study used data from the 8th National Health and Nutrition Examination Survey (2019-2020) and excluded cases with missing values in variables for the total number of respondent participants of 15,469. Finally, 2,850 subjects aged 65 or older were selected for final analysis. This study examined the relationship between experiences of unmet medical needs, attempting to confirm the relationship between single-person households and unmet medical needs through subgroup analysis considering gender, age, and household income. Results: According to the results, in the case of single-person households, the odds ratio (OR) for unmet medical needs was significantly higher at 1.60 times (95% CI: 1.16-2.21). Upon conducting subgroup analyses for gender, age, and household income quintiles, the OR was significantly higher at 2.24 times (95% CI: 1.14-4.41) for males and 1.48 times (95% CI: 1.02-2.14) for females, statistically significant in both cases. For individuals aged 65-69, the OR was significantly higher at 1.90 times (95% CI: 1.04-3.47), but for those aged 70-74 and over 75, it was not statistically significant. In the case of households with 'low' income, the OR was higher at 1.62 times (95% CI: 1.16-2.26), and for 'middle' income, it was significantly higher at 3.21 times (95% CI: 1.08-9.51). Conclusion: This study confirmed that the experience of unmet medical care is high among men who make up single-person households and low-income seniors. Therefore, this study suggests that policies to expand medical services and support welfare for single-person households should be established to resolve these problems, showing that health policies that take into account individual and regional characteristics are needed to improve medical accessibility for single-person households.

뇌성마비 환자의 주거 환경과 재활 접근성에 관한 연구 (Study of the Residential Environment and Accessibility of Rehabilitation for Patients with Cerebral Palsy)

  • 목적: 뇌성마비 환자의 주거 환경과 재활 접근성을 조사하여 현재 시행되고 있는 장애인 주거 관련 법률에 대한 문제점에 대해 고찰해보고 장애인 건강권에 관한 법률 제정의 기초 자료를 제공하고자 한다. 대상 및 방법: 주거, 재활, 접근성 3개의 키워드로 문헌을 검색하여 주거 환경, 재활 접근성이라는 2개의 아이템을 선정한 후 설문지를 작성하였다. 문항은 총 51문항을 작성하였고 그중 24문항은 리커트 척도, 27문항은 다지선다형으로 구성되어 있다. 뇌성마비 환자와 보호자를 대상으로 설문조사를 시행하였다. 결과: 연구대상자 100명 중 재가보호 중인 환자는 93명, 시설보호 중인 환자는 7명으로 나타났다. 재가보호 중인 경우 65%는 아파트에 거주하고 있었고, 주로 2층 이상의 지상층에 거주하고 있었으며 그중 40%는 엘리베이터가 없는 건물에 거주하고 있었다. 연구 대상자의 Gross Motor Function Classification System (GMFCS) 단계에 따라 GMFCS I, II, III은 보행가능군, IV, V는 보행불가군으로 나누어 군별로 느끼는 어려움을 조사하였다. 재가보호 중인 경우 두 군 모두 혼자서 재활센터 방문하는 것을 가장 어려워했고, 시설보호 중인 경우 보행가능군은 혼자 시설 밖으로 나가기, 보행불가군은 혼자서 화장실 이용하기를 가장 어려워하는 것으로 나타났다. 응답자의 83%는 뇌성마비 환자에게 재활이 필요하다고 생각하지만, 실제로 재활을 받는 뇌성마비 환자는 33%에 불과했다. 재활을 받는 경우, 평균 주 3.6회, 회당 39분 동안 치료를 받는다고 응답하였다. 결론: 주거약자법은 접근로와 관련된 규정이 없고 장애인등편의법에서는 공동주택에 대한 주 출입구 접근로의 항목만 규정하고 있다. 하지만 본 연구에서 단독주택에 거주하는 뇌성마비 환자는 18%로, 이들은 접근로가 확보되지 않은 주거 환경에서 살고 있다. 뇌성마비 환자들이 고층으로 이동하기 위한 수단에 대한 법률은 없으며, 실내로 진입하기 위한 접근로에 관한 법률은 미흡한 실정이다. 현재 시행 중인 장애인등편의법, 주거약자법 그리고 2017년 12월에 시행된 장애인건강권법의 성공적인 시행을 위해서는 장애인과 보호자가 현실적으로 느끼는 어려움을 반영한 시행 규칙이 제정되어야 할 것이다. 이를 위해 본 연구 결과를 바탕으로 대규모 설문을 통해 뇌성마비 환자의 주거와 재활에 대한 조사가 필요하다고 생각한다.

도서지역 보건지소 공중보건의사의 응급의료 경험 및 대처능력 고찰 (The Experience and Competence of Physicians Who Provide Emergency Health Care at Public Health Sub-Centers on Remote Islands in Korea)

  • 본 연구는 도서지역에 있는 47개 보건지소에 근무하는 의과 공중보건의사 79명을 대상으로 응급의료 수행 현황 및 지식수준 등을 파악하고자 하였다. 전체 79명의 대상자 중 35명이 설문에 응답하여 44.30%의 응답률을 보였다. 지난 6개월간 도서지역 보건지소에 근무하는 공중보건의사 중 58.68%가 응급의료 질환을 경험하였고, 평균 1.92건의 응급처치를 수행한 것으로 나타났다. 하지만, 실제 응급처치 능력에 대해서는 일부 생명과 직접 연관된 응급처치 능력에 자신이 없음을 호소하였다. 보건지소 근무 의사 중 20.25%만이 전문의 자격을 갖춘 의사였는데, 응급의료 관련 질환을 처치하는 데 있어 전문의가 일반의와 비교하면 지식수준이 유의하게 높은 것을 확인할 수 있었다. 또한, 지식수준이 높다 하더라도 장비 및 의료지원 등 부족으로 실제 처치 능력으로 이어지지 못하는 것을 알 수 있었다. 일부 섬의 경우 1명의 공중보건의사만 배치된 일도 있었으나 일반의 1인, 전문의 1인을 배치하는 것이 타당하다는 의견이 많았고, 공중보건의사 배치 이후 부족한 지식 및 기술을 습득하기 위한 교육 및 지원체계가 필요하다는 의견이 다수 있었다. 도서지역은 지리적으로 고립되어 있고, 이용할 수 있는 보건의료기관에 대한 선택권이 적다는 점에서 보건지소의 역할이 무엇보다 중요하다. 특히 응급질환 발생시 보건지소를 이용하는 주민이 많다는 측면에서도 보건지소 응급의료 기능을 강화하는 방안을 마련하는 것이 필요하겠다. 응급의료 기능을 효율적으로 수행하고 질 높은 서비스 제공을 위하여 적절한 인력, 의료장비를 갖추도록 하고, 응급의료에 대한 매뉴얼 개발 및 현장교육 체계를 정비하여 최신의 지식과 기술을 보급하는 방안을 강구하여야 한다.

한국농촌보건(韓國農村保健)의 문제점(問題點)과 개선방안(改善方案) (Innovative approaches to the health problems of rural Korea)

  • The categories of national health problems may be mainly divided into health promotion, problems of diseases, and population-economic problems which are indirectly related to health. Of them, the problems of diseases will be exclusively dealt with this speech. Rurality and Disease Problems There are many differences between rural and urban areas. In general, indicators of rurality are small size of towns, dispersion of the population, remoteness from urban centers, inadequacy of public transportation, poor communication, inadequate sanitation, poor housing, poverty, little education lack of health personnels and facilities, and in-accessibility to health services. The influence of such conditions creates, directly or indirectly, many problems of diseases in the rural areas. Those art the occurrence of preventable diseases, deterioration and prolongation of illness due to loss of chance to get early treatment, decreased or prolonged labour force loss, unnecessary death, doubling of medical cost, and economic loss. Some Considerations of Innovative Approach The followings art some considerations of innovative approaches to the problems of diseases in the rural Korea. 1. It would be essential goal of the innovative approaches that the damage and economic loss due to diseases will be maintained to minimum level by minimizing the absolute amount of the diseases, and by moderating the fee for medical cares. The goal of the minimization of the disease amount may be achieved by preventive services and early treatment, and the goal of moderating the medical fee may be achieved by lowering the prime cost and by adjusting the medical fees to reasonable level. 2. Community health service or community medicine will be adopted as a innovative means to disease problems. In this case, a community is defined as an unit area where supply and utilization of primary service activities can be accomplished within a day. The essential nature o the community health service should be such activities as health promotion, preventive measures, medical care, and rehabilitation performing efficiently through the organized efforts of the residents in a community. Each service activity should cover all members of the residents in a community in its plan and performance. The cooperation of the community peoples in one of the essential elements for success of the service program, The motivations of their cooperative mood may be activated through several ways: when the participation of the residents in service program of especially the direct participation of organized cooperation of the area leaders art achieved through a means of health education: when the residents get actual experience of having received the benefit of good quality services; and when the health personnels being armed with an idealism that they art working in the areas to help health problems of the residents, maintain good human relationships with them. For the success of a community health service program, a personnel who is in charge of leadership and has an able, a sincere and a steady characters seems to be required in a community. The government should lead and support the community health service programs of the nation under the basis of results appeared in the demonstrative programs so as to be carried out the programs efficiently. Moss of the health problems may be treated properly in the community levels through suitable community health service programs but there might be some problems which art beyond their abilities to be dealt with. To solve such problems each community health service program should be under the referral systems which are connected with health centers, hospitals, and so forth. 3. An approach should be intensively groped to have a physician in each community. The shortage of physicians in rural areas is world-wide problem and so is the Korean situation. In the past the government has initiated a system of area-limited physician, coercion, and a small scale of scholarship program with unsatisfactory results. But there might be ways of achieving the goal by intervice, broadened, and continuous approaches. There will be several ways of approach to motivate the physicians to be settled in a rural community. They are, for examples, to expos the students to the community health service programs during training, to be run community health service programs by every health or medical schools and other main medical facilities, communication activities and advertisement, desire of community peoples to invite a physician, scholarship program, payment of satisfactory level, fulfilment of military obligation in case of a future draft, economic growth and development of rural communities, sufficiency of health and medical facilities, provision of proper medical care system, coercion, and so forth. And, hopefully, more useful reference data on the motivations may be available when a survey be conducted to the physicians who are presently engaging in the rural community levels. 4. In communities where the availability of a physician is difficult, a trial to use physician extenders, under certain conditions, may be considered. The reason is that it would be beneficial for the health of the residents to give them the remedies of primary medical care through the extenders rather than to leave their medical problems out of management. The followings are the conditions to be considered when the physician extenders are used: their positions will be prescribed as a temporary one instead of permanent one so as to allow easy replacement of the position with a physician applicant; the extender will be under periodic direction and supervision of a physician, and also referral channel will be provided: legal constraints will be placed upon the extenders primary care practice, and the physician extenders will used only under the public medical care system. 5. For the balanced health care delivery, a greater investment to the rural areas is needed to compensate weak points of a rurality. The characteristics of a rurality has been already mentioned. The objective of balanced service for rural communities to level up that of urban areas will be hard to achieve without greater efforts and supports. For example, rural communities need mobile powers more than urban areas, communication network is extremely necessary at health delivery facilities in rural areas as well as the need of urban areas, health and medical facilities in rural areas should be provided more substantially than those of urban areas to minimize, in a sense, the amount of patient consultation and request of laboratory specimens through referral system of which procedures are more troublesome in rural areas, and more intensive control measures against communicable diseases are needed in rural areas where greater numbers of cases are occurred under the poor sanitary conditions.
