병원에서는 의료 기록저장 시스템 EMR (Electronic Medical Record)을 통하여 개인 정보 및 건강 정보를 저장 및 관리한다. 그러나 병원의 정보 공유를 위한 다양한 서비스를 제공함에 따라 취약점과 위협이 증가하고 있다. 따라서 본 논문에서는 EMR에서 환자 정보의 전송으로 인한 개인 정보 유출을 방지하기 위한 모델을 제안하였다. 환자의 의료 기록이 저장된 병원으로부터 환자 정보를 안전하게 수신 및 전달할 수 있는 권한을 부여하기 위한 방법을 OAuth 권한 위임 토큰을 사용하여 제안하였다. OAuth Token에 의사 정보와 환자가 원하는 기록 열람 제한을 적용하여 전달함으로써, 안전한 정보 전달이 가능하도록 프로토콜을 제안하였다. OAuth Delegation Token은 환자 정보를 열람할 수 있는 권한, 범위, 파기 시점 등을 작성하여 전달 가능하다. 이를 통하여 안전한 환자 정보 전달 및 환자 정보 재사용 금지를 방어할 수 있다. 또한, 불법적인 환자 정보 수집을 방지하고 전달 과정에서 발생할 수 있는 개인 정보의 유출을 방지한다.
오픈노트는 환자-의사 간 온라인 진료기록 공유라고 정의할 수 있으며, 환자가 자신의 진료기록에 언제든 접근할 수 있도록 허용한 시도이다. 진료기록의 확장으로써 오픈노트 도입의 필요성을 확인하기 위해, 본 연구는 의료서비스의 일부를 담당하는 병원 종사자를 대상으로 진료기록과 오픈노트에 대한 인식과 태도를 조사하였다. 연구 결과 자신의 진료기록을 읽어보는 것은 건강상태를 이해하는데 도움이 되며, 병원 종사자들은 오픈노트의 유용성에 대체로 동의하였고 참여의향이 있는 것으로 나타났다. 한편 대상자들은 진료기록이 위·변조될 가능성이 있다고 인식하였다. 결론적으로 환자가 자신의 진료기록을 자유롭게 읽어보도록 허용함으로써 건강상태를 이해하고 자가건강관리를 향상시킬 수 있으며, 진료기록에 대한 불필요한 오해와 불신을 해소할 수도 있다. 오픈노트의 건강상의 효용성 뿐 만 아니라 의사에 대한 신뢰에 미치는 영향을 생각할 때, 실험적 검증을 위한 오픈노트 시범사업을 제언한다.
IHE(Integrating the Healthcare Enterprise)에서는 환자의 전자진료기록 공유를 위하여 XDS.b(Cross-Enterprise Document Sharing.b) 프로파일을 개발하였다. 그러나 XDS.b 프로파일의 Document Source와 Document Consumer는 모바일 환경에서는 사용하기에는 부적당하기 때문에 모바일 환경에서 XDS.b infrastructure에 쉽게 접근할 수 있는 MHD(Mobile access to Health Documents) 프로파일을 개발하였다. 본 논문에서는 전자진료기록 표준인 HL7 CDA 문서 등록을 위한 XDS.b Document Source의 Provide and Register Document Set-b[ITI-41] 트랜잭션과 MHD Document Source의 Put Document Dossier[ITI-65] 트랜잭션을 모바일 환경에서 구현하고 메시지의 크기를 비교하였다. 구현된 결과를 통하여 MHD ITI-65 트랜잭션의 메시지 크기가 XDS.b ITI-41 트랜잭션 메시지 보다 80% 정도 감소함을 알 수 있었다.
Purpose: The purpose of this study was to evaluate the effects of reminiscence therapy on depressive symptoms in older adults with dementia using a systematic review and meta-analysis. Methods: Randomized controlled trials (RCTs) published from January 2000 to January 2018 were searched through Research Information Sharing Service (RISS), Korean Studies Information Service System (KISS), Korean Medical Database (KMbase), KoreaMed, PubMed, Cochrane Library, Cumulative Index to Nursing and Allied Health Literature (CINAHL), and Ovid MEDLINE. Two researchers independently performed the search, selection, and coding. Comprehensive Meta-Analysis 3.0 was used for meta-analysis, and Review Manager program 5.3 was used for quality assessment. Results: Out of the 1,250 retrieved articles, 22 RCTs were selected for analysis. The overall effect size of reminiscence therapy for mitigating depressive symptoms in older adults with dementia was -0.62 (95% Cl: -0.92 to -0.31). The effect size was greater in older adults under 80, those with less disease severity, and those for whom the therapy session lasted less than 40 minutes. Conclusion: Reminiscence therapy is an effective non-pharmacological therapy to improve depressive symptoms in older adults with dementia. Because its effectiveness is also influenced by age, disease severity, and application method, it is necessary to consider treatment designs based on individual characteristics as well as methodological approaches.

COVID-19 poses a major risk to global health, highlighting the importance of faster and proper diagnosis. To handle the rise in the number of patients and eliminate redundant tests, healthcare information exchange and medical data are transmitted between healthcare centres. Medical data sharing helps speed up patient treatment; consequently, exchanging healthcare data is the requirement of the present era. Since healthcare professionals share data through the internet, security remains a critical challenge, which needs to be addressed. During the COVID-19 pandemic, computed tomography (CT) and X-ray images play a vital part in the diagnosis process, constituting information that needs to be shared among hospitals. Encryption and image steganography techniques can be employed to achieve secure data transmission of COVID-19 images. This study presents a new encryption with the image steganography model for secure data transmission (EIS-SDT) for COVID-19 diagnosis. The EIS-SDT model uses a multilevel discrete wavelet transform for image decomposition and Manta Ray Foraging Optimization algorithm for optimal pixel selection. The EIS-SDT method uses a double logistic chaotic map (DLCM) is employed for secret image encryption. The application of the DLCM-based encryption procedure provides an additional level of security to the image steganography technique. An extensive simulation results analysis ensures the effective performance of the EIS-SDT model and the results are investigated under several evaluation parameters. The outcome indicates that the EIS-SDT model has outperformed the existing methods considerably.

This study aims to explore improvement measures for the linkage system between the police and social welfare agencies in responding to domestic violence and child abuse issues in South Korea by comparing and analyzing the current systems in South Korea, the United States, and the United Kingdom. Despite the legal and institutional foundations for victim protection and perpetrator correction in each country, the effectiveness of actual cooperation faces several limitations. In South Korea, the level of cooperation is insufficient due to the lack of information sharing, role conflicts, and ambiguity of cooperation procedures. The United States and the United Kingdom operate more systematic cooperation programs, showing notable achievements in mental health crisis intervention and domestic violence response. To improve the linkage system in South Korea, this study proposes the following measures: (1) developing a clear cooperation protocol, (2) conducting regular education and training programs, (3) establishing an integrated information system, and (4) strengthening the victim-centered approach. Implementing these measures will contribute to more effective responses to domestic violence and child abuse issues, ultimately leading to a healthier and safer society by reducing social costs through victim protection and perpetrator correction.
Objectives: The aim of this study was to examine self-management status, nutritional knowledge, barrier factors in dietary management and needs of nutritional management program for women with Gestational Diabetes Mellitus (GDM). Methods: A total of 100 women with GDM were recruited from secondary and tertiary hospitals in Seoul. The questionnaire composed of general characteristics, status of self-management, dietary habits, nutrition knowledge, barrier factors in dietary management, needs for nutrition information contents and nutritional management programs. Data were collected by a self-administered questionnaire. All data were statistically analyzed using student's t-test and chi-square test using SAS 9.3. Results: About 35% of the subjects reported that they practiced medical nutrition and exercise therapy for GDM control. The main sources of nutrition information were 'internet (50.0%)' and 'expert advice (45.0%)'. More than 70% of the subjects experienced nutrition education. The mean score of nutrition knowledge was 7.5 point out of 10, and only about half of the subjects were reported to be correctly aware of some questions such as 'the cause of ketosis', 'the goal of nutrition management for GDM', 'the importance of sugar restriction on breakfast'. The major obstructive factors in dietary management were 'eating more than planned when dining out', 'finding the appropriate menu when dining out'. The preferred nutrition information contents in developing management program were 'nutritional information of food', 'recommended food by major nutrients', 'the relationship between blood glucose and food', 'tips on menu selection at eating out'. The subjects reported that they need management program such as 'example of menu by calorie prescription', 'recommended weight gain guide', 'meal recording and dietary assessment', 'expert recommendation', 'sharing know-how'. Conclusions: Based on the results of this study, it is necessary to develop a program that provide personalized information by identifying the individual characteristics of the subjects and expert feedback function through various information and nutrition information contents that can be used in real life.
본 연구는 정보를 빠르게 수용하고 활용하는 대학생들이 정보보호의 중요성과 건강정보에 관한 인식을 알아보고 대학교에서 개인정보보호 교육의 필요성을 확인하고자 경기도 소재 대학교 3개 학과 학생 전수를 대상으로 설문조사를 수행하였다. 조사결과 개인정보의 민감도는 민감정보, 건강정보, 일반개인정보 순으로 높았다. 민감정보는 주민번호, 주소 및 휴대전화번호에서, 건강정보는 진단명, 검사결과, 과거병력, 투약정보에서 민감하게 반응하였다. 여학생이 남학생보다, 성년이 미성년보다, 고학년일수록, 개인정보보호교육 경험자가 교육받은 적이 없는 응답자에 비하여 개인건강정보에 대한 인식수준이 높고 민감도 차이를 나타났다. 또한, 개인정보의 제공의사는 공공 및 의학연구보다 영리목적일 경우 제공불가라고 응답한 비율이 높았다. 특히 학생들은 주민번호, 성병과 같은 진단명의 공개나 과거 병력이 민감하게 반응하여 개인의 프라이버시 침해를 우려하였다. 보건 전공계열 학생이 기타 계열의 학생보다 의학연구나 교육 공공목적에 정보제공의사가 높아 교과 학습을 통해 공공목적이나 의학연구에 있어서 정보제공의 중요성에 관한 인식이 높음을 알 수 있었다. 건강정보는 개인에게 있어서 매우 민감한 정보이지만 의학 발전이나 공중보건의 측면에서 적절한 정보의 활용 또한 고려되어야 한다. 다양한 개인정보 유출 피해가 증가하고 있으므로, 정보주체로서 개인정보보호 의식 함양을 위해 대학 교양교육과정 또는 정보관련 교과목에 개인정보보호 교육을 포함시킴으로써 개인정보보호에 대한 인식수준을 높이는 것이 필요하다.
최근 병원에서는 불치병을 갖고 있는 환자에게 의료서비스를 제공하기 위해서 체내에 장치를 부착하여 환자상태를 모니터링하는 체내삽입장치를 사용하고 있다. 그러나, 유헬스케어 환경을 구성하고 있는 병원관계자가 무분별하게 환자의 생체정보를 악용하여 환자의 생명에 위협을 줄 수 있는 문제점들이 나타나고 있다. 본 논문에서는 병원 관계자의 권한레벨에 따라 환자의 생체정보를 사용하기 위한 연합 ID 기반의 인증 모델을 제안한다. 제안 모델은 서로 다른 인증 식별 체계가 사용되고 있는 병원에서 다양한 형태로 존재하는 다수의 ID 정보를 연합하여 병원 간 건강/의료 정보 공유시 불필요한 개인 정보 노출 없이 익명성을 보장받을 수 있다. 특히, 환자 정보에 쉽게 접근할 수 있는 병원관계자의 악의적 행위에 대해서 환자 정보를 안전하게 보호하기 위해서 접근권한에 대한 레벨을 부여함으로써 제 3자가 쉽게 접근하지 못하도록 한다.
현재 U-Healthcare는 헬스, 치료중심의 건강관리 개념에서 향후 예방중심의 Wellness로 전문화 되고 있다. 그 중 Wellness는 U-Healthcare의 핵심 분야로 발전됨에 따라 휘트니스 시스템 분야의 개발이 대두되고 있다. 기존의 휘트니스 시스템에서는 트레이너와 사용자 간의 1:1 상담만이 가능했고, 트레이너의 수가 제한되어, 특정 시간대에 코칭을 받을 수 있는 사용자의 수가 늘어남에 따라 개인이 받을 수 있는 코칭 시간이 줄어드는 문제점이 존재하였다. 이에 본 논문에서는 휘트니스 시스템에서의 사용자의 BMI(Body Mass Index) 지수를 DB 테이블의 필드 값으로 그룹화 하여 관리하고, 트레이너가 BMI 지수가 상이한 다수의 사용자를 효율적으로 코칭할 수 있도록 하는 메커니즘을 설계하고자 한다.
