우리나라 노인의 우울과 주관적 구강건강 관련성: 국민건강영양조사 제6기 2차(2014년) 자료 이용 (Association between depression and poor oral health in Korean elderly: the six Korean national health and nutrition examination survey (KNHANES VI-2))

  • Objectives: The purpose of this study was to examine the association between depression and poor oral health in Korean elderly using Korean version of the Patient Health Questionnaire-9 (PHQ-9) for assessment of depressive symptoms. Methods: This study used the data from Korean National Health and Nutrition Examination Survey (KNHANES VI-2). The study included 1,454 elderly Korean aged over 65. Variables included demographic characteristics (gender, age), socioeconomic factor (income, education), systemic diseases, oral health related factor (tooth brush, dental products), health related factor (alcohol drinking, smoking), and depression. Logistic regression analysis was used as sequential models. Effects were quantified as odds ratios (OR) and 95% confidence intervals (CI). Results: From frequency analysis, being female, primary school or less, non-alcohol drinking, poor oral health were significantly related to depression. In the multiple logistic regression model, depression was significantly associated with poor oral health (OR=1.96, CI=1.15-3.53) after adjustment for other covariates including demographic characteristics, socioeconomic factor, systemic diseases, oral health related factor, and health related factor (OR=1.91, CI=1.13-3.27). Conclusions: Depression had an influence on the poor oral health after adjustment as confounding variable in the elderly. It should be focused on the health promotion for the elderly vulnerable to depression and poor oral health. The development of the mental health and oral health should be established.

종합건강진단 수진자들의 건강진단에 대한 인식 및 태도에 관한 연구 (A Study on Perception and Attitudes of Examinees of Health Screening Center towards Health Examination)

  • 본 연구는 종합건강진단 수진자들의 건강진단에 대한 인식 및 태도를 조사하여 예방진료 행위에 대한 이용형태를 분석하고자 1995년 3월부터 1995년 5월까지 영남대학교 의과대학 부속병원 종합건강진단센터를 방문한 총 468명을 대상으로 설문조사를 실시하였으며, 분석한 결과는 다음과 같다. 조사대상자 468명 중 40-49세가 48.3%로 가장 많았으며, 대졸이상이 68.6%이었다. 학력이 높고 사회경제적으로 안정된 연령층에서 건강진단에 대한 관심도가 높았다. 현재 종합건강진단을 이용하고 있는 대다수가 자발적 동기보다는 기업체의 직원복지 차원에서 이루어지고 있고, 자발적으로 건강진단을 받은 137명 중에서는 건강유지를 위해 검진을 받았다고 응답한 경우가 43%로 제일 높았다. 응답자의 대부분인 95.7%가 종합건강진단이 질병 조기발견에 효과가 있다고 응답했으며, 92.5%는 종합건강진단이 유익성이 있는 것으로 인식하고 있었다. 종합건강진단에 소요되는 시간과 절차가 적당하다고 생각하고 있었으며, 적정건강진단 간격을 묻는 물음에서는 1년이 가장 많았다. 또한 검진 추가항목으로는 뇌검사, 신경계통 검사, 위내시경검사, 각종 암검사에 대한 의견이 높게 나타났다. 건강유지를 위한 실천여부와 수진 횟수와의 관계 및 적정 건강진단 간격과 수진횟수와의 관계는 통계학적으로 유의한 관련성이 있었다(p<0.01). 종합건강진단 수가에 대한 의견과 적절 비용을 묻는 물음에서는 현재비용이 비싸다고 응답한 경우가 61.8%로 가장 높았으며, 적절한 비용으로는 15만원 정도가 적당하다고 보는 의견이 가장 많았다. 건강진단 결과에 대한 판정시 의사와 상담하고 싶은 내용으로는 각종 성인병에 대한 정보가 가장 많았으며, 다음으로 정기적 건강관리 내용에 대한 문의나 가족들의 건강문제에 대한 상담 등이 많은 수를 차지하였다. 요망사항으로는 예약기간의 단축, 검진비용의 인하 및 상세한 설명에 대한 요구의 순으로 나타났다. 이상의 결과로 보아 종합건강진단을 질병의 예방 및 조기발견의 측면에서 예방진료기능이 높은 것으로 인식되고 있으며, 수진자들의 의견을 반영하여 상담내용의 내실화를 기하고 수진자가 선택할 수 있는 건강진단 항목의 개발과 수진자 교육을 위한 보건교육 교재 등을 개발함으로써 단순한 검사 결과의 판정이나 일회성 건강진단의 수준을 극복하고 보다 지속적이고, 포괄적인 건강관리를 위한 기전과 방안이 필요하다고 생각된다.

당뇨병 유병자 중 혈당 미조절군의 주관적 건강인식 관련 요인: 제4-6기 국민건강영양조사 자료(2007-2015)를 중심으로 (Factors Associated with Self-Rated Health among Poor Glycemic Control Group with Diabetes Mellitus: The 4th-6th Korea National Health and Nutrition Examination Survey (2007-2015))

  • Background: This study aimed to properly manage diseases such as blood sugar control so that patients with diabetes can benefit from both medication and health activities. Also, these health practices are greatly influenced by self-rated health, a subjective assessment of health status. Because self-rated health does not necessarily match the objective health status, it is important to identify which factors affect self-rated health. Methods: For the study, the data was gathered from the 4th-6th National Health Nutrition Survey (2007-2015). Out of the total 73,353 participants in the survey, 2,303 patients with uncontrolled blood sugar with an HbA1c level of more than 7% were selected for the final study. Dependent variables fell into two categories depending on how the participant reported whether he or she was in good health or not. Independent variables included socio-demographics, health behavioral, and health status factors. This study performed logistic regression analysis. Results: Out of 2,303 participants, 18.1% reported that their heath was 'good,' despite the fact that their blood sugar level was not controlled. After running a logistic regression model, the odds ratio of groups that perceive subjective health awareness as good was higher in the groups of people as below: in the people over 60 years old; in the people who graduated from a junior college or higher than those who had a level of education of primary school completion or less; in the people living in Chungnam than those living in Seoul; and in the group with hypertriglyceridemia. Conclusion: The study identified factors associated with those failed to perceive the blood sugar level as a severe health problem despite of the fact that blood sugar was not controlled. To improve public health, diabetes management policies need to be addressed to population groups with these problems above.

보건소등록결핵환자(保健所登錄結核患者)에 대(對)한 사회의학적(社會醫學的) 조사연구(調査硏究) (A Follow-up Survey on the Socio-medical Status of the Drop-out Cases of Tuberculosis in Jeonju Health Center)

  • A socio-medical survey was carried out on 1,108 cases of pulmonary tuberculosis who had registered and 220 cases unregistered at Jeonju health center in 1973, during from June 1 to July 31, 1974. As the results of this survey, the following conclusions were obtained. 1. Of the total 1,108 cases of pulmonary tuberculosis there were 708 new cases, while remaining 400 were old cases and rate of registration was 4.0 per thousand person. 2. The highest rate of registration of the newly diagnosed pulmonary tuberculosis could be found in bacteriological examination while 54.4 per cent in X-ray examination. 3. As for the educational status of the tuberculosis patients, primary school graduates contituted the greatest proportion or 64.6 per cent, middle school graduates 13.6 per cent, high school graduates 6.8 per cent and collge graduates only 0.7 per cent. 4. By age group of the cases, at age of 20 to 29 years occupied 23.6 per cent (262 out of 1,108 cases), 17.9 per cent at age of 40 to 49 years. 5. The greatest proportion or 38.4 per cent of the cases had no occupation. 6. The living standard of the patient's household, low class constituted 60.6 per cent of the total households. 7. By distribution of residential area, farming area was 5.0 per thousand person, 4.0 in downtwon and suburban areas. 8. The greatest proportion or 70.0 per cent of positive cases in X-ray examination was unregistered in August, 63.6 per cent in July 1973. 9. 220 out of 1,108 cases(19.9%) were unregistered pulmonary tuberculosis in X-ray and bacteriological examinations. 10. For age groups of unregistered caes, most prevalent age group was 30 to 39. 11. Regarding on the cases of unregistration, 'indifference for disease' occupied highest rate with 31.9 percent' and 'private secret' with 15.7 per cent. 12. Of the total 457 cases drop-outed, there were 78 complete heald cases while remaining 207 inactive.

Vitamin D deficiency is an independent risk factor for cardiovascular disease in Koreans aged ${\geq}50$ years: results from the Korean National Health and Nutrition Examination Survey

  • Vitamin D deficiency is a risk factor for metabolic syndromes. We examined whether vitamin D deficiency altered the prevalence of cardiovascular disease (CVD) in older Koreans. Cross-sectional analysis of data from the Korean National Health and Nutrition Examination Survey IV 2008-2009 was used to examine the association between serum 25-hydroxyvitamin D (25(OH)D) levels and the prevalence of CVD in a representative population-based sample of 5,559 men and women aged ${\geq}50$ years. CVD was defined as angina pectoris, myocardial infarction, or stroke. The prevalence of CVD (7.0%) in the older Korean population was lower than that in the older US population, although average serum 25(OH)D levels were much lower in the Korean population. Additionally, serum 25(OH)D levels did not differ significantly between the CVD and non-CVD groups. However, subjects in the lowest category (< 25 nmol/l) of serum 25(OH)D level had the greatest prevalence of CVD, about two-fold higher than subjects in the highest category (> 75 nmol/l), after adjusting for age, gender, body mass index, education level, residence location, and region. The prevalence of other risk factors for CVD, including higher waist circumference, fasting glucose, low-density lipoprotein (LDL) cholesterol, and triglyceride levels and lower high-density lipoprotein (HDL) cholesterol levels, was also higher among subjects in the lowest category than among those in the highest category. In conclusion, low serum 25(OH)D may be an independent risk factor for CVD in older Koreans.

Nutrient intake and dietary quality of Korean adults according to chronic obstructive pulmonary disease (COPD): Based on the 2012~2014 Korea National Health and Nutrition Examination Survey

  • Purpose: Food intake of COPD patients decreases because of appetite loss, depression, and breathing problems while eating, which increase the likelihood of patients becoming malnourished. This study investigated the nutrient intake and dietary quality of people with and without COPD in Korea. Methods: There were 2,160 adults aged 50 and over who participated in the $5{\sim}6^{th}$ Korea National Health and Nutrition Examination Survey. The subjects were divided into the COPD group and non-COPD group according to the rate of forced expiratory volume at 1 second (FEV1) against forced vital capacity (FVC). Consumption of food and nutrients were calculated based on the nutrition examination survey. Statistical analyses were conducted using SPSS 19.0. Results: Among individuals in their 50s, intake of vitamin $B_1$ in the COPD group was significantly less than the NCOPD group, while intake of sodium in the COPD group was significantly greater than the NCOPD group. Among individuals in their 60s, the energy intake of the COPD group was significantly less than that of the NCOPD group. The COPD group showed significantly less intake of vitamin $B_1$ and vitamin C, but greater intake of sodium than the NCOPD group. In the group aged 70 and over, The COPD group had significantly less intake of vitamin $B_1$ than the NCOPD group and showed lower nutrient adequacy ratio values in protein, vitamin $B_1$, vitamin $B_2$, and iron than the NCOPD group. Conclusion: The COPD group consumed less energy and certain nutrients, and their dietary quality was poor compared to the NCOPD group. Thus, more detailed research is required to understand the concrete relationship between COPD and malnutrition.

Dietary factors related to hypertension risk in Korean adults-data from the Korean national health and nutrition examination survey III

  • Regional differences between large cities and rural areas are observed in the Korean National Health and Nutrition Examination Survey (KNHANES). This present study was conducted to evaluate the effect of dietary factors on hypertension risk in Korean populations, especially residents of the Chungcheong province which was not in metropolitan area, using KNHANES III. A total of 544 adults aged ${\geq}$ 19 years were placed into either the normotensive or the hypertensive group. Subject characteristics, BMI, blood pressure, and nutrient intakes were compared between the two groups using a chi-square test and t-test. We estimated odds ratios (ORs) using multiple logistic regression, adjusted for energy intake and selected covariates. There were significant differences in age, education level, alcohol consumption, and BMI between the normotensive and hypertensive groups. We found decreased ORs for the medium versus lowest tertile of calcium intake (multivariate OR=0.43, 95% CI: 0.21-0.88), for the highest versus lowest tertile of calcium intake (multivariate OR=0.43, 95% CI: 0.20-0.90) with significant trends in risk (P=0.040), and for the medium versus lowest tertile of potassium intake (multivariate OR=0.43, 95% CI: 0.20-0.89). Subjects with the highest sodium/calcium ratio had a 2.10-fold greater risk of hypertension compared to the subject with the lowest, with significant trends in risk (P=0.002). Adequate calcium and potassium intake should be encouraged and regional differences should be considered in making a healthy plan for hypertension management.

  • This study was conducted to investigate nutrient consumption by Korean adults in various places. To accomplish this, we used the 1998-2009 Korea National Health and Nutrition Examination Survey (KNHANES). Subjects of this investigation were over 19 years and the study included 37,160 people. The meals were categorized as breakfast, lunch, dinner, and snacks consumed at home, or while eating-out. Investigation of the rate of consumption at serving places based on daily meals and years showed that eating-out generally increased with time. The consumption of meals prepared at home was higher than that of meals consumed anyplace else in 1998, 2001, 2005, and 2007-2009. However, the rate of consumption of home meals decreased from 1998 to 2007-2009, while the rate of eating-out increased during this period. Annual nutrient intake according to serving places with respect to meals, energy, fat, and sodium were significantly lower in home meals than those consumed elsewhere in 2007-2009 relative to 1998. The sodium intake and energy distribution ratio of fat in meals consumed while eating-out increased significantly from 1998 to 2007-2009. The energy, fat and sodium intake and energy contribution ratio of fat consumed in meals at institutions was significantly higher in 2007-2009 than in 1998. Based on these results, additional research is required to develop guidelines for dietary life improvement at each serving place and to address education and policies for balanced nutrition intake.

한국성인의 김치 섭취에 관한 연구: 국민건강영양조사 5기(2010~2012) 자료를 이용하여 (A Study on the Kimchi Consumption of Korean Adults: Using Korea National Health and Nutrition Examination Survey (2010~2012))

  • The purpose of this study was to analyze average kimchi intake, general characteristics, frequency of daily meal intake, intakes of vegetables and fruits, and nutrient intakes in four serving size groups based on data from the Korea National Health and Nutrition Examination Survey 2010~2012. The results showed an average amount of kimchi intake in subjects of 115.6 g, ranging from 0 g to 605.94 g. For daily meal intake except snacking according to kimchi serving size, all daily meal intakes increased significantly with increasing kimchi serving size (p<0.0001), and tendency of kimchi intake increased with more eating-out. As kimchi serving size increased, total intakes of vegetables and salted vegetables increased significantly (p<0.0001). However, unsalted vegetables intake did not show significant difference. Intake of fruits also increased with increasing kimchi serving size. As the serving size of kimchi increased, intakes of energy, carbohydrates, protein, fat, sodium, and potassium increased significantly (p<0.0001). For intake of sodium, intakes of all groups exceeded 2,000 mg, which is the recommended level for Koreans. Moreover, the fourth serving size group consumed three times (6,546.35 mg) more sodium than the recommended level.

  • The purpose of this study were to analyze the staple food consumption trend of Koreans using KNHANES(Korea National Health and Nutrition Examination Survey) data. Nutrient intake of 38,840 participants of 1998~2008 KNHANES, over 6 year old were analyzed. For the analysis of major staple food intakes, twenty four hour recall data was used. The results were analyzed by SAS Program, as follows. Despite westernized diet, ranking in the primary energy source of food showed similar patterns. But total calory intake was reduced over the past 11 years and percentage of carbohydrate intake was increased. Relative proportions of protein and fat intake was reduced. The major carbohydrate and protein source of Koreans was rice and grains. Pork and ramen were the major fat source food. Although intake levels were different by gender, age and areas, carbohydrate of rice was primary source of energy in Korea.
