Association between hemoglobin glycation index and cardiometabolic risk factors in Korean pediatric nondiabetic population

  • Purpose: The hemoglobin glycation index (HGI) represents the degree of nonenzymatic glycation and has been positively associated with cardiometabolic risk factors (CMRFs) and cardiovascular disease in adults. This study aimed to investigate the association between HGI, components of metabolic syndrome (MS), and alanine aminotransferase (ALT) in a pediatric nondiabetic population. Methods: Data from 3,885 subjects aged 10-18 years from the Korea National Health and Nutrition Examination Survey (2011-2016) were included. HGI was defined as subtraction of predicted glycated hemoglobin ($HbA1_c$) from measured $HbA1_c$. Participants were divided into 3 groups according to HGI tertile. Components of MS (abdominal obesity, fasting glucose, triglycerides, high-density lipoprotein cholesterol, and blood pressure), and proportion of MS, CMRF clustering (${\geq}2$ of MS components), and elevated ALT were compared among the groups. Results: Body mass index (BMI) z-score, obesity, total cholesterol, ALT, abdominal obesity, elevated triglycerides, and CMRF clustering showed increasing HGI trends from lower-to-higher tertiles. Multiple logistic regression analysis showed the upper HGI tertile was associated with elevated triglycerides (odds ratio, 1.65; 95% confidence interval, 1.18-2.30). Multiple linear regression analysis showed HGI level was significantly associated with BMI z-score, $HbA1_c$, triglycerides, and ALT. When stratified by sex, age group, and BMI category, overweight/obese subjects showed linear HGI trends for presence of CMRF clustering and ALT elevation. Conclusion: HGI was associated with CMRFs in a Korean pediatric population. High HGI might be an independent risk factor for CMRF clustering and ALT elevation in overweight/obese youth. Further studies are required to establish the clinical relevance of HGI for cardiometabolic health in youth.

Convergent Factors Affecting Depression of Drinkers by Age (연령에 따른 음주자 우울의 융합적 영향요인)

  • The present study was conducted to determine convergent factors affecting depression in drinker by age. This was a cross-sectional secondary data analysis study that used the raw data from the Korea National Health and Nutrition Examination Survey in 2014 & 2016. The subjects of the present study were 4,446 middle-aged drinker. Variables related to general characteristics, physical and psychological factors were selected. After a sampling plan compound file was created using IBM SPSS 23.0 program, data were analyzed by giving weights. Depression was influenced by gender (30s, 50-60s), income (30-40s, 60s), education (30-40s), number of family members (30-60s), job (30-60s), eating level (30-40s, 60s), hypertension (40s), DM (30-40s), BMI (30-40s), smoking (30s, 50s), exercise (30s), weight control (60s), drinking binge (30-60s), subjective health (30-60s), subjective body awareness (30-50s), stress (30s-60s), quality of life (30-60s). It is required to design preventive interventions and management plans for depression, considering these factors comprehensively.

A Study on the Febrile Illness in Autumn among Farmers in Gyeongju-si - Scrub typhus, Leptospirosis, HFRS - (경주시 일부 농업인의 가을철 발열성 질환 실태조사 - 쯔쯔가무시증, 렙토스피라증, 신증후군출혈열 -)

  • Objectives: This study aimed to assess the status of Scrub typhus, Leptospirosis and HFRS(hemorrhagic fever with renal syndrome) among farmers with febrile illness. Methods: We involved a total of 841 farmers who had febrile illness(508 and 333 village residents were, respectively, three and four district of Gyeongju city) selected during autumn of 2014 and 2015. Data were collected by survey questionnaires and blood sample examination. Results: Serum response rate for Scrub typhus and Leptospirosis was 1.5% each and 1.9% for HFRS. Serum response rate for Scrub typhus was significantly higher for tick-bite cases(38.5%), while Leptospirosis was significantly higher for those who were in fruit-planting work(23.1%). Similarly, serum response rate was significantly higher for HFRS who were working in venyl green house work(25.0%). Conclusions: Government authority should develop effective and efficient preventive strategies to create awareness of infectious diseases among farmers. Extending information, education and communication be reached to farmers that could change their perception and help early diagnosis and treatment and reduce the disease burden and its complication.

Correlation between Metabolic Syndrome and Allergic Rhinitis (대사증후군과 알레르기 비염의 상관관계)

  • Metabolic syndrome and allergic rhinitis are two important chronic diseases that affect people all over the world. Metabolic syndrome very often induces other diseases and the prevalence of allergic rhinitis is currently increasing. Aging is known to affect the pathogenesis of various diseases such as metabolic syndrome and allergy. This study examined the effects of metabolic syndrome and the prevalence of allergic rhinitis depending on different age groups and several general characteristics. This study was conducted by using the data from the Korea National Health and Nutrition Examination Survey from the 4th, 5th, 6th, and 7th-1 (2007~2016). The data used for this study was obtained from 51,854 people. In all age groups, our studies have shown that people without metabolic syndrome have a higher prevalence of allergic rhinitis than those people with metabolic syndrome, and the younger age group had the higher prevalence of allergic rhinitis (P<0.05). Overall, the results of this study found a significant effect on the prevalence of allergic rhinitis in each category of age, general characteristics (physical activity, smoking and drinking) and the presence or absence of metabolic syndrome (P<0.05). The evidence found in this study will help to understand the correlation between metabolic syndrome and allergy, and specifically allergic rhinitis.

The Relationship between the Serum Aspartate Aminotransferase/Alanine Aminotransferase Ratio and Pulse Pressure in Korean Adults with Hypertension (대한민국 고혈압 성인에서 아스파르트산 아미노전이효소/알라닌 아미노전이효소 비율과 맥압의 관련성)

  • The present study was conducted to assess the relationship between aspartate aminotransferase/alanine aminotransferase (AST/ALT) ratio and pulse pressure in Korean adults with hypertension. Data from 1,515 adults from the sixth Korean National Health and Nutrition Examination Survey (KNHANES VI-3, 2015) were analyzed. There were several key findings in the present study. First, aspartate aminotransferase (odds ratio [OR], 1.018; 95% confidence interval [CI], 1.002 to 1.033), alanine aminotransferase (OR, 0.982; 95% CI, 0.969 to 0.996), and aspartate aminotransferase/alanine aminotransferase ratio (OR, 1.367; 95% CI, 1.027 to 1.819) were the independent factors determining high pulse pressure. Second, after adjusting for related variables [age, gender, smoking, alcohol consumption, regular exercise, total cholesterol (TC), triglycerides (TGs), high-density lipoprotein-cholesterol (HDL-C), fasting plasma glucose (FPG), body mass index (BMI), and waist circumference (WC)], the ORs of high pulse pressure with the 1st quartile as a reference were significantly higher in the 4th quartile of aspartate aminotransferase/alanine aminotransferase ratio [1.632 (95% CI, 1.113~2.393)]. The high pulse pressure was positively associated with aspartate aminotransferase and alanine aminotransferase/alanine aminotransferase ratio in Korean adults with hypertension, but was inversely associated with alanine aminotransferase.

A Study on the Prevalence and Risk of Hyperuricemia according to Sitting time and High-Risk Drinking by Occupational group (직업군별 좌식시간과 고위험음주에 따른 고요산혈증의 유병률과 위험도에 관한 연구)

  • This study was conducted to analyze with a focus on gender whether the prevalence of hyperuricemia varies depending on sitting time and high-risk drinking by occupational group. For this study, the Korea National Health and Nutrition Examination Survey data were used, and 16,366 people were selected. The chi-square independence test and logistic regression model were used as the analysis method. The prevalence and risk of hyperuricemia by sitting time were different in the 'agricultural, forestry and fishery skilled workers' only in men. On the other hand, the prevalence with high-risk drinking, both men and women showed differences in 'managers, experts and related workers' and 'office worker'. Also, only women have differences in 'service and sales workers', 'technicians, equipment, machine operation and assembly workers' and 'unemployed'. These results inform men have a higher prevalence and risk of hyperuricemia and suggest that health care policies and medical services are needed to prevent it by occupational group.

Predictors of Breastfeeding in Adult Women: a Secondary Data Analysis of The Seventh Korea National Health and Nutrition Examination Survey(KNHANES VII-3), 2018 (성인 여성의 모유수유 예측요인: 국민건강영양조사 제7기 3차년도(2018)의 2차 자료 분석)

  • This study was conducted to identify the factors affecting breastfeeding in adult women based on socio-demographic and health-related characteristics using the results of KNHANES VII-3. The data was analyzed by applying weights, using the complex sample analysis method of the SPSS Win 22.0 program. Breastfeeding was influenced significantly by age, education level, and the current smoking status. Women in the age group of 30-39 years had a breastfeeding rate that was 3.06 times higher than 19-29 year old women. Also, the breastfeeding rates for women with an education level under elementary school were 4.70 times higher than those with a college degree or higher education level and non-smokers and ex-smokers had levels 2.51 times higher than smokers. Therefore, for mothers under the age of 30, education on breastfeeding should be further strengthened. For mothers over 40 years old, lactation support to increase the milk supply should be provided as well as assistance toward care for other children in the family. Also, better-educated mothers will need to learn effective breastfeeding methods in limited environments such as work-life, and a social atmosphere needs to be established that is considerate to lactating women. Furthermore, education related to pregnancy and lactation should be provided for use in smoking cessation programs for female smokers.

Relationship between Hypertension and the Declining Renal Function in Korean Adults (한국 성인에서 고혈압과 신기능 저하와의 관련성)

  • This study examined the association between hypertension (HTN), estimated glomerular filtration rate (eGFR), and urine microalbumin/creatinine ratio (ACR) in Korean adults. Data for 8,922 adults (3,941 men and 4,981 women) aged ≥20 years from the Sixth Korean National Health and Nutrition Examination Survey VI (2013~2014) were analyzed. In men, after adjusting for the related variables, the odds ratios (ORs) of HTN [systolic blood pressure (SBP) ≥140 mmHg, diastolic blood pressure (DBP) ≥90 mmHg, or use of HTN medications] were significantly higher in the decreased eGFR group [eGFR <60 mL/min/1.73 ㎡, 1.98 (95% CI, 1.21~3.24)], elevated ACR group [ACR ≥30 mg/g, 2.03 (95% CI, 1.54~2.69)], and decreased eGFR plus elevated ACR group [eGFR <60 mL/min/1.73 ㎡ and ACR ≥30 mg/g, 6.03 (95% CI, 2.82~12.92)] than in the normal group (eGFR ≥60 mL/min/1.73 ㎡ and ACR <30 mg/g). In women, after adjusting for the related variables, the ORs of HTN were significantly higher in the decreased eGFR group (2.29, 95% CI, 1.27~4.13), elevated ACR group (2.22, 95% CI, 1.68~2.94), and decreased eGFR plus elevated ACR group (10.77, 95% CI, 3.89~29.82) than the normal group. In conclusion, HTN was associated with a decreased eGFR and elevated ACR in Korean men and women. In addition, the prevalence of HTN increased greatly when a decreased eGFR and elevated ACR occurred simultaneously.

Bone mineral density in type 2 diabetic patients aged 50 years or older in men and postmenopausal women in Korea

  • Relationship between bone mineral density (BMD) and type 2 diabetes is still inconsistent. Recently, many epidemiologic data show that fracture risk is increased in type 2 diabetic patients regardless of BMD status. In this study, we used nation-wide data from 2008 to 2011 Korea National Health and Nutrition Examination Survey (KNHANES) to analyze the BMD status in patients with type 2 diabetes compared to non-diabetics. We included subjects aged 50 years or older in men (N=2,959, 2,430 without diabetes, 529 with type 2 diabetes) and postmenopausal women (N=2,902, 2,479 without diabetes, 423 with type 2 diabetes). Subjects with history of medication for osteoporosis or with illness or malignancy affecting bone metabolism were excluded. Data of anthropometric measurements and demographic characteristics were collected by trained examiner. Serum was separated from peripheral venous blood samples obtained after 8 hours of fasting. BMD was measured at lumbar spine and femur using dual-energy X-ray absorptiometry (DXA). There was a significant positive association between lumbar spine BMD and type 2 diabetes after adjusting age, gender, body mass index, monthly house income, education level, physical activity, daily calcium intake and vitamin D concentration by multiple regression analysis in all subjects. In the subgroup analysis by gender, this association was maintained both in male and female after adjusting those confounding factors. However, femur BMD was not different between type 2 diabetic and non-diabetic subjects. In conclusion, lumbar spine BMD was significantly higher in type 2 diabetic patients aged 50 years or more in men and postmenopausal women compared to non-diabetic subjects.

The association between body composition and bone mineral density in subjects aged 50 years or older in men and postmenopausal women in Korea

  • The effect of body composition such as lean mass and fat mass on bone mineral density (BMD) is complex and still controversial. In this study, we investigated the relationship between body composition and bone mineral density using nation-wide data from 2008 to 2011 Korea National Health and Nutrition Examination Survey (KNHANES) in 2,139 men and 2,193 postmenopausal women aged 50 years or older. Subjects with history of medication for osteoporosis or with diseases or malignancy affecting bone metabolism were excluded. Data of anthropometric measurements and demographic characteristics were collected by trained examiner. Fasting blood sample was obtained for blood chemistry analysis. BMD of the lumbar spine, total femur, and femoral neck, and body composition such as total lean mass (TLM), total fat mass (TFM), truncal fat mass (TrFM) were measured using dual-energy X-ray absorptiometry (DXA). There were significant positive correlations between body composition indices such as lean mass and fat mass with BMD. In multiple regression analysis, TLM was positively associated with BMD after adjusting age, body mass index, monthly house income, education level, physical activity, daily calcium intake and vitamin D concentration in both men and postmenopausal women. BMD at lumbar spine and femur in lowest quartile of TLM was significantly lower than other quartiles after adjusting those confounding factors in both gender. TrFM was negatively associated with total femur BMD in male and femur neck BMD in postmenopausal women after adjusting confounding factors. In conclusion, TLM is very important factor in maintaining BMD in subjects aged 50 years or older in men and postmenopausal women.
