The Determinants of the Use of Opportunistic Screening Programs in Korea (민간검진 서비스의 이용현황과 결정요인)

  • Objectives : Both organized and opportunistic screening programs have been widely used in Korea. This paper examined the determinants of the use of opportunistic screening programs in Korea. Methods : The subjects were a national stratified random sample of 10,254 people aged 45 or older from the first wave of the Korean Longitudinal Study of Ageing in 2006. A logit model was used to examine the determinants of the use of opportunistic screening programs in terms of the demographic and socioeconomic characteristics, the type of health insurance and the health status. Results : Thirteen point seven percent of the individuals received opportunistic screening programs within 2 years from the time the survey was conducted in 2006. The individuals who graduated from college or who had even more education were 3.0 times more likely to use opportunistic screening programs compared with the individuals who were illiterate. The individuals who resided in urban areas and who had religious beliefs were more likely to receive opportunistic screening programs compared with their counterparts. Those who were in the first quartile for the total household assets were 2.6 times more likely to use opportunistic screening programs than those who were in the fourth quartile for the total household assets. Privately insured people were 1.6 times more likely to use opportunistic screening programs than those who were not insured. Finally, the individuals who self-assessed their health status as worst were 2.1 times more likely to use opportunistic screening programs compared individuals who self-assessed their health status as best. Conclusions : This study suggests that opportunistic screening programs can be an indicator for whether or not an individual is among the advantaged group in terms of their socioeconomic characteristics and type of health insurance.

Analysis of Wage Determinants of Care Workers (요양보호사 임금결정요인 분석)

  • Background: In this study, wage status and wage determinants of care workers were analyzed. Methods: The analysis used database (DB) of long-term care institutions, DB of long-term care institutions, DB of long-term care workers, DB of health insurance qualification, and contribution possessed by National Health Insurance Services. We analyzed the wage status of the care workers from 2009 to 2016 through basic analysis and estimated the factors affecting the wage of the long-term care facilities' care workers using pooled ordinary least squares. Results: The monthly average wage of care workers was raised from Korean won (KRW) 1.37 million in 2009 to KRW 1.52 million in 2016, and the working hours were shortened by 20 hours from 207 hours to 187 hours. Hourly wages increased by KRW 1,329 from KRW 6,831 in 2009 to KRW 8,160 in 2016. The average monthly wage of care workers was affected by gender, age, years of employment, monthly working hours, establishment type, city size, institutional size, the grade of the institution, and management status. In particular, the wage level of the care workers was high when the larger the size of the institution, the better the management status (fill rate), the establishment type is "government and local government" and "corporation," the institutional rating is high, and the facility manager has the first grade of the social worker license. Conclusion: The government should consider aggressive policies to improve the treatment of care workers as well as the quality of long-term care services so that there will be more long-term care facilities that are guaranteed social publicity above a certain level.

Determinants of Self-rated Health in Sedentary Older Adults (규칙적 신체활동을 수행하지 않는 고령자의 주관적 건강평가에 영향을 미치는 요인)

  • Purpose: The objective of this study were to examine the determinants of self-rated health, specially focused on the effect of functional capacity of community dwelling sedentary older adults on self-rated health. Method: The data has been collected from 654 community-dwelling sedentary older adults (mean age: 75 years) during the period from April to June in 2007. The data were collected by the in-person interview and direct measurement of functional capacity. The data were analyzed using chi-square test and multiple regression analysis with the SPSS 9.1 program. Result: The elderly rated their health as very good (3%), good (28%), fair (38%), poor (29%0, and very poor (2%). The higher average daily walk minutes ($\beta$=0.12, p<.01), number of chair stand ($\beta$=0.10, p<.05), scores of self-efficacy ($\beta$=0.16, p<.001) and the lower number of disease ($\beta$=-0.44, p<.001) show better self-rated health. Conclusion: Self-rated health is the most commonly used indicators in social epidemiology and geriatric research because it has been known as the good predictor of mortality and reflects health related disability. The finding suggested that daily walking habits, lower body strength, physical self-efficacy should be considered to improve the senior's self-perception of health. The community-based intervention associate increase these factors should be considered.

Determinants of selecting a doctor in specialized medical institutions and general hospitals (종합전문요양기관과 종합병원의 선택진료 결정요인)

  • This research was performed to investigate the determination factors of medical service to cover the fee for selecting a doctor which is one of the most important causes of debilitating national health insurance in Korea. Data was from Korea Health Panel and analyzed by Dutton(1986)'s medical service model which was an extended Anderson Model and was widely used in the researches on determination factors of medical service. The results were as follows; In the determinants of selecting a doctor in specialized medical institutions and general hospitals, patients with serious diseases selected doctors more often than other patients. By industrial accident compensation insurance law and enforcement ordinances, insurance covers the fee of selecting a doctor in the hospitals appointed by Labor Welfare Corporation for the patients in critical conditions under industrial accident compensation insurance, while health insurance patients pay the fee themselves for selecting a doctor in all cases. It is suggested that patients with serious diseases proved by medical opinion be provided with health care insurance in selecting a doctor and that the health insurance benefit coverage be enhanced by staged lowering of patient's cost-sharing.

  • This study tried to explore the determinants of health of the youth in Korea. Especially, it explored how the socio demographic characteristics and delinquent behaviors factors of the Korean youth affect their health. This study used the 2009 Korean Youth Health Risk Behavior On-line Survey for this purpose. It selected 35,668 cases of senior high school students from the raw data and conducted a secondary data analysis. This study conducted frequency analysis, t-test, F-test, and regression analysis. Gender, living with parents, economic status, school type, academic record, internet addiction, problem drinking, drug use were significantly determinants of the health of the youth. Findings may be used for improving health of the Korean youth.

A Multilevel Model Analysis on the Determinants of Smoking Cessation Success Rates (다층모형을 통한 금연성공에 영향을 미치는 요인 분석)

  • Objectives: In this study, a multilevel analysis model has been designed to investigate the effect of personal characteristics associated with smoking cessation on anti-smoking determinants with a goal of finding out the factors which have influence on smoking cessation among the entrants of smoking cessation clinic in a public health center. Methods: A total of 253,136 male smokers who received smoking cessation services for more than six(6) months in a smoking cessation clinic of public health center from July 16, 2007 to July 15, 2008 were examined. For technical analysis, SPSS Version 2.0 has been used. For multilevel analysis on smoking cessation determinants, in addition, HLM 7.0 has been adopted. Results: According to the unconditional model of multilevel analysis, the success rates of smoking cessation among the entrants of a smoking cessation clinic were 47.3%. In an unconditional slope model test to which regional variables were added, a negative effect was observed in average smoking amount, total smoking period, nicotine dependence and services while a positive effect was found in age, stress and type of social security in terms of the log of the odds of smoking cessation. In a conditional model test, a positive effect was observed in Non-Smoking Campaigns (NSC) and Frequency of Counseling (FC) in terms of the log of the odds of smoking cessation in regional variables. Conclusions: It is important to approach smokers individually and, at the same time, build healthy environment for a local community to increase smoking cessation rates among the entrants of smoking cessation clinic in a public health center.

Individual Determinants of Hospital Days m Community-dwelling Elders (지역사회 노인의 입원기간에 영향을 주는 개인요인의 분석)

  • This study examines individual determinants of hospital days in community-dwelling elders by estimating a linear structural equation model based upon Andersen's behavioral model. Data were collected through a national survey of elders in Korea in 1994. The subjects for this secondary data analysis were 1687 non-institutionalized elders aged 60 years or older. Except for the effect of age and gender on family help. the predisposing components had direct effects on the enabling components. Of the effects of the enabling components. family help had a direct effect on self-evaluated health; economic status had a direct effect on chronic disease and self-evaluated health; and access had a direct effect on chronic disease. functional health status and self-evaluated health. Of the enabling components. residence and family help directly affected hospital days. Self-evaluated health revealed the greatest direct effect on hospital days followed by functional health. Overall. the effects of the predisposing components and the enabling components on hospital days were not prominent. Since the model explained only $4\%$ of the variance in hospital days. the magnitude of the effect of the need components cannot be judged in terms of equity of distribution of health services. In particular. the effect of family help in the model reveals the importance of family support in health management of elders. The implications of these analyses for improving the proposed model of hospital days were discussed.

  • This study focused on 'The Health' which should considered as a precondition for the local residents by reinvestigating the indicators of happiness after Young-Do ward formulate a community social welfare plan. There is purpose of this study as below. I searched an general attribute and the indicators of happiness, then looked how does results from questions about health and factors of the indicators of happiness can affects the health with some kinds of elements. Five hundred people from the inhabitants of Young-Do ward who is living in Young-Do district were separated by using the random sampling method as the subject of the study. The investigation took effect from October to November of 2010 and an investigator visited the inhabitants of a ward with structured questionnaire. An investigator had one-on-one talk with inhabitants of Young-Do ward while the investigation and I also collected the data through self-administered questionnaire as well. I found the physical elements influence in the health much more than the outside factors from the study. Higher satisfaction of their career, relationship between friends and co-workers, work environment and married life reflects the fact that higher self-contentment means high quality health level.
