A Study on u-Health care System Design based on Context-awareness (상황인식 기반의 u-Health Care 시스템 설계에 관한 연구)

  • This paper presents an u-health Care system based on context-awareness information. Extended project induced an education for creative engineering design. It could occur one of the issue for life care or wellness in recently life. Especially u-health Care system help on achieving this object as an efficient care-training with user blood pressure or with echocardiogram, electromyogram, electroencephalogram, or Electrocardiography. In this paper, development project show an u-health care system based on this parameters of the context-awareness with network service and analysis provide with working memory(DB) for user data base applications.

Policy Development on Health Administration System in the Era of Local Autonomous Government (지방자치제에 따른 보건의료사업을 위한 보건소 모델개발연구)

  • As the WTO system launches through the agreement of Uruguay Round, the Government has to revise the office regulations or reform the system. Also, Integrating and Coordinating the like affair in health care (i. e., children's home, industry health, school health, health manpower, the administration of health center, the administration on food hygiene, health environmental education, and so on.) which is now scattered into some government departments like the Ministry of Labor, the Ministry of Education, the Ministry of Home Affairs, the Ministry of Agriculture, and the Ministry of Environment, the Government has to prevent unspecialty, inefficiency, inconsistency, and uneconomy. The Government has to review and adopt above suggested the Proposal 1),2),3),4) of the Health Centers on the basis of the local autonomy law and it will help the successive settlement of the local autonomy system in Korea. According to the suggested proposal, the Central Government mainly takes charge of the Macro affairs as hardware, and transfer the Micro affairs as software into the Local Governments to attempt the appropriate functional allocation. To achieve it successfully, the Central Government also has to do the financial support, manpower training and technical support, allocation of health care resources, direction and control, research and development and the health care plan on the macro level. Local Governments which divided into the wide local government and basic local government also have to do their best for health improvement of the community societies like plan of health care program, implementation of health care service program, taking charge of the affairs of health insurance, activation of community residents' participation and security of health care resources etc. To achieve this goal, the Government have to be more active and reformative, the related social and health agencies and educational agencies have to cooperate and support for the goals, and especially, the community residents have to participate actively and voluntarily, When all these conditions promote, local health care administration will be developed, and health level of community residents will be secured. And going one step forward, the country and people will be more healthy

Model Development of Nursing Care System for Women's Health : Based on Nurse-Midwifery Clinic (여성의 건강을 위한 간호전달체계 모형개발 - 조산원 중심으로 -)

  • The purposesof the study are to analyze the community nursing center in U.S.A and to develop the model of nursing care system based on nurse-midwifery clinic in community for women's health in Korea. 1. In America nursing center is defined as nurse-anchored system of primary care delivery or neighborhood health center. Nursing centers are identified the following four types: (1) community outreach centers, which are similar to traditional public health clinics: (2) institutional-based centers following the mission of a large institution, such as a hospital or university: (3) wellness/health promotion centers, which offer screening, education, counseling, triage, and health maintenance services: and (4) independent practice. Nursing centers are a concept of services provided by nurses in practice arrangements in a community. Nursing centers offer a variety of services, ranging from primary care provided by advanced practice nurses with medical acute management and nursing care to the more traditional education, health promotion, screening wellness and coordination services. Some services, such as the care provided by advanced practice nurses are reimbursed under various insurance plan in some instances and states, where as others, such as preventive and educational services, are not. Thus, lack of reimbursement has threatened the survival of some centers. Licensing of nursing centers varies by state and program and accreditation of nursing centers is also limited. 52% of centers are affiliated with another facility and 48% are freestanding centers. The number of registered nurse at the nursing centers ranges from just one to 115, with a mean of eight RNs peragency and a median of three. Nursing centers avail ability varies: 14% are open 24 hours, 27% have variable short hours, 23% are open 6-7 days per week, and 36% are open Monday- Friday. As the result of my visiting three health centers in Seattle and San Francisco, the women's primary care nurse practitioners focus on a systematic and comprehensive assessment of the health status of women and diagnosis and management of common physical and psychosocial health concerns of women in ambulatory settings. Therapeutic nursing strategies are directed toward self-care, risk reoduction, health surveillance, stress reduction, healthy nutrition, social support, healthy coping, psychological well-being, and pharmacological therapy. They function as primary care providers for the well ness and illness care of women from adolescence through the older adult years and pregnant families. 2. In Korea a nurse-midwife practices independently for pregnant women's health including childbearing family at her own clinic in community. Her services are reimbursed under national health insurance but they are not paid on a fee-for-service schedule covering items. Analyzing the nursing centers in America, I suggest that nurse-midwifery clinics offer primary care for women and home care for chronic ill patients. The health law and health insurance policy should be reovised in order to expand nurse-midwife's and home care nurse's roles at nurse-midwifery clinic.

The Attitude of Medical Practitioners to the Chronic Disease Care System (개원의의 만성질환관리제도에 대한 수용태도)

  • The purpose of this study was to research the opinion and acknowledgement about the clinical chronic disease care system subject to Medical Practitioners that copied the internal medicine and family medicine as a treatment subject and was fulfilled in the purpose of providing a basic data for the improvement and stable establishment of the chronic disease care system and the high blood pressure and diabetes mellitus patient's promotion of health. Results from the research the internal medicine was 79% and the subject's average age was 52.3 years, the proportion that the high blood pressure and the diabetes mellitus took was average 28% and the interest about the system was 65.9%. In the chronic disease care system expansion status the opposition was 75.7%, and the detail field knowledge level was 56.9%. In the opinion field about the system, all fields were shown negative. The chronic disease care system is an important government policy enforcing purposed to effectively manage the yearly increasing chronic disease, but the due to the cold attitude of Medical Practitioners that must actively accept it, the stable establishment of the system is being concerned. Therefore even if it's a good policy if it cant gain the support and agreement of the accepter, in the future when trying to implement a system there should be no idleness in the effort to gain cooperation and understanding to relevant involved ones.

  • This study was conducted to develop a home care nursing management system based on the validated and useful data base found through literature review. The contents and structure according to a development procedure for a computer system were as follows. 1. A data base on home care nursing patients was accumulated by putting data respectively in both steps and fields - from selection criteria. basic information. prescription. plan of home visits. to application of nursing process. 2. Accumulated data was classified and designed to search by basic information. drug/injection prescription. examination prescription, treatment prescription. supply. and a record of the nursing process. 3. Various forms of retrieval including graphs were elaborated in terms of diagnosis and intervention aspects.

The impact of continuous positive airway pressure on radiation dose to heart and lung during left-sided postmastectomy radiotherapy when deep inspiration breath hold technique is not applicable: a case report

  • Deep inspiration breathing hold (DIBH) compared to free-breathing (FB) during radiotherapy (RT) has significantly decreased radiation dose to heart and has been one of the techniques adopted for patients with breast cancer. However, patients who are unable to make suitable deep inspiration breath may not be eligible for DIBH, yet still need to spare the heart and lung during breast cancer RT (left-sided RT in particular). Continuous positive airway pressure (CPAP) is a positive airway pressure ventilator, which keeps the airways continuously open and subsequently inflates the thorax resembling thoracic changes from DIBH. In this report, authors applied CPAP instead of FB during left-sided breast cancer RT including internal mammary node in a patient who was unable to tolerate DIBH, and substantially decreased radiation dose the heart and lung with CPAP compared to FB.

Analyzing the Relative Importance for the Development Plan of the Public Health Care System (공공보건의료체계 발전 방안에 대한 상대적 중요도 분석)

  • Objectives: The purpose of this study is to demonstrate empirically through a specialist AHP analysis what factors should be more important in the development of the public health care system. In addition, we will use Analytic Hierarchy Process (AHP) method for experts to achieve research purpose. Methods: The data analysis method of this study is as follows. First, we set up three metrics in order to measure the relative importance between the factors to be improved for the development of the public health care system and each of the sub-factors. A total of nine measurements (items) were set by combining the three measurement criteria for each measurement index. Second, the relative importance and priority analysis use the AHP analysis. Third, the subjects of this study were 15 experts in the field of public health care. The statistical processing was performed using the Expert Choice 2000 statistical program. Results: In order to development of the public health care system, experts ranked the most important as improvement in the systematic aspect of public health care (56%) as the first priority. Next, the relative importance analysis of the measurement items considering the multiple-weights of the sub-factors is as follows. The strengthen institutional improvement (revitalization of secondary public function hospital) was the number one, strengthen cooperation between agencies was the second, and Re-establishing the role of local public health care system was the third place. Conclusions: Considering the relative importance, factors that are considered to be important in the first place may not be improved as the best policy alternative due to limitations in spatial, temporal, financial, and institutional aspects. In this case, we suggest that we should choose the best policy alternative by using prioritization considering relative weights.

Awareness of Dementia National Responsibility of Elders: Oral Health Items

  • Background: Dementia is a condition in which a person who has been living a normal life suffers from various cognitive impairments in memory, words, and judgment that considerably disrupt daily life. The oral care ability and subjective oral status of elderly individuals with dementia are lower than those of a healthy person. The oral health care of individuals admitted to nursing homes inevitably falls to nursing assistants and nursing care staff. This study aimed to investigate the need for oral health management items of and to provide basic direction for the future of the Dementia National Responsibility System. Methods: Elders aged 65 years and over were selected from a comprehensive welfare center. A total of 155 questionnaires were analyzed. The questionnaire consisted of 15 items about general status, 9 items about recognition of the Dementia National Responsibility System, 5 items of the subjective recognition of oral health, and 6 items of the correlation between oral health and dementia. Results: Among our subjects, 71.0% answered that they did not know about the Dementia National Responsibility System, 78.7% answered that they think they need the system, and 81.9% think that they should add dental health items to the Dementia National Responsibility System. The response to the need for dementia national responsibility, oral health items in the Dementia National Responsibility System, and oral specialists all showed scores of >4 points. The need for the Dementia National Responsibility System, oral health items, and specialists were found. Conclusion: It is necessary to include oral health care items in the Dementia National Responsibility System so that elderly individuals with dementia can receive the needed oral health care.

A Relationship of Care Time with Functional Status and Patients Characteristics among Patients in Long-term Care Hospitals (장기요양환자에서 환자 특징 및 기능상태와 환자돌봄 시간과의 관련성)

  • Objectives : The aim of this study was to investigate the functional status variables related to the care time of health professionals for patients in long-term care facilities. Methods : The functional stati of 1001 patients in 8 long-term care hospitals were examined by the Resident Assessment Instrument for Long-term Care Facility Version 2.0. The care time of health professionals for patients was calculated using data from a self-reported task survey by nurses, auxiliary nurses, private aides, doctors, physiotherapists and social workers. Results : The average care time per diem was 240.6 minutes. The care time by doctors, nurses and private aides were 11.0, 71.0 and 139.5 minutes, respectively. The lower the function of activities of daily living (ADL) and the greater the symptoms of extensive services, special care and clinical complexity, the more care time was served. On the contrary, the greater the symptoms of nursing rehabilitation, depression, cognitive disorder, behavior problem and psychiatry/mood disorder, the less care time was served. Age and gender were not significantly related to the care time. Conclusions : Developing a case mix classification system for elderly long term care patients may be helpful for both of patients and health care providers. The ADL, extensive services, special care and clinical complexity of variables should be considered in the development of a case mix system for the long term care of patients in Korea.

  • The article reviewed the elderly health care management problems in policy development issue of the nation. Policy of Korean government on elderly health care has still not yet developed. The main stream of policy which is effective on elderly welfare policy is 'Elderly care are responsible by their families'. Now only those aged whose family members are not able to take care of their parents are receive custodial care at the non-profit nursing homes. This article examined the main stream policy in pro and cons aspects in relation to social changes such as: demographic changes. family structure changes. attitude changes. health care delivery system. and political settlement view points in connection with medical insurance program. Finally. a model for the elderly health care management was proposed which will provide chronic care services at the community level. such as nursing homes. day care centers. day hospitals, respite care units, and special care institute for dementia.
