Development of the Standard Blood Inventory Level Decision Rule in Hospitals (병원의 표준 혈액재고량 산출식 개발)

  • Two major issues of the blood bank management are quality assurance and inventory control. Recently, in Korea blood donation has gained popularity increasingly to allow considerable improvement of the quality assurance with respect to blood collection, transportation, storage, component preparation skills and hematological tests. Nevertheless the inventory control, the other issue of blood bank management, has been neglected so far. For the supply of blood by donation barely meets the demand, the blood bank policy on the inventory control has been 'the more the better.' The shortage itself by no means unnecessitate inventory control. In fact, in spite of shortage, no small amount of blood is outdated. The efficient blood inventory control makes it possible to economize the blood usage in the practice of state-of-the-art medical care. For the efficient blood inventory control in Korean hospitals, this tudy is to develop formulae forecasting the standard blood inventory level and suggest a set of policies improving the blood inventory control. For this study informations of $A^+$ whole bloods and packed cells inventory control were collected from a University Hospital and the Central Blood Bank of the Korean Red Cross. Using this informations, 1,461 daily blood inventory records were formulated.48 varieties of blood inventory control environment were identified on the basis of selected combinations of 4 inventory control variables-crossmatch, transfusion, inhospital donation and age of bloods from external supply. In order to decide the optimal blood inventory level for each environment, simulation models were designed to calculate the measures of performance of each environment. After the decision of 48 optimal blood inventory levels, stepwise multiple regression analysis was started where the independent variables were 4 inventory control variables and the dependent variable was optimal inventory level of each environment. Finally the standard blood inventory level decision rule was developed using the backward elimination procedure to select the best regression equation. And the effective alternatives of the issuing policy and crossmatch release period were suggested according to the measures of performance under the condition of the standard blood inventory level. The results of this study' were as follows ; 1. The formulae to calculate the standard blood inventory level($S^*$)was $S^*=2.8617X(d)^{0.9342}$ where d is the mean daily crossmatch(demand) for a blood type. 2. The measures of performace - outdate rate, average period of storage, mean age of transfused bloods, and mean daily available inventory level - were improved after maintenance of the standard inventory level in comparison with the present system. 3. Issuing policy of First In-First Out(FIFO) decreased the outdate rate, while Last In-First Out(LIFO) decreased the mean age of transfused bloods. The decrease of the crossmatch release period reduced the outdate rate and the mean age of transfused bloods.

A Survey on Asbestos Exposure Possibility in Indoor and Outdoor Environments of Childcare Centers (어린이집 실내·외 석면노출 가능성 조사에 관한 연구)

  • Objectives: Because of its properties such as resistance to heat, chemicals and corrosion; tensile strength; sound absorption; and affordable price, asbestos has been widely used as a building material, fire resistant and retardant, thermal and heat insulator, soundproofing material, and electrical insulation. Since the prolonged inhalation of asbestos can cause serious illnesses such as lung cancer, mesothelioma, and asbestosis after an incubation period of 20 to 40 years, the mineral was classified as a Group 1 carcinogen by the International Agency for Research on Cancer, an intergovernmental agency forming part of the World Health Organization. Children and infants are more at risk than are adults if they are exposed to carcinogens, due to aweaker immunity that has not yet been fully developed. Most childcare centers are operated all day and children tend to spend a great amount of time in the centers. This is why it is important for them to be systematically isolated from environments that may expose them to asbestos. Materials: In order to understand both indoor and outdoor hazards to which children may have been exposed, the study focused on actual surveys of asbestos used in childcare centers, paying special attention to slate-roofed buildings in the vicinity of the centers. Results: A survey of a total of 211 childcare centers showed that the buildings of 18.1% of the centers contained asbestos, with 60.53% of the material being found in classroom ceilings. "Tex" was the most used material for ceilings, making up 89.47% of all ceilings. An outdoor survey showed that childcare centers in Daegu Metropolitan City had an average of 143 slate-roof buildings within a distance of 1km. Conclusions: Buildings housing mainly toddlers, children, teenagers and others more vulnerable to the toxicity of asbestos are not subject to asbestos investigation by law. A legal and practical basis for asbestos control is required for such buildings. In particular, housing materials which contain asbestos in day care centers require asbestos control. GIS should be used to identify the location of buildings with slate roofing materials in the vicinity of daycare centers in order to gauge toxicity of exposure to asbestos caused by potential asbestos friability possibility in outdoor conditions.

The affective effect of odor products in relaxation and working contexts (휴식 및 집중 환경에서 방향 제품의 감성적 영향)

  • As the consumer market for odor products grows, companies producing healthcare products are beginning to pay more attention to the emotional aspect of an odor product in order to differentiate their products from competitors. In the following research, the affective effect of odor product was investigated while focusing on relaxation and working contexts using orange and pine scents, since these are typical odors in current domestic market. Two empirical studies were carried out. First, in experiment I, 18 subjects, all of whom were university students, spent 20 minutes sitting comfortably on a sofa while electrocardiogram assessments were made. After a five-minute break, in experiment II, the same subjects were provided with both arithmetic and geometric questions and their electroencephalogram readings was recorded from eight channels. All subjects participated in three sessions - no odor, an orange scent, and then a pine scent - with a minimum time interval of 24 hours. The results show that in the context of a pine scent, both the activation ratio of subjects' parasympathetic system and those of the Sensory Motor Rhythm waves and Mid Beta waves were at the highest peak. Therefore, the pine scent helped the subjects to feel more comfortable and more focused at the same time. In other words, it gave them a state of meditated attention. In addition, it was found that the right brain was activated twice the intensity when the subjects worked through the geometric questions, whereas both sides of the brain were activated in equal magnitude during the process of arithmetic tasks. This replicates previous studies of the functional aspect of the right brain - being responsible for spatial and creative thinking.

Institutionalization of a Patient-Sitter Program in Acute Care Hospitals (보호자 없는 병원 제도화 방안)

  • In an effort to resolve the burden of patients hiring patient sitters, this study sought to review the Ministry-of-Health-and-Welfare-initiated pilot program of running hospitals without patient-sitter to identify its background, operation method, performance results, and limitations. Based on the review, the study derived the necessity of introducing a hospital system without patient-sitter as well as its operation and systemization methods. The ministry-initiated pilot programs were conducted twice: in 2007, and in 2010. A review of the 2007 pilot program revealed that the patients and families' satisfaction score with nursing services was 9.1 points (on a 10-point scale), their intention to reuse the service was 97.8%, and their intention to recommend the service was 98.0%, all high scores. Appropriate nursing manpower, derived from the 2007 pilot project, indicated 2.3 patients per nurse and 4.0 patients per nurse aid. The 2010 pilot project results indicated that the patients and families' satisfaction was high at 8.0-9.1 points (on a 10-point scale), and that the intention to reuse and recommend the service was also high. Compared with the 2007 pilot project, however, the types of medical institutions and the nurse to patient ratios were diverse, offering limitations. In conclusion, to systemize hospitals without patient-sitter, it is necessary to develop policies designed to establish criteria for the appropriate nurse to patient ratio and skill-mix, to standardize the work, to prepare finances for securing nursing staff, to evaluate the nursing demands, and to monitor the quality management.

Differences on Satisfaction of Healthcare Applications by Smartphone Users' Characteristics (스마트폰 사용자 특성에 따른 헬스케어 애플리케이션 만족도 차이)

  • This study examined the satisfaction difference in the aspects of awareness, information quality, and availability in healthcare applications according to smartphone user's characteristics. Data collection was conducted between September 17th and September 21st 2014 on the 193 people who had used healthcare applications, and the data collected were analyzed with descriptive statistics, t-test, One-way ANOVA, and Kruskal-Wallis test using SPSS WIN 20.0. Differences in satisfaction of healthcare application were observed according to the general characteristics, and according to age, education level, and income, as well as for healthcare application use characteristics, according to operation system, total use period, the number of installed healthcare applications, the sources of main information, use frequency, and reasons of no use. To establish strategies for the effective spread of mobile healthcare and development and distribution of healthcare applications, it will be necessary to confirm smartphone users' characteristics and it is important to reflect them in the strategies. In addition, it is necessary to develop and implement a range of strategies to ceaselessly induce the users' motives of use and to improve their degree of satisfaction to secure the constant use of developed health care applications.

A Study of Trends in Continuing Education Published in the Korean Nurse (간호협회지를 통해 본 보수교육의 역사적 경향 1962년 ~ 1995년)

  • This historical research was conducted to analyze and categorize the titles which were presented in the journal, The Korean Nurse, from August 1962 to October 1995. Titles which were published with the purpose of educating graduate nurses and to update 0 their nursing knowledge to improve professional practice were included. There were 348 articles published from the beginning of publication in August, 1962 to October, 1995. All of the journals were reviewed except nine missing journals which were not available in any library. According to the characteristics of the articles in the periodical, the articles were divided into three periods. In each of the three peroids there were five categories: Subject, Clinical Practice, Fundamentals of Nursing Science, Nursing Administration, Others. These categories were adopted from Kim's(1994) division system which was developed to analyze nurse's insurance education program. The special feature peroid was from August, 1962 to December, 1974. In this period the articles were presented in an unorganized manner in the area of special feature or main issue. The largest area was the subject category(44%). The second largest area wes the fundamental of nursing science category(31%). From May, 1975 to December, 1977, the articles with the educational purposes were published in a designated area called continuing education. This period was labelled as the continuing education period. Among the published articles in this period, 45% focused on the subject category and 45% on the fundamentals of nursing science category. In this period the articles were focusing on nurses 'work in specific areas such as industry, nurses' aid schools, and nursing administration, articles on physical assessment first started to appear. The written continuing education period was from January 1978 to October, 1995. All the educational articles published in this area were analyzed and categorized into five areas as for the other periods. 48% of the articles focused on the subject category. In the mid-eighties, the term nurse specialist first.appeared and ten years later in 1990, the titles were subdivided into more specific titles, such as, home nursing, industrial nursing, emergency nursing, 23% were in the fundamental nursing science category and they dealt with nursing process, nursing theories, theory development. For the content analysis, three articles, one from each period, dealing with cardiovscular diseases were selected for comparsion. First, the special feature period, the title of the article was on diet therapy for cardiovascular disease patients, but instead the content was about rest, hygiene, medication, observation. They recommended rings to prevent bed sores, which is now considered as a cause for bed sores. In the continuing education period, the content was more focused on rehabilitation rather than general nursing .care. It became more specific, systematic, and organized compared with the special feature period. In the written continuing education period, the focus was on rehabilitation, not broadly, but very specifically on exercise. The further research on the content analysis is recommended along with a comparison of the trends in the Journal of Nurses Academic Society.

Anti-Reflection Coating Technology Based High Refractive Index Lens with Ultra-Violet Rays Blocking Function (반사방지 코팅기술 기반 자외선 차단 기능의 고굴절률 안경렌즈)

  • Ultra-violet rays are very harmful to eye health care. The blocking of ultra-violet rays and a reduction of optical reflection in the visible light range, which is to increase the share of transmitted light, and avoid the formation of ghost images in imaging, are important for the applications of polymer eyeglasses lenses. In this study, the high-refractive index polymer lenses, n=1.67, were fabricated by injection-molded method with the xylene diisocyanate monomer, 2,3-bis-1-propanethiol monomer, and benzotriazol UV absorber (SEESORB 709) mixture. To reduce the reflection of the polymer lens surfaces, multi-layer anti-reflection (AR) coatings were coated for both sides of the polymer lens using an E-beam evaporation system. The optical properties of the UV blocking polymer lens were characterized using a UV-visible spectrometer. The material properties of the thin films, which were composed AR coating layers, refractive index, and surface roughness, were analyzed by ellipsometry and atomic force microscopy. As a result, the fabricated polymer lens perfectly blocked ultra-violet rays below 395 nm with a blocking rate greater than 99%.

Model Verification of a Safe Security Authentication Protocol Applicable to RFID System (RFID 시스템에 적용시 안전한 보안인증 프로토콜의 모델검증)

  • RFID is an automatic identification technology that can control a range of information via IC chips and radio communication. Also known as electronic tags, smart tags or electronic labels, RFID technology enables embedding the overall process from production to sales in an ultra-small IC chip and tracking down such information using radio frequencies. Currently, RFID-based application and development is in progress in such fields as health care, national defense, logistics and security. RFID structure consists of a reader that reads tag information, a tag that provides information and the database that manages data. Yet, the wireless section between the reader and the tag is vulnerable to security issues. To sort out the vulnerability, studies on security protocols have been conducted actively. However, due to difficulties in implementation, most suggestions are concerned with theorem proving, which is prone to vulnerability found by other investigators later on, ending up in many troubles with applicability in practice. To experimentally test the security of the protocol proposed here, the formal verification tool, CasperFDR was used. To sum up, the proposed protocol was found to be secure against diverse attacks. That is, the proposed protocol meets the safety standard against new types of attacks and ensures security when applied to real tags in the future.

EPIDEMIOLOGICAL EVALUATION OF PERIODONTAL STATUS IN KOREAN ADULTS (한국인의 치주조직상태에 관한 역학조사)

  • To evaluate the periodontal status of Korean adults, 3,988 adults(20-69 years old) who lived in Kwangju, Chonnam, Taegu, and Kyungbuk districts were periodontally examined by plaque index(PI), calculus index(CI), gingival index(GI), periodontal disease index (PDI), and papillary bleeding index(PBI). The periodontal examination charts were grouped according to age and sex. And subsequently 200 periodontal examination charts in each age group were randomly selected according to sex. The differences of the indices among age groups and those between male and female in each age group were statistically analyzed by Chi-Square test. In PI, CI, GI, PDI, and PBI, there was no statistical significance in the difference between male and female in each age group(p>0.05). In male, PI of 60s age group($1.44{\pm}0.68$) was significantly higher than that of any age group(p<0.05), and in female PI of 20s age group($0.86{\pm}0.39$) was significantly lower than that of any other age group(p<0.05). CI of 60s age group in both male($1.67{\pm}0.70$) and female($1.63{\pm}0.91$) was significantly higher than that of any age group(p<0.05). GI of 20s age group in both male($0.85{\pm}0.66$) and female($0.67{\pm}0.60$) was significantly lower and GI of 60s age group in both male($1.37{\pm}0.60$) and female($1.44{\pm}0.84$) was higher than that of any age group(p<0.05). PDI in male was the lowest in 20s age group($0.70{\pm}0.73$), and was the highest in 60s age group($4.93{\pm}1.96$) (p<0.05). In female PDI of 20s age group($0.71{\pm}0.74$) was significantly lower than that of any age group(p<0.05). In male PBI of 20s age group($0.38{\pm}0.31$) was significantly lower than that of any age group(p<0.001). PBI in female was the lowest in 20s age group($0.35{\pm}0.30$) (p<0.001), and was the highest in 60s age group($1.09{\pm}0.76$) (p<0.05). The data insist that all the young populations of 20s age group should be interested in routine check for early diagnosis and prevention of periodontal disease, and a special education program of periodontal health care for young generation must be prepared, and the periodic recall check system for periodontal examination of middle age group must be supported by government.

A Geographical Study on the Behavior Changes of Telemedicine Participants in Terms of Time and Space (시공간 관점에서 본 원격진료 이용자 행태 변화에 관한 연구)

  • This research aims to examine the behavior changes of telemedicine participants with regard to time-space reconfiguration and to address the implications of telemedicine in terms of extensibility and restrictions (ambilaterality). According to the results of this research, telemedicine can lead to behavior changes in telemedicine participants, particularly patients. However, it is difficult to anticipate the time-space reconfiguration of telemedicine participants drastically. In other words, although telemedicine minimizes patients' burden of accessibility to and utilization of medical institutions, it requires the patients to visit medical institutions at least once due to the restricted application of telemedicine related to technological problems, the characteristics of medical practice and mutual stakes among the medical institutions involved in telemedicine. And physicians (telemedicine providers as mediators between medical specialists and patients) and medical specialists (as the ultimate telemedicine providers) do not evidence considerable changes in their behaviors, except for offline meetings for information sharing and medical training. Because the present telemedicine system does not require simultaneity between physicians, patients and medical specialists. Furthermore, present telemedicine operation is absorbed into existing medical activities as a health care delivery method. These phenomena are due to 1) the interests among medical institutions and the limitation or generalization of telemedicine technologies to stimulate regional-based telemedicine operation and 2) the goal of face-to-face interactions between patients and doctors, which is to avoid misdiagnosis and side effects. Finally, medical activities related to telemedicine do not differ from general medical activities. The ambilaterality of telemedicine in terms of extensibility and the restriction of time-space reconfiguration is an unsettled problem in the ICT technologies of medical services.
