Background: There were a few studies which were conducted to know about the behavior of the chronic obstructive pulmonary diseases (COPD) patients. The aims of this study was to explore the behaviour of COPD patients, such as awareness and impact of disease, the pathway of visiting doctors, and the treatment pattern and preference. Methods: A face-to-face interview of 300 subjects with COPD was conducted. Results: The most concerned symptom which made the respondents to visit the hospital was 'breathlessness' (78%). Only 58% of them knew the exact diagnosis. Seventy-three percent of them visited the hospital 'once a month' or 'once every 2 month'. They have made 12.8 prescheduled visits to the hospital in the past 1 year. Unscheduled visits and hospital stay figured to two in the past year. Only 11% of respondents felt they were currently in good health. 'Severe' and 'very severe' COPD patients perceived their health to be in a worse condition than 'mild' and 'moderate' COPD patients. When conditions worsened, 42% of patients were hospitalized. The most common prescription treatment was a fixed combination of inhaled corticosteroids and long-acting ${\beta}2$ agonists (48%), followed by a long acting anticholinergics (38%). Conclusion: Over forty percent of the patients didn't know exactly about their condition. Most of them had a negative attitude toward their current health status. Doctors need to know more about COPD patients in terms of their attitude toward the disease, impact of the disease, interaction with healthcare professionals and treatment related problems.
Verruco-papillary lesions (VPLs) of the oral cavity described in the literature involve a spectrum of conditions including squamous papilloma, verruca vulgaris, focal epithelial hyperplasia, condyloma, proliferative verrucous leukoplakia and verrucous carcinoma. A majority of the VPLs are slow growing, benign in nature and have a viral aetiology. Virus associated benign mucosal outgrowths are not too difficult to diagnose either clinically or by microscopy. Apart from virus-associated lesions, VPLs harboring malignant potential or behaviour such as verrucous carcinoma, proliferative verrucous leukoplakia, oral verrucous hyperplasia (OVH), oral papillary squamous cell carcinoma (PSCC) and oral conventional squamous cell carcinoma with papillary features (CSCC) need to be further clarified for better understanding of their predictable biologic behavior and appropriate treatment. Current understanding of potentially malignant VPLs is perplexing and is primarily attributed to the use of confusing and unsatisfactory terminology. In particular, the condition referred to as oral verrucous hyperplasia (OVH) poses a major diagnostic challenge. OVH represents a histopathological entity whose clinical features are not well recognised and is usually clinically indistinguishable from a verrucous carcinoma and a PSCC or a CSCC. A consensus report published by an expert working group from South Asia as an outcome of the 'First Asian Regional Meeting on the Terminology and Criteria for Verruco-papillary Lesions of the Oral Cavity' held in Kuala Lumpur, Malaysia, recognised the clinical description of these OVH as a new entity named 'Exophytic Verrucous Hyperplasia'. Previously described clinical features of OVH such as the 'blunt' or 'sharp' variants; and the 'mass' or 'plaque' variants can now collectively fall under this newly described entity. This paper discusses in detail the application of the standardized criteria guidelines of 'Exophytic Verrucous Hyperplasia' as published by the expert group which will enable clinicians and pathologists to uniformly interpret their pool of OVH cases and facilitate a better understanding of OVH malignant potential.
화재현장에서 발생하는 연기는 일반적으로 연소가스와 더불어 여러 가지 성분의 유기증기 및 미연탄소로부터 생성된 미세입자로 되어 있다. 연기는 호흡기로 흡입되어 장단기 인체피해를 유발한다. 화재 시 발생하는 유독가스나 입자에 의한 피해를 예측하기 위해서는 연기입자의 호흡기관 내 이송과 침전, 그리고 입자를 매개로 한 유독가스의 흡수와 흡착등의 현상 파악이 필수적이며 이에 연기입자의 크기 및 형상은 이와 같은 현상의 변화에 중요한 변수로서 작용한다. 이 연구에서는 화재 시 발생하는 연기입자의 흡입에 의한 인체피해 특성을 예측하기 위하여 가연물 및 화재 조건에 따른 연기입자의 크기 및 형상에 대한 분석을 수행하였다. 국제표준에 따른 연소생성물 분석을 위하여 ISO/TS 19700 기준에 따라서 등속연소로를 제작하고 각 가연물에 대하여 연소로 온도 및 당량비 조건으로 정해지는 4가지 대표적인 화재조건, 즉, 저온불완전연소, 완전불꽃연소, 불완전불꽃연소, 고온불완전연소 조건에서 연기입자를 발생시켰다. 발생된 연기입자는 다단 충돌집진기를 이용하여 채집한 다음 투과전자현미경으로 크기 및 형상을 분석하였다.
The purpose of this study was to investigate discomforts and sexual life and to identify the relation between the discomforts and sexual life with low back pain. The data were collected from March 2 through July 31, 2001. Four hundred forty-two questionnaires were returned (response rate=88.0%). Analysis of the data was done with SPSS PC+ and use descriptive statistics, $x^2$-test, t-test, ANOVA. regression. The statistics shows that over than 80% of the adults experienced lumbago at least one time in their life, and Back pain is known as one of the most common complaints made by the patients of all ages in the general hospital or local medical clinics throughout. However, in certain case it leads to a chronic condition which can cause a great deal of problems in management and in financial burden to individuals and society. The result of this study was summarized as follows: 1) It appeared that regarding the distribution of gender, male was the higher(63.6%) then that of female, the portion of forties was 28.5%. Sitting for long time was 23.1% in men and 21.7% in women. Unknown reason including sexual behaviour was 12.9% in men and 15.5% in women. Patients treated medicine and physical therapy were 36.4%. In level of educational background, the rate of high school was 31.0%, technical college was 28.5%. The highest proportion by occupation was 18.3% of office workers, occupation posture was 41.9% of sitting. 2) Men(26.0%) and most of women(34.8%) were not satisfied in the explanation satisfaction rate of sex life concerned disease. 23.8% in men and 23.6% in women considered flexibility of waist good. Man(33.3%) and most of woman(35.0%) considered that Health education is necessary. 32.7% in men and 27.3% in women did't mind educator is whoever. Preventing of lower back pain(LBP) and proper Health education of sex life are demanded in daily life. 3) 58.0% of man and 64.0% of woman mostly had a posture which is man over woman. 28.5% in men and 27.8% in women considered that proper information finding of LBP and sex life was very few and few. 37.7% in men and 42.7% in women have acquired information about sex life flung their friends. 4) The number of sex life was decreased from 2.96 0.98 to 2.61 1.63 and also the time of sex life was decreased from 3.65 1.89 to 226 1.64. The satisfaction rate of sex life changed from 3.60 0.86 to 2.77 1.10. In the number of sex life, The non correct group was 2.62 1.91 and the correct group was higher in 2.68 1.65. In the time of sex life, The non correct group was 2.02 1.47 and the correct group was higher in 229 1.65. The satisfaction rate of sex life was 2.76 0.86 in non correct group and 2.88 1.10 in correct group. So there was a difference. 5) In the satisfaction rate of sex life, Men who have a lower back pain were higher than women and no attack group was higher than attack group. As they had many sex life, the satisfaction rate was higher significantly in statistics. As the time of sex life was short, the satisfaction rate was lower significantly in statistics. As the age was low, the demand rate of Health education was high and as means of patient who had a lower back pain was high, the demand rate of Health education was high. As the patient who had a lower back pain had a long married life, the demand rate of Health education was high and as education level was high, the demand rate of Health education was high. It is necessary to provide patients with conservative treatment, educational teaching, and training to prevent further injuries in the future. In general, it is important to educate the public how to prevent back injuries and how to treat themselves in an onset period to prevent further injuries sliding into a chronic state. Sexuality is an integral part of normal and healthy relationships, but patients are unable to enjoy sex because they are riot able to get into a comfortable position due to back pain. Many conditions of the spine can make certain positions uncomfortable. Health educator should make the education program of the discomforts and the sexual pattern for low back pain in workplace and/or hospital. Further study Is needed on how to integrate the educational program on sexuality into the total rehabilitation program.
Full-scale shake table seismic experiments and low-amplitude vibration tests on a masonry building are carried out to assess its seismic performance as well as study the effectiveness of a new multifunctional textile material for retrofitting masonry structures against earthquakes. The un-reinforced and the retrofitted with glass fiber reinforced polymer (GFRP) strips masonry building was subjected to a series of earthquake excitations of increasing magnitude in order to progressively induce various small, moderate and severe levels of damage to the masonry walls. The performance of the original and retrofitted building states is evaluated. Changes in the dynamic characteristics (lowest four modal frequencies and damping ratios) of the building are used to assess and quantify the damage states of the masonry walls. For this, the dynamic modal characteristics of the structure states after each earthquake event were estimated by performing low-amplitude impulse hammer and sine-sweep forced vibration tests. Comparisons between the modal results calculated using traditional accelerometers and those using Fiber Bragg Grating (FBG) sensors embedded in the reinforcing textile were carried on to investigate the reliability and accuracy of FBG sensors in tracking the dynamic behaviour of the building. The retrofitting actions restored the stiffness characteristics of the reinforced masonry structure to the levels of the original undamaged un-reinforced structure. The results show that despite a similar dynamic behavior identified, corresponding to reduction of the modal frequencies, the un-reinforced masonry building was severely damaged, while the reinforced masonry building was able to withstand, without visual damage, the induced strong seismic excitations. The applied GFRP reinforcement architecture for one storey buildings was experimentally proven reliable for the most severe earthquake accelerations. It was easily placed in a short time and it is a cost effective solution (covering only 20% of the external wall surfaces) when compared to the cost for full wall coverage by GFRPs.
Background: Cigarette per day (CPD) use is a key smoking behaviour indicator. It reflects smoking intensity which is directly proportional to the occurrence of tobacco induced cancers. Self reported CPD assessment in surveys may suffer from digit bias and under reporting. Estimates from such surveys could influence the policy decision for tobacco control efforts. In this context, this study aimed at identifying underlying factors of digit bias and its implications for Global Adult Tobacco Surveillance. Materials or Methods: Daily manufactured cigarette users CPD frequencies from Global Adult Tobacco Survey (GATS) - India data were analyzed. Adapted Whipple Index was estimated to assess digit bias and data quality of reported CPD frequency. Digit bias was quantified by considering reporting of '0' or '5' as the terminal digits in the CPD frequency. The factors influencing it were identified by bivariate and logistic regression analysis. Results: The mean and mode of CPD frequency was 6.7 and 10 respectively. Around 14.5%, 15.1% and 15.2% of daily smokers had reported their CPD frequency as 2, 5 and 10 respectively. Modified Whipple index was estimated to be 226.3 indicating poor data quality. Digit bias was observed in 38% of the daily smokers. Heavy smoking, urban residence, North, South, North- East region of India, less than primary, secondary or higher educated and fourth asset index quintile group were significantly associated with digit bias. Discussion: The present study highlighted poor quality of CPD frequency data in the GATS-India survey and need for its improvement. Modeling of digit preference and smoothing of the CPD frequency data is required to improve quality of data. Marketing of 10 cigarette sticks per pack may influence CPD frequency reporting, but this needs further examination. Exploring alternative methods to reduce digit bias in cross sectional surveys should be given priority.
The Ting Kau Bridge in Hong Kong is a cable-stayed bridge comprising two main spans and two side spans. The bridge deck is supported by three towers, an end pier and an abutment. Each of the three towers consists of a single reinforced concrete mast strengthened by transverse cables and struts. The bridge deck is supported by four inclined planes of cables emanating from anchorages at the tower tops. In view of the heavy traffic on the bridge, and threats from typhoons and earthquakes originated in areas nearby, the dynamic behaviour of long-span cable-supported bridges in the region is always an important consideration in their design. Baseline finite element models of various levels of sophistication have been built not only to match the bridge geometry and cable forces specified on the as-constructed drawings but also to be calibrated using the vibration measurement data captured by the Wind and Structural Health Monitoring System. This paper further describes the analysis of axle loading data, as well as the generation of random axle loads and simulation of vibrations of the bridge using the finite element models. Various factors affecting the vehicular loading on the bridge will also be examined.

Breast cancer is the most common cancer among women globally. This study was conducted to compare the awareness of breast cancer and the practice of breast self-examination (BSE), clinical breast examination (CBE) and mammography screening among rural females in Pahang and Perak. A cross-sectional study was carried out in five selected rural districts of Pahang and Perak. Two hundred and fifty households were randomly selected and interviewed face to face using a semi-structured questionnaire. The majority of residents from both states were Malay, aged between 50 and 60 years and had a secondary level of education. Malay women aged 40-49 years and women with a higher level of education were significantly more aware of breast cancer (p<0.05). About half of these women practiced BSE (60.7%) and CBE (56.1%), and 7% had underwent mammography screening. The results of this study suggest that women in Pahang and Perak have good awareness of breast cancer and that more than half practice BSE and CBE. The women's level of education appears to contribute to their level of knowledge and health behaviour. However, more effort is needed to encourage all women in rural areas to acquire further knowledge on breast cancer.

The objective of this study is to investigate the contamination levels and dispersion patterns of arsenic and heavy metals and to estimate the bioaccessible fraction of the metals in soil and plant samples in the vicinity of the abandoned Songcheon Au-Ag mine. Tailings, soils, plants (Chinese cabbage, red pepper, soybean, radish, sesame leaves, green onion, lettuce, potato leaves, angelica and groundsel) and waters were collected around the mine site. After appropriate preparation, all samples were analyzed for As, Cd, Cu, Pb and Zn by ICP-AES and ICP-MS. Elevated levels of As and heavy metals were found in tailings. Mean concentrations of As in agricultural soils were higher than the permissible level. Especially, maximum level of As in farmland soil was 513 mg/kg. The highest concentrations of As and Zn were found in Chinese cabbage (6.7 mg/kg and 359 mg/kg, respectively). Concentrations of As, Cd, and Zn in most stream waters which are used for drinking water around this mine area were higher than the permissible levels regulated in Korea. Maximum levels of As, Cd and Zn in stream waters were 0.78 mg/L, 0.19 mg/L and 5.4 mg/L, respectively. These results indicate that mine tailings can be the main contamination sources of As and heavy metals in the soil-water system in the mine area. The average of estimated bioaccessible fraction of As in farmland soils were $3.7\%$ (in simulated stomach) and $10.8\%$ (in simulated small intestine). The highest value of bioaccessible fraction of metal in farmland soils was $46.5\%$ for Cd.
While extensive efforts have been made in the past to develop finite element models (FEMs) for concrete-filled steel tubular columns (CFSTCs), these models may not be suitable to be used in some cases, especially in view of the utilisation of high strength steel and high strength concrete. A method is presented herein to predict the complete stress-strain curve of concrete subjected to tri-axial compressive stresses caused by axial load coupled with lateral pressure due to the confinement action in square and rectangular CFSTCs with normal and high strength materials. To evaluate the lateral pressure exerted on the concrete in square and rectangular shaped columns, an accurately developed FEM which incorporates the effects of initial local imperfections and residual stresses using the commercial program ABAQUS is adopted. Subsequently, an extensive parametric study is conducted herein to propose an empirical equation for the maximum average lateral pressure, which depends on the material and geometric properties of the columns. The analysis parameters include the concrete compressive strength ($f^{\prime}_c=20-110N/mm^2$), steel yield strength ($f_y=220-850N/mm^2$), width-to-thickness (B/t) ratios in the range of 15-52, as well as the length-to-width (L/B) ratios in the range of 2-4. The predictions of the behaviour, ultimate axial strengths, and failure modes are compared with the available experimental results to verify the accuracy of the models developed. Furthermore, a design model is proposed for short square and rectangular CFSTCs. Additionally, comparisons with the prediction of axial load capacity by using the proposed design model, Australian Standard and Eurocode 4 code provisions for box composite columns are carried out.

제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
