The Tectonic Characteristics of ′KAN′ in the head residence of Uisong Kims (의성 김씨종택에 있어서 간의 건축적 특성)

  • The purpose of this study is to explain the tectonic characteristics of 'Kan' which is an unit of composition in the head residence of Uisong Kims. Here, the 'Kan' is assumed as a conceptual framework to organize a house and also a maneuvering tool to control the spatial order with relationship between the whole and the part. Not limited to the simple conceptual aspects, but the interrelated elements such as the site conditions, modules, functions, structure and aesthetic factors. Through the process of the study, the role of 'Kan' is proved to be a tectonic tool for self-regulating architectural order by relation matrix of elements. 'Kan' is not only a fixed measuring tool for functions and material of a house but also a flexible device to control the building composition. 'Kan' is certainly a relative subject for measurement of form, space and aesthetic quality.

A Correlation Study on the Relationship between Nursing Performance and Leadership Style as perceived by Head Nurses and Staff Nurses (수간호사의 리더쉽에 대한 수간호사와 일반간호사의 지각정도와 간호업무성과간의 관계 연구)

  • The purpose of this study was to identify relationships between the nursing performance of staff nurses and the leadership styles of head as perceived by the head nurses and the staff nurses. The subject consisted of 33 head nurses and 200 staff nurses who were working at SNUH between July 1, and July 31, 1991. Leadership styles as perceived by head nurses and staff nurses were measured using an instrument adapted by Park (1988) . Evaluations of the nursing performance of staff nurses were done by head nurses utilizing a questionnaire adopted by Park(1988). Data were analysed using percentage distribution, ANOVA, Scheffe test and Pearson Correlation Coefficient. The results are summarized as follows : 1. The majority of the head nurses and staff nurses perceived the head nurse as having a tendency to exercise high level authority and benevolence oriented leadership. 2. The majority of the staff nurses carried out their nursing performance in a highly skilled manner. 3. There were significant differences in perceptions of leadership style between head nurses and staff nurses. 4. “Authority” and “Benevolence” in leadership style as perceived by the head nurses were related very weakly to the nursing performance of the staff nurse. 5. “Benevolence” of leadership style as perceived by the staff nurses was related very weakly to the nursing performance of the staff nurse. 6. Group comparisons of nursing performance and leadership style found that high nursing performance by the staff nurse was related in the group with high “Authority” as perceived by the head nurse and low “Authority” as perceived by the staff nurse. From the above findings, this study suggests the following : 1) Further studies are needed to control extraneous variables. 2) Efforts in leadership development are needed to enhance nursing performance. 3) Factor exploration study are needed to seek factors that contribute to the perception gap between the head nurse and the staff nurse.

Differentiation of Malignant and Benign Cervical Lymph Nodes with Color and Pulsed Doppler Ultrasonography (색채 및 펄스 도플러 초음파를 이용한 경부 림프절의 양성과 악성의 감별)

  • Objectives: The clinical efficacy of the color and pulsed Doppler ultrasound with spectral waveform analysis for differentiation of malignant from benign cervical lymphadenopathy was prospectively evaluated in cervical lymphadenopathy. Materials and Methods: Color and pulsed Doppler ultrasound examination was prospectively performed in 32 cervical lymph nodes in 28 patients. These 10 nodes from 10 patients were malignant and 22 nodes from 18 patients were benign, proved by operation, biopsy, and follow-up examination. Another 12 lymph nodes from 12 normal volunteers were evaluated as control group. The peak systolic velocity (PSV), minimal diastolic velocity (MDV) , and resistive indexes (RI) of arterial flows within the 32 lymph nodes were assessed to differentiate the malignant from benign nodes with pulsed Doppler ultrasonography. The results were qualitified with one-way ANOVA and Bonferroni method of multiple comparison. Results: The mean values of PSV of malignant, benign, and control nodes were 38.2(10.1-134)cm/sec, 23.3(9-38.5) cm/sec and 11.8(6.7-18.1) cm/sec, respectively. The mean values of MDV of them were 0.9(-7.5-10.7)cm/sec, 9.7(2.9-18.6)cm/sec and 6.5(3.7-9.3)cm/sec, respectively. However, there was no statistical significance in differentiation of malignant from benign nodes with PSV and MDV. The mean values of RI of malignant, benign, and control nodes were 0.99(0.80-1.30), 0.59(0.46-0.77) and 0.45(0.38-0.50), respectively. RI value of 0.8 is suggestive value for discrimination of malignant from benign lymphadenopathy during examination of color Doppler ultrasound of cervical lymphadenopathy. Conclusion: Color and pulsed Doppler ultrasound examination with spectral waveform analysis may be quite helpful in the differentiation between benign and malignant alterations of cervical lymph nodes.

  • The purpose of this study was to investigate the effect of wearable sensor wear on the muscle contraction of cervical erector spinae and upper trapezius causing the forward head posture induction in order to reduce the stress induced by the use of smartphone. This study was to investigate the muscle activity of healthy adults in the 20th to 30th generations by dividing them into the control group using the smartphone, the non-wearing group conscious the posture of the head posture, and the wearing group wearing the wearable sensor. There were no differences in muscle activity between cervical erector spinae and upper trapezius compared to the control, non - wearing, and wearing groups. In addition, the changes in muscle activity of cervical erector spinae muscles were increased in all groups, but the muscle activity of upper trapezius muscles were in the wear group compared to the non-wear group and the control group, but there was no statistical significance. That is, wear of the wearable sensor may be effective in controlling the conscious posture, but it may cause the compensation of another part.

Effect of Low-level Laser Therapy on Propylthiouracil-induced Hypothyroidism Model Mice: A Pilot Study

  • Background and Objectives Hypothyroidism is the most common endocrine disease. On the other hand, there is no treatment that can improve the thyroid function. Low-level laser therapy (LLLT) can improve the cellular activity. The effect of hypothyroidism is not obvious. This study examined the effects of LLLT on the thyroid gland function of a propylthiouracil (PTU)-induced hypothyroidism mouse model. Materials and Methods Twenty-five male ICR mice were distributed into five groups of five animals each: Negative control (none PTU animal) and positive control (PTU animal) of unirradiated animals, and three experimental groups with LLLT (3J, 6J, and 12J). Each mouse was exposed to a distinct dose of a 632-nm laser once a week for three rounds. The positive control group and three LLLT groups were induced into a hypothyroidism state by PTU administration. The animals' thyroid-stimulating hormone and thyroxine levels were measured using an ELISA kit, and their thyroid tissue was harvested and analyzed after sacrifice at the end of the experiment. The hormone level and morphological changes in the tissue of the five groups were compared. Results The thyroid hormone levels in the control group and LLLT groups were similar. On the other hand, the thyroid tissue of the LLLT groups showed some morphological changes that were similar to those of iodine deficiency thyroid. Conclusion LLLT did not affect the thyroid gland function in PTU-induced hypothyroidism mice.

The Cooling System for Head up Display (Head up Display용 냉각시스템)

  • Head up display’s cooling system is auto-diagnosed resulting from the external environment. The quantity of heat depending on this Head up display’s cooling system layout determines the speed of FAN for system cooling. In other words, a system’s heat quantity is planned through the air density depending on altitude, the amount of wind in air depending on FAN control condition, and the algorithm that is proportional to delta temperature. To detect the altitude, we use the criteria of delta T, which is determined by the subtracted value of LED junction temperature, and atmospheric temperature that is recorded on the Head up display system. Depending on the classification of delta T value, the altitude section is determined. While we can use GPS as the tool to detect the altitude, we should predict the change of the air density as the altitude alters, and should not just measure the altitude. And the value of delta T is used as the criterion of detecting the altitude for increasing the cooling efficiency of the car’s inner Head up display system with reflecting the speed of the FAN dependent upon the air density. In our theory, altitude is depending on the value of delta T and stabilizing or maintaining the system’s temperature by changing FAN’s rpm depending on determined value of altitude.

  • There are three sub-processes associated with the assembly of the valve seat and cylinder head; heat fitting, cold fitting, and shrink fitting. In the heat fitting stage, the cylinder head is heated to a specified temperature and then squeezed toward the outer diameter of the valve seat. The cold fitting process cools the valve seat and safely squeezes it toward the inner diameter of cylinder head. However, these methods increased the installations & running cost and curtailed productivity. To address these problems, we analyzed the shrink fitting process using the contact pressure caused by fitting interference between the outer diameter of the valve seat and the inner diameter of the cylinder head. In this study, a closed form equation for predicting the contact pressure and fitting load is proposed. For quality control of the assembly line, principal factors of the shrink fitting process influenced in contact pressure were simulated by the FEM. Actual loads measured in the field showed good agreement with the results obtained by theoretical and finite element analysis.

Preoperative Evaluation and Significance of Carotid Resection in Advanced Cervical Metastatic Cancer (진행된 경부 전이암에서 경동맥 절제를 위한 술전검사와 절제술의 의의)

  • Objectives: We studied what is the role of elective carotid artery resection in the management of advanced cervical metastatic cancer. Materials and Methods: 5 patients with elective carotid artery resection in advanced metastatic cervical cancer were reviewed retrospectively. The patients underwent complete neuroradiologic evaluation, including CT/MRI. angiography, duplex doppler, balloon occlusion test with EEG, and brain SPECT for determination of compatible collateral circulation after carotid artery resection. Results: Perioperative complication were appeared in 2 patients those were middle cerebral arterial infarction and mediastinal bleeding. Postoperative mortality rate was 20%. 4 patients recurred within 1 year. Conclusion: Preoperative collateral study rarely provide whether resection carotid artery or not. Elective carotid artery resection cannot provide locoregional control of tumor and don't promote survival.

Effects of Head and Neck Massage on Anxiety, Pain, and Discomfort in Hysterectomy Patients (두경부 마사지가 자궁적출술 환자의 불안, 통증 및 불편감에 미치는 효과)

  • Purpose: This study determined the effect of head and neck massage on anxiety, pain, and discomfort for hysterectomy patients. Methods: Subjects consisted of 48 hysterectomy patients at K hospital. Experimental group (n=23) received head and neck massage at 8 minutes per massage for five times: (at admission, before sleeping on preoperative day, at 4 hours after operation, before sleeping on operation day, before sleeping on the first postoperative day). Control group (n=25) received conventional treatment. As an effectiveness of this intervention, state of anxiety and BP was measured before sleeping on preoperative day. Pain and discomfort were measured before sleeping on the day of surgery and then on the first postoperative day. Data was analyzed descriptive statistics using $x^2$ test, Fisher's exact test, paired t-test, unpaired t-test, repeated measures ANOVA and Bonferroni multiple comparison. Results: 1. State of anxiety score and BP at post-treatment decreased significantly in the experimental group. 2. Pain score in experimental group was lower than that in control group, with no significant difference; whereas, 3. Discomfort level decreased significantly in the experimental group. Conclusion: Results indicate that head and neck massage could be an effective intervention for reducing preoperative anxiety and postoperative discomfort in hysterectomy patients.

Simultaneous Neck Dissection in Transoral $CO_2$ Laser Surgery for Supraglottic Cancer (성문상부암에서의 경구강 $CO_2$ 레이저 수술과 동시경부청소술)

  • Objectives : In $CO_2$ laser surgery for supraglottic cancer, neck dissection is generally done in second stage. We investigated simultaneous neck dissection with primary resection could be available in laser supraglottic surgery. Material and Methods : We analyzed 13 patients with supraglottic cancer who were treated with transoral supraglottic laryngectomy and simultaneous neck dissection from 2001 to 2007. Tumor stage, extent of laser surgery, histological results, survival rate, local control rate, complications, and functional results were reviewed. Results : 5-year local control rate, survival rate and disease specific survival rate from the neck was 100%, 69.9%, 100% respectively. Tracheotomy was done in all 13 cases. One patient had a long tracheotomy indwelling (191 days). In the rest of 12 patients average decanulation time was 7.4 days(1-22 days). Nasogastric tube was inserted in 5 cases, and average oral intake was possible in 3.5 days(1-16 days). Average hospital days was 29.7 days. There was no serious complication associated with neck dissection. Conclusion : Simultaneous neck dissection with primary laser resection for supraglottic cancer is oncologic sound and can be performed without significant surgical morbidity.
