Numerical Analysis on Performance of Dental Air Turbine Handpiece for Various Reflection Angles of Turbine Rotor (터빈 로터 반사각에 따른 의료용 에어터빈 핸드피스의 성능에 관한 수치 해석)

  • Dental high-speed air turbine handpieces have been used as a dental cutting tool in clinical dentistry for over 50 years, yet little study has been reported on their flow and performance analysis. Therefore it is necessary to investigate turbine for the performance improvements of an air turbine handpiece. This paper presents pressure on turbine rotor and flow analysis in air turbine handpiece using CFD (computational fluid dynamics). Characteristics on each flow and pressure for four various reflection angles of turbine rotor are presented, and then performance change is analyzed about air turbine handpieces by CFD results.

  • The design process of the dental handpiece is described. The parameters of the high speed air turbine are estimated. The effect of supply hole on the stiffuess and damping of the air bearing for handpiece is studied numerically. The Reynolds equation is solved by using the divergence formulation and the perturbation method. The test rig is built and the test procedure is developed for the turbine rotational speed measurement by using Fourier transform of noise generated by the turbine during steady operation.

  • Recently, research on bone fracture and osteoplasty using ultrasonic bone surgery unit has been ongoing in the fields of dentistry, plastic surgery, and otorhinolaryngology. However, detailed data evaluation with ultrasonic bone surgery unit has not been conducted to date. In this study, we developed handpiece moving system (HMS) for cutting performance evaluation. In the experimental setup of HMS, a handpiece was immobilized, and bone samples from cortical bone of bovine leg were prepared. Also, the experimental process was described in detail, and a basic experiment was carried out to evaluate the cutting performance. Future study is required on all experimental process conditions by HMS.

DISTRIBUTION OF AIRBORNE BACTERIA BY HANDPIECE AEROSOL CONDITIO (핸드피스 분무조건에 따른 부유세균 기균(氣菌) 의 분포)

  • In recent years, cross-contamination has become one of the noticeable issues in dental clinic. Two major routes of contamination are the direct-contamination through blood and oral secretion and the indirect-contamination through dental office equipments. Especially, air-contamination through air-floating pollutant in a confined space like hospital, and also contamination through aerosol ejected from high-speed handpiece in a dental office was interested. The purpose of this study was to understand risk of bacterial infection through aerosol from handpiece in a dental office, which will help the practitioner with prevention of contamination during dental treatment. The main findings are as follows. 1. In a comparative test, the group using handpiece has higher bacterial number than the group not using handpiece with significant statistical difference(P<0.01). 2. The group using handpiece with rubber dam has lower bacterial number than the group using handpiecewithout rubber dam with significant statistical difference(P<0.01). 3. Comparing the group using drainage water with the group using distilled water as a handpiece water source results in 22.4 cfu and 17.0 cfu respectively but the difference is no statistically significant(P>0.05). 4. Measuring cfu at 0.5m and 1.5m distance, 0.5m distance showed higher bacterial number with statistical significance(P<0.01). 5. Classification of bacterial types showed the largest bacterial number came from gram-positive micrococcus(73.9%), and gram-negative micrococcus, gram-negative bacillus, and gram-positive bacillus follow in descending order.

  • High-speed air turbine handpieces have been used as a dental cutting tool in clinical dentistry for over 50 years, but little study has been reported on their performance analysis. Therefore, the effect of turbine blade shape on performance characteristics of dental air turbine handpiece were studied using CFD in this paper. Computations have been performed for five different positions of turbine blade by using frozen rotor method that is one of steady-state method. The characteristics of turbine blade for shapes and reflection angles were analyzed. As a result of the computation, torque is increased by increasing the reflection angle of turbine blade.

  • Subcutaneous emphysema and pneumomediastium is a relatively uncommon phenomenon. Most case of pneumomediastium are caused by iatrogenic injury on the cervical region and chest during tracheostomy. It is also well known that emphysema may occur secondary to dental treatment using high-speed air turbine handpiece, but there have been few cases of emphysema extended to involving the mediastinum. These complications are reported to occur mainly in patients after dental procedures, in particular during mandibular third molar extraction. Early recognition and conservative treatment of these problems is essential in preventing life-threatening complications such as airway obstruction, mediastinitis, pneumothorax and cardiac failure. As we report a case of 25-year-old woman with subcutaneous emphysema and pneumomediastium after mandibular third molar extraction using high-speed air turbine handpiece.

  • In this paper, the effect of turbine shape on the flow characteristics and performance of dental air turbine handpiece were studied using CFD. Computations have been performed for many different cases of the angle between the center-line of nozzle and turbine rotor by using frozen rotor method that one of steady-state method. The characteristics of turbine rotor for three different types (flat, concave, gull) were analyzed. Additionally, the turbine rotor of gull type that has better performance than others was computed for other reflected angles.

Extensive Bilateral Subcutaneous Emphysema after Dental Treatment: Two Case Reports

  • We report two rare cases of extensive bilateral subcutaneous emphysema that occurred during ordinary dental procedures. An air-driven high-speed handpiece, routinely used in dental procedures may cause subcutaneous emphysema when high pressure air is introduced into the loose connective tissues below the dermal layer. The first case occurred with surgical extraction of the lower third molar. The air introduced into the fascial spaces near the surgical field spread to the contralateral spaces, as well as the neck and chest areas. The second case also showed extensive bilateral subcutaneous emphysema caused by the introduction of compressed air from the handpiece during crown preparation without any invasive procedure. Cases where the emphysema extends beyond the treatment site to involve the contralateral cervicofacial areas have been rarely reported. Predicting the occurrence of subcutaneous emphysema is difficult, so it is important to exercise caution during routine dental treatment. If significant bilateral cervicofacial swelling is suspected to be due to subcutaneous emphysema, prompt diagnosis with securing the patient's airway will be necessary.

A STUDY OF THE CRAZING OF TOOTH STRUCTURE ASSOCIATED WITH PLACEMENT OF PINS (Pin장착시(裝着時)에 치질귀열(齒質龜裂)에 미치는 영향(影響)에 관(關)한 연구(硏究))

  • The purpose of this study was to determine the extent of tooth structure crazing when pinhole were drilled in dentin at various distances from the dentionenamel junction, and when pins were inserted by hand wrench and Auto-klutch handpiece. Sixty freshly extracted teeth were embedded in blocks of cold-curing acrylic resin to a level 1mm apical to the cementoenamel junction, the crown of the teeth were removed to a level 1.5mm coronal to the cementoenamel junction. After being sectioned, the cut surface of the teeth were smoothed, and they were divided into 4 groups. Group I : 4 pinholes were drilled with self-threading Minim spiral drill and pins were inserted with hand wrench in fifteen teeth. Group II : 4 pinholes were drilled with self-threading Minim spiral drill and pins were inserted with Auto-klutch handpiece in fifteen teeth. Group III : 4 pinholes were drilled with self-threading Regular spiral drill and pins were inserted with hand wrench in fifteen teeth. Group IV : 4 pinholes were drilled with self-threading Regular spiral drill and pins were inserted with Auto-klutch handpiece in fifteen teeth. The cut surface were then examined microscopically for the presence or absence of craze line. The results were as follows: 1. The self-threading Minim pin groups (Group I & II) produced less craze lines than did the self-threading Regular pin groups (Group III & IV). 2. In the self-threading Minim pin groups, there was more craze lines with hand wrench (Group I) than with Auto-klutch handpiece (Group 11). 3. In the self-threading Regular pin groups, there was more craze lines with hand wrench (Group III) than with Auto-klutch handpiece (Group IV). 4. Placement of the pinhole at 0.0mm from the dentionenamel junction produced more craze lines than placed at 0.5mm, 1.0mm (p<0.05), however there was no statistically significant between the 0.5mm and 1.0mm distance.
