부인과 질환으로 유발된 만성 골반통 환자에 대한 한방치료 치험 3례 (Chronic Pelvic Pain Patients with Gynecological Diseases Treated by Korean Medicine Treatment: Three Case Reports)

  • Objectives: The purpose of this study is to report the effect of Korean medicine treatment on chronic pelvic pain with gynecological diseases. Methods: Three female patients with gynecological diseases who suffered from chronic pelvic pain got Korean medicine treatment such as Boheo-tang-gagambang, acupuncture, Jungsongouhyul pharmacopuncture, moxibustion, fumigation therapy with Hyeonhosaek-tang and Sasangja-tang and physiotherapy during hospitalization. Numerical Rating Scale(NRS) of pains and percentage of other symptoms were measured every morning. Results: In three cases, after 9, 15, and 22 days of treatment in each case, pelvic pain decreased by more than 50% and other symptoms were also improved. Conclusions: These cases showed that Korean medicine treatment of chronic pelvic pain with gynecological diseases was effective for alleviating symptoms.

부인암 환자의 삶의 질 예측요인 (Factors Influencing on Quality of Life in Gynecological Cancer Patients)

  • Purpose: The purpose of this study was to measure the quality of life (QOL) and to identify the factors influencing QOL in gynecological cancer patients. Methods: The subjects of this study were 242 people who were receiving medical therapy or follow-up after surgery from one general hospital in Daegu. Data were collected from August 1, 2010 to January 31, 2011. A questionnaire including questions on QOL, distress score, distress problem, depression, anxiety, insomnia, perceived health status and body image were completed by the subjects. Results: The mean score of QOL was $70.68{\pm}13.40$. Religion, job, presence of spouse, level of education, household income, financial compensation, disease stage and recurrence were the significant factors related to QOL. Distress score, distress problem, depression, anxiety, insomnia, perceived health status and body image were also significant factors influencing QOL. Sixty eight percent of the variance in subjective overall QOL can be explained by body image, distress problem, distress score, anxiety, level of education and perceived health status (Cum $R^2$=0.689, F=76.316, $p$ <.001). Body image was the most important factor related to QOL. Conclusion: An integrative care program which includes general, disease-related and psychosocial characteristics of patients is essential to improve QOL in gynecological cancer patients.

  • Objectives: The purpose of this study is to investigate the relationship between granulocyte/lymphocyte ratio and dysmenorrhea, history of gynecological disease, heart rate varibility(HRV). Methods: From May 1st 2011 to July 10th 2011, a total of 40 female subjects (age 20-39) were recruited. We evaluated the menstruation condition by questionnaires including VAS(Visual Analog Scale), VRS(Verbal Rating Scale) and MVRS (Multidimensional Verbal Rating Scale), also measured subject's WBC differential count and HRV. Results: 1. There was no statistically significant difference in granulocyte/lymphocyte ratio according to menstruation cycle, duration, amount, color, and blood clot. 2. There was no statistically significant difference in VAS, VRS, MVRS mean scores between normal group and abnormal group according to granulocyte/ lymphocyte ratio. 3. The past prevalence of gynecological disease of abnormal group showed significantly higher compared with that of normal group($x^2$=6.578, p=0.010). 4. LF/HF ratio and granulocyte ratio significantly showed positive correlation ($r_s$=0.311 p=0.048) and LF/HF ratio and lymphocyte ratio significantly showed negative correlation($r_s$=-0.319 p=0.045). Conclusion: These findings suggest that the gynecologic diseases are related to disorder of autonomic nervous system, but not dysmenorrhea. And the balance state of sympathetic/parasympathetic nerve infered through granulocyte/lymphocyte ratio and HRV seems to be consistent. However, reconfirmation through further studies is needed.

맥진기를 활용한 부인과 질환 대상 임상연구 고찰 (A Review on Clinical Studies on Gynecological Disease by Using a Pulse Diagnosis Device)

  • Objectives: The purpose of this study was to survey the status of clinical use of a pulse tonometric device (PTD).Methods: We searched domestic and international articles using the PTD from online medical databases including OASIS, RISS, DBpia and PubMed. We selected articles on clinical application of PTD but excluded articles on mechanical design or software programming for developing a new PTD. Finally we found 13 articles and classified the articles according to the disease of clinical study.Results: Out of the 13 articles, 5 were focused on the dysmenorrhea. Three articles were focused on the infertility, two article were focused on the post mastectomy. According to the results, the pulse energy has been widely used in research as the primary outcome.Conclusions: We found out that until now the researches on clinical application of PTD mainly had been performed for producing a variety of PTDs. This results of this study will be used as a useful information during perform a clinical study and clinics. We suggest that the standard operating procedure for PTD will be developed, and researches for development and its application of various new contents will be performed.

Perception of Patients with Cancer towards Support Management Services and Use of Complementary Alternative Medicine - a Single Institution Hospital-Based Study in Saudi Arabia

  • Background: To evaluate the perception of cancer patients toward treatment services and influencing factors and to inquire about the use of complementary alternative medicine (CAM). Materials and Methods: Information was obtained through pre-tested structured questionnaires completed by cancer patients during treatment at King Abdulaziz University Hospital, Jeddah, Saudi Arabia. Results: Of 242 patients, 137 (64.6%) accepted to enter this study. Most were Saudi (n=93, 68%), female (n=80, 58%), educated at university (n=71, 52%), married (n=97, 72%) and with breast cancer (n=36, 26%). One-hundred (73%) patients were satisfied with the services provided; 61% were Saudi. Ninety-four (68%) respondents were satisfied with the explanation of their cancer. Twenty-eight (21.6%) patients received CAM, of them 54.0% received herbal followed by rakia (21.0%), nutritional supplements/vitamins (7.0%) and Zamam water (18.0%), with significant differences among them (p =0.004). Seven (5%) patients believed this therapy could be used alone; 34 (25%) patients believed it could be used with other treatments, regardless of whether they themselves used this therapy. Fifty-three (53%) satisfied patients felt they received enough support; 31 (58%) patients received support from family and friends; 22 (41.6%) patients received support from the health-care team. Patients who received information about their disease from their physicians and those who felt they had enough support were more satisfied. The patients who took alternative treatment were older age, mostly female and highly educated but values did not reach significance. Conclusions: We stress enhancing the educational and supportive aspects of cancer-patient services to improve their treatment satisfaction and emphasize the need for increasing the educational and awareness programs offered to these patients.

Ovarian Transposition for Stage Ib Squamous Cell Cervical Cancer - Lack of Effects on Survival Rates?

  • Background: To investigate the impact of ovarian transposition (OT) on survival rates of the patients with stage Ib squamous cell cervical cancer. Materials and Methods: Ninety-two subjects who underwent a radical hysterectomy including oophorectomy were evaluated. For nineteen (20.7%), OT was performed. Patients were divided into two groups, OT versus oophorectomy alone. The primary end-point of this study was to investigate the impact of OT on tumor recurrence rate and time, 5-year disease-free survival (DFS) and overall survival (OS). These comparisons were performed for subgroups including patients who received radiotherapy versus who did not. Statistical analyses were conducted using the Chi-square test, T-test and Mann-Whitney test. OS was examined using the Kaplan-Meier method. $P{\leq}0.05$ was considered to be statistically significant. Results: The median follow-up period was 89 months for OT and 81 months for the oophorectomy group (p>0.05). Both groups experienced similar recurrence rates (31.6% vs. 26.4%, p=0.181). The median duration from surgery to recurrence, and surgery to death were also similar between the groups (p>0.05). The 5-year DFS and OS rates were both 68.4% for the OT group, and 73.6% and 77.8% for the oophorectomy group (p=0.457 and p=0.307, respectively). While the 5-year DFS rate was not statistically significant between the OT and oophorectomy groups who did not receive radiotherapy (p=0.148), the 5-year OS rate was significantly higher in the oophorectomy group (95.4% vs 66.7%, respectively) without radiotherapy (p=0.05). The 5-year DFS and OS rates were statistically similar between the groups who received adjuvant radiotherapy (p>0.05). Conclusions: Ovarian transposition has not significantly negative effect on the survival rates when adjuvant radiotherapy will be applied, while 5-year OS may be less in OT group if radiotherapy is not mandatory.

  • Objectives: The purpose of this review is to analyze the domestic study trends of clinical use of Digital Infrared Thermal Imaging (DITI) as a diagnosis instrument on obstetrics and gynecological diseases in Korean literature. Methods: We searched for clinical studies using DITI in the journal of Korean obstetrics & gynecology, National Digital Science Library, Research Information Sharing Service, Korean Medical Database up to May 2020. After searching studies, we selected studies and analyzed according to disease. Results: 18 clinical studies were published in two kinds of academic journals. There were 4 studies about dysmenorrhea and menopausal symptoms, 2 studies about postpartum disease and hypercryalgesia, 1 study about amenorrhea, premenstrual syndrome, leukorrhea, infertility, polycystic ovary syndrome and hot flush. Conception vessel was frequently used for DITI measurement, followed by stomach meridian. The temperatures of abdomen (CV4, CV12), face (HN3), upper and lower limb (PC8, LU4, LR3, ST32) were commonly measured. Conclusions: This study shows that DITI could be one of effective diagnosis instrument for obstetrics and gynecological diseases. More well-designed clinical studies using DITI will be needed.

Functional Status in Turkish Women with Gynecological Cancer

  • Background: Functional status is the ability to perform daily activities. Little is known about quality of life and health status of gynaecological cancer patients. The present study therefore aimed to evaluate the functional status of women receiving treatment for gynecological oncological disease while not hospitalised. Materials and Methods: This descriptive study covered 42 patients monitored by the Gynecological Oncology Unit in 2011. Data were collected using the Functional Living Index-Cancer and analysed with the chi square test, independent samples t-test, Mann-Whitney U test, one-way ANOVA test and the Kruskal-Wallis H test. Results: Of the 42 cases, 66.7% had been diagnosed within the previous year and 90.5% were undergoing chemotherapy. The most severe symptoms experienced were pain (35.7%), fatigue-weakness (40.5%) and nausea and vomiting (56.5%). Daily activities where the most difficulty was experienced were housework (28.6%), average pace walking (31.0%), carrying more than 5 kg (28.6%). The mean Functional Living Index score was quite high ($103.5{\pm}24.1$). FLIC-C scale scores did not vary with the educational status, diagnosis duration, and family history of cancer (p>0.05). Conclusions: Evaluation of the functional status of gynecological cancer patients and how they cope with problems should indicate to healthcare professionals what help can be given to maintain quality of life.

Quantitative Analysis of Lysophosphatidic Acid in Human Plasma by Tandem Mass Spectrometry

  • Analysis of lysophosphatidic acids (LPAs) is of clinical importance as they can serve a potential marker for ovarian and other gynecological cancers and obesity. It is critically important to develop a highly sensitive and specific method for the early detection of gynecological cancers to improve the overall outcome of this disease. We have established a novel quantification method of LPAs in human plasma by negative ionization tandem mass spectrometry (MS-MS) using multiple reaction monitoring (MRM) mode without the conventional TLC step. Protein-bound lipids, LPAs in plasma were extracted with methanol : chloroform (2:1) containing LPA C14:0 as an internal standard under acidic condition. Following back extraction with chloroform and water, the centrifuged lower phase was evaporated and reconstituted in methanol. The reconstituted solution was directly injected into electrospray source of MS/MS. For MRM mode, Q1 ions selected were m/z 409, 433, 435, 437 and 457 which corresponds to molecular mass [M-H]- of C16:0, C18:2, C18:1, C18:0 and C20:4 LPA, respectively. Q2 ions selected for MRM were m/z 79, phosphoryl product. Using MS/MS with MRM mode, all the species of LPAs were completely separated from plasma matrix without severe interferences. This method allowed simultaneous detection and quantification of different species of LPAs in a plasma over a linear dynamic range of 0.01-25 ㎛olL-1 . The detection limit of the method was 0.3 pmol/mL, with a correlation coefficient of 0.9983 in most LPAs analyzed. When applied to the plasmas of normal and gynecological cancer patients, this new method differentiated two different groups by way of total LPA level.

한방여성의학에서 추나요법을 활용한 국내 연구 동향 분석 (A Review of the Domestic Study Trends on Obstetrics & Gynecological Diseases with Chuna Manual Therapy)

  • Objectives: The aim of this study was to analyse the domestic trends of Chuna manual therapy on Obstetrics and Gynecological Diseases in Korean literature. Methods: We searched for papers which had Chuna manual therapy through 2 related journals and 5 Korean web databases. All relevant papers were selected and extracted to be analyzed according to their study designs, academic journals, and target disease. Results: Nine papers belonged to four types of study designs, systematic review, randomized or non-randomized controlled trial and case report. And they were published in four kinds of academic journals. There were four papers about postpartum pain, two low back pain during pregnancy, and a stress urinary incontinence. The exact Chuna techniques were explained in only seven papers. All of papers have been shown that Chuna manual therapy is useful for obstetrics and gynecological diseases. Conclusions: It has been identified that Chuna manual therapy could be a good treatment for obstetrics and gynecological diseases. However, more clinical and well-designed studies with Chuna manual therapy will be needed.
