정신지체장애환자에서 Computer-Guided Surgery를 이용한 임플란트 식립 및 즉시 보철물 장착: 증례보고 (Immediately loaded dental implants with fixed prostheses using a computer-guided surgery in a mental retardation patient: a case report)

  • 통상적인 임플란트 치료는 식립 후 약 3 - 5개월 뒤 임플란트 상부 보철물을 제작한다. 이러한 치료과정의 단점으로는 환자가 임플란트 식립 후 보철물이 장착되는 최종 치료 시점까지의 기간 동안 유지력과 저작능력이 떨어지는 임시의 치를 사용하여 생활해야 한다는 것이다. 최근의 임플란트 식립 후 즉시 부하 보철물 치료는 시술 적응증에 따른 제한이 있긴 하지만, 잦은 치과 방문이 어렵고 진료에 대한 협조도가 낮은 정신적, 신체적 장애 환자들에게 더욱 유용하다고 생각된다. 본 증례에서는 삼차원 컴퓨터 유도 수술법을 이용한 임플란트 식립 후 즉시 부하 보철치료를 정신지체장애 환자에서 시행하였으며, 시술 후 좋은 경과를 보였기에 이를 보고하는 바이다.

  • Purpose: Heterotopic calcification is the abnormal deposition of calcium salts in tissues other than bone and enamel, and it occurs in the form of dystrophic calcification or metastatic calcification. This deposition can occur under many conditions, but in some rare cases, it may develop in burns and nonhealed scars. It is difficult to treat the combination of heterotopic calcification and ulceration in scar tissues by using conservative therapy and to determine the margin of excision in such cases. Our study proposes the use of intraoperative C-arm-guided mapping of lesions with heterotopic calcification, and adequate excision of ulcers in chronic scars where heterotopic calcification is also observed. Methods: This study included 2 patients and was conducted from January 2010 to July 2010. The first patient was a 63-year-old woman who presented with atypical calcium deposits and chronic ulceration in the lower one-third region of the right leg. The second patient was a 38-year-old man who presented with a nonhealing ulcer that had developed on the right leg 3 months earlier he had a history of 40% scalding burns on the entire body. Surgery is the most reliable method for treating heterotopic calcification therefore, both patients were treated using intraoperative C-arm-guided marginal mapping of heterotopic calcification, followed by release of contracture, and eventually split-thickness skin grafting. Results: Plain radiographs of the leg showed spotty radiopaque areas in the hard part of the scar well superficial to the underlying bones. Histopathological analysis revealed multiple foci of calcified deposits, increased fibrosis, and inflammation in the scar tissue. Surgery-related complications were not observed. Conclusion: C-arm guided excision of calcified scars and the release of contracture can cure nonhealing ulcers and may therefore prevent recalcification.

  • The authors describe the experience with the interactive image-guided Zeiss SMN system, which has been applied to 20 patients with various intracranial lesions during one year. Preoperative radiologic evaluation was CT scan in 6 cases, MRI in 14 cases. In all except one case, average fiducial registration errors were less than 2mm. There was no statistical difference in registration error between CT and MR image. This system considered to be relatively stable with respect to soft and hardware. Also it was useful for the designing of the scalp incision and bone flap and assessing the extent of resection in tumors, especially in gliomas. Moreover, it was helpful to evaluate complex surgical anatomy in skull base surgery.

Computed tomography-guided 3D printed patient-specific regional anesthesia

  • Classic anesthetic techniques for the inferior alveolar nerve, lingual nerve, and long buccal nerve blockade are achieved by estimating the intended location for anesthetic deposition based on palpation, inspection, and subsequent correlation for oral anatomical structures. The present article utilizes computed tomography (CT) data to 3D print a guide for repeatable and accurate deposition of a local anesthetic at the ideal location. This technical report aims to anatomically define the ideal location for local anesthetic deposition. This process has the potential to reduce patient discomfort, risk of nerve damage, and failed mandibular anesthesia, as well as to reduce the total anesthetic dose. Lastly, as robotic-based interventions improve, this provides the initial framework for robot-guided regional anesthesia administration in the oral cavity.

  • 고관절 주변에 발생하는 석회화 건염은 그 발생빈도가 극히 드믈지만 증상이 발생하면 매우 심한 통증으로 일상 생활이 힘들어진다. 저자들은 급성 고관절 통증과 관절 운동 장애를 호소하는 환자 4례에서 석회화 건염을 진단하고 초음파 감시하 다발성 천공술을 시행하여 만족할 만한 결과를 경험하였기에 이를 보고하고자 한다.

  • Head and neck abscess usually requires hospitalization, intravenous antibiotic therapy, and surgical incision and drainage. Open surgical drainage may result in unsightly scars. We report two cases of a 56-year-old man with a facial abscess and a 47-year-old man with a parotid abscess. The patients were successfully treated with ultrasound-guided angiocatheter irrigation and drainage without scar. Ultrasound-guided angiocatheter irrigation and drainage can be a simple, safe and effective alternative procedure to open surgery in the management of the selected head and neck abscesses.

최소침습 방사능 유도 부갑상선 수술 (Minimally Invasive Radio-guided Parathyroid Surgery)

  • Introduction: Since 1996, Dr. James Norman has successfully performed mimimally invasive radio-guided parathyroidectomy(MIRP) using intraoperative nuclear mapping with $^{99m}Tc$ sestamibi scanning and radioactivity detection probe. Objectives: We aimed to introduce this new surgical technique and evaluate it's efficacy by our own experiences. Method: From May to October 1999, five consecutive patients with primary hyperparathyroidism underwent parathyroidectomy by using modified MIRP technique. $^{99m}Tc$ sestamibi scanning was performed 1.0 or 1.5 hour before operation. After intraoperative localization of the tumor under the guidance of quantitative gamma counting with a NEVIGATOR probe, an unilateral small skin incision(3.0-4.0cm) was placed. Without a skin flap, the strap muscle was directly divided with the use of a Harmonic scalpel. After careful dissection, the parathyroid tumor was removed. Result: In all patients, a single adenoma could be easily detected and removed by this new technique. Mean incision length was 3.2cm(3.0-4.0cm) and operative time ranged from 40 to 110minute. All the patients were discharged within 2 days of surgery without any complication. Conclusion: This new operative technique could become the most minimally invasive alternative to the standard operative procedure for parathyroid adenoma.

임플란트 가이드 수술시 고려사항 (Consideration of computer-guided implant surgery)

  • 우리나라 치과의 CBCT 보급률은 세계적으로 높은 편으로, 최근 1만대 보급을 돌파하고 있는 것으로 파악된다. 또한 최근 각 치과 제조업체마다 DLP방식의 In-House방식의 3D 프린터 보급률 또한 가파르게 상승하고 있다. 이에 맞춰 최근 개원가에서 CBCT와 구강 스캔 정보를 이용한 컴퓨터 가이드 임플란트 수술의 활용도가 증가하고 있는 상황이다. 현재 컴퓨터 가이드를 이용한 임플란트 수술 방법에 대해 문헌상 고찰을 통해 리뷰해보고, 또한 정확성 및 신뢰성이 보장되는 가이드 수술을 위한 고려사항에 대해 정리해보고자 한다.

  • When jaundice persists for more than 14 days postnatally, the early diagnosis of surgical jaundice is important for the prognosis in extrahepatic biliary atresia after draining procedure. The role of diagnostic laparoscopy to differenctiate medical causes of jaundice from biliary atresia is evaluated in this report. Four patients with prolonged jaundice have been included in this study. When the gallbladder was not visualized we proceeded to laparotomy. In patients with enlarged gallbladder visualized at laparoscopy, laparoscopic guided cholangiogram was performed, and laparoscopic liver biopsy was done for those who had a patent biliary tree. Two patients had small atretic gallbladder and underwent a Kasai hepato-portoenterostomy. One patients showed a patent gallbladder and common bile duct with atresia of the common hepatic and intrahepatic ducts, and they underwent a Kasai hepatic-portoenterostomy. One patient showed an enlarged gallbladder and laparoscopic-guided cholangiogram were normal. Laparoscopic liver biopsy was performed. There were no complications. Laparoscopy with laparoscopic-guided cholangiogram may be a valuable method in accurate and earlier diagnosis in an infant with prolonged jaundice.

마우스에서 히알우론산 나트륨이 항염효과에 미치는 영향에 관한 연구 (The effect of hyaluronic acid on anti-inflammatory action in mouse)

  • Purpose: The purpose of this study was not only to evaluate the relative mRNA expression of interleukin-$1{\beta}$(IL-$1{\beta}$), cyclooxygenase2 (COX-2) and prostaglandin E2 (PGE2) by RT-PCR analysis but to observe pattern of edema by light microscopic and electron microscope after topical apply of hyaluronic acid in inflammation-guided mouse. Material and methods: Mice of this study were devided into 4 groups: Control group (no inflammation guided), Positive control (inflammation guided + vaselin apply), Protopic group (inflammation guided + protopic apply), Hyaluronic group (inflammation guided + hyaluronic acid apply). Results: Hyaluronic group showed less expressions of IL-$1{\beta}$, COX-2, PGE2 than those of positive control & protopic group. Hyaluronic group revealed a decreased inflammation than positive control & protopic group in Light Microscope. Hyaluronic group appeared decreased edema of ear compare to positive control & protopic group in Elecron Microscope. Conclusion: It was considered that hyaluronic acid has an antiinflammatory effect for intercepting the gene expression of cytokines related to inflammation.
