고유수용성신경근촉진법 운동이 만성허리통증환자의 통증과 기능장애지수에 미치는 영향 (The Effects of Proprioceptive Neuromuscular Facilitation Exercise on the Pain and Functional Disability Index of Patients with Chronic Lower Back Pain)

  • Purpose: This study attempts to identify the effects of stretching and core exercise using proprioceptive neuromuscular facilitation (PNF) on the pain and functional disability index of patients with chronic lower back pain. Methods: A total of 20 patients with chronic lower back pain were randomly divided into either the experimental group (n=10), who received PNF stretching and core exercise, or the control group (n=10), who received conventional physiotherapy. Both interventions were applied three times a week for six weeks. The visible analogue scale (VAS) was measured in order to determine the level of pain, while the Oswestry Disability Index (ODI) was used to measure the change in the functional disability index. We conducted a paired t-test to compare the within-group change before and after the intervention. To compare the between-group difference, we used an independent t-test. The statistical significance level was set at ${\alpha}=0.05$ for all the variables. Results: The experimental group showed a significant within-group change in both the VAS and the ODI (p<0.01). The control group also showed a significant change (p<0.01). A significant difference was observed between the experimental group and the control group with regard to the change in both the VAS and the ODI after the interventions (p<0.01). Conclusion: In this study, the application of stretching and core exercise using PNF for subjects who complain of chronic lower back pain proved effective in reducing both pain and functional disability. We therefore expect that this intervention can be applied in the future as a useful program for patients with chronic lower back pain.

도수치료와 치료적 운동이 만성 요통의 여성 노인의 만성 통증과 신체 기능에 미치는 영향 (Effects of Manual Therapy and Theraputic Exercise on Chronic Pain and Body Function in Elder Women with Chronic Low Back Pain)

  • Background: The purpose of this study is to examine the effects of manual therapy and therapeutic exercise on pain and body function in elder women with chronic low back pain. Methods: The patients with chronic low back pain were randomly divided 2 group. The experiment group (n=11) was taken manual therapy and therapeutic exercise program. The control group (n=10) was taken physical therapy program(hot pack with 30 minutes and ICT with 15 minutes and ultra sound with 15 minutes). Both groups intervention performed 3 session during 6 weeks. We measured the quadruple visual analogue scale, flexibility, static balance, leg strength subject of symptom before and after experiment. The significant test pared t-test according to applying the manual therapy and therapeutic exercise group and control group between group used independent t-test. Results: Pain assessment in the experimental group was significantly different between time and group. Flexibility test groups were not significantly different between time and group. The static balance was not significantly different for the time. Strength tests were also not significantly different for the time. Conclusions: According to the results, manual therapy and therapeutic exercise group it was found to be helpful in pain, flexibility, static balance, not strength in elderly patients with chronic low back pain.

크로커다일 호흡 운동이 비특이성 요통을 가진 환자들의 통증과 체간 근긴장도 및 근경직도에 미치는 영향 (Effects of Crocodile Breathing Exercise on Pain, Muscle Tone, and Muscle Stiffness in Patients with Non-specific Low Back Pain)

  • PURPOSE: This study was to investigate the effects of crocodile breathing exercise on pain, muscle tone, and muscle stiffness of non-specific low back pain patients. METHODS: The subjects were 37 patients with nonspecific low back pain. The patients were divided into two groups. The experimental group (EG) performed crocodile breathing exercise and the control group (CG) performed chest expansion breathing exercise. The intervention was conducted for 10 minutes every day for a total of eight weeks. Pain was measured using a VAS. Muscle tone and stiffness were measured using Myoton PRO. Two points were measured for muscle tone and muscle stiffness. They were measured at 30mm from the spinous process of the L1 and T10 vertebra. RESULTS: Pain, muscle tone and muscle stiffness at the T10 level showed a significant decrease in both EG and CG. Muscle tone and muscle stiffness in the L1 level area decreased significantly in the experimental group but the control group did not show any difference. The only statistically significant difference was observed in the L1 muscle tone between the pre and post intervention values. CONCLUSION: This study suggests that Crocodile breathing is a good method for managing pain, muscle tone, and muscle stiffness in non-specific low back pain patients.

관동맥 조영술 후 요통완화를 위한 운동요법과 경피적 전기 신경자극의 효과 (The Effects of Exercise Therapy and Transcutaneous Electrical Nerve Stimulation for the Alleviation of Low Back Pain After Coronary Angiography)

  • Background and Purpose: After the coronary angiography procedure, patients are required to remain on bed rest to reduce the risk of bleeding and hematoma formation at the puncture site. This prolonged bed rest in the supine position is difficult for many patients, who frequently complain of low back pain. The purpose of the study was to determine whether a specially designed exercise therapy and transcutaneous electrical nerve stimulation (TENS) had an effect on the alleviation of low back pain. Method: Sixty-two patients were assigned to one of three groups : specially designed exercise therapy plus TENS plus general nursing care (exercise group N=21), general nursing care plus TENS (TENS group, N=23) or general nursing care (control group, N=18). The exercise therapy consisted of five movements including stretching, pelvic tilting, knee to chest, modified situps and trunk rotation with minimizing the motion of the puncture site. The severity of low back pain was assessed by a visual analogue scale(VAS) every two hours. The use of analgesic and any development of bleeding or other complications were monitored as well. The level of serum ${\beta}$-endorphin was determined before and after the three interventions. Result: The pain score of the exercise group was significantly lowered compared to that of the other groups. There was no difference in the serum ${\beta}$-endorphin level among three groups. Analgesic were less frequently taken by the exercise group. However the incidence of bleeding complications was not significantly different among the three groups. Conclusion: Exercise therapy is more effective than general care or TENS in alleviating low back pain of the patients with coronary angiography.

만성 요통 환자에서의 휴대용 저출력 레이저 치료기의 통증 및 기능 효과 (Efficacy of Portable Low Power Laser Therapy on Pain and Functions in Chronic Low Back Pain)

  • Objective: A prospective, assessor-blinded, randomized controlled trial was conducted in patients with chronic low back pain to evaluate the efficacy of portable low power laser therapy (LPLT) and the effect when combined with exercise therapy on pain and functions. Method: 60 patients were recruited and 56 patients, excluding 4 dropouts, were randomly allocated to the LPLT group (Group 1: 19 patients), placebo laser therapy with exercise group (Group 2: 18 patients), and LPLT with exercise group (Group 3: 19 patients). Laser therapy and exercise was performed five times a week for 4 weeks. Visual analogue scale (VAS), Schober test, lumbar range of motion (ROM) measures (flexion, extension and lateral flexion), Oswestry Disability index (ODI) were measured at baseline, at 4 weeks after intervention, and at 6 weeks after 2 weeks of no intervention. Results: Statistically significant improvements were noted in all group by time interaction with respect to all outcome parameters (p<0.05). All parameters in each group improved not only in the period of treatment (4 weeks), but also in the final evaluation (6 weeks) 2 weeks after the end of treatment. Post-hoc analysis showed statistically significant difference between the LPLT with exercise group and the other groups in all outcome parameters except for the ODI at 4 weeks and at 6 weeks. Conclusion: Portable LPLT is effective treatment in reducing pain and improving lumbar ROM and with exercise is more effective than laser or exercise monotherapy for the chronic low back pain patients.

단축발 운동을 적용한 벽 스쿼트 운동이 엎침발을 동반한 만성 허리통증환자의 통증과 골반 정렬에 미치는 영향 (The Effect of Wall-squat with Short-Foot Exercise on Pain and Pelvic alignment of Chronic Low Back Pain with Pronated Foot)

  • PURPOSE: This study examined the effects of wall-squat with short-foot exercise on pain, dysfunction, and pelvic alignment in chronic low back pain patients. METHODS: Thirty outpatients diagnosed with chronic low back pain and pronated foot were enrolled in this study. The patients were divided randomly into a wall-squat with short-foot exercise group (WS; n = 15) and a normal wall-squat exercise group (NW; n = 15). These groups performed their respective exercises 15 times, for three sets, three times a week over six weeks. The visual analogue scale (VAS) was used to measure the subjects' pain, and the Roland-Morris disability questionnaire (RMDQ) was used to measure the subjects' dysfunction. A navicular drop test (NDT) was used to measure the subjects' arch height. To assess the patients' pelvic alignment, their lordosis, sacral tilt, lumbar width, sacral width, ilium length, and ilium width were measured by X-ray imaging. RESULTS: Both the WS and NW groups exhibited significant decreases in their VAS and RMDQ scores after exercise (p < .05). The WS group exhibited significant increases in their arch height (p < .05). Significant differences in the VAS, sacral tilt, sacral width, and ilium length were observed between the WS and NW groups (p < .05). CONCLUSION: These results suggest that wall-squat exercise is effective in decreasing the level of pain and dysfunction in chronic low back pain patients. In addition, the wall-squat with short-foot exercise is considered more effective in improving the pelvic alignment than without short-foot exercise. This can be an effective method for the non-pharmacological and non-surgical treatment of chronic low back pain

Change of Pain, Lumbar Sagittal Alignment and Multifidus after Sling Exercise Therapy for Patients with Chronic Low Back Pain

  • Purpose: This study examined the effects of sling exercise therapy on vertebral alignment, VAS, muscle activity, and multifidus of patients with chronic low back pain. Methods: Simple random sampling was used to divide the patients (n=116) into the sling exercise therapy group (SETG) and conservative physical therapy group (CPTG), with each group provided a intervention program in 3 sessions a week for 12 weeks. The lumbar lordosis angle (LLA), lumbar intervertebral disc angle (LIVDA) for vertebral alignment, lumbar muscle activity, and multifidus atrophy were measured before and after the intervention. Results: SETG showed significant changes in LLA, LIVDA of rate of change (delta score), and in relieving pain. The right-left balance gap for the lumbar dynamic muscle activity decreased after the intervention. The SETG showed significant changes in the grade of lumbar multifidus atrophy. Conclusion: The sling exercise therapy program is an effective exercise therapy method on vertebral alignment, muscle activity, recovery from multifidus atrophy, and pain relief for patients with chronic low back pain.

  • Purpose : The purpose of this study is to investigate that the Effects of Pain, Lumbar Flexibility and Abdominal Obesity of Patient with Low Back Pain and Abdominal Obesity after Lumbar Strengthening Exercise. Methods : The selected subjects of this study were 17 patients who had diagnosis on low back pain and body mass index was more than $25kg/m^2$ and lumbar length was more than 90cm. They divided into a control group with 10 patients doing lumbar stabilization exercise and an experimental group with 7 patients ding simple lumbar strengthening exercise in order to compare degrees of oswestry disability index and lumbar stability, lumbar length was measured before the exercise, in 4 weeks and 8 weeks after exercises respectively. Result : lumbar strengthening exercise on patients with low back pain and abdominal obesity affected improving function of lumbar and decrease of pain with abdominal obesity. Conclusion : Then lumbar stabilization exercise than simple strengthening exercise affected decrease of Visceral fat tissue on abdominal obesity.

스위스 볼을 이용한 등척성, 등장성 운동이 요부 안정화 근육의 근활성도 및 통증에 미치는 영향 (The Influence of a Core Stability Exercise Program using Swiss Ball on Muscle Activity and Pain in the Lower Back)

  • Purpose : The purpose of this study is to examine the influence of a core stability exercise program using Swiss Ball on muscle activity, muscle thickness, maximum muscular strength, and pain in the trunk region during a 6-weeks program involving participants who have lower back pain. Methods : A total of 21 males between the ages of 20 and 33 years old were divided into 3 Swiss Ball exercise groups. Group 1 performed isometric exercises, group 2 performed isotonic exercises, and group 3 performed mixed exercises. Measurements were taken prior to starting the exercise program and after completing the program at the 6-week period using ME6000 to measure muscle activity and VAS to measure pain reduction. Comparisons were made using a paired t-test and ANOVA on SPSS 10.0. Results : There was a statistically significant effect in muscle activity for group 1 and group 3. Secondly, there was a statistically significant effect in pain reduction for group 1, group 2, and group 3. Conclusion : We found that isometric exercises performed by group 1 were effective in improving muscle activity and pain reduction.

요통환자에 있어 신전운동이 통증감소에 미치는 효과 (The Effect of Extension Exercise on the Reduction of Pain in Patients with Low Back Pain)

  • The purpose of this study was to determine whether extension exercise is effective in reducing low back pain. Nine patients with low back pain were randomly assigened to one of two groups-one was an experimental group in which the patient were treated with extension exercise(McKenzie method) and conservative therapy. The other was a control group in which the patients were treated with conservative therapy only. Treatment was performed for a period of ten days during Which we examed each patient three times: the first exam was done at the begining of treatment, the second after fives days, and the third after ten days. We used 'The Oswestry low back pain disability questionnaire'(r=.99) as the examination tool. The results were as follows: 1. Both experimental and control groups showed nonsignificant differences before and after treatment, and the between group difference was also non-significant(P>.05). 2. There was no effective reduction of low back pain by MecKenzie's extension exercise(p>.05). We admit that our study protocol had several shortcomings. If they had been accounted for, the result might have been different. Further study should be done with a better experimental design, a larger sample, and a longer experimental duration.
